1.The Effect of Acupuncture for Women with Ovulation Disorder by Glucose Metabolism and Menstrual Cycle

Purpose : Recently, it has been reported that insulin resistance was revealed among non-PCOS women with ovulation disorder. There have been many relevant reports so far that glucose metabolism and lipid metabolism were improved by acupuncture therapy. In this research, therefore, it was examined if acupuncture therapy is effective for glucose metabolism or menstrual cycle disorder of women with ovulation disorder.
Method : We had thirty three adult female measured their basal body temperature (BBT) for three months. Six of them whose BBT showed a normal menstrual cycle were put into a control group, and seven who didn’t show the high temperature phase were put into an abnormal group. Each group was given 75g of glucose tolerance test (OGTT) and their blood glucose level and insulin level were compared. Also, the abnormal group had acupuncture therapies for about a year, and their menstrual cycle, blood glucose level and serum insulin level were compared between before and after the series of therapies.
Results : The comparison of the blood glucose level at 30 minutes, 60 minutes, 120 minutes and serum insulin level at 120 minutes between the control group and the abnormal group showed that they were significantly higher in the abnormal group. (The blood glucose level at 30 minutes : 113±16.0 mg/dl vs. 142.4±22.0 mg/dl, p<0.01, the blood glucose level at 60 minutes : 91.6±21.2 mg/dl vs. 128.2±47.9 mg/dl, p<0.01, the blood glucose level at 120 minutes : 84.1±9.1 mg/dl vs. 123.2±32.6mg/dl, p<0.01, serum insulin level at 120 minutes : 35.3±14.5 μU/ml vs. 55.0±17.5μU/ml, p<0.01) The comparison between before and after the therapies of the abnormal group showed that they were significantly lower at the blood glucose level at 30 minutes, 120 minutes and serum insulin level at 30 minutes, 120 minutes. (The blood glucose level at 30 minutes : 142.4±22.0 mg/dl vs. 119.8±26.6mg/dl, p<0.01, the blood glucose level at 120 minutes : 123.2±32.6 mg/dl vs. 94.2±18.7 mg/dl, p<0.01, serum insulin level at 30 minutes : 74.2±50.0μU/ml vs. 54.5±29.1μU/ml, p<0.01, serum insulin level at 120 minutes : 55.0±17.5μU/ml vs. 37.3±13.3μU/ml, p<0.05) The number of days of menstmal cycle became significantly shorter and the high temperature phase became significantly longer after the series of therapies. (Cycle : 110.8±124.7 days vs. 37.7±13.0 days, p<0.03, number of days of high temperature : 0 days vs. 7.5±4.5 days, p<0.03)
Conclusion : It was suggested that acupuncture has a possibility to work effectively for improving glucose metabolism disorder and menstrual cycle for women with ovulation disorder.
2.Gender differences in kinematics during landing motion and effect of the prevention program for lower extremity injuries on kinematics in junior basketball players

Anterior Cruciate Ligament Injury often occurs as a result of knee valgus collapse during landing or pivoting in sports activity. Previous studies reported that the risk of ACL injuries was reduced by jump and balance training, and those training can be effective as the prevention program for the ACL injuries. But those studies often focused on only adult athletes, and there are few studies focused on junior athletes. The purpose of this study is to investigate the pattern of landing movement in junior athletes, and to verify the effect of the prevention program. One-hundred and ten junior basketball players (boys; 61, girls;49, age ranging 12 to 15) were subjected in this study. We measured the knee flexion, valgus angles and jump height during continuous vertical jump. After measuring, they executed a prevention program for 12 weeks. The motion pattern of the knee during jump test were compaired between before and after prevention program. Female athletes showed greater knee valgus angle at initial landing phase and grater maximum knee valgus angle than those of males. As a result of the prevention program, maximum knee valgus angle was significantly decreased in female athletes. The greater angle of knee valgus in female may increase the risk of ACL injuries. Present study suggests that the prevention program is useful for reducing the risk of ACL injury. Increment of jump height in male players after this program might be considered that this training program has also an effect of performance improvement.
3.Successful Treatment of Necrotizing Fasciitis and Toxic Shock Syndrome by Hip Amputation and Endotoxin Hemoadsorption

Background: Necrotizing fasciitis is a rare but severe condition associated with high mortality. We encountered a patient with severe and rapidly progressing necrotizing fasciitis. Patient: A 40-year-old male was hit by a tractor and received a wide laceration wound spanning the length of his posterior thigh. Soon after the accident, the wound was washed and debridement was performed. Two days postoperatively, we observed septic changes in the wound and diagnosed this condition as necrotizing fasciitis. Consequently, the patient's leg was amputated at the thigh. The patient, however, developed toxic shock syndrome after the amputation. Endotoxin adsorption using a polymyxin B-immobilized fiber column was performed for 2 days. Finally, a hip joint amputation was performed after 11 days, following which the patient's general condition gradually improved. Discussion: Treatment for necrotizing fasciitis should be initiated promptly. Early debridement is associated with a significant decrease in mortality. In severe conditions, endotoxin and cytokine removal by blood purification is one of the most effective treatments. Although group A streptococci are widely known as "flesh-eating bacteria," we should also consider a wide variety of pathogenic organisms to be the probable cause of severe necrotizing fasciitis. Conclusion: Management of necrotizing fasciitis requires careful investigation as well as an aggressive therapeutic approach, which may include urgent surgical intervention. In addition to surgery, endotoxin adsorption therapy should be considered.

4.Characteristics of pre-existing physical factors associated with the onset of Osgood–Schlatter disease in junior soccer players

The aim of this study is to investigate the characteristics of pre-existing physical factors associated with the onset of Osgood-Schlatter disease (OSD) in junior soccer players. Fifty-nine junior soccer players of the elementary school were included in this study. The boys who had been diagnosed as OSD were excluded. At the time of the first investigation, the subjects were examined through physical evaluation and an ultrasonography. The subjects were followed up one and a half years, and the tibial tuberosity was classified according to the growth stages using the ultrasonography. After the end of follow-up period, the subjects were divided into two groups, those with signs and symptoms of OSD that appeared during the follow up period and those without that. The Mann-Whitney U test was used for the comparison between OSD group’s and control group’s initial assessments. The players who were followed up for one and half years were 38 people. 5 knees of the 4 players were clinically diagnosed as OSD (OSD group), and the remaining 34 players with no symptoms of the OSD constituted the control group. The range of the bilateral hip external rotation, straight leg raise of the non-dominant side, and knee flexion in the OSD group were significantly lower than those in the control group (p < 0.05). We concluded that a decrease in specific joint flexibility might be related with the onset of OSD.
