1.The Hidden Salmon Patch: Ocular Lymphoma Mistaken as Vogt Koyanagi Disease

Ocular inflammation from various causes may have similar clinical presentation thus careful clinical evaluations are
mandatory particularly when the disease appear to be resistant to treatment. This paper reports a case of ocular
lymphoma which was initially treated as Vogt Koyanagi Harada disease at a different centre. A 65-year-old
Sudanese man complained of gradual worsening left eye vision. Careful ocular and slit lamp examination revealed a
conjunctival lesion with choroidal infiltration as well as exudative retinal detachment. Computed tomography scan
(CT scan) showed left eye axial proptosis, and a homogenous enhancing mass at the posterior coat of the globe.
Tissue biopsy of the conjunctival lesion revealed marginal Zone B cell (MALT type), low grade, non-Hodgkin’s
lymphoma. AS the ocular signs and symptoms progressed, chemotherapy was initiated. The proptosis, exudative
retinal detachment, disc hyperemia and swelling improved after chemotherapy. The post treatment CT scan showed
reduction of the posterior ocular coat mass. The possibility of malignant lymphoma should be considered in patients
with resistant uveitis despite administration of corticosteroid. Ocular lymphoma is an indolent tumour with good
prognosis. Careful ocular examination, adequate imaging studies followed by early surgical biopsy will contribute to
early diagnosis.

2.Flourescein Angiogram and Funduscopic Features of Radiation Induced Retinopathy: A Descriptive Value of Retinopathy Severity in Patients with Nasopharyngeal Carcinoma

Radiation retinopathy (RR) is a known complication after radiotherapy for
Nasopharyngeal Carcinoma (NPC). This study aims to relate the relationship of RR and radiation dose in patients with NPC through assessment with clinical
funduscopy and fundus fluorescein angiogram (FFA). A cross sectional study was
conducted on patients with NPC who had completed radiotherapy treatment in
the Oncology Clinic, Universiti Kebangsaan Malaysia Medical Centre (UKMMC).
Eighty two eyes of 42 patients were examined and the prevalence of RR was found
to be 35.4%. The severity of RR is strongly associated with the dose of radiation to
the retina (Spearman correlation value=0.48; p<0.001). The common features of
RR assessed by FFA were telangiectatic vessels (26.2%) and capillary non-perfusion
(14.3%). Retinal neovasularization occurred in 10.7% of eyes. The level of visual
deterioration correlated with the severity of RR with 26% of eyes experiencing a
visual acuity of 6/18 or worse. More than one third of patients developed RR, with
radiation maculopathy being the commonest cause for significant visual loss. FFA
is a useful tool in detecting early signs of radiation retinopathy and maculopathy.

4.Use of the National Early Warning Score (NEWS) to Identify Acutely Deteriorating Patients with Sepsis in Acute Medical Ward.

INTRODUCTION:
The National Early Warning Score (NEWS) is well established in acute medical units to identify acutely deteriorating patients and is shown to have good prognostic value. NEWS, however, has only been used in the Emergency Department as a triage tool. We aimed to evaluate the validity of NEWS in Acute Medical Ward (AMW) that treats predominantly acute infection-related conditions to the Internal Medicine service.
MATERIALS AND METHODS:
We undertook a retrospective cohort study and analysed NEWS records of all patients admitted to AMW at Singapore General Hospital between 1 August 2015 and 30 July 2017. The outcome was defined as deterioration that required transfer to Intermediate Care Area (ICA), Intensive Care Unit (ICU) or death within 24 hours of a vital signs observation set.
RESULTS:
A total of 298,743 vital signs observation sets were obtained from 11,300 patients. Area under receiver operating characteristic curve for any of the 3 outcomes (transfer to ICA, ICU or death) over a 24-hour period was 0.896 (95% confidence interval, 0.890-0.901). Event rate was noted to be high above 0.250 when the score was >9. In the medium-risk group (score of 5 or 6), event rate was <0.125.
CONCLUSION
NEWS accurately triages patients according to the likelihood of adverse outcomes in infection-related acute medical settings.
