1.The effect and prognosis study on super-elder patients with myocardial infarction by Shexiang baoxin pillls

Objective To explore the effect of Shexiang baoxin Pillls on super‐elder patients with myocardial infarction ,and observe and analyze its prognosis .Methods Two hundred super‐elder patients with myocardial infarction were selected in the hos‐pital from December 2013 to November 2014 ,they were randomly divided into control group(n=100) and observation group(n=100) .The control group used conventional drug treatment ,and the observation group treated with Shexiang baoxin pillls on the ba‐sis of the control group .Three months wasr a treatment period .To observe the clinical therapeutic effect ,echocardiography was used to detected the left ventricular wall thickness ,left ventricular end‐diastolic diameter (LVEDD) ,left ventricular ejection fraction (LVEF) before and after treatment .Observed 6 minutes walking distance .Total cholesterol (TC) ,blood lipid ,low density lipopro‐tein (LDL‐C) ,c‐reactive protein (CRP) ,and other indicators were detected .Results The total effective rate of the observation group was [72 .00% (72/100)] was higher than the control group [65 .00% (65/100)] ,P<0 .05 .The re‐hospitalization rate of the observation group was [23 .00% (23/100)] was lower than the control group [34 .00% (34/100)] ,P<0 .05 .The left ventricular posterior wall thickness and LVEDD of the observation group after treatment were(10 .12 ± 0 .40)mm and(49 .11 ± 1 .39)mm , which were lower than the control group after treatment [(11 .06 ± 0 .52)mm and (51 .36 ± 1 .46)mm] ,LVEF (% ) of the observa‐tion group after treatment was (0 .51 ± 0 .12)% ,which was higher than the control group after treatment (0 .47 ± 0 .11)% ,all P<0 .05 .TC ;LDL‐C and CRP of the observation group after treatment were (5 .19 ± 0 .32) mmol/L ,(2 .83 ± 0 .30) mmol/L and (54 . 11 ± 4 .83) mg/L ,they were lower than the control group after treatment [(5 .48 ± 0 .37) mmol/L ,(3 .10 ± 0 .33) mmol/L and (62 . 38 ± 4 .36) mg/L] ,all P<0 .05 .6 min walking distance of the observation group after treatment was (376 .38 ± 19 .41)m ,it was higher than the control group after treatment(331 .04 ± 22 .18)m ,P<0 .05 .The complications of the observing group[38 .00% (38/100)] was lower than the control group after treatment 47 .00% (47/100) ,P<0 .05 .Conclusion There is better clinical effect on super‐elder patients with myocardial infarction by Shexiang baoxin Pillls ,which can reduce hospitalization rates again ,improve left heart function and blood lipid levels ,reduce complications .
2.Study on Spectrum-effect Relationship of Antipyretic Effect of Radix Bupleuri Injection

OBJECTIVE:To study the spectrum-effect relationship of the antipyretic effect of Radix Bupleuri injection. METH-ODS:HPLC method was used to establish the fingerprint of Radix Bupleuri injection. The antipyretic effect of 10 batches of Radix Bupleuri injection on fever rats induced by dried yeast were determined respectively. The fingerprint peaks were screened,along with the temperature value of different time points(0,10,15,30,40,55,70 min). The principal components were extracted by principal component method and the indirect relationship between the fingerprint and antipyretic effect was analyzed,depending on the principal component correlation coefficient matrix. RESULTS:There were 39 common peaks in fingerprint(similarity>0.85), No. 8,12,14,19,26,31,34,35 and 39 common peaks with large peak area were included in the study. Four principal compo-nents were extracted by principal component analysis(87% of total variant). First principal component showed that the active com-ponents of the 12th peak may be related to the antipyretic effect of 6 to 13 hours. The second principal component showed that the active components of the 26th peak may be related to the antipyretic effect of 0.5 to 5 hours. The third principal component showed that the similar effect of the active components could be caused by 34th,35th and 39th peaks. The fourth principal component sug-gested that there were some similarities between the 14th and the 31st peaks. CONCLUSIONS:Radix Bupleuri injection have obvi-ous improvement for fever rats. There is certain corresponding relation between HPLC fingerprint and antipyretic effect of Radix Bu-pleuri injection .
3.Role of Notch1 signal pathway in osteogenic differentiation of human periodontal ligament stem cells

