OBJECTIVE:To explore the mode,methods and role of clinical pharmacists providing pharmaceutical care for pa-tients with natural killer/T-cell lymphoma (NK/TL) complicated with diabetes. METHODS:Clinical pharmacists participated in SMILE chemotherapy plan for a NK/TL patients with diabetes mellitus,evaluated potential chemotherapy risk during first chemo-therapy,implemented individual medication monitoring and put forward suggestion. It included insulin injection 4 U,ivgtt,to con-trol the increase of blood glucose induced by dexamethasone;sodium bicarbonate 40 ml,ivgtt,bid to alkalize urine;allopurinol 0.1 g,po,tid to inhibit uric acid;fluid infusion 4 000 ml/d to prevent renal failure;mesna 0.4 g/time to protect urinary passage;monitorring potential allergic reaction and arrest of bone marrow;providing medication education on the application of calcium foli-nate in SMILE chemotherapy regimen. RESULTS:Physicians adopted the suggestions of clinical pharmacist,and chemotherapy was carried out smoothly. No ADR was found,and patients manifested good medication compliance. CONCLUSIONS:Clinical pharmacists participate in chemotherapy,evaluate potential risk and provide individual medication monitoring,which optimize ther-apy plan,reduce medication risk and guarantee the safety of drug use.

Objective To study the X-ray findings and evaluate the value of X-ray in diagnosis of stress fracture of the pubic ramus. Methods The X-ray findings of 66 cases (18 males and 48 females, ranging in age from 18-21 years, mean age 19.6 years) with stress fracture of the pubic ramus were retrospectively analyzed. Fifty-five cases were military recruits undergoing basic training, and 11 army veterans were undergoing intensive goose step training. Results Sixty-two cases had stress fractures of the inferior pubic ramus including right-sided in 26, left-sided in 30, and bilateral in 6. Four had ipsilateral inferior and superior pubic rami fractures. With 2 weeks after onset, the radiographs were normal in 3 cases. The 66 cases had either a slight transverse fissure (34 cases) or a small dense callus (65 cases) or both during the examination period of 4 weeks to 8 months. Then the calcium resorbed at the margins of the fracture and at the same time the callus increased and surrounded the bone. Finally, the resorptive zone disappeared and the callus became homogeneous. This callus formation persisted for several months and disappeared gradually. Conclusion X-ray examination is the first imaging method of choice for detecting stress fracture of the pubic ramus. With combining clinical data, correct diagnosis can be made in the majority.

To investigate the clinical diagnosis and treatment of stress fracture of the pubic ramus caused by military training, clinical data of 66 cases (18 males and 48 females, ranging from 18～21 years, mean age 19 6 years ) with stress fracture of the pubic ramus were retrospectively analysed. 55 cases were military recruits undergoing basic training, 11 army veterans undergoing intensive goose step training. 66 cases complained of chronic groin pain which increased in activity and relieved while at rest. The symptom occurred 3～10 weeks after the training began. The pain often lasted 1 week ～14 months. 22 cases who sought medical advice in medical team were mistaken for muscular injury. There were 76 stress fractures of the pubic ramus in 66 soldiers. 56 had stress fractures of one inferior pubic ramus, 6 both inferior pubic rami, and four had ipsilateral inferior and superior pubic rami fractures. Stress fracture of the pubic ramus is not rare, but it's quite easy to be neglected or misdiagnosed. The key to its diagnosis is getting a deeper understanding and making an adequate and early imaging examination in light of the clinical data.

Generalized granuloma annulare is a rare skin disease presenting generalized eruption with a distinctive histologic picture. The age of onset of generalized granuloma annulare differs from that of localized granuloma annulare. Most of the patients with generalized granuloma annulare were in the fifth to seventh decades and cases of generalized granuloma annulare in infancy or in early childhood have been rarely reported. We herein report two cases of generalized granuloma annulare in 45- and 18-month-old boys, who is the youngest patient yet reported in the Korean literature. The histopathologic findings were compatible with granuloma annulare and all lesions completely involuted in two months after administration of topical or systemic corticosteroids.

