Chinese Journal of Rehabilitation Theory and Practice 2009;15(2):173-175
Objective To evaluate the efficacy and safety of sufentanil used for patient-controlled subcutaneous analgesia (PCSA) with different models after craniotomy. Methods Sixty ASA Ⅰ~Ⅱ patients undergoing craniotomy were randomly divided into two groups with or without background infusion. Sufentanil 0.12 mg was used via patient-controlled analgesia (PCA) and no loading dose was administered. The PCA device was programmed background infusion of 1 ml/h, bolus dose of 0.5 ml in group of using continuance plus PCA model (group CP) and bolus dose of 0.75 ml without background infusion in group P. Hour limit of 3 ml/h, lockout time of 15 minutes were set in two groups. Postoperative pain was assessed at rest and when moving, using Visual Analogue Scale (VAS) at 2 h, 20 h, 24 h, 44 h and 48 h after operation. Side-effects were also recorded systematically during the first 48 h after surgery. Results No significant differences were observed in patients' physical status, fentanyl consumption during operation and anaesthetic time. There were no significant differences in VAS scores between the two groups within 48 h after operation, but the incidence of complaining postoperative pain was significantly more in group P than that in group CP at 20 h and 24 h after surgery (P<0.05). The cumulative analgesic consumption of sufentanil in group CP and group P were 98.73±5.96 μg and 57.25±9.73 μg, respectively (P<0.05). Compared with group P, the mean arterial pressure in 20 h and 24 h were significantly lower in group CP (P<0.05). The sedation degree and the incidence of nausea, vomiting in two groups had no significant differences. No restrain of respiration occurred in two groups. The patients' satisfactory rate with PCA in group CP and group P were 90% and 80%, respectively. Conclusion PCSA with sufentanil and continuance plus PCA model is an effective and safe analgesic method after craniotomy.

Objective To compare the effect of thin-sliced enhanced CT scanning and 3D-DSA data sources in the 3 D printing data reconstruction of intracranial arteriovenous malformation (AVM ). Methods Five patients with AVM were selected prospectively,3 were Spetzler-Martin grade II and 2 were grade III. Two of them used 256-slice spiral CT thin slice enhanced scanning. Three used the 3D-DSA rotating imaging, and the DICOM raw data of the examination results were extracted. Digital processing was performed by using the Mimics software, and the 3 D printing was performed according to the ratio of 1∶1 obtaining the solid model and the effects were compared. Results Using the data source 3 D printing of 256 slice spiral CT thin-slice enhanced scan could obtained skull and blood vessel image information and could reveal the smallest diameter of 0. 9 mm vessel, however, the fine branch structures of the vessel were difficult to distinguish. The 3D printing based on 3D-DSA data, although the digital subtraction did not have the skull data information, the vascular branches showed more abundant. It could reveal the smallest diameter of 0. 9 mm vessel. Conclusions Using the CT thin-slice enhanced scan or 3D-DSA data source can obtain reconstruction images of AVM nidus, and 3D-DSA shows that the better effect for spatial structure of AVM nidus. It is helpful to the design of preoperative treatment scheme and the development of corresponding auxiliary tools.

In order to develop safe training intensity and training methods for the passive balance rehabilitation train- ing system, we propose in this paper a mathematical model for human standing balance adjustment based on T-S fuzzy identification method. This model takes the acceleration of a multidimensional motion platform as its inputs, and human joint angles as its outputs. We used the artificial bee colony optimization algorithm to improve fuzzy C--means clustering algorithm, which enhanced the efficiency of the identification for antecedent parameters. Through some experiments, the data of 9 testees were collected, which were used for model training and model results validation. With the mean square error and cross-correlation between the simulation data and measured data, we concluded that the model was accurate and reasonable.