Objective To explore the effect of Notch1 signal pathway on the osteogenic differentiation in human periodontal ligament stem cells(PDLSCs) under dynamic strain.Methods PDLSCs were separated from freshly extracted teeth then identified and prolifed.Notch1 signal pathway was regulated by chemicals.Dynamic strains were applied to PDLSCs with the tension plus system.Then Notch1 signal pathway key factor Notch intracellular domain(NICD),osteoblastic related indexes alkaline phosphatase(ALP) and bone morphogenetic proteins 2 (BMP2) were detected by western blot expression.The deformation rate of stress parameters was 0 to 12%,and the frequency was 0.1 Hz.The loading time was 0 h,6 h,12 h and 24 h.Results As Notch1 signal pathway was activated,the expression of osteogenic markers ALP and BMP2 both reduced (P<0.05).On the contrary, the expression of osteogenic markers ALP and BMP2 both increased obviously (P<0.05) as Notch1 signal pathway was inhibited.Conclusion The activated Notch1 signal pathway will inhibit osteogenic differentiation of PDLSCs under dynamic tensile.
4.SUPRAGLOTTIC HORIZONTAL PARTIAL LARYNGECTOMY FOR LARYNGEAL CARCINOMA: A Report of 41 Cases

From 1982 to 1988, we have performed supraglottic horizontal partial laryngectomy for 41 cases of laryngeal carcinoma. 19 of them were males and 22 females. 40 cases of squamous cell carcinoma accounted for 97.6%,and 75.6% of cases were in second or third stage. All the cases have been followed up. The three-year survival rate is 93.7%, the five-year survival rate is 86.5%. 95% of the patients retained their phonation ability. Decannulation rate of tracheostomy tube is 65.9%. Normal deglutition ability is found in 90.2% of cases. Indications for operation, postoperative complications, late results, and failure of removal of tracheostomy tube are discussed.
5.The application of the proton and carbon ion treatment equipment imaging system in prostate cancer therapy

Objective To evaluate the accuracy of the patient positioning in prostate cancer therapy with the kilo-volt X-ray imaging system. Methods From June to August in 2014,14 patients with prostate cancer were treated by carbon ion beam with the Siemens proton and carbon ion equipment. X-ray orthogonal images was made twice by the imaging system before treatment. All the images were fused with the planning digitally reconstructed radiograph ones by automatic and manual method. We got positioning error data in six directions:lateral, longitudinal, vertical, isocentre, roll and pitch. The data were analyzed to study the situation. Results A total of 644 pairs of images from 14 patients were obtained during treatment. The average error in six directions is 0. 34 cm,0. 30 cm,0. 79 cm,0. 3°,1. 1° and 0. 5°.The patients′positioning error had no obvious change in all directions with fractions. There was no significant deviation between the two groups of images,either. But error in pitch and vertical direction is much larger. Conclusions The bony landmarks of the orthogonal filed images are clear. Photography lasts short time by the imaging system. DRR images are 3D and we can get images in six directions,but the development of soft tissue is weak. Positioning error we get cannot be match with the one of actual situation.
6.Treatment and clinical analysis of larngeal function preserring surgery in hypophngeal carcinoma

