1.Evaluation of uncertainty in determination of cetrimonium bromide residue in polysaccharide vaccines by ultra performance liquid chromatography-mass spectrometry

Objective To determine the cetrimonium bromide(CTAB)residue in polysaccharide vaccines using ultra performance liquid chromatography-mass spectrometry(UPLC/MS-MS),and analyze and evaluate the uncertainty of the determination results.Methods By establishing a mathematical model,the sources and values of uncertainty introduced in the measurement process were analyzed,the uncertainty components of each influencing factor were calculated,and the standard uncertainty and expanded uncertainty were synthesized to form an uncertainty report.Results At 95% confidence interval,the expanded uncertainty was 0. 002 8 mg/kg. The determination result of CTAB residue in polysaccharide vaccine was reported as(1. 000 6 ± 0. 002 8)mg/kg(k = 2,confidence interval p = 95%).Conclusion The main factors affecting the accuracy of determination results are the preparation of standard solution and the introduction of recovery rate,which should be focused on and controlled in the experiment process to make the detection results more reliable.
2.Annual advance of the research on gonadal disorders from August 2013 to July 2014

[Summary] Over the past year, researches in the field of gonadal diseases have achieved rapid advances. Considerable literatures concerning the diagnosis and treatment of these diseases have been published. We herely reviewed and analyzed literatures which may be helpful to clinical practice in the future, while the defects and deficiencies in this field in China were also discussed with some suggestions proposed for the future.
3.Metastatic solid pseudopapillary tumor of the pancreas

Objective To analyze pathologic features, therapeutic strategies and prognosis of patients with metastatic solid pseudopapillary tumor of the pancreas (m-SPTP). Methods Twenty five patients with m-SPTP undergoing radical resection between June 1985 and Dec 2005 were retrospectively analyzed. Kaplan-Meier analysis was used to screen out risk factors of the prognosis of m-SPTP patients.Results Twenty-three postoperative patients were followed up until Dec 2007. The follow-up rate was 92% and the median follow up time was 78 months. The 5-year overall survival rate was 82%. Kaplan-Meier analysis showed that five variables including age, tumor size, pathologic features, number of metastatic lesions and lymphatic metastasis were related to overall survival. Conclusions Solid pseudopapillary tumor of the pancreas is low-grade malignant potential with a favorable prognosis, but not for the m-SPTP.Patients of ≥40 years old, tumor size (≥6 cm), pathologic features (including presence of areas with diffuse growth pattern and tumor necrosis), multiple metastatic lesions and lymphatic metastasis have poor prognosis.
4.Establishment of the system of standards for assessing the managing capacity of hospital directors

We defined the prerequisite managing capacity of hospital directors through a comprehensive set of methods including literature review, interview of specialists and questionnaires. There were altogether 7 assessment items with 36 assessment indexes. The acquired assessment items and indexes were edited as Likert Scales and as signed to extprts concerned. The weight ofthe assessment indexes was obtained through a statistical analysis of the results. Hence the pre iluninary establishment of the system of standards for assessing the managing capacity of hospital directors.
5.CT Features of Atypical Primary Carcinoma of the Fallopian Tube and Ovarian Cystadenocarcinoma

Purpose To summarize the CT features and clinical characteristics of atypical primary fallopian tube cancer (PCFT) and ovarian cystadenocarcinoma (OCA), in order to improved the diagnosis. Materials and Methods CT findings and clinical data of 12 patients with atypical PCFT (PCFT group) and 20 patients with OCA (OCA group) were retrospectively studied. All patients were confirmed by pathology. Serum CA125 level, clinical characteristics and CT features of two groups were compared. Results Serum CA125 level of PCFT group and OCA group were (486.13±23.89) U/ml and (1606.94±62.86) U/ml, respectively. There was statistic difference between the two groups (P<0.01). There was significant differences on vaginal bleeding and vaginal discharge between the two groups (P<0.01). There were 4 cases with solid mass, 8 cases with cystic solid mass in PCFT group. And in OCA group, 16 cases with cystic solid mass and 4 cases with cystic mass were revealed, which showed statistic differences between the two groups (P<0.05). Some other findings also showed statistic differences including the diameter of ovarian vein on the affected side, the diameter of the round ligament of uterus on the affected side, tumor volume, and CT values of the mass in venous phase and delay phase (P<0.05 or P<0.01). However, there were no statistic significance in the size of metastatic lymph nodes, the mean CT value of the mass on plain scan and in the arterial phase (P>0.05). Conclusion Clinical findings of serum CA125 level, vaginal bleeding, and vaginal discharge combine with imaging findings of diameter of the ovarian vein in the affected side, diameter of ipsilateral round ligament of uterus, tumor volume, CT values of the mass in venous and delay phase would be the key points for differential diagnosis of PCFT and OCA.
6.Evaluation of endoscopic ultrasound for preoperative staging of ampullar tumor

