1.Children’s old Monteggia fracture:Annular ligament reconstruction or not?

BACKGROUND:Radial head re-dislocation is one of the alarming complications of old Monteggia fracture. Many treatment methods have described the necessary of fracture and annual ligament reconstruction during treatment, but stil unclear.
OBJECTIVE:To emphasize the practicality of annular ligament reconstruction through comparing the children’s treatment in two groups.
METHODS:Twelve patients with children’s old Monteggia fracture were included for retrospective analysis. Al the patients were treated with radial head open reduction and internal fixation. Among them, five cases received ulna extending osteotomy and annual ligament reconstruction, and seven cases received ulna extending
osteotomy without annual ligament reconstruction. The course was 3-18 months from damage to the clinical
manifestations (an average of 9 months). Ten cases with Bado type Ⅰ and two cases were Bado type Ⅱ andⅢ. The reliability of ligament reconstruction was evaluated before and after treatment with Kim standard.
RESULTS AND CONCLUSION:Al the 12 cases were fol owed-up for 22 months (8-26 months). Evaluation
according to the Kim standard after treatment showed excel ent in 10 cases without abnormalities and pain, the range of motion>250 °, and no elbow was limited, the activities of daily living was 25 points;good in two cases with mild deformity and mild or intermittent pain, the range of motion was 200°-250° with elbow limited, activities of daily living was 15 points;no poor (serious deformity, pain, range of motion<200°, elbow severely limited, activities of daily living was 0 points). Fol owed-up lasted for 22 months and found that the average healing time of ulnar
osteotomy was 8 weeks (6-14 weeks). The wounds were healed primarily without infection and nonunion. The
results indicate that ulna extending osteotomy has great significance in keeping stability of radial head, and annular ligament reconstruction is determined by the time of injury, surgical procedures and intraoperative radial head stability.
2.Observation on Aloe Juice Reducing Skin Injury of Face and Neck Caused by Radiotherapy in Nasopharyngeal Cancer Patients

Objective To observe the effect of the aloe juice reducing skin injury of face and neck caused by radiotherapy in nasopharyngeal cancer patients. Methods Eighty nasopharyngeal cancer patients admitted in hospital from March 2007 to January 2009 were randomly recruited into a control group and a treatment group. The treatment group (n=48) was treated by smearing on face and neck skin with aloe juice after the conventional fractionation radiotherapy. The control group (n=32) received no therapeutic measures after the conventional fractionation radiotherapy. Results In the treatment group there were 21 patients and 13 patients had I degree and II degree of skin lesion respectively; and in the control group, there were 5 patients and 10 patients had the I degree and II degree of skin lesion respectively. There was a significant difference between the two groups (χ~2= 10.6. P<0.05). Conclusion Aloe juice can reduce the injury of skin caused by radiotherapy.
3.Diagnosis and Interventional Therapy by Spiral CT in Rupture of Hepatocellular Carcinoma

Objective To investigate the diagnosis and the curative effects of interventional therapy by spiral CT in rupture of hepatocellular carcinoma(HCC). Methods 46 patients were checked by spiral CT and abdominal puncture of non-condensable blood. All the patients were treated by the transcatheter arterial embolization (TAE) with iodinated oil and/or gelatin sponge. Results Characteristic findings were detected in 31 patients,of which 7 patients were chiefly detected by spiral CT. The bleeding of ruptured HCCs was controlled effectively in all the cases treated by TAE without serious complications. Conclusion Spiral CT is susceptive to the diagnosis of the ruptured HCC. TAE can be the chiefly treatment in ruptured HCC with fewer serious complications.
4.The study on the relatioship between on ischemia modified albumin and coronary heart disease

