1.The relationship between plasma glucose transporter expression and ERK in diabetic patients with obesity

Objective To investigate the relationship between plasma glucose transporter (Glut) expression and ERK in diabetic patients with obesity. Methods Subjects in this study included type 2 diabetes (T2DM ) with overweight or obesity(T2DM+ OB/OW group ,n=20) ,T2DM with normal weight (T2DM group ,n=22) ,normal glucose with overweight or obesity(OB/OW group ,n=20) ,normal glucose with normal weight (NC group ,n=21). Automatic biochemical analyzer was used to detect the glucose concentrations ;colorimetry was used to detect blood lipid;ELISA was used to detect the level of insulin;Western blot was used to evaluate the expression of Glut1 Glut2 and Glut4 ;RT‐PCR was used to evaluate the mRNA expression of Glut2 ,Glut4 and ERK. Results Fasting plasma glucose (FPG )and two‐hour postprandial plasma glucose (2 hPG)in group T2DM+ OB/OW[(8.9 ± 3.7) ,(20.1 ± 3.5)mmol/L]and T2DM[(9.3 ± 2.8) ,(18.48 ± 4.59)mmol/L]were higher than in group OB/OW[(4.10 ± 0.90) ,(6.60 ± 0.75)mmol/L ]and NC[(3.9 ± 0.7) ,(6.7 ± 0.8)mmol/L](P<0.01).Hemoglobin A1c(HbA1c) in group T2DM+ OB/OW [9.90 ± 3.19)% ] and T2DM [(8.68 ± 2.75)% ] were higher than in group OB/OW [5.05 ± 0.37)% ] and NC[(5.47 ± 0.59)% ](P<0.01).FIns in group OB/OW [(7.98 ± 2.01)μIU/ml] were higher than in T2DM group[(5.91 ± 3.02)μIU/ml]and NC[(5.49 ± 1.36)μIU/ml](P< 0.05).Total cholesterol and triglyceride in T2DM + OB/OW group[(5.06 ± 0.97) ,(3.52 ± 2.68)mmol/L] , T2DM[(5.13 ± 1.27) ,(3.79 ± 3.56)mmol/L]and OB/OW[(4.67 ± 0.83) ,(1.87 ± 0.95)mmol/L]were higher than in group NC[(3.51 ± 0.89) ,(1.41 ± 1.21)mmol/L](P<0.01). Glut1 Glut2 ,Glut4 expression in T2DM+OB/OW group ,T2DM were lower than those in group OB/OW ,NC.Glut2 expression in T2DM+ OB/OW group was significantly lower than in T2DM group.Glut2 and Glut4 mRNA expression in group T2DM+ OB/OW ,T2DM were significantly lower than in group OB/OW ,NC.ERK mRNA expression in group T2DM + OB/OW ,T2DM were significantly higher than those in group OB/OW and NC.Conclusion Glut1 ,Glut2 and Glut4 expression are lower in diabetic patients with obesity ,and there is a negative correlation between the expression of ERK and Glut2 and Glut4.
2.Management and Recognition of Anatomical Variations of the Calot's Triangle during Laparoscopic Cholecystectomy

Objective To summarize the anatomical variations of the Calot's triangle and explore the best method to manage the variations during laparoscopic cholecystectomy(LC).Methods From December 2006 to December 2008,158 patients with anatomical variation of the Calot's triangle received LC,the clinical data of the cases were reviewed retrospectively.Results Among the cases,15 patients were converted to open surgery because of Ⅰ type Mirizzi syndrome(3 cases),Ⅱ type Mirizzi syndrome(4 cases),low location of the convergence of the cystic duct and the common bile duct(2 cases),cystic duct opening into the posterior wall of the common bile duct(2 cases),the cystic duct and common bile duct sharing 2-cm lateral wall(1 case),severe adhesion of the Calot's triangle(2 cases),and hemorrhage of the posterior cystic artery(1 case).The LC were completed in 143 patients,among which 5 cases had postoperative complications,including biliary leakage in 1 case(cured by a second operation),bleeding at the puncture sites in 2 patients,infection of the puncture site in 1 case,and residual cystic stones in 1 case(cured by ERCP in 2 weeks).Conclusions Knowledge of the anatomical variations of the Calot's triangle is the key to LC.Different surgical strategies should be carried out according to the dissection of the Calot's Triangle area,and the location of the common hepatic duct and common bile duct.
3.Effect of xinmailong on metabolism of oxygen free radicals and content of lipofuscin in brain and hepatic tissues

