1.Study of the effect of glycosylphosphatidylinositol-specific phospholipase D on atherosclerosis

Objective To investigate the effect of glycosylphosphatidylinositol specific phospholipase D (GPI-PLD) on artherosclerosis.Methods The GPI-PLD activities and mononuclear cells GPI-PLD gene mRNA expression was detected in 102 patients with coronary artery disease and 121 healthy adult.The GPI-PLD highly expressing cell model was constructed and induced by ox-LDL,the HUVEC and HUVEC transfected with pcDNA3.1(+) as blank and control group,respectively.Before and after the induction,the change of cellular function and biological features was detected.Results The peripheral blood GPI-PLD activity of patients with coronary heart disease and normal control were 31.80±4.21 and 44.32±4.50,and the IA ratio of GPI-PLD mRNA expression of mononuclear cells were 0.92±0.16 and 1.53±0.14,respectively.The activity of GPI-PLD and mRNA expression in patients with coronary artery disease were decreased up to 28.2% (t=21.31,P<0.01) and 39.9% (t=30.36,P<0.01) as compared with healthy control.The adhesion cells,ET-1,reactive oxygen,malondialdehyde (MDA) and the apoptosis rate of GPI-PLD overexpressing HUVEC were lower as compared with blank [29.59=1.40,3.51 ± 0.45,(50.63 ±4.22) ng/L,0.043±0.011,(3.32±0.44) nmol/L vs.41.39±2.15,10.57±1.12,(59.35±4.45)ng/L,0.052±0.011,(5.01±0.69) nmol/L],and as compared with control [42.68±2.45,9.92±1.03,(61.11±4.12) ng/L,0.051±0.007,(4.89±0.71) nmol/l] after inducing by ox-LDL; while the level of nitrogen noxidium (NO) was higher than blank [(29.88± 1.37)μmol/L vs.(21.76±1.02)μmol/L] and control(23.43±0.85)μmol/L groups.Conclusions The expression and activity of GPI-PLD in patients with coronary artery disease are lower than health people.Stable high expression of GPI-PLD is beneficial to vascular endothelial cell injury repairment and prevent the occurrence and development of atherosclerosis.
2.Effects of solution-focused brief therapy on self-perceived burden and quality of life in patients with stroke

Objective To investigate the effects of solution-focused brief therapy on self-perceived burden and quality of life in patients with stroke.methods 60 patients with stroke recruited at the department of rehabilitation of a hospital in Chengde city from January to June 2016 were divided into control group (n=30) and experimental group (n=30).Both groups received conventional nursing care and rehabilitation treatment.In addition,the experimental group received psychological intervention of solution-focused brief therapy,2-3 times a week,30-45 minutes for each time.The self-perceived burden scale(SPBS),memorial university of Newfoundland scale of happiness(MUNSH) and short from 36 health survey questionaire(SF-36) were used to assess the patients before and after intervention.Result s(1)The differences of the scores of SPBS ((39.27±5.83) vs (22.40±4.03),t=16.248,P<0.01) and MUNSH ((4.20±4.50) vs (31.10±3.66),t=-24.136,P<0.01) for the experimental group before and after the intervention were statistically significant.There were statistically significant differences in scores of SPBS ((22.40±4.03) vs (35.30±2.63),t=-14.683,P<0.01) and MUNSH ((31.10±3.66) vs (19.67±3.64),t=12.127,P<0.01) between the experimental group and control group after the intervention.(2)The scores of physiologic functioning,general health,vitality,social functioning,role-emotional and mental health for the experiment group were higher than before(t=-23.427--6.624,all P<0.01).After the intervention,the scores of physiologic functioning,general health,vitality,social functioning,role-emotional and mental health in the experiment group were higher than those in control group(t=5.705-12.806,all P<0.01).Conclusion Solution-focused brief therapy can effectively reduce the self-perceived burden of stroke patients and improve their quality of life.
3.Effect of fast capacitance compensation method on improving the action potential firing accuracy of nerve cell.

Patch-clamp is used to study all sorts of ionic channels and their regulations with measuring pA current of cell ionic channel, but the fast capacitance (C-fast) compensation and slow capacitance (C-slow) compensation transient currents are caused by measuring objects and measuring instruments themselves which will change the properties of action potentials. The present paper firstly discusses the C-Fast transient currents affecting membrane capacitance and membrane potential, and then draws a conclusion that the changes of membrane potential affect the properties of action potential through analyzing the changes of membrane potential in H-H model. Based on this conclusion, we discuss the influence mechanisms mainly through the analysis of traditional C-fast compensation errors, and focus discussion on the shape of electrode capacitance affecting C-fast. This method can not only improve the compensation speed greatly, but also improve the compensation precision from the electrode shape as much as possible.

