Objective To explore the efficacy of prophylactic bilateral internal iliac arteries balloon occlusion in treatment of women with pernicious placenta previa complicated with placenta accreta.Methods A retrospective analysis was conducted on 41 cases of pernicious placenta previa with placenta accreta admitted to Guangzhou Women and Children's Medical Centre from January 2010 to December 2012.The study group (n=13) underwent prophylactic bilateral internal iliac arteries balloon placement before cesarean section and occlusion after delivery.The control group (n =28)received conventional haemostasis during cesarean section.The amount of blood loss and blood transfusion during the operation,the perioperative hemoglobin level,operation time and duration of hospitalization of the two groups were compared with t test or Chi square test.Results The volume of intraoperative hemorrhage of the study group was lower than of the control group [(1429±875) ml vs (4600± 2090) ml,t=6.840,P=0.000],the amount of intraoperative blood transfusion in the study group was also lower [(920±438) ml vs (3600± 1225) ml,t=10.251,P=0.000].Operation time and postoperative duration of hospitalization of the study group were shorter than those of the control group [(197±45) min vs (284±44) min,t 5.850,P=0.000; (6.7±1.3) d vs (8.2± 2.2) d,t=2.272,P=0.029].There was no statistical difference on hysterectomy rate between the two groups [11/13 vs 89％(25/28),x2=0.181,P 0.670)].In addition,two cases of reoperation,one case of pulmonary edema,two cases of coagulation disorder,one case of venous thrombosis in lower limbs,one case of renal dysfunction and pulmonary edema were reported in the control group,but none in the study group.Conclusions.Prophylactic bilateral internal iliac arteries balloon occlusion is effective in reducing intraoperative blood loss,transfusion,and relative complications in patients with pernicious placenta previa complicated with placenta accreta.

OBJECTIVE: To research population pharmacokinetics of biapenem in critical patients after repeated dosing. METHODS: Blood samples were collected according to the different time points after intravenous administration of 300 mg for many times in the group of critical patients. High-performance liquid chromatography (HPLC) was used to determine the drug concentration in plasma.And pharmacokinetic parameters was caculated. RESULTS: The main pharmacokinetic parameters for critical patients were as follows: ρmax was (6.66±2.93)mg·L-1, Tmax was (0.51±0.04) h, AUC0-∞ was (18.98±16.95) mg·h·L-1, T1/2 was (2.06±1.93) h, Cl was (20.9±17.4) L·h-1, Vd was (46.43±3.5) L. CONCLUSION: The pharmacokinetic parameters of biapenem in critical patients with a significant difference was found in healthy people. So need according to pharmacokinetic characteristics of patients to develop personalized anti-infection plan.

Objective: To study the correlation between the blood routine indicators and CD₄⁺ T, CD₈⁺ T lymphocytes in AIDS patients without antiviral treatment, and to find a simple, cheap and effective monitoring indicator. Methods: A study of 333 AIDS patients without antiviral treatment was performed.The methods of regression analysis and other statistical methods were applied to research the correlation between WBC, W-SCC and CD₄⁺ T, CD₈⁺ T cell count among HIV/AIDS. Results: The rate of routine blood abnormalities among 333 HIV/AIDS patients was very high, the main test indicators within the reference range only accounted for 30.63%.The proportion of late diagnosis infected with HIV among women was higher than that of males.Significant correlations were observed between CD₄⁺ Tcell count and WBC, W-SCC, W-LCC, RBC, HGB, PLT(r=0.408, 0.541, 0.157, 0.337, 0.338, 0.166). The equation between W-SCC (x) and CD₄⁺ cell count (y) was y=120.913x+ 90.339. Conclusion There is a strong correlation between CD₄⁺ T lymphocyte count and lymphocyte count in AIDS patients without antiviral treatment.It can be used as an important indicator of immune status in patients with AIDS.At the same time, we should strengthen the propaganda of AIDS prevention, promote AIDS early detection, early treatment, and improve the quality of life of AIDS patients.

