Objective To explore the value of procalcitonin (PCT)and lipopolysaccharide (LPS)in identifying pathogens and evaluating therapeutic efficacy of hospital-acquired pneumonia (HAP).Methods A total of 110 HAP patients were enrolled in a prospective study,patients were divided into gram-negative bacterial infected HAP group (G- infected group,n=50),gram-positive bacterial infected HAP group (G+ infected group,n=30),and control group (nontypical pathogen or virus infected group,n =30).Serum levels of PCT,LPS and C-reactive protein (CRP)of patients were dynamically detected,receiver operating characteristic (ROC)curve and area under the curve (AUC)were adopted to assess the value of PCT and LPS in predicting pathogenic bacteria causing HAP. Results PCT and LPS levels of G - infected group were (3.43 ±1 .15)ng/mL and (0.20 ±0.08)EU/mL respec-tively,which were higher than G+ infected group ([0.42±0.12]ng/mL and [0.05±0.02]EU/mL respectively)and control group([0.14±0.08]ng/mL and [0.02 ±0.01 ]EU/mL respectively)(all P <0.05 ).Levels of PCT and CRP of G- infected group before and after therapy were both significantly different ([3.43±1 .15]ng/mL vs [0.63 ±0.22]ng/mL,[47.26±30.35]mg/L vs [9.21 ±6.54]mg/L,respectively)(both P <0.01).The levels of PCT, LPS,and CRP in moderate and severe patients were all significantly higher than mild patients ([5.43±1 .05]ng/mL vs [0.72±0.32]ng/mL,[0.33±0.07]EU/mL vs [0.09 ±0.04]EU/mL,[57.46 ±20.15 ]mg/L vs [8.25 ± 5.24]mg/L,respectively)(all P <0.05).Sensitivity and specificity of combined detection of PCT and LPS in dif-ferentiating gram-negative bacteria infected VAP from gram-positive bacteria infected VAP were 95.83% and 96.15% respectively,AUC was 0.95.Conclusion PCT and LPS have certain value in identifying pathogens of HAP,combined detection of PCT and LPS can increase specificity in identifying HAP type,and assess the efficacy of antimicrobial therapy in accordance with the dynamic change.

Objective Abnormal activation of mitogen-and stress-activated kinase (MSK1) plays an important role in the development of various cancers.This study was to explore the effect of small interfering RNA (siRNA)-mediated MSK1-silencing on the proliferation of human nasopharyngeal carcinoma (NPC) cells and its underlying mechanism.Methods The siRNA vector targeting MSK1 was constructed and transfected into CNE2 cells, and the NPC cell line stably expressing MSK1 was established.Then the cells were divided into a blank control (without transfection of the plasmid), a negative control (with stable transfection of the negative control plasmid), and an experimental group (with stable transfection of the positive recombinant plasmid).The expressions of MSK1 mRNA and protein were detected by real-time quantitative PCR and Western blot, respectively, the proliferation of the cells determined by CCK-8 and colony formation assays, the cell cycles analyzed by flow cytometry, the level of histone H3 phosphorylation at Ser10 examined by Western blot, and The transcriptional activity and expression of the c-jun protein measured by dual-luciferase reporter gene assay and Western blot.Results Compared with the blank control, the inhibition rates of cell proliferation at 48, 72 and 96 hours were significantly reduced in the experimental group (P<0.05), and so were the colony formation ability of the cells (P<0.01) and the expression and transcriptional activity of the c-jun protein (P<0.05).In comparison with the negative control, the experimental group showed significant decreases in the rate of cell growth after 24 hours, the inhibition rates of cell proliferation at 48, 72 and 96 hours (P<0.05), the number of formed colonies ([221.00±20.08] vs [99.67±15.57] / 300 cells, P<0.01), the proportion of S-phase cells (P<0.01), and the expression of the c-jun protein in the CNE2 cells ([100.00±0.00] vs [48.77±10.71] %, P<0.05), but a remarkable increase in the percentage of G0/G1-phase cells (P<0.01).Furthermore, histone H3 phosphorylation at Ser10 was markedly reduced (P<0.01) but no significant change was observed in the expression of the total c-jun protein in the experimental group.Conclusion Knockdown of MSK1 using siRNA can significantly inhibit the growth and proliferation of CNE2 cells, which may be closely related to the decreased phosphorylation of histone H3 and subsequently down-regulated transcriptional activity of c-jun.

