1.Clinical Study on Fire-needle Therapy for Rheumatoid Arthritis by Adopting WHOQOL-BREF Questionnaire

Objective To observe and compare the effects of fire-needle therapy, acupuncture, acupoint thread embedding on the quality of life (QOL) and clinical efficacy in patients with rheumatoid arthritis (RA).Method Ninety RA patients were randomized into a fire-needle therapy group, an acupuncture group, and an acupoint thread embedding group,30 cases in each group. The fire-needle therapy group was intervened by fire-needle acupuncture, the acupuncture group was by ordinary acupuncture treatment, and the acupoint thread embedding group was by acupoint thread embedding. The fire-needle therapyand ordinary acupuncture were given once every other day, and the acupoint thread embedding was given once every 2 weeks, 1 month as a treatment course. The therapeutic efficacy was evaluated before the treatment, and respectively after 1 and 2 treatmentcourses, and the World Health Organization Quality of Life (WHOQOL)-BREF questionnaire was adopted for evaluation.Result After 1 or 2 treatment courses, the scores in the domain of physical health were significantly changed compared to those before treatment in each group (P<0.01,P<0.05). After 1 treatment course, the scores of physical health and psychological domain in the fire-needle therapy group were significantly higher than those in the acupoint thread embedding group (P<0.01). After 2 treatment courses, the scores of physical health and psychological domain in the fire-needle therapy group were significantly higher than those in the other two groups (P<0.01,P<0.05). Regarding the domain of social relationships, the score in the fire-needle therapy group after 1 treatment course was significantly higher than that before treatment (P<0.05); the score in the acupoint thread embedding group after 2 treatment courses was significantly higher than that before treatment (P<0.05). There were no significant between-group differences in comparing the score in the domain of social relationships (P>0.05). The scores in environment domain in the fire-needle therapy group and acupuncture group were significantly higher than those before treatment (P<0.01); the scores in the fire-needle therapy group and acupuncture group were superior to that in the acupoint thread embedding group after 1 treatment course (P<0.01);there were no significant between-group differences in comparing the score in environment domain (P>0.05). Conclusion Fire-needle therapy is an effective approach in treating RA, and it can significantly boost the QOL of the RA patients.
2.Advances on enteral nutrition of children with acute pancreatitis

Acute pancreatitis with high decomposition and metabolic state, is one of the common acute abdominal pain in children. The patients are vulnerable to malnutrition during the illness. Reasonable nutrition therapy has an important role in pancreatitis management. Compared with parenteral nutrition,enteral nutrition,especially early enteral nutrition within 48 hours,can maintain the gut barrier function,reduce the incidence of enterogenic infection and multiple organ function failure,shorten length of hospital stay and re-duce the hospitalization expenses. Enteral nutrition should be initiated first,if feasible. Enteral nutrition,either by nasojejunal tube or via nasogastric tube,is believed to be safe and effective.
3.Clinical value of NMP22 and cytology in bladder cancer

Objective To investigate the clinical applications of nuclear matrix protein 22 (NMP22) and urine cytology in early diagnosis,monitoringrecurrence and determining prognosis of bladder cancer.Methods Ninty-six urine specimens,including 45 cases before the resection of bladder cancer (pathologically confirmed),20 cases after the resection of bladder cancer and 31 cases with benign urinary tract condition,were both selected in detecting NMP22 by enzyme-linked i mmunosorbent assay (ELISA),and the results were compared with urinary cytology by x2 test.Results The NMP22 content of 45 cases before the resection of bladder cancer was 9.3 to 112.5 U/mL,the median was 48.7 U/mL.The NMP22 content of 31 cases with benign urinary tract condition was from 2.1 to 14.7 U/mL,the median was 7.9 U/mL.The NMP22 content of 20 cases after the resection of bladder cancer was from 4.3 to 18.7 U/mL,the median was 8.9 U/mL.The median of NMP22 before the resection of bladder cancer was significantly higher than the median in patients with benign urinary,the difference was statistically significant (P <0.05).Considering NMP22 ≥ 10 U/mL as the critical value,the sensitivity of the NMP22 in diagnosing bladder cancer was 82.2% and the specificity was 70.9%.And the sensitivity of urine cytology was 31.1% and the specificity was 100%.The recurrence of 9 cases was confirmed by cystoscopy in 20 cases after the resection of bladder cancer.Conclusion The NMP22 can be a effective biomarker in the early screeningand postoperative follow-up of bladder cancer.

