1.Primary myoepithelial carcinoma of the preauricular area in a young female: a rare case report.

A female patient with a dark red mass in the of preauricular area for over 3 months was hospitalized. Enhanced CT scan and ultrasonography showed that the mass had clear boundaries and was rich in blood supply. The patient was misdiagnosed as preauricular hemangioma, then the mass was removed as a benign tumor and found irrelevant to parot, SMA(+), Vim(+), S-100(+), indicating myoepithelial carcinoma. So a radical excision was performed to get negative incised margin. The patient didn't undergo postoperative chemotherapy but demonstrated no evidence of recurrence over a 12-month follow-up.

2.Pingyangmycin Local Injection Under Suspension Microlaryngoscopy for Hemangioma at Laryngopharyngeal and Laryngeal in 22 Cases

Objective To study the therapeutic effect of pingyangmycin local injection under suspension microlaryngoscopy for treatment of laryngopharyngeal and laryngeal hemangioma. Methods The total of 44 cases of patients with laryngopharyngeal or laryngeal hemangioma were randomly divided into the experimental group ( n=22 ) and the control group (n=22). They were treated with bleomycin 5 mg or pingyangmycin 4-8 mg injection under suspension microlaryngoscopy in the hemangioma, respectively. If the treatments were not curative, repeated one more times in intermittent 15 d, but not more than 3 times. Therapeutic effect and adverse effect were recorded and analyzed. Results The total effective rate (95. 4%)in the experimental group was significantly better than that (59. 1%) in the control group (P<0. 05),and side reaction in two groups showed no significant variation. Conclusion Pingyangmycin local injection for treating laryngopharyngeal and laryngeal hemangioma is effective, less adverse reaction.
3.Clinical trials of tulobuterol patch in the treatment of acute exacerbation of chronic obstructive pulmonary disease in the elderly

Objective To evaluate the efficacy and safety of tulobuterol patch in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD) in the elderly. Methods Totally 165 elderly patients with moderate to severe AECOPD were divided into trial group and control group randomly.83 patients of trial group were treated with tulobuterol patch,2 mg once a day,and fluticasone inhalation 250 μg twice a day; 82 patients of control group were treated with inhalation of salmeterol/fluticasone 250 μg/50 μg twice a day.Basic therapy was the same in two groups. Results After 10-14 days' treatment,forced expiratory volume in one second(FEV1),peak expiratory flow (PEF),6-min walking distance and symptom scores were ( 1.30 ± 0.31 ) L,(245.3 ± 56.1 ) L/min,(263.0±53.2)m,(33.2±12.1)scores in trial group,and (1.21±0.23)L,(213.9±58.4) L/min,(230.0±45.6)m,(37.8± 14.5) scores in control group,respectively.The lung function,6 minute walk distancc and symptom score were improved (t=2.120,3.521,4.279,2.212,all P<0.05).The frequencies of rescue medication,waking-up suffocating at night and the days of hospital stay were deceased significantly in trial group as compared with control group[(2.5 ± 0.6) time/d,( 1.8 ± 0.5)time/week,(12.9±1.6)dvs.(2.90.8) time/d,(2.2±0.7) time/week,(14.1±1.8) d,t=3.610,4.219,4.524,all P<0.05].The incidence of adverse reactions was not significant difference between two groups[8 cases(9.6%)vs.7 cases(8.5%),P>0.05] and the adverse reactions were mild.Conclusions Tulobuterol patch is a newly formulated,effective and safe medication for the treatment of acute exacerbation of AECOPD.
4.Effect of Stereotactic Aspiration Combined with Drug Therapy in the Treatment of Cerebral Hemorrhage Induced by Antithrombotic Drugs

Objective:To observe the effect of stereotactic aspiration combined with corresponding drug therapy in the treatment of cerebral hemorrhage induced by different drugs. Methods:Totally 66 patients with hemorrhage induced by antithrombotic drugs were randomly divided into the control group and the treatment group with 33 cases in each. The control group was given corresponding drug therapy according to the pathogenesis of cerebral hemorrhage, and the treatment group was treated by stereotactic hematoma aspiration additionally. Daily life ability, NIHSS and prognosis improvement were compared between the two groups before and after the treat-ment. Results:After the treatment, NIHSS of the treatment group was ( 10. 17 ± 2. 97 ) and Barthel index score was ( 59. 97 ± 12. 10), which were both better than those of the control group (P<0. 05). The prognosis of the treatment group was also better than that of the control group (P<0. 05). There was no severe adverse drug reaction in the two groups. Conclusion:For different drug in-duced cerebral hemorrhage, corresponding drug therapy should be carried out, and combining with stereotactic hematoma aspiration, the therapeutic regimen can effectively improve neurological deficits and activities of daily living of the patients, and then improve the prognosis of the patients with promising safety.
5.Effect of total flavone in Sanguis Draconis on platelet aggregation,thrombus formation and myocardial ischemia