BACKGROUND: Chinese medicine xinmailong (XML) injection is a kind of new nucleoside compound which comes from American giant cockroach of insects. A lot of studies show that XML has obvious effect on cardiovascular activity which can treat acute or chronic congestive heart failure and shock and improve microcirculation.OBJECTIVE: To observe the effect of XML on anti-aging of mice.DESIGN: Controlled observational study.SETTING: Department of Physiology and Pathology, Basic Medical College of Dali College.MATERIALS: The experiment was carried out at the Functional Laboratory of Basic Medical College between July 2002 and September 2004. A total of 40 ICR mice of both sexes were provided by Kunming Medical College,Yunnan Naturally Occurring Drugs Key Laboratory. XML injection was provided by Yunnan Tengchong Pharmacy Factory [batch number: 94 (ZL) 03].METHODS: A total of 40 ICR mice were randomly divided into 4 groups:young control group, aging control group, XML group at high dosage and XML group at low dosage with 10 in each group. All mice were fed routinely with standard feed. Mice in young control group aged 3 months and weighed (20±2) g; meanwhile, mice in other groups aged 20 months and weighed (40±3) g. From the 20th month, mice in XML groups were intraperitoneally injected with 6 mg/kg and 8 mg/kg, respectively, for 30 days with once every day. Mice in young and aging control groups were intraperitoneally injected with the same volume of saline. On the 31st day,heads of mice were cut off to obtain blood which was maintained at hepalean anti-coagulated tube. Brains and livers were obtained rapidly for homogenate with 10% saline. Activities of catalase (CAT), superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) in serum, content of malondialdehyde (MDA) in brain tissue and content of lipofuscin in cerebral and hepatic tissue were measured according to introduction of kits.MAIN OUTCOME MEASURES: ① Activities of CAT, SOD and GSHPx in serum; ② content of MDA in brain tissue and contents of lipofuscin in cerebral and hepatic tissues.RFSULTS: A total of 40 animals were involved in the final analysis without any loss. ① Activities of CAT, SOD and GSH-Px in serum of mice were lower in aging control group than those in young control group; however,activities of CAT, SOD and GSH-Px in serum of mice were increased in the two XML dosage groups, and there was significant difference from those in aging control group (P < 0.05 or P < 0.01). ② Content of MDA in brain tissue and contents of lipofuscin in cerebral and hepatic tissues of mice in aging control group were higher than those of mice in young control group, but those in the two dosage groups were decreased, which was significant difference from those of mice in aging control group (P < 0.01). However, there was no significant difference between the two dosage groups.CONCLUSION: XML has an effect on anti-aging through improving metabolism of free radicals.
4.An analysis of death-related risk factors in patients with severe acute pancreatitis

ObjectiveTo identify the risk factors related to the mortality of severe acute pancreatitis.Methods Clinical data of 57 cases of severe acute pancreatitis in our hospital from August 1997 to February 2004 were collected and analyzed retrospectively. There were 32 males and 25 females. The median age was 51 years. Cases were divided into death group and survival group according to their outcome. The characteristics, severity and surgical therapy were analyzed. ResultsEleven patients died, the mortality was 19.3%。 Thirty cases underwent surgery, 8 died, the mortality was 26.3%.Factors which were of significant differences between the two groups by univariate analysis were age, Ranson score, APACHEⅡscore, ascites, MODS, ARDS, heart failure and renal failure. Multivariate logistic regression showed that independent risk factors included age, ascites, MODS. ConclusionsIn case of severe acute pancreatitis,the independent risk factors related to mortality were age, ascites and MODS.
5.Primary study of radiofrequency ablation for early breast cancer with negative sentinel lymph node