OBJECTIVE: To explore the effect of cadmium on bone formation and its mechanism in male mice. METHODS: i) The specific pathogen-free C57 BL/6 J wild-type male mice were divided into control group and cadmium exposure group, with 10 mice in each group. Mice in the cadmium exposure group were intraperitoneally injected with 2 mg/kg body weight cadmium chloride twice a week for eight weeks, and mice in the control group were intraperitoneally injected with the same amount of 0.9% sodium chloride solution. After that, the bone mineral density and bone microstructure of the femur of mice were detected by a high-resolution microcomputed tomography scanner. ii) Human SV40-transfected osteoblast cells(hFOB1.19) were divided into control group and cadmium exposure group. The cadmium exposure group was treated with 0.5 μmol/L cadmium chloride solution, cells in the control group were given an equal volume of α-low-limit minimal medium. After culture, the differentiation and mineralization ability of hFOB1.19 cells were analyzed by alkaline phosphatase(ALP) and alizarin red staining, respectively. Cell viability was examined by CCK-8 assay. The protein expression of Janus kinase 2(JAK2), total signal of transducers and activator of transcription 3(tSTAT3) and phosphorylated signal transducers and activator of transcription 3(pSTAT3) was detected by Western blotting. The expression of osteoblastic marker genes ALP, Runt-related transcription factor 2(RUNX2) and osteocalcin(OCN) was detected by real-time quantitative polymerase chain reaction.RESULTS: Compared with the control group, the average bone mineral density decreased(P<0.01), bone volume fraction decreased(P<0.05), the bone trabecula thickness became thinner(P<0.01), the number of bone trabecula decreased(P<0.05), trabecular bone spacing increased(P<0.05) in the femur of mice in cadmium exposure group. The viability of hFOB1.19 cells was decreased [(100.0±10.8)% vs(49.1±8.2)%, P<0.01]. The differentiation ability of osteoblasts was reduced and the mineralization was inhibited. The relative expression levels of JAK2 and pSTAT3 in cells decreased(all P<0.05) and the relative expression levels of osteoblast marker genes ALP, RUNX2 and OCN decreased(all P<0.01).CONCLUSION: Cadmium can induce mice bone loss, which may be due to its inhibition of osteoblastic function by reducing the expression of JAK2 and STAT3 proteins.

BACKGROUND: Stem cells intra-articular injection stagey indicated a potential therapeutic effect on improving the pathological progress of osteoarthritis (OA). However, the long-term effect of stem cells intra-articular injection on the cartilage regeneration remains unclear. miR-29a-3p is predicted to be a critical target for inhibiting insulin-like growth factor-1 expression and may aggravate the progression of OA. METHODS: In this study, we investigated the therapeutic efficacy of intra-articular injection of bone marrow mesenchymal stem cells (BMSCs) transfected with miR-29a-3p inhibitor in OA. RESULTS: miR-29a-3p inhibitor transfection did not influence cell viability of BMSCs, while the chondrogenic differentiation potential of BMSCs was significantly improved. Interestingly, intra-articular injection of BMSCs with miR-29a-3p inhibition significantly prevented articular cartilage degeneration by up-regulating the expression of Sox 9, Col-2a1, aggrecan and down-regulating the expression of matrix metalloproteinase, as well as relieved pain in OA. CONCLUSION The double effects on cartilage protection and pain relief indicated a great potential of intra-articular injection of miR-29a-3p inhibitor-transfected BMSCs for the treatment of OA.

【Objective】 To investigate the willingness of whole blood donors to donate apheresis platelets and analyze its influencing factors. 【Methods】 A total of 400 whole blood donors from Kunshan Blood Station and Leshan Blood Station from January to May, 2023 were surveyed by random sampling, and their willingness to donate apheresis platelets were analyzed by univariate analysis and binary logistic regression analysis. 【Results】 A total of 386 valid questionnaires were collected, with a recovery rate of 96.5%. Among the 386 whole blood donors, 177 were willing to donate apheresis platelets, accounting for 45.9%. Univariate analysis showed that gender, age, education level, blood donation times, average blood donation volume, adverse reactions to blood donation, understanding of platelets, and family members' attitude towards blood donation were the main factors affecting the willingness of whole blood donors to donate platelets, and binary logistic regression analysis showed that gender, age, blood donation times, average blood donation volume, understanding of platelets and family members' attitude towards blood donation were the main factors. 【Conclusion】 Targeted recruitment of whole blood donors should be conducted to recruit more apheresis platelet donors, so as to meet clinical demand of apheresis platelets.

BACKGROUNDMost of the studies on traditional Chinese medicine (TCM) 'spleen' deficiency syndrome in the recent 30 years were conducted only on the basis of single functional index, neglecting the study on the pathophysiologic internal relationship between spleen deficiency syndrome and gastric diseases in modern medicine. But it was at the subcellular molecular biological level that we explored the pathophysiologic basis of classification of spleen deficiency in chronic gastritis by detecting the bioactive substances in gastric mucosa nuclei and mitochondria.

METHODSBy means of optical microscope, scanning electron microscope (SEM), transmission electron microscopy (TEM) and histochemical staining, we conducted histopathological, subcellular ultrastructural analysis and nuclei and mitochondrial ultrastructural analysis of gastric mucosa of 188 spleen deficiency patients and of 42 voluntary blood donors. At the same time, bioactive substances were measured by means of X-ray energy dispersive analysis system (EDAX) image analysis system, radioimmunoassay method and chemiluminescence method.

RESULTSThe content of cAMP, superoxide dismutase (SOD), Zn and Cu in gastric mucosa, and the content of Zn and Cu in mitochondria decreased progressively in order of groups: healthy control (HC), spleen Qi deficiency without organic lesion (F-SQD), spleen Yang deficiency without organic lesion (F-SyangD), disease without symptoms group, spleen Qi deficiency with organic lesion (G-SQD), spleen Yang deficiency with organic lesion (G-SyangD), spleen Yin deficiency (SyinD) and spleen deficiency with Qi stagnation (SDQS), chronic spleen deficiency gastritis (CSG) and chronic atrophic gastritis (CAG); decreased in order of HC, intestinal metaplasia (IM)Ia, IMIb, IMIIa and IMIIb, P < 0.05. The content of DNA, Zn and Cu in nuclei progressively increased in order mentioned above, P < 0.05.