Objective:To evaluate the therapeutic effect and the prognosis of the laryngeal function preserving surgery (LFPS) and total laryngectomy in hypophngeal carcinoma.Method:Ninety-three cases of laryngopharynx carcinoma were retrospectively analyzed from September 1974 to September 2006.Among which 57 cases were primary pyriform sinus cancer,20 cases were postero-cricoid cancer and 16 cases were postero-pharyngeal wall cancer.53 cases were treated by LFPS,and 40 cases were treated by total laryngectomy.The treatment effectiveness,complication,survival rate and repair materials were analysed.Result:Adopting Kaplan-Meier survival analysis,the 3-year survival rates were 69.9%and 5-year survival rates were 43.0%.The 3-year survival rates of LFPS and Non-LFPS were 73.6% and 67.5%, and the 5-yenr survival rates of LFPS and Non-LFPS were 49.1% and 32.5%. There was no significant difference between the two groups(χ~2=2.566,P>0.05). Single element analysis indicated survival rates concerned with T-stage(χ~2=9.764,P<0.05), neck lymphatic transfer(χ~2=10.472,P<0.01) and the degree of pathological differentiation(χ~2=25.894,P<0.01).Cox regression analysis suggested that T-stage,the degree of pathological dfferentiaton and wheather going through the surgical operation were the independent elemant of the patient's prognosis.There was no significant difference between LFPS and No-LFPS in the neoplasms location, complication,neoplasms residuum and neoplasms recurrence.Conclusion:LFPS did not affect the survival rates and LFPS was feasible.LFPS can increase the living quality of laryngopharynx careinoma patients.
7.Analysis on behavioral characteristic and the knowledge-attitude-belief-practice of rural community patients with chronic obstructive pulmonary diseases

Objective To know social and demographic characteristics,behavioral characteristics,as well as knowledge-attitude-belief-practice(KABP) and its influential factors of rural community chronic obstructive pulmonary diseases(COPD) patients. Methods Cluster sampling all COPD patients of fangcun and mape township of tongshan county in 2007. A face-to-face interview was performed to investigate the COPD patients in their home,and t test,one-way analysis of variance and multiple linear regression were used to analyze the behavioral characteristics and K.ABE Results The main Patients were old person aged 60-80,with mean of(63 ± 15)years old. The active and passive smoking rate were 49.5% and 35.9%,respectively,and very significance between male and female Patients (X2 = 101. 365,P< 0. 05). 62. 9%abstained from smoking mainly owing to having COPD. About 31.9% of the patients had used the firewood and coal to cook for longer than 30 years. 24. 2% often built their body by exercise; however,only 8. 9%participated in convalescence. The mean KABP score of Patients was (38 ± 8). The Patients have lower knowledge to COPD, but give much more concerns to their own diseases. Univariate and multivariate analysis indicated that the main influential factors of COPD patients KABP were education, family attitude, income,sex and the distance between their homes and health stations. Conclusions Community COPD patients have high smoking rate,long firewood and coal cooking history,so active interventions should be took to those risk factors. KABP level of the COPD patients was not good, more health instructions including prevention,treatment and convalescing of COPD need to be gave to COPD patients.
8.Disease perception and awareness in patients with chronic obstructive pulmonary disease in rural Xuzhou

Objective To evaluate perception and awareness of disease knowledge in patients with chronic obstructive pulmonary disease (COPD) in rural area of Xuzhou City. Methods Perception and awareness of COPD, including its nature, control, medication use, impact on their quality of life, experience of its acute exacerbation, and so on, in 392 patients with COPD were evaluated with self-designed ad hoc questionnaire. Proportion and percentage were used in statistical analysis. Results According to the GLOD standards, 58 of 392 patients with COPD could be classified as stage Ⅰ (14. 8%), 164 as stage Ⅱ (41.8%), 103 as stage Ⅲ (26.3%) and 67 as stage Ⅳ (17.1%), respectively. Only 14 patients (3.6%) had heard COPD, but all of them did not know the definition of COPD and its acute exacerbation. Lung function tests and health education had never been performed for all these patients. One hundred and twenty-six patients (32. 1%) did not understand that smoking is a risk factor for COPD, 343 patients (87. 5%) had never heard that COPD could be prevented and controlled. Only 95 patients (24. 2%) did regular physical exercises. Seventy-six patients (19. 4%) in stable stage did not take theophylline regularly, and none of them took inhalers or nebulizer drugs regularly or temporally, or oxygen therapy and pulmonary rehabilitation at home. During acute exacerbation, 261 patients (66.6%) could take theophyUine or analogous drugs and antibiotics. One hundred and five patients (26. 8%) were admitted to hospital during the past year due to respiratory symptoms. Physical activities were restrained in 224 patients (57.1%), 91 patients (23. 2%) had a little bit feeling of depression sometimes and social communication was impacted in 232 patients (59. 2%). Conclusions Patients with COPD in rural Xuzhan, Jiangsu province were poor in knowledge of COPD, and their treatment during stable stage and acute exacerbation did not meet the standards. COPD had significant impact on their daily life, and education and management for the patients should be strengthened.
9.Investigation of the peripheral blood mononuclear cells expressing HLA-G of patients with rheumatoid arthritis

