1.Assessment of Fatigue among Citrus Fruits Farmers in an Island of the Inland Sea of Japan

We investigated the status of fatigue in a cohort of about 300 citrus fruits farmers in western Japan. The Fatigue Questionnaire for Farmers developed by the Rural Life Research Institute in Tokyo was used in this study. The severity of fatigue was assessed with the fatigue scale for the Questionnaire. The major results obtained were as follows;1. Fatigue scores at the farming season were not so high. It was assumed that the amount of agricultural work was small at that year because the citrus fruits crop was below average.2. The scores of fatigue, labor burden, satisfactory and health priority of the subjects at the farming season were not much high in comparison with those of the slack season. However, the fatigue situation indicated different tendencies between the male and female subjects.3. Comparison among three surveys including this study on farmers producing different agricultural products showed different fatigue situations.4. Forty percent of the questionnaires recovered could not be used for this analysis because of unanswered items. It was suggested that there is need of developing a questionnaire easier to answer for subjects.

2.Suggestions and recommendations from health care professionals involved in regional palliative care program

The aim of this study is to collect the suggestions and recommendations of health care professionals involved in the regional palliative care program (OPTIM-study). A total of 101 multidisciplinary health care professionals who participated in the intervention program were interviewed, and 107 meaningful units were obtained from 89 valid interviews. The responses were categorized into “suggestions regardless of profession” (n=59), including “Participate in a multi-disciplinary conference to expand the network of people”, “Try to understand the situation of others”, “Seek support from others when you cannot solve the problem by yourself”, “Pursue all possibilities before giving up”, and “Do not try too hard”. As suggestions to those engaged in each profession, “Suggestions to community pharmacies” and “Suggestions to care managers” are frequently described. An examination of suggestions by the participants in the regional palliative care program could provide some insights to improve community palliative care.
3.What is the greatest impact of the regional palliative care program?: the OPTIM-study

The purpose of this study was to identify the greatest impact of the regional palliative care program on community health care professionals. Interviews were conducted involving 101 people who became involved in the intervention program implemented in 4 areas across Japan, and 96 valid responses were collected. The following were cited as the greatest impact: [I developed a network of people, and realized the importance of collaboration] (n=61; “I was able to develop an interpersonal relationship” and “Now I understand the significance of collaboration”), [My knowledge and skills regarding palliative care were improved] (n=18; “Knowledge and support helped me respond to patients with confidence” and “I have come to think that there is more to palliative care than terminal care”), [I rediscovered my role through a wide variety of experiences] (n=10), [Both collaboration and palliative knowledge/skills meant a lot to me] (n=4), [What I experienced in this program will help me play my role] (n=2), and [Patients and their families became satisfied] (n=1). The community palliative care program was most effective in facilitating collaboration, and helped participants develop knowledge and skills concerning palliative care.
4.Construction and Evaluation of an Outpatient Prescription Discrimination System Using GeneralPurpose Database Software

Objective: It is very important that, to avoid, pharmacists-check medication being taken by patient. In the Hokkaido University Hospital we used commercial drug identification software at the start of outpatient prescription identification duty and reported the outcome. Furthermore, we filled in another hand-written check sheet with the drug’s name, whether or not it is used in our hospital, alternative drugs, and the dosage and administration. Because of the risk of drugs being entered by mistake, we built a database for drug identification and distinguished the outpatient’s prescriptions. With this system it is possible integrate identification reports and check sheet using one style, automatically. We also to smoothly rationalize duties by planning correct communication between the medical staff. At the same time, we analyzed the case that was able to intervene in reasonable use of medical supplies with a past identification report as a result of pharmacists distinguishing outpatient prescriptions.
Design and Methods: This system was constructed using Microsoft® Access, which is a general-purpose database software. Also, the medical supply database that we used for this system uses “Drugs in Japan Ethical Drugs DB (supervised by Drugs in Japan Forum)” published by JIHO Co., Ltd.
Results: By using this system, we were able to reduce the time required to identify the drugs and make the report. The result of a questionnaire carried out on doctors and a nurses and medical staff revealed that more than 90% of the respondents claimed, “the report is easy to refer.” Likewise, we analyzed a report of the previous year and recognized that medical staff could not find the inappropriate use of prescriptions for outpatients in about 17.5%.
Conclusion: This system improved the efficiency of outpatient prescriptions practices, and it became clear that it could be used convincingly as a tool to share appropriate drug information between medical staff and pharmacists, more precisely. In addition, feedback from medical staff suggested that it might prevent the risk of problems surrounding outpatient prescriptions, from the viewpoint of the pharmacist.
5.A BASIC STUDY FOR ESTABLISHING A SUITABLE PRESCRIPTION OF EXERCISE ON THE LONG DISTANCE WALKING

The present study was performed to investigate the effect of long distance walking upon physical functions for healthy male subjects. The first experiment was carried out in 1977, the second, in 1978, and the third, in 1979 in each autum season. At the first experiment, subjects began walking upon having breakfast, then ate and drank noting during first 30km, and took some foods during another 26km. Twelve hours and fifty minutes of time was spent for 56km walking including 15, 60, and 30 minutes rest periods in between. In the second experiment, subjects ate nothing after dinner of the previous evening and 35km walking took 6 hours during which some rest periods were taken for blood sampling. The third was 80km walking for two days. Subjects walked 40km in 8 hours in the first day and on the following day they walked 40km in ten hours and twenty minutes. They took foods and rest freely.
From the view points of the changes in blood and urinary recordings obtained from those three experiments, the influences of long distance walking on the body indicate as follows ;
1) In case of long distance walking while nothing to eat and drink, the rate of serum saturated fatty acids composition decreased and unsaturated fatty acids increased.
2) In a fasting state, blood glucose gradually decreased, LDH activity increased and blood lactate, GOT and GPT activity presented no significant changes on long distance walking.
3) It was shown that the longer distance to walk, the lesser tendency in blood triglyceride even though subjects take a carbohydrate rich diet while walking.
4) In fasting state, less than 35km walking without foods, the stress to the body seemed not to be critical.
5) More than 40km walking in a day, even though taking meals, would result a significantly severe stress for body.
6) As long as usual meals are taken, 80km walking for two days continuously, 40km in each day, would not result in such a physical stress over the following days.

In the U-40 column, we conducted a questionnaire survey of the U-40 generation on the theme of the specialist system for cardiovascular surgery and reported on the present condition and problems in obtaining certification. With the introduction of a new system, off the job training and participatory training using extracorporeal circulation techniques were newly mandated. In this article, we report the results and discussion of the questionnaire survey regarding the present condition of extracorporeal circulation training for the U-40 generation and the pros and cons of training programs.

At the 51st Annual Meeting of the Japanese Society of Cardiovascular Surgery, U-40 Special Project, we looked back on the U-40 generation and more senior doctors to see what kind of future visions, problems and careers they have had. We conducted a questionnaire for the purpose of visualizing the future prospects of the U-40 generation. In this article, we report the results.
