1.THE RELATIONSHIPS OF ANKLE LIGAMENTOUS INJURIES WITH PHYSICAL AND POSITIONAL CHARACTERISTICS IN COLLEGE FOOTBALL PLAYERS

The purpose of this study was to investigate the relationships of ankle ligamentous injuries with physical and positional characteristics in college football players. Specific hypothesis was addressed whether medial ankle ligamentous complex (MALC) injury was often caused by oversized players, offensive or defensive linemen.
53 subjects participated in this study. We examined their previous history of ankle ligamentous injuries and classified them into three groups: (1) the lateral ankle ligamentous complex (LALC) in-jury group; (2) the MALC injury group; and (3) the no ankle ligamentous (NAL) injury group. Regarding MALC injury, we obtained the following results. First, MALC injury occupied 35% of total ankle ligamentous injuries. Second, both body weight and BMI of the MALC injury group were signi-ficantly the heaviest and largest of the groups. Lastly, OL, LB, and DL occupied highest occurring percentage (75%) of MALC injury.
We concluded that MALC injury tended to be incurred by the players with heavier body weight and/or larger BMI. Players OL, LB, or DL also suffered MALC injury. Thus, these results suggest that body weight, BMI and position are considered as the risk factor for MALC injury.
3.Characteristics of Keio University Hospital's Kampo Clinic Judging from the First Visit Patients

Background : Since 2002, Keio University Hospital's Kampo Clinic has promoted itself actively through the media, public presentations, faculty outreach and an internet home page. However, the relative value of these promotions is unknown. Additionally, the range and prevalence of presenting concerns, and the ages and the gender ratio of the patients served at Keio is unknown.Objective and Methods : To better understand and better serve the patients, the medical charts of every new patient who presented to Keio University Hospital's Kampo Clinic from November 2004 to November 2005 (n=791) were retrospectively analyzed for 1) referral source, 2) age, 3) gender, and, 4) disease category.Results : The internet webpage was by far the best source of new patient referrals. The out-of-hospital referral rate to the clinic was remarkably low. Women exceeded men by a 3 : 1 ratio. Most women were in their thirties but male patients were fairly evenly distributed across the age spectrum. Patients under 16 and over 70 were gender-balanced. The vast majority of patients presented with general medicine/pediatric, dermatological or gynecological problems.Conclusion : A patient-oriented internet home page provides a good source of new patient referrals. Given the low rate of referrals from outside hospitals and physicians, additional outreach directed at internal medicine, dermatology or obstetrics/gynecology physicians appears warranted.

4.Cardiac Rupture Caused by Blunt Trauma: Pitfalls in Diagnosis and Treatment.

Diagnosis of cardiac rupture caused by blunt trauma is sometimes difficult when multi-organ injuries are associated with profound shock. Only prompt diagnosis and urgent treatment can save the patients. We have encountered 16 cases of blunt cardiac rupture, including 4 that survived in the past 10 years. All cases of cardiopulmonary arrest on arrival were unable to be resuscitated. Also, all cases of profound hemorrhagic shock caused by multi-organ injury succumbed. Among the cases in which the region of cardiac rupture was identified, only cases of injury to the right heart chambers were saved. No case of rupture in the left heart chambers survived. It is definitely important to suspect cardiac injury and make a prompt diagnosis to save patients with blunt chest trauma. Ultrasonic cardiography is highly effective for urgent diagnosis of cardiac rupture because it can be performed readily, noninvasively and, therefore, repeatedly.
5.Extra-anatomical Bypass Grafting Combined with Bilateral Renal Artery Reconstruction for a Case with Atypical Coarctation Due to Aortitis Syndrome

A 16-year-old girl with aortitis syndrome under treatment with a low dose of prednisolone was admitted because of severe headache and intermittent claudication. Angiography revealed diffuse stenosis of the thoracoabdominal aorta and the bilateral renal arteries. Extra-anatomical bypass grafting from the ascending to the abdominal aorta was first made with a 14mm woven Dacron graft through a midline sternolaparotomy. Bilateral renal arteries were difficult to dissect due to periarterial adhesion. Bypass grafting for the left renal artery could be performed with a 5mm external velour wrap-knit Dacron graft (Sauvage, Bionit); however, the right renal artery was so thin that bypass was made with a 4mm EPTFE graft which was demonstrated to be occluded by follow-up angiography 3 years after surgery. The postoperative course has been uneventful and she has been free from symptoms up to now. The good long-term function of the bypass graft from the ascending aorta holds promise for diffuse coarctation of the thoracoabdominal aorta due to aortitis syndrome.
6.Arterial Reconstruction with Prosthetic Grafts in the Bilateral External Iliac Artery of Infant.