Objective To evaluate endoscopic ultrasound for preoperative staging of ampullar tumors.Methods A total of 31 patients with ampullar tumors who underwent surgery from 2010 to 2015 were retrospectively reviewed.B-US,CT,MRCP and EUS were performed in all patients.The diagnostic rates of EUS and other imaging technology,and the diagnostic accuracy of preoperative staging and postoperative pathological staging were compared.Results Diagoses of 31 patients of ampullar tumors patients undergoing preoperative endoscopic ultrasonography assessment T staging were as the following:uT1,4 cases; uT2,15 cases; uT3,10 cases; uT4,2 cases.Compared with the postoperative pathological diagnosis,the T staging accuracy was 90.3% (28/31),anong which 1 case was overestimated,2 cases were underestimated.The accuracy of endoscopic ultrasound in preoperative diagnosis of ampullar lesions was superior to other detection technology.Conclusion EUS is a valuable diagnostic tool for patients with suspected ampullar tumors.
7.Pituitary abscess:a review of 7 cases and some points about its diagnosis

Objective To analyze the clinical features,auxiliary examination,diagnosis and treatment of pituitary abscess.Methods A retrospective analysis was done in 7 cases of pituitary abscess.All cases’ final diagnose were confirmed by histopathological results.Results 1.Four cases were diagnosed as primary pituitary abscess,the others 3 cases as secondary pituitary abscess which was associated with pituitary adenoma,pituitary cyst,craniopharyngeal duct tumor respectively.2.Clinical features:Of 7 cases,fever was determined in 6 cases,visual disturbance or temporal hemianopia in 5 cases,headache associated with anorexia and nausea and vomitting in 4 cases.High white blood cell counts was present in 3 cases,high PRL levels in 4 cases.Three cases were associated with hypothyroidism,2 cases with hypoadrenocorticism.MR image showed isointense or hypointense signal on T1WI and isointense or hyperintense signal on T2WI in pituitary abscess.Postcontrast T1WI showed a ring-shaped enhancement.Two cases were diagnosed by MR images and clinical features before surgery,the others by surgical histopathology.3.Treatment:All cases were treated with surgery,followed by antibiotics therapy.After operation all symptoms were improved except for optic nerve function.Conclusion It is suggestive of pituitary abscess when any patient present firstly with fever,headache,visual or eyeshot disturbance,partial or whole hypopituitarism associated with saddle area mass.Earlier diagnosis,timely surgery and postoperative proper antibiotics therapy are the keys to the treatment of pituitary abscess.
8.Hyperoxia solution improves oxygenation in patients with hypoxemia after orthotopic liver transplantation

Objective Post-operative hypoxemia occurred frequently after extubation in patients who underwent orthotopic liver transplantation (OLT) in ICU. This study is to investigate the efficiency of hyperoxia solution in improving oxygenation. Methods Ten OLT cases, who post-operatively manifested hypoxemia after extubation, were enrolled in this study. Hyperoxia Ringer's solution (500ml) prepared by Jing-Shu-Yang (an apparatus of oxygen supply, Vanform Co., Shandong) was infused at a rate of 200ml/hr. Hemodynamic parameters, blood gas, and blood lactate were determined at the beginning, end of infusion, and 1 hour after the infusion, with recording of the following data: heart rate (HR), mean arterial pressure (MAP), pulmonary arterial pressure (PAP), and pulmonary capillaries wedged pressure (PCWP). Results Compared with the levels at the beginning of infusion, SpO2 and SvO2 increased significantly at the end of the infusion of 500 ml hyperoxia Ringer's solution, and they returned to the baselines 1 hour later, although regular Ringer′s solution was coutinuously given by the same infusion rate. HR, MAP and PCWP were kept stable with the solution infusion. Notably, PAP was decreased markedly at the end of infusion, and blood lactate level remained unchanged after the intervention. Conclusion Hyperoxia Ringer's solution infusion improved oxygenation in OLT patients who suffered from acute lung injury with hypoxemia postoperatively. PAP decreased markedly as hypoxemia was corrected.