Objective To investigate two single nucleotide polymorphism sites of IRF5 and to de-tect their relationship with SLE in a population from Shandong province. Methods The polymorphisms (rs2004640 G/T,rs10954213 G/A) were detected with PCR-RFLP in 92 eases of SLE and 88 healthy con-trols. The genotype and allele frequencies were calculated and analyzed. Results The genotype frequencies Of GG, GT and TT in rs2004640 site in SLE were 0. 198, 0.521 and 0.281, respectively. The difference was significant between SLE and centrol (X2 = 8.73, P < 0.05). The genotype frequencies of GG, GA and AA in rs10954213 site in SLE were 0. 318, 0. 409 and 0.273, respectively. The differenee was significant between SLE and control (X2 = 6. 36, P < 0. 05). Conclusion The polymorphism of rs2004640, rs10954213 in IRF5 may be associated with SLE in the population of Han nationality from Shandong province of China.

Objective To compare the value of transrectal ultrasonography(TRUS) and magnetic resonance(MR) in preoperative estimation of circumferential resection margin (CRM) of mid-low rectal cancer.Methods Both TRUS and MR were performed prior to surgery in 111 patients with mid-low rectal cancer.The CRM was evaluated and compared with the postoperative pathologic findings.Results The accuracy of TRUS and MR for CRM was 91.9％(102/111) and 85.6％(95/111),respectively,there was no statistical difference between them (P =0.137).Their specificities were 94.0％ (79/84)and 84.5％ (71/84),there was statistical difference between them(P =0.046).The consistency of the results between TRUS and pathology was better than that between MR and pathology(Kappa1 =0.783,Kappas =0.652).Conclusions TRUS is proved to have an important diagnostic value for CRM of mid-low rectal cancer.

Objective To explore the correlation of the anti-stress ability of nuclear power plant operators with their heart rate,blood pressure and mental health.Methods A total of 136 male nuclear power plant operators received the continuous performance test (CPT) and mental health test (MHT),while 37 male senior students were used as the control group.Heart rate and blood pressure were recorded before and after the test.The nuclear power plant operators were divided to high score group and low score group by CPT scores.The correlation of the anti-stress ability and blood pressure,heart rate and their mental health was analyzedd.Results The scores of CPT and MHT test with the operators were significantly higher than those of the control group(t =- 1.25,6.25,P ＜ 0.05).After the test,the heart rate,systolic pressure and diastolic pressure of the operators were all significantly lower than those of the control group ( t =- 2.07,3.28,2.74,P ＜ 0.05 ).The CPT score of the high score group was lower in the heart rate,systolic pressure and diastolic pressure than the low score group(t=-3.15, -2.78, -2.54,P ＜0.05),while the scores of CPT and MHT were better(t =0.63,6.90,P ＜ 0.05).CPT scores were negatively correlated with the increasing range of the heart rate,systolic pressure and diastolic pressure( r =- 0.69,- 0.94,and - 0.62,P ＜ 0.05 ),and positively correlated with the MHT scores ( r =0.54,P ＜ 0.05 ).Conclusions To a certain extent,CPT and MHT test can reflect the anti-stress ability and be used as one of the bases to select nuclear power plant operators and to assess their competence.

Objective To explore the status of nuclear power plant operators in mental health and its correlation with emotional stability,liveliness,anxiety and urgency.Methods 255 male operators were randomly selected from three nuclear power bases,meanwhile 61 undergraduates were used as control group.The mental health and neurobehavioral evaluation system of Chinese nuclear power plant operators was developed by Second Affiliated Hospital of Soochow University,which was used to assess mental health of the subjects.The scores of mental health personality factors were recorded,together with four main personality factors including emotional stability,liveliness,anxiety and urgency.Results The score of lie was lower than 8 which showed all inspected groups were normal. 1.57％ (4/255) operators had psychological disorders,3.92％ ( 10/255 ) had poor mental health,27.84％ ( 71/255 ) had general mental health,66.7％ (170/255) had excellent mental health,whereas 9.84％ (6/61)for control group had psychological disorders.Obvious difference was observed in the final scores between the nuclear power plant operators and control group.The former gained higher scores on mental health,emotional stability,and lower scores on anxiety and urgency ( t =3.437,4.423,- 2.493,- 2.093,P ＜ 0.05 ).Both groups aged over 27 years and with length of service over 5 years were awarded higher scores on mental health,emotional stability( t =2.585,2.349 ; t =2.606,2.947,P ＜ 0.05 ),lower scores on anxiety and urgency (t =- 3.407,- 2.138 ; t =- 2.941,- 2.256,P ＜ 0.05).The mental health was positively correlated with emotional stability and liveliness ( r =0.721,0.650,P ＜ 0.05),but negatively correlated with anxiety and urgency(r =- 0.809,- 0.693,P ＜ 0.05 ).Conclusions The majority of nuclear power plant operators had excellent psychological quality,but some factors should be paid more attention,such as different ages and length of service time.