Objective To evaluate the value of ischemia modified albumin (IMA) in the diagnosis of coronary heart disease (CHD) and the correlation between IMA levels and the pathological scope of coronary atherosclerosis .Methods A total of 88 coro-nary heart disease(CHD) patients diagnosed by coronary arteriongraphy (CAG) were selected ,including stable angina pectoris (16 cases)(SA group) and acute coronary syndrome (72 cases)(ACS group) .ACS group was further divided into 3 groups :unstable an-gina pectoris(30 cases)(UA group) ,non-ST-elevation myocardial infarction (19 cases) (NSTEMI group) ,ST-elevation myocardial infarction(23 cases) (STEMI group) .According to the results of CAG ,CHD patients were divided into 3 groups:1 vessel lesion group ,2 vessels lesion group and 3 vessels lesion group ,and 60 healthy physical cases were selected as control group .Results IMA in the CHD group was significantly higher than in control group (P<0 .05) .IMA in the ACS group was higher than SA group (P<0 .05) .IMA in the NSTEMI group and STEMI group were significantly higher than UA group (P<0 .05) .IMA levels and the degree of coronary atherosclerosis were significantly positively correlated (P<0 .05 ,r=0 .570) .When the cut-off value of IMA was 80 .10 U/mL ,the area under the curve was 0 .869 ,the specificity was 98 .4% and the sensitivity was 65 .9% .Logistic regression a-nalysis showed that IMA was an independent predictor of CHD .Conclusion IMA has a high negative predictive value in the diag-nosis of CHD ,there was a significantly positive correlation between IMA level and the pathological scope of coronary atherosclero-sis .

6.Comparison of the efficacy of laryngeal mask of Supreme and laryngeal mask of i-gel for the airway man-agement in patients undergoing elective lumbar vertebral surgery in prone position

Objective To compare the efficacy of laryngeal mask of Supreme and i-gel for the airway management in patients undergoing elective lumbar vertebral surgery in prone position. Methods A total of 264 patients,ASA physical status Ⅰ or Ⅱ,scheduled for elective lumbar verte-bral surgery in prone position under general anesthesia were divided into two groups (n =132)using a random number table:laryngeal mask of Supreme group (group S)and laryngeal mask of i-gel group (group I).The laryngeal mask of Supreme or i-gel were respectively placed in group S and group I im-mediately after anesthesia induction according to manufacturers’specification.Volume-controlled ven-tilation was used to observe the ventilation quality.The number of laryngeal mask insertion,laryngeal mask insertion time and the ventilation quality in supine and prone position were recorded,as well as the airway seal pressure.The position of laryngeal mask was evaluated by fibero ptic examination in both supine and prone positions.The blood on the laryngeal mask was noted.Finally,the occurrence of laryngeal mask related complications was observed.Results One patient in group I was excluded due to the failure of laryngeal mask insertion after three attempts.No significant difference in type of laryngeal mask,insertion time,adjusted cases,peak airway pressure,and fiber optic examination in both supine and prone positions between two groups.Compared with group S,group I required signif-icantly more attempts of insertion (P < 0.01 ).Nevertherless,ventilation quality in prone position was improved and airway seal pressure in supine and prone position increased (P <0.01 or P <0.05) in group I.No significant difference was found in the occurrence of laryngeal mask related complica-tions.Conclusion In the airway management of patient undergoing elective lumbar vertebral surgery in prone position,laryngeal mask of i-gel is more difficult to be inserted but can maintain higher air-way seal pressure compared with laryngeal mask of Supreme.
7.EFFECTS OF CROCODILE BONE GELATIN ON STRESS REACTION AND BLOOD-FORMING FUNCTION IN MICE

To investigate the effects of Crocodile Bone Gelatin(CBG) on stress reaction and blood forming function in mice. Results showed CBG increased staying power for high temperature and low temperature, but didn′t increase anti exhaustion ability and hypoxia tolerance of mice. CBG increased platelet count, didn′t increase white blood cell count on Cyclophosphamidum causing blood forming functional obstruction.
8.Determination of Capsaicin in Different Kinds of Dried Capsicums by RP-HPLC

Objective To determine the content of capsaicin in different kinds of dried capsicums.Methods A RP-HPLC method was established.The chromatographic column was DiamonsilTM C18(250 mm ? 4.6 mm,5 ? m) and the column temperature was 30 ℃.The mobile phase consisted of methnol ∶ 0.1 % phosphoric acid aqueous solution(70 ∶ 30) with a flow rate of 1.0 mL? min-1.The detection wavelength was 281 nm.Results A good linearity of capsaicin was in the rang of 0.100 6~ 4.024 0 ? g.The average recovery was 101.18 % and RSD was 1.81 %(n=6).Conclusion The method can be used to determine the content of capsaicin in different kinds of dried capsicums.
9.Value of anti-mutated citrullinated vimentin antibodies in diagnosing rheumatoid arthritis