Objective To investigate the expression of autophagy and the effect of complement C5a/C5aR pathway on autophagy induced by renal ischemia reperfusion injury (IRI).Methods MaleWT and C5aR gene knockout (BALB/C background) mice were selected.The model of renal IRI was established by occluding bilateral renal pedicles with microaneurysm clamps.Mice were divided into wild type BALB/C (WT) group and C5aR gene knock out (C5aRKO) group.The pathology of kidney was assessed by HE staining.The levels of BUN and KIM-1 were detected 24 h after reperfusion.The expression of the autophagy-associated protein (LC3 Ⅱ/LC3 Ⅰ and P62) was measured by Western blotting and immunofluorescence.In vitro,human renal tubular epithelial cells (HK2) were cultured.The expression of LC3 in HK2 cells was investigated by immunofluorescence and Western blotting after being treated with recombinant C5a or C5a combined with C5aR antagonist (C5aRA).Results As compared with WT group,the severity of kidney injury was obviously reduced in C5aRKO group (P＜0.05).After ischemia-reperfusion,the expression of autophagy-related protein LC3 gradually increased with the reperfusion time prolonged.The level of autophagy induced by ischemia-reperfusion was significantly reduced in C5aRKO group as compared with WT group (P＜0.05).In addition,the expression of autophagy-related protein LC3 Ⅱ in HK2 cells was increased with the augment of C5a stimulation concentration in vitro.Blockage of C5aR pathway by C5aRA led to a significant decrease in autophagy (P ＜ 0.05).Conclusion Complement C5a/C5aR pathway promotes renal IRI-induced autophagy.

Objective Objective Structured Clinical Examination (OSCE) is one of the most important methods for evalua-ting the medical students′clinical ability .The aim of this study was to analyze the value of OSCE on practice examination in the depart -ment of cardiothoracic surgery . Methods Through the use of standardized patients and the six-station clinical examination , we as-sessed the clinical skills of interns in the department of cardiothoracic surgery . Results OSCE could appraise interns′clinical ability objectively and accurately , which obtained the recognition from students . Conclusion OSCE is applicable to the clinical skills tes-ting in the department of cardiothoracic surgery .

AIM To prepare D-α-tocopherol polyethylene glycol 1000 succinate (TPGS)-modified artesunate liposomes and to investigate the in vitro anti-tumor activity.METHODS The liposomes prepared by thin-film dispersion method were characterized by transmission electron microscopy and particle size analyzer,and the encapsulation efficiency was determined by ultrafiltration centrifugation.The liposomes' cytotoxicity to human hepatoma HepG2 cells was evaluated by MTT method.RESULTS The average particle size,PDI,Zeta potential,encapsulation efficiency,drug loading of the liposomes were 126.7 nm,0.182,-10.1 mV,78.8％ and 18.38％,respectively.The liposomes displayed a significant inhibition on HepG2 cells with the IC50 value of 0.034 μmol/mL.CONCLUSION Compared with non-TPGS-modified artesunate liposomes,the TPGS-modified artesunate liposomes prepared by this method afford smaller vesicle size,better stability and higher encapsulation efficiency with stronger in vitro anti-tumor activity.

Objective To study the prevalence of atopic dermatitis (AD) in children in major cities of China. Method The children aged 1-7 were surveyed with questionnaire in ten cities of our country. Results In a total population of 49 241, there were 1 371 AD patients (768 males and 603 females) discovered. The total prevalence was 2.78%. The prevalence of males and females was 3.03% and 2.53%, respectively, The total standardized prevalence (S.P.) was 3.07%. We have performed the prick test on 70 patients with atopic lermatitis and at the same time measured the levels of serum IgE and peripheral eosinophil counts. The following results were obtained: 1. Patients with atopic dermatitis showed higher rates of positivty in the prick tests, espe cially to inhalant, allergens. 2. The levels of IgE were increased in these patients and those who were positive to at least one allergen in the prick tests showed higher levels of IgE ha those who were negative in the prick tests. 3. The patients with atopic dermatitis with a past history of allergic rhinitis demonstrated higher rates of skin test positivity. 4.