4.Demand for traditional Chinese medicine healthcare services among the elderly in community:A perspective of Andersen's behavioral model

Objective:
To learn the demand for traditional Chinese medicine(TCM)healthcare services among the elderly people in community based on Andersen's behavioral model.
Methods:
The people aged 60 years or above in six communities of Changqing Street,Xiacheng District,Hangzhou were recruited. Based on Andersen's behavioral model,a questionnaire was developed to investigate their demand for TCM healthcare services. The logistic regression model was used to analyze the influencing factors for the demand,and the advantage analysis method was used to figure out the importance of the influencing factors.
Results:
A total of 360 questionnaires were sent out and 352(97.78%)valid questionnaires were recovered. There were 215(61.08%)people with demand for TCM healthcare services,ranking first in TCM health intervention and assessment(68.84%),followed by TCM health consultation and guidance(42.79%). The results of multivariate logistic regression analysis showed that age of 70 years old and over(OR:1.958-2.767,95%CI:1.087-6.493),monthly income of 2 000 yuan and over(OR:2.757-3.409,95%CI:1.356-6.555),modest family relation(OR=0.152,95%CI:0.076-0.306),severity of disability(OR:2.980-4.332,95%CI:1.545-11.906)and severity of depression(OR:3.792-17.347,95%CI:1.972-68.020)were the influencing factors for TCM healthcare services demand of the elderly. Among them,the importance weights of the demand factors(disability and depression),the ability factors(monthly income and family relation)and propensity factors(age)were 47.59%,34.02% and 18.39%,respectively.
Conclusions
Factors influencing the demand for TCM healthcare services in the elderly include age,monthly income,family relation,disability and depression,with the last two factors of the most importance.
5.The Value of Tumor Treating Fields in Glioblastoma

Glioblastoma (GBM) is one of the most common tumors of the central nervous system, which is the most lethal brain cancer. GBM treatment is based primarily on surgical resection, combined with radiotherapy and chemotherapy. Despite the positive treatment, progression free survival and overall survival were not significantly prolonged because GBM almost always recurs. We are always looking forward to some new and effective treatments. In recent years, a novel treatment method called tumor treating fields (TTFields) for cancer treatment has been proposed. TTFields devices were approved by the Food and Drug Administration (FDA) for adjuvant treatment of recurrent and newly diagnosed GBMs in 2011 and 2015, respectively. This became the first breakthrough treatment for GBM in the past 10 years after the FDA approved bevacizumab for patients with relapsed GBM in 2009. This paper summarized the research results of TTFields in recent years and elaborated the mechanism of action of TTFields on GBM, including cell and animal experimental research, clinical application and social benefits.
6.Changes in force associated with the amount of aligner activation and lingual bodily movement of the maxillary central incisor.

OBJECTIVE: The purposes of this study were to measure the orthodontic forces generated by thermoplastic aligners and investigate the possible influences of different activations for lingual bodily movements on orthodontic forces, and their attenuation. METHODS: Thermoplastic material of 1.0-mm in thickness was used to manufacture aligners for 0.2, 0.3, 0.4, 0.5, and 0.6 mm activations for lingual bodily movements of the maxillary central incisor. The orthodontic force in the lingual direction delivered by the thermoplastic aligners was measured using a micro-stress sensor system for the invisible orthodontic technique, and was monitored for 2 weeks. RESULTS: Orthodontic force increased with the amount of activation of the aligner in the initial measurements. The attenuation speed in the 0.6 mm group was faster than that of the other groups (p < 0.05). All aligners demonstrated rapid relaxation in the first 8 hours, which then decreased slowly and plateaued on day 4 or 5. CONCLUSIONS: The amount of activation had a substantial influence on the orthodontic force imparted by the aligners. The results suggest that the activation of lingual bodily movement of the maxillary central incisor should not exceed 0.5 mm. The initial 4 or 5 days is important with respect to orthodontic treatment incorporating an aligner.