Object To investigate the effect of total flavone in Sanguis Draconis (tFSD) on platelet aggregation, thrombus formation and myocardial ischemia in rat and rabbit. Methods Adopting the experimental models including in vitro platelet aggregation in rat and rabbit, in vivo thrombus formation in rat internal vein, and acute myocardial ischemia by ligating the coronary artery. Results In vitro, tFSD could, to a certain degree, inhibit the platelet aggregation induced by ADP and PAF. Moreover, tFSD could markedly restrain artificial thrombus formation in rat internal vein and the area of acute myocardial ischemia after ligating the coronary artery. Conclusion tFSD has the function of promoting blood circulation and removing blood stasis, anti thrombosis, together with platelet aggregation inhibition.
6.Urinalysis of transforming growth factor-β1 in old patients with diabetic nephropathy

ObjectiveTo search detecting urine transforming growth factor-β1(TGF-β1) of type 2 diabetes mellitus(DM) in diagnosing early diabetic nephropathy in old patients.MethodsTGF-β1,microalbumin,microglobulin α1 and microglobulin β2 in urine from 53 cases of old type 2 diabetics and 27 cases of normal controls were detected.ResultsTGF-β1 was(21.2±7.32) ng/mmolCr in diabetics whose urine albumin were normal and was(58.79±25.93) ng/mmolCr in diabetics whose urine microalbumin was abnormal(P<0.01).Both were significantly higher than that of the normal control which was(7.79±3.58) ng/mmolCr(P<0.01).TGF-β1 was direct remarkably correlative with microablumin(r=0.5415),microglobulin α1(r=0.6282) and microglobulin β2(r=0.3248)(P<0.01 respectively),but was nothing with glycohemoglobin(r=0.2304,P>0.05).ConclusionTGF-β1 detection may be used as a sensitive and reliable guideline to diagnose early diabetic nephropathy in old diabetics.
7.Effects of Hyperbaric Oxygen on Cognition after Leukoaraiosis

Objective To study the effect of hyperbaric oxygen on cognitive dysfunction in patients with leukoaraiosis (LA). Methods 48 cases of moderate or severe LA with cognitive dysfunction were divided into 2 groups: the experimental group accepted hyperbaric oxygen (HBO) besides routine therapy, while the control group accepted routine therapy only. Both groups were assessed with Montreal cognition assessment (MoCA). Results The scores of MoCA in patients in the experimental group improved after treatment(P<0.05), and was more than that of the control group(P<0.05). Conclusion HBO can improve the cognitive function in patients with LA.
8.Short-term effect of laparoscopy in combination with fast-track colorectal surgery on colorectal cancer in the elderly

Objective To evaluate the safety and feasibility of laparoscopy in combination with fast track colorectal surgery (FTCS) in the treatment of colorectal cancer in the elderly.Methods A total of 123 patients were randomly divided into 3 groups:the laparoscopy plus FTCS group (n=41),the laparoscopy group (n=41) and the laparotomy group (n=41).Parameters for measuring surgical quality,recovery and postoperative complications were analysed.Results No significant differences were found in age,gender,tumor location,anesthesia ASA classification,American Joint Committee on Cancer (AJCC) Tumor Node Metastasis (TNM) staging,Eastern Cooperative Oncology Group (ECOG) score or complications between the three groups (P>0.05 for all).There were no differences in blood loss,operative time,time required to resume defecation or number of lymph nodes dissected between the laparoscopy plus FTCS group and the laparoscopy or laparotomy group (P>0.05 for all),but time taken to initiate postoperative ambulation,time taken to resume flatulence,time taken to start intake of liquid food and length of hospital stay were shorter in the laparoscopy plus FTCS group than in the other groups (P < 0.05 for all).The incidence of postoperative complications was 12.2% or 5/41 in the laparoscopy plus FTCS group,which was lower than in the laparoscopy group (34.1% or 14/41) and in the laparotomy group (68.3% or 28/41) (x2 =5.549 and 28.826,P=0.018 and 0.01,respectively),a statistically significant difference was also found between the latter two groups (x2 =9.567,P =0.002).Conclusions Laparoscopy in combination with FTCS is safe and effective in the treatment of colorectal cancer in the elderly.
9.Visual function assessment after correction of enophthalmos deformity caused by orbital blow-out fractures with porous high-density polyethylene implantation