Objective To investigate the expression of HLA-G in lymphocytes,CD3+ T cells,and CD14+ monocytes from patients with rheumatoid arthritis (RA) and their associations with disease activity,disease duration,medication used and clinical parameters.Methods We studied 68 RA patients as well as 63 healthy controls.HLA-G expression was analyzed by flow cytometry in peripheral blood mononuclear cells (PBMCs).Mann Whitney U tests and Spearman rank correlation coefficient test were used for statistical analysis.Results We found that lymphocytes as well as CD3+ T cells,and CD14+ monocytes in RA patients showed a diminished expression of HLA-G compared with healthy controls [1.98%(1.17%-3.66%) vs 2.93% (1.70%-5.40%),U=1624.5,P<0.05; 1.35%(0.57%-2.79%) vs 2.16%(1.15%-4.45%),U=1387,P=0.000; 22.65% (10.59%-36.81%) vs 32.76% (20.73%-44.62%),U=1526,P<0.01].Spearman analysis showed that the levels of HLA-G+ lymphocytes as well as CD3+ HLA-G+ T cells was negatively associated with DAS28 score(r=-0.288,P=0.017; r=-0.373,P=0.001),but no significant correlation was detected between the levels of CD14+HLA-G+monocytes and DAS 28 score (P>0.05).In addition,the levels of expression of HLA-G was not correlated to disease duration,medication used,ESR,CRP,RF,anti-CCP and ANA (P>0.05).Conclusion Decreased expression of HLA-G are detected in lymphocytes,CD3+ T cells,and CD14+ monocytes from pa-tients with RA,and the abnormality of HLA-G may play an important role in the development of RA.
10.The nongenomic effects of progesterone in repressing iNOS activation through P38MAPK pathways in gonococci-infected polymorphonuclear leukocytes and the clinical significance.

Progesterone has nongenomic effects on inducible nitric oxide synthase (iNOS), which is mediated by mitogen activated protein kinase (MAPK) pathways. This effect is supposed to have some potential association with asymptomatic gonococcal infections in women by immunological depression. In this study, polymorphonuclear leukocytes (PMNs) challenged by gonococci were used to study the nongenomic effects of progesterone. The activation of iNOS was assessed by measuring [(3)H] L-arginine converses to [(3)H] L-citrulline, and the activity of MAPK was detected by Western blot. It was found that the activity of iNOS and the yields of NO were enhanced significantly in gonococci-challenged PMNs compared with the controls (P<0.01). Progesterone could repress the activation of iNOS through P38MAPK pathway within PMNs (P<0.05), which could be blocked by SB203580 (P<0.01), but not by actinomycin D (P>0.05). It was also found subsequently that in the serum specimens collected from gonococci-infected but asymptomatic women, the progesterone level was higher than that in women with severe symptoms (P<0.01). Moreover, the yield of NO had an inverse correlation with progesterone. With these results it suggested that the rapid nongenomic effects of progesterone may inhibit iNOS activation and NO yields mediated by P38MAPK pathways, which were supposed to be concerned with asymptomatic women infected with gonococci.