7.Role of invasive intracranial pressure monitoring in treatment of bilateral posttraumatic acute diffuse brain swelling

Objective To investigate the instructive role and clinical effect of invasive intracranial pressure monitoring in treating bilateral posttraumatic acute diffuse brain swelling(PADBS).Methods A total of 52 consecutive patients with bilateral PADBS managed under invasive intracranial pressure monitoring between October 2009 and December 2010 were enrolled as the study group.Another 53 patients with bilateral PADBS managed with non-intracranial pressure monitoring from February 2007 to September 2009 were set as the control group.The clinical outcomes of the two groups were compared.Results The ratios of good recovery[Glasgow Outcome Scale(GOS)=5 points]and severe disability(GOS=3 points)were 59.6%(31/52)and 11.5%(6/52)respectively in the study group,but 35.9%(19/53)and 28.3%(15/53)respectively in the control group(P<0.05).The death rates of the study and control groups were 5.8%(3/52)and 9.4%(5/53)respectively(P>0.05),and the average hospital stay was(34.35±17.50)days and(42.43±22.17)days respectively(P<0.05).Conclusion Durative monitoring of invasive intracranial pressure in treatment of bilateral PADBS can improve prognosis,shorten hospital stay and therefore is worthy of clinical application.
8.Risk factors of traumatic arterial and venous cerebral infarction in patients with moderate or severe craniocerebral trauma

Objective To provide theoretic support for preventing traumatic arterial and venous cerebral infarction after craniocerebral trauma by probing into the related risk factors.Methods The clinical data of 154 pateints with moderate or severe craniocerebral trauma treated by decompressive craniectomy were studied retrospectively.Univariate analysis was carried out on 13 related factors including gender,age,Glasgow Coma Score(GCS)on admission,pupil status,morphological changes of ambient cisterns,brain midline,associated injury,blood pressure,traumatic superficial cerebral veins injury,platelet count,plasma D-dimer value,dosage of dehydrating agent and perioperative fluid balance.Then,the logistic multiple regression analysis was made on significant indexes with SPSS 10.0.Results Univariate analysis showed that seven factors including pupil status,GCS on admission,age,associated injury,perioperative blood pressure,morphological changes of ambient cisterns and brain midline were significantly correlated with traumatic arterial cerebral infarction(P < 0.05)and that three factors including traumatic superficial cerebral veins injury,plasma D-dimer value and associated injury were significantly correlated with traumatic venous cerebral infarction(P < 0.05).Logistic multi-factors regression analysis showed that mydriasis and hypotension might be the independent risk factor of traumatic arterial cerebral infarction and that traumatic superficial cerebral veins injury might be the independent risk factors of traumatic venous cerebral infarction.Conclusion The pupil status,GCS on admission,age,associated injury,perioperative blood pressure,morphological changes of ambient cisterns and brain midline are the risk factors of traumatic arterial cerebral infarction,with mydriasis and hypotension as independent risk factors.Traumatic superficial cerebral veins injury,plasma D-dimer value and associated injury are the risk factors of raumatic venous cerebral infarction,with traumatic superficial cerebral veins injury as independent risk factor.
9.Etiological factor distribution in elderly outpatients with vertigo/dizziness

Objective To clarify the distribution of etiological factors in elderly versus non-elderly outpatients with vertigo/dizziness for optimizing the diagnosis and therapy. Methods We retrospectively analyzed data of outpatients with vertigo/dizziness in Shaanxi Provincial People's Hospital from April 2015 to April 2017 and conducted diagnoses in accordance with the currently wide-accepted diagnostic criteria. Results A total of 3 356 patients with chief complains of vertigo/dizziness were recruited ,and their top seven etiological factors were benign paroxysmal positional vertigo (n= 1 320 ,39.3%) ,chronic subjective dizziness(n=680 ,20.3%) ,vestibular migraine(n=386 ,11.5%) ,posterior circulation ischemia (n=213 ,6.4%) ,Meniere's disease (n = 138 ,4.1%) ,vestibular neuritis (n= 121 ,3.6%) ,and vestibular paroxysmia(n=76 ,2.3%). The top four etiological factors for the elderly patients (n=1 255)were benign paroxysmal positional vertigo (n= 498 ,39.7%) ,chronic subjective dizziness (n= 279 ,22.2%) ,posterior circulation ischemia(n=161 ,12.8%) ,and vestibular migraine(n=73 ,5.8%) ;while the top four etiological factors for non-elderly patients (n= 2 101)were benign paroxysmal positional vertigo (n= 822 ,39.1%) , chronic subjective dizziness(n=401 ,19.1%) ,vestibular migraine(n=313 ,14.9%) ,and vestibular neuritis(n=105 ,5.0%). The detection rate in elderly patients versus non-elderly patients was significantly higher in chronic subjective dizziness (22.2% vs.19.1%,P= 0.032 )and in posterior circulation ischemia (12.8%vs.2.5%,P=0.000) ,and was significantly lower in vestibular neuritis (1.3% vs.5.0%,P= 0.000 ) ,in vestibular migraine(5.8% vs.19.4%,P = 0.000)and in other causes (1.0% vs.2.7%,P = 0.002) . Conclusions The ratio of posterior circulation ischemia is markedly higher in elderly outpatients than in non-elderly outpatients ,whereas the ratios of vestibular migraine and vestibular neuritis in elderly patients are lower than in non-elderly outpatients.
10.Evaluation and application of automated quality control of automatic pipeline in clinical biochemical and immunological detection