5.Advances in nutrition screening and nutritional assessment among critically ill children

Malnutrition is common in critically ill children.Nutrition prescription is hampered by various factors,and the nutritional status of these children often deteriorates during hospitalization.Nutrition therapy for critically ill children must be based on proper nutrition screening and nutritional assessment.Adopting dynamic nutritional assessment is an effective method to monitor patients under nutrition therapy and inform the prescription and administration of nutrition therapy.This article reviews the recent advances in nutrition screening and nutritional assessment in critically ill children.
6.Assessing patient's capacity to consent in mental health practice:A review

Capacity assessment is of significant priority for medical treatment and clinical research invol-ving mental disorder patients,as to protect an individual's fundamental rights.Clinical judgment of individual's ca-pacity to consent most focus on medical situation,well-designed capacity assessment instruments should be used to support experienced clinical judgment.Capacity to consent can be adequately captured by measuring the constructs of understanding,appreciation,reasoning and communicating /expressing a choice.A large number of instruments designed by foreign developers to assess capacity to consent need validation and implementation in China,the in-strument designed by Chinese researchers has not been widely agreed.Establishing an effective capacity assessment instrument with psychometric properties (validity and reliability),which considering the clinical practice and culture background in china would be a breakthrough in methodology.
7.Effects of emulsified isoflurane on apoptosis in human neuroblastoma cells : the role of c-Jun N-terminal kinase

Objective To investigate the effects of emulsified isoflurane on apoptosis in human neuroblastoma cells and the role of c-Jun N-terminal kinase (JNK) in it.Methods The human neuroblastoma SHSY-5Y cells were seeded in 96-well plates or dishes and then randomly divided into 8 groups using a random number table:control group(group C,n=24),different concentrations of lipid emulsion groups(LE1 groups [n =24],LE2 group [n =24] and LE3 group [n =72]),different concentrations of emulsified isoflurane groups (EI1 group [n=24],El2 group [n=24] and EI3 group [n=72]),and emulsified isoflurane + JNK inhibitor SP600125 group (group EI-SP,n =24).At 24 h after the cells were plated,in LE1-3 groups,30% lipid emulsion was added to the culture medium with the final concentrations of 0.395 6,0.791 2 and 1.582 4 μl/ml,respectively;in EI1-3 groups,8% emulsified isoflurane was added with the final concentrations of 0.56,1.12 and 2.24 mmol/L,respectively;in group EI-SP,emulsified isoflurane was added with the final concentration of 1.12 mmol/L,and SP600125 was added at 1 h before addition of emulsified isoflurane with the final concentration of 10 μmol/L;the cells were cultured normally in group C.At 6,12 and 24 h of incubation in EI3 and LE3 groups,and at 24 h of incubation or culture in the other groups,the morphology of cells was detected,the cell viability was measured using methyl thiazolyl tetrazolium assay,and the expression of JNK,phosphorylated JNK (p-JNK) and cytochrome c (Cyt c) was detected by Western blot.Results Compared with group C,the cell viability was significantly decreased,and the expression of p-JNK and Cyt c was significantly up-regulated at 24 h of incubation in group EI2 and at 12 and 24 h of incubation in group EI3,the cell viability was significantly decreased (P<0.05),and no significant change was found in the expression of p-JNK and Cyt c in group EI-SP,and no significant change was found in the cell viability and expression of p-JNK and Cyt c in LE1-3 and EI1 groups (P>0.05).Compared with group EI1,the cell viability was significantly decreased,and the expression of p-JNK and Cyt c was significantly up-regulated at 24 h of incubation in EI2.3 groups (P<0.05).Compared with group EI2,the cell viability was significantly decreased,and the expression of p-JNK and Cyt c was significantly up-regulated at 24 h of incubation in group EI3,and the cell viability was significantly increased,and the expression of p-JNK and Cyt c was significantly down-regulated in group EI-SP (P<0.05).There was no significant difference in JNK expression between the eight groups (P>0.05).Conclusion High concentrations of emulsified isoflurane can induce apoptosis in neurons only when applied for a long time,while low concentrations do not have the effect when applied for a short time.The mechanism by which emulsified isoflurane induces neuronal apoptosis is related to activation of JNK pathway.

Zebrafish has been widely used as an important model system in research fields of genetic and develop -mental biology over the past 20 years.Similar to the mammalians and other vertebrate animals , zebrafish also has various tissue barriers.In recent years, more and more important progress of tissue barrier studies have been achieved using ze -brafish as in vivo model, such as blood-brain barrier.These findings contribute to the understanding of the mechanisms of diseases caused by the disorders of physiological barriers .It also helps with the modulation of the permeability of tissue bar-riers for drug delivery .This review summaries recent progress of zebrafish applications in the study of tissue barriers , such as blood-brain barrier, blood-retina barrier, epidermal barrier, etc.
9.Professor LIN Guohua's experience of gold implantation at acupoint for rheumatoid arthritis.

Based on the pathogenesis and symptom of rheumatoid arthritis (RA), professor LIN Guohua's unique opinion and method for RA in clinical treatment are summarized and analyzed. In the opinion of Professor Lin, RA is considered as "Jinbi" and "Gubi" in TCM, which is caused by deficient root with superficial excess. Based on the symptoms of RA, attention should be focused on lung-kidney diagnosis and treatment, and gold and catgut implantation at acupoint can be mutually combined, which is aimed to provide a special and effective method for clinical treatment of RA.