CONCLUSIONSThe quantitative changes of gastric mucosal cAMP, SOD, Zn, Cu, of mitochondrial Zn, Cu and of nuclear DNA, Zn and Cu are not only the substance base on which the lesion of gastric mucosa tissue structure occurs, but also the substance base on which spleen deficiency is classified. G-SQD and G-SyangD were more likely to be found in low-grade or middle-grade CSG and CAG, while SyinD and SDQS in middle-grade or high-grade CSG, CAG and IMIIb.

Objective: To investigate current status and associated factors of preschool children’s physical fitness, in order to provide scientific basis for improving preschool children’s physical fitness. Methods: A total of 3 240 preschool children aged 3-6 years old in Kunshan city were selected through cluster sampling method. They were surveyed about physical fitness and influencing factors. Results: The number of excellence of preschool children’s physical fitness was 269, and the rate was 8.30 percent. The excellence rates of preschool children’s physical fitness in girls, high grade, non-residency in Jiangsu Province were higher(10.87%, 10.96%, 14.88%), and the excellence rate of preschool children’s physical fitness in premature group was lower(4.31%)(P<0.05). Further unconditioned logistic regression analysis found that girls, middle and high grade and non-residency in Jiangsu Province were the protective factors for the excellence of preschool children’s physical fitness, OR values were 1.96, 1.94, 2.45 and 1.87, respectively; premature was a risk factor for the excellence of preschool children’s physical fitness, OR value was 0.47. Conclusion Preschool children in Kunshan have poor physical fitness, especially in boys, low grade and premature groups. Education department and health department should work together to improve the preschool children’s physical fitness.

ObjectiveTo evaluate and compare the curative effect and the safety of sclerotherapy of venous malformation in children using absolute ethanol and pingyangmycin.MethodsEighty children with venous malformation were treated by sclerotherapy with absolute ethanol or pingyangmycin under general anesthesia during February of 2009 to May of 2011. Diagnostic criteria included the presence of a bluecolored lesion at birth,with gradual increase in its size associated with a positive postural test.MRI showed characteristic imaging features.Eighty patients were divided into two groups by randomization.Forty patients were treated with absolute ethanol,and the remaining 40 patients with pingyangmycin. The therapeutic effects and side effects were observed and recorded during a follow-up period of 6 to 24 months ( average 15 months).Treatment results weredividedinto four categories: cure, basiccure, effective, and ineffective,and the former three categories were considered effective in treatment.Chi-square test was used to compare the rate of effective treatment and rate of complications in these two groups.ResultsThe effective patients treated with absolute ethanol was 38 (95.0％,38/40),and the effective patients with pingyangmycin was 26(65.0％,26/40),x2 test ( x2 =11.25,P ＜ 0.01 ) difference was statistically significant.Eight patients developed skin necrosis; 4 patients serious local swelling,2 patients muscle fibrosis,1 patient cerebral embolism in the absolute alcohol group.Two patients developed postoperative fever and vomiting and 2 skin necrosis in the pingyangmycin group,the difference was statistically significant (x2 =12.29,P ＜0.01 ). ConclusionsSclerotherapyiseffectiveforthetreatmentofvenousmalformationsin children.Absolute ethanol is more effective,but associated with more complications,such as skin necrosis or as serious as cerebral embolism,than pingyangmycin.

Objective To evaluate the safety and efficacy of percutaneous ethanol embolization (PEE) in treating arteriovenous malformation (AVM) of foot in children.Methods The clinical data of 11 sick children with arteriovenous malformation of foot,who were treated with PEE in authors' department during the period from January 2007 to January 2016,were retrospectively analyzed.The clinical symptoms,the type of tumor nidus,the therapeutic effect and the postoperative complications were analyzed.Results The 11 sick children included 6 boys and 5 girls,with a mean age of 9 years.Foot mass was seen in 8 children,pain in 8 children,claudication in 3 children and ischemic skin ulcer in one child.Cho Ⅲ b type was observed in 10 children and Cho Ⅱ type in one child;Yakes Ⅳ type was found in 10 children and Ⅱa type in one child.A total of 30 PEE procedures were performed,with an average of 2.7 times per case.The children were followed up for 6-48 months,with a mean of 24 months.Evaluation of curative effect showed that complete cure was achieved in 7 children and partial remission in 3 children,the effective rate was 90.9％.Treatment failure (showing no remission) was seen in one child,and no deterioration of disease was observed.Complications included transient blood oxygen decrease in operation (n=1),toe ischemia (n=1),postoperativeblister (n=1) and local skin ulcer (n=l),no severe complications were observed.Conclusion PEE is a safe and effective treatment for arteriovenous malformation of foot in children.