AIM: To investigate the effects of apelin-13 on proliferation, migration and capillary-like tube formation of a monkey choroid / retinal endothelial cell line, RF/6A, to clarify whether apelin-13 could promote retinal angiogenesis in vitro.METHODS: RF/6A cells in good conditions were administrated with DMSO (the control group), apelin-13 at 0.1μmol/L (low dose group) or apelin-13 at 1μmol/L (high dose group).Cell proliferation, migration and capillary-like tube formation were detected by using the MTT assay, scratch assay and matrigel assay, respectively, at 24h after plating the cells.RESULTS: Cell proliferation was promoted in both low and high dose apelin-13 groups compared to the control cells (P<0.05);the cell migration distance of both apelin-13 groups was significantly greater than that of the control group (P<0.05);and the number of capillary-like tube structures of both apelin-13 groups was significantly larger than that of the control cells (P<0.05).In addition, cell proliferation, migration and tube formation increased as the concentration of apelin-13 increased.CONCLUSION: Apelin-13 could obviously promote the angiogenesis capacity of RF/6A cells, suggesting that apelin-13 was an important pro-angiogenic factor in retinal endothelial cells.

Objective It was reported that pretreatment with small dose of lipopolysaccharide (LPS) could protect the animal from lethal dose endotoxin. The purpose of this study was to investigate the effects of pretreatment with small dose of LPS on cardiac function in endotoxemic rats and the possible mechanism. Methods Sixty male Wistar rats weighing 200-280 g were randomly divided into 3 groups: group I normal saline(NS) ( n = 12); group Ⅱ LPS (n = 24) and group Ⅲ LPS-pretreatment ( n = 24). Group Ⅱ and group Ⅲ were further divided into 3 groups: 2 h,4 h and 6 h subgroups based the time when blood sample was taken and the animal was sacrificed. The table shows the LPS given in the 3 groups:groupⅠⅡ Ⅲ0hNSNSLPS 0.25 mg?kg-1ip24 hNSNSLPS0.5mg?kg-1ip96 hNSLPS 10 mg?kg-1 Ⅳ LPS 10 mg?kg-1 TVThe animals were anesthetized with 3 % pentobarbital 30 mg?kg ip and intubated. Right femoral artery and vein were cannulated for MAP monitoring and fluid infusion. Cardiac catheter was placed in the left ventricle for measurement of left ventricular systolic pressure (LVSP) and?dp/dt max. Blood samples were taken 2 h,4 h and 6 h after intravenous LPS (10 mg?kg-1) for determination of plasma levels of L-lactate-dehydrogenase (LDH) and creatine kinase (CK) . The animals were sacrificed right after blood sampling and the heart was removed for determination of myocardial HSP 70 expression using immuno-histochemical staining. Results LVSP and?dp/dt max gradually decreased 1h after intravenous administration of LPS in group Ⅱ (LPS group); while in group DI (pretreatment group) the cardiac contractility was maintained and LVSP,?dp/dt max did not decrease as compared with the baseline value. Plasma LDH concentration and CK activity increased significantly 4 h and 6 h after intravenous IPS in group Ⅱ . The plasma LDH and CK levels were significantly lower in groupⅢthan those in group Ⅱ ( P

The specification of decoction pieces and quality uniformity are the important factors to influence the efficacy of clinical medicine. Considering the deficiency of diversity, poor quality uniformity and confusion of decoction pieces specifications, we first propose a new idea of precision decoction pieces (PDP) based on clinical demands and fresh-processed technology. In order to explain the idea, a study case of aconite SUP is provided, including the optimized specification design, processing technology, extraction effects, quality uniformity, and toxic and efficacy variation and so on. The results showed that preparing 5 mm PDP by fresh-cutting is rather simple and practicable, with high efficiency and large yield; then, this technology could significantly decrease the ingredients loss and increase the efficacy components; moreover, it was helpful for achieving the quality uniformity and best extraction effects. This work revealed the quality superiority of PDP, and provided a good strategy and example for the standard of decoction pieces specification and modernization of processing technology.