Objective To explore the personality features of nuclear power plant operators and the influencing factors thereof.Methods Minnesota Multiphasic Personality Inventory was used to examine the personality features of 136 nuclear power plant operators randomly selected from 2 cooperative units,all males.The results were compared with the nationwide norms and subsequently an inter-block contrast analysis was carried out.Results Obvious difierence was observed in the final scores between the nuclear power plant operators and nationwide norms.The former got higher scores on hysteria(t=3.05,P<0.05).and lower scores on hypochondriasis,depression,morbid personality,masculinity-femininity,paranoia,psychasthenia,schizophrenia,hypomania,and social introversion(t=7.47,7.47,7.3 1,2.23,15.09,16.15,19.28,7.88,11.10,P<0.05).The scores on hypochondriasis,depression,schizophrenia,and social introversion of those with the length of services over 3 years were all significantly higher than those ofwith the length of services less than 3 years(t=3.25,2.51,2.76,3.00,P<0.05).The scores on hypochondriasis,depression,psychop8thicdeviate,and social introversion of the operators aged over 30 were all significantly higher than those of the operators aged below 30(t=2.36,2.35,2.01,2.54,P<0.05).Conclusions The psychological quality of the nuclear power plant operators is superior to that of the general population.

Objective To understand the operators'psychosomatic health status in nuclear power plant;and provide the scientific basis of measures for preventing and reducing mental disorders in operators.Methods The Psychosomatic Health Battery(PSHB) was used to assess the psychosomatic health status in 109 operators who were random selected from Qinshan nuclear power plant,etc.They were tested from lie,emotional stability,liveliness,tension,apprehension,mental health,such as psychopathic deviatesuch 7 personality traits.Results Lie < 8,all inspected groups were normal.Psychopathic deviate:98.2% for normal group 0.9% for both of groups occurred possible mental health problems and confirmed mental health problems ;Mental health:80.7% (88/109) for fine mental health ones,29.4% (32/109) for those with excellent mental health,51.4% (56/109) for good mental health ones,13.8% (15/109) for general mental health ones,5.5% (6/109) for poor mental health ones.Age factor could influence the mean values of the factors of apprehension,tension,mental health and psychopathic deviate.Correlation analysis showed that there was a correlation between tension and psychopathic deviate(r = 0.664 ,P < 0.01),and the other correlation coefficient was between apprehension and mental health(r =-0.789 ,P < 0.01).Conclusions There is an excellent condition of psychosomatic health in most of the operators,however,there are still a very small percentage of psychosomatic disorders among these operators,to improve the quality of their psychosomatic health,psychological counseling should be particularly strengthened to those with problems of psychosomatic health.

BACKGROUND:Porous gradient hydroxyapatite/zirconia composite, which can adhere to a variety of cel s, such as osteoblasts, bone marrow mesenchymal stem cel s and chondrocytes, can induce and promote fracture healing or replace bone defects. OBJECTIVE:To review the development and application of porous gradient hydroxyapatite/zirconia composite in bone tissue engineering. METHODS:Articles related to porous gradient hydroxyapatite/zirconia composite in bone tissue engineering were retrieved in CNKI and PubMed databases (1971-01/2014-12). The key words were“porous bioceramic, bone tissue engineering, bone morphogenetic protein-2, induced pluripotent stem cel s”in Chinese and English, respectively. A total of 54 articles based on inclusion criteria and exclusion criteria were obtained for the review. RESULTS AND CONCLUSION:Porous gradient hydroxyapatite/zirconia composite can provide a scaffold to induce natural y forming bone growing to fil the three-dimensional pores, thus realizing the perfect integration of tissue-engineered bone material and host-bone tissue. As zirconia functions as an enhancer of hydroxyapatite, high-quality artificial bone materials, which have elastic modulus, fracture toughness and structure similar to human bone, can be prepared by adjusting the proportion and porosity of materials. Additional y, the new hydroxyapatite/zirconia foamed ceramics as tissue-engineered bone carrying bone morphogenetic protein-2/chitosan gel sustained release system and bone marrow mesenchymal stem cel s derived from induced pluripotent stem cel s wil be expected to increase bone formation and bone fusion rates significantly in the future.