Prosthetic grafts have been employed in a limited number of pediatric patients with peripheral vascular lesions. We treated an iatrogenic obstruction of bilateral external iliac arteries in a child. The patient was a six-year-old girl whose chief complaints were intermittent claudication, and lower limb pain when exposed to cold. She had a history of coarctation complex for which she had undergone repeated catheterizations by puncture of both femoral arteries in her infancy. At the age of four, two-stage operation was performed: resection of the coarctation and end-to-end anastomosis, and direct closure of ventricular septal defect. Angiography performed through the brachial artery demonstrated obstruction of the bilateral external iliac arteries. Both right and left femoral artery were visualized through the collateral artery from the ipsilateral internal iliac artery. Because she became afflicted with lower limb ischemia a revascularization procedure was indicated. A 6mm expanded polytetrafluoroethylene (ePTFE) bypass graft was implanted bilaterally between the internal iliac artery and the common femoral artery with end-to-side anastomosis. Ischemic symptoms disappeared postoperatively and MR-angiogram performed nine days after surgery also showed the patency of the graft. It is a great concern, however, that the length of the graft may become relatively shorter with the growth of the patient. It is also anticipated that the lower limbs may suffer relatively insufficient blood flow in the future.
7.The Function of the Vitamin D Receptor and a Possible Role of Enhancer RNA in Epigenomic Regulation of Target Genes: Implications for Bone Metabolism

Vitamin D (VD) is essential for bone health, and VD or its analogues are widely used in clinics to ameliorate bone loss. The targets and mode of VD anti-osteoporotic actions appear to be different from those of other classes of drugs modulating bone remodeling. VD exerts its biological activities through the nuclear VD receptor (VDR)-mediated transcriptional regulation of target mRNA and non-coding RNA genes. VD-induced gene regulation involves epigenetic modifications of chromatin conformation at the target loci as well as reconfiguration of higher-order chromosomal organization through VDR-mediated recruitment of various regulatory factors. Enhancer RNAs (eRNA), a class of non-coding enhancer-derived RNAs, have recently emerged as VDR target gene candidates that act through reorganization of chromatin looping to induce enhancer-promoter interaction in activation of mRNA-encoding genes. This review outlines the molecular mechanisms of VD actions mediated by the VDR and suggests novel function of eRNAs in VDR transactivation.

8.Complete Recovery from Generalized Anxiety Disorder Through Anchusan

We had a female patient who had stagnation of qi and recovered not only by adjusting qi but also by warming interior cold. A 67-year-old woman visited our clinic for generalized anxiety disorder and whole body overstrain on the basis of domestic problems. She had taken anxiolytic, which brought about some relief. She also suffered from uterine cancer, and had operation of uterine resection. After operation, she took radiation therapy, which caused ileus. Although she took daikenchuto, ileus sometimes recurred. At first, we diagnosed her condition as stagnation of liver qi and deficiency of liver blood according to Kampo medical findings, and prescribed yokukansankachimpihange. However, she felt little effectiveness. We focused on her condition as spleen-stomach deficiency cold, and switched to anchusan, which gradually improved her symptom and led to discontinuation of daikenchuto and anxiolytic. She continued to take it for 7 years, and gradually reduced. She stopped taking medication 2 years later, and has had no recurrence ever since. It suggests that anchusan was effective in warming her latent severe interior cold. We should consider anchusan as a therapeutic candidate for stagnation of qi and interior cold, although it is well known Kampo medicine that is suitable for stomach pain.
9.The Association of Fever with Total Mechanical Ventilation Time in Critically Ill Patients.

This research aims to investigate the impact of fever on total mechanical ventilation time (TVT) in critically ill patients. Subgroup analysis was conducted using a previous prospective, multicenter observational study. We included mechanically ventilated patients for more than 24 hours from 10 Korean and 15 Japanese intensive care units (ICU), and recorded maximal body temperature under the support of mechanical ventilation (MAX(MV)). To assess the independent association of MAX(MV) with TVT, we used propensity-matched analysis in a total of 769 survived patients with medical or surgical admission, separately. Together with multiple linear regression analysis to evaluate the association between the severity of fever and TVT, the effect of MAX(MV) on ventilator-free days was also observed by quantile regression analysis in all subjects including non-survivors. After propensity score matching, a MAX(MV) ≥ 37.5℃ was significantly associated with longer mean TVT by 5.4 days in medical admission, and by 1.2 days in surgical admission, compared to those with MAX(MV) of 36.5℃ to 37.4℃. In multivariate linear regression analysis, patients with three categories of fever (MAX(MV) of 37.5℃ to 38.4℃, 38.5℃ to 39.4℃, and ≥ 39.5℃) sustained a significantly longer duration of TVT than those with normal range of MAX(MV) in both categories of ICU admission. A significant association between MAX(MV) and mechanical ventilator-free days was also observed in all enrolled subjects. Fever may be a detrimental factor to prolong TVT in mechanically ventilated patients. These findings suggest that fever in mechanically ventilated patients might be associated with worse mechanical ventilation outcome.