Objective To assess the diagnostic value of anti-mutated citrullinated vimentin antibodies (anti-MCV) for rheumatoid arthritis (RA), and compare it with anti-cyclic citrullinated peptide antibodies (anti-CCP), rheumatoid factors (RF). Methods Commercially available enzyme-linked immunosorbent assay (ELISA) kit was used to detect anti-MCV antibodies in a group of 177 RA patients, 46 patients with other rheumatic diseases, and 48 healthy blood donors. At the same time, anti-CCP, RF were detected. T test and χ2 test were selected. Results The average concentration of anti-MCV was (523±376) U/ml in RA, (96± 55) U/ml in patients with other rheumatic diseases, (34±18) U/ml in healthy controls. Different threshold levels (20, 40, 60, 80, 100, 120, 140 U/ml) for positive results were calculated bythe areas under the ROC curve (the areas were 0.521, 0.706, 0.769, 0.791, 0.816, 0.826, 0.822), then the best diagnosis efficacy for RA was determined as more than 120 U/ml. At this level, the sensitivity and the specificity for anti-MCV were 80.1% and 80.9% for RA diagnosis. The positive and negative predictive value were 92% and 67.8%. Comparing with anti-CCP, anti-MCV showed comparable specificity but higher sensitivity. And it's also better than RF apparently. If all 3 antibodies were detected at the same time, or anti-MCV combine with one of them, the sensitivity would increase to 95.7%. In addition, Anti-MCV showed positive in 32 of 67(55.2%) patients with RA whose anti-CCP was negative, meanwhile 31 of 59 (52.5%) patients with RA whose RF was negative. Conclusion RF and anti-CCP are complementary in diagnosing RA. The combination detection of RF and anti-CCP could significantly improve the specificity for the diagnosis of RA.
10.Effect of five-flavor Sophora falvescens enteric-coated capsules on intestinal flora in rats with ulcerative colitis

Objective:
To explore the impact of five-flavor Sophora flavescens enteric-coated capsules (FSEC) on the intestinal flora of rats with ulcerative colitis (UC), so as to provide the reference for the mechanism of FSEC in treating UC.
Methods:
Forty SPF-grade Wistar rats were randomly divided into the control group, the model group, the mesalazine group and the FSEC group. Except the control group (0.9% sodium chloride solution), the other 3 groups used 3% dextran sulfate sodium (DSS) for 7 days to establish UC model. After successful modeling, the control group and the model group were given 2 mL/kgbw of 0.9% sodium chloride solution by gavage for 2 weeks, while the mesalazine group and the FSEC group were given 2 mL/kgbw of mesalazine suspension (0.2 g/kg) and FSEC granule suspension (2.16 g/kg), respectively. Pathological changes of colon tissue were observed after hematoxylin-eosin (HE) staining. Rat fecal samples were collected, and 16S rDNA high-throughput sequencing and bioinformatics analysis were performed on intestinal flora. The α and β diversity of intestinal flora among the four groups were compared, and the dominant flora was screened using LEfSe analysis.
Results:
Compared with the control group, the model group showed a significant loss of colonic crypts and a large infiltration of inflammatory cells. Compared with the model group, the mesalazine group and the FSEC group exhibited a slight loss of colonic crypts, a small amount or an absence inflammatory cell infiltration, and improved tissue damage. The α-diversity analysis showed that compared with the control group, the Chao1 and Shannon indices in the model group increased, while the Simpson index decreased; compared with the model group, the Chao1 and Shannon indices in the mesalazine group and the FSEC group decreased, and the Simpson index increased(all P<0.05). The β-diversity analysis showed that the sample distance between the FSEC group and the control group were more closer than that between the model group and the control group. LEfSe analysis results showed that the dominant bacteria in the model group were mainly from the Alistipes and Oscillospira. In the FSEC group, the dominant bacteria were from the Ruminococcus and Prevotella.
Conclusion
FSEC can improve the structures of intestinal flora, increase the abundance of beneficial bacteria such as Ruminococcus and Prevotella, reduce the abundance of pathogenic bacteria such as Alistipes, and alleviate the inflammatory response in UC rats.