BACKGROUND: Enophthalmos deformity is the most common complication caused by orbital blow-out fractures, and others are diplopia and worsening of visual acuity. Since the therapeutic result of orbital blow-out is not satisfactory and many complications exist after operation, it is still a dispute to select implantation materials and therapeutic regimens.OBJECTIVE: To observe the therapeutic effect and assess the improvement of visual function by surgical reconstruction with porous high-density polyethylene (Medpor) for the correction of enophthalmos deformity caused by orbital blow-out fractures.DESIGN: A pre-and postoperative controlled study.SETTING: Beauty Center for Trauma Repair,Plastic Surgery Hospital,Peking Union Medical College, Chinese Academy of Medical Science .PARTICIPANTS: Totally 56 patients with orbital blow-out fractures who had enophthalmos deformity caused by fists or traffic accidents, treated at Beauty Center for Trauma Repair,Plastic Surgery Hospital, Peking Union Medical College, Chinese Academy of Medical Science, were selected in this study from December 1996 to March 2004. Final diagnosis were made with case history, X-ray film, two-demensional and three-dimensional CT before operation. 24 cases were accompanied with other areas fractures such as zygoma and nasal bone, 34 cases with diplopia, 35 cases with visual acuity worsening after injured.METHODS: ①Material implantation: Exposure of the orbital floor, inferior and medial walls could be performed through a 2 mm inferior subciliary incision of 3 cm long. To approach the orbital rim via a dissection plane anterior to the orbital septum, sub-periosteal dissection was then performed over the orbital rim, and along the orbital floor to the orbital apex. Mobilized the soft tissue from the bone throughout the entire area of fractures and re-position it to its proper position. Took Medpor (Type 6331) sheets as the implantation materials, trimmed Medpor sheets according to the radian and anatomic form, and 2 mm larger than the defect rim was needed.If other operations were needed during the operation, they could be done.Mannitol and dexamethasone should be used just postoperatively to decrease edema of the orbital contents and reduce inner orbital excessive pressure. ②Functional evaluation standard: Diplopia: completely disappear meant recovered, less diplopia residual meant improvement, no improvement meant inefficacy. Enophthalmos: marked improvement meant the degree of enophthalmos stabilizated at below 2 mm, less improvement meant stabilizated at above 2 mm.MAIN OUTCOME MEASURES: ①Improvement of enophthalmos; ②Improvement of diplopia ; ③Improvement of visual acuity.RESULTS: ①All 56 cases of enophthalmos deformities caused by orbital blow-out fractures improved greatly. ② Of all the 34 patients with diplopia, 27 recovered. ③ 9 patients' visual acuity of 35 improved with different degrees. No diplopia or visual acuity worsening occurred. With a follow-up ranging from 2 months to 5 years, the degree of enophthalmos stabilizated at below 2 mm, and no relapse and other complications occurred.CONCLUTION: Medpor has such advantages as better histocompatibility,fewer complications and better visual function improvement, so it is the preferred implantation material for correcting enophthalmos deformity caused by orbital blow-out fractures.
10.Megavoltage cone-heron CT in the use of head and neck dose calculation

Objective To evaluate the feasibility and accuracy of performing dose calculation on megavoltage cone-beam CT(MVCBCT) in the head and neck. Methods MiniCTQC phantom was imaged using MVCBCT scanner, and the MVCBCT value density calibration curve was established. Conventional CT and MVCBCT image of phantom and nasopharyngeal carcinoma(NPC) patient were acquired respectively. Two kinds of single field plan were designed for conventional CT image of phantom,and IMRT plan was used for conventional CT image of a NPC patient. The conventional CT plans were copied to MVCBCT image. The dose distribution was calculated for targets and normal tissues using the MVCBCT value density calibration curve,and compared with that of conventional CT. Results For all the cases,the differences between the calculated dose distributions using MVCBCT and CT were less than 3% and 3 mm in single field plan. In IMRT plan, DVHs of conventional CT and MVCBCT were in excellent agreement. The biggest difference between conventional CT and MVCBCT was 95 cGy with the error of 1.4%. On the isocenter plane,the passing rate was 95.5% ,99.4% ,93.8% ,98.7%, 100% ,94.5% ,97.3% ,95.6% ,99.3% and 99.4% for the beam angle of 0°,45°,90°,120°,160°,200°,240°,280° and 320°. Conclusions Performing dose calculation using MVCBCT in head-and-neck region was feasible, and the dose distributions on the conventional CT and MVCBCT were in excellent agreement.