Objective:To assess the applicability of fully automatic pipeline automated testing for internal quality control (automated quality control).Methods:Stability, assay efficiency and implementation costs of 18 biochemical tests, 5 immunoturbidimetric tests and 11 chemical illuminescent tests in the Department of Laboratory Medicine of Peking Union Hospital from January 2019 to July 2022 were evaluated using automated quality control implementation methods. The detailed method is as follows: quality control materials for biochemical, immunoturbidimetric and chemiluminescent tests were stored in the refrigerator in the pipeline which was controlled by the intermediate software, and were automatically retrieved and tested as pre-set followed by documenting and storing. The quality control setup for the biochemical tests included refreshing quality control materials daily and weekly,both of which were paralleled for 3 months. The on-line storage stability of quality control materials in the pipeline was evaluated by comparing the coefficients of variation ( CV) of the quality control results between the two patterns. Effect of automated quality control application was evaluated using 6 indicators, including the results′ variation of automatically performed and manually performed quality controls, the out-of-controlled rate, the consumption of quality control materials, the change of staff workload, the impact on the testing time of the first sample, and the failure rate of automated quality control. Results:(1) Storage stability of quality control materials in the pipeline: under the pattern of weekly refresh of the biochemical quality control materials, except for total carbon dioxide (TCO 2) (the CVs of low and high level quality control were respectively 20.24% and 21.82%) and sodium (the CV of low level quality control was 1.51%) that were greater than the allowable variation set by the laboratory, the CVs of the rest tests meet the lab requirements on the allowable variations. (2) The results′ variation of quality control in automatically performed and manually performed control patterns: in the patterns of daily refresh of biochemical quality control materials and weekly refresh of immunoturbidimetric and chemiluminescent quality control materials, the CVs of both low and high levels of quality control were lower in the automatically performed control pattern than that in manually performed pattern for 8 chemiluminescent items of dehydroepiandrosterone sulfate, estradiol, follicle stimulating hormone, luteinizing hormone, serum ferritin, serum folic acid, vitamin B12 and testosterone, 3 immunologic items of complement 3, C reactive protein and immunoglobulin G, and 10 biochemical items of alkaline phosphatase, glucose, calcium, chloride, potassium, lactate dehydrogenase, sodium, urea, low density lipoprotein cholesterol, and adenosine deaminase. The out-of-control rates of biochemistry, immunoturbidimetric and chemiluminescence tests in both quality control patterns conformed with the clinical routine work requirements. (3) Comparison of quality control materials′ consumption: compared with manually performed quality control, weekly consumption of automatically performed chemiluminescent quality control materials decreased 37.5% (from 8 ml to 5 ml); weekly consumption of automatically performed immunoturbidimetric quality control materials decreased 33.3% (from 3 ml to 2 ml). (4)Comparison of staff workload and first sample testing time: compared with manually performed quality control, automatical quality control reduced manual work by about 156 steps per week, and the daily initial testing time was earlier by 15 min on average. The failure rate was 54.5% (37/64) during the early-stage application of the automated quality control which dropped to 10.2% (13/128) in the late-stage. Conclusion:The results of automated quality control detected in the pipeline system meet the quality indicators′ requirements of the laboratory, and the application of automated quality control can improve the quality control, save costs, reduce workload, and improve work efficiency.