Objective:To summarize the preliminary experience of percutaneous radiofrequency ablation(RFA) in the treatment of early breast cancer(the length of the lesion<15 mm) with negative sentinel lymph node(SLN), and to evaluate its feasibility and efficacy.Methods:From October 2016 to November 2019, 18 patients with early breast cancer(the length of the lesion<15 mm) treated in the First Affiliated Hospital of Zhengzhou University underwent FNAC of axillary SLN under the guidance of CEUS were enrolled. Patients with pathologically negative axillary SLN were treated with percutaneous RFA of breast cancer. The complete ablation rate and the relapse rate were evaluated. Intraoperative pain, postoperative complications and postoperative patient satisfaction were analyzed.Results:Eighteen patients underwent CEUS-guided FNAC of SLN, of which 1 case was positive and 17 cases were negative. Seventeen breast cancer lesions of the 17 negative patients underwent percutaneous RFA. The length of the lesions was 3.3-14.0 (8.2±3.9)mm, the complete ablation rate was 100%, the ablation time was 9-18 (12.8±2.7)min. During the follow-up of 9-46 months, no patient developed tumor progression. There were no serious complications related to RFA after operation. The patients′ satisfactory rate for postoperative appearance was 100%.Conclusions:RFA of early breast cancer(the length of the lesion<15 mm) with negative SLN is feasible and has the advantages of safety, minimal invasion and beauty.
6.Hepatectomy for hepatic hemangiomas close adjoining the portal hepatis and vital blood vessels

Objective To explore the surgical modality and safety of hepatectomy for hepatic hemangiomas close adjoining the hepatic portal and vital blood vessels. Methods From June 2005 to June 2010 17patients of hepatic hemangiomas underwent hepatectomy. Data were retrospectively analyzed.Results All the 17 cases were operated on successfully. Six cases were treated with anatomic liver lobectomy including right hemibepatectomy through liver hanging maneuver by anterior approach in 2 cases,under right liver blood vessel blochade and anatomic right posterior hepatectomy in 2 cases, left hemihepatectomy in 2 cases. Eight cases were treated by hemangiomas enucleation, in 3 cases hemangioma was enuleated through liver parenchyma splitting under intermittent hepatic blood inflow exclusion. There was no postoperative mortality, postoperatively pleural effussion occured in 5 cases,wound infection occured in 1 case, and pulmonary infection occured in 1 case, all the cases were cured. Conclusions Different operation styles should be applied according to the position, size of hepatic hemangiomas close adjoining the hepatic portal and the important blood vessels.
7.Building a comprehensive health assessment indicators systems for health management of military officers

In line with the new WHO definition for health and reference of comprehensive health assessments of elderly overseas, the paper screened and set the weight for comprehensive health assessment indicators for military officers, establishing a general health assessment model as a result 402 military officers were subject to Comprehensive Health Assessments, and the outcomes were compared with subjective assessment of experts. The results of both methods are highly coincidental. It proves Comprehensive Health Assessment as a scientific quantitative health assessment method, with promising perspectives in health management in the future.

With the progress of science and technology and the increase of clinical demand, medical robots have developed rapidly and played a important role in promoting the medical cause. Service robot is a branch of medical robot, which is mainly oriented to medical service and assistance needs, and has been applied in many medical scenarios and achieved demonstration effects. This research first describes the development of medical service robots, and then summarizes the key technologies and clinical applications of robots. Finally, it points out the challenges and directions that medical service robots face at present, and puts forward prospects for their further development in the medical field.
