Background: Femoral shaft fracture, in company with the development of morden society and the increase in means of transport, is increasing in the number of case and the complicated degree of injury. Objectives: 1. To note indication for interlocking intramedullary SIGN nailing in femoral shaft fracture in adult patients. 2. To assess result of interlocking intramedullary SIGN nailing in treatment of femoral shaft fracture in adult patients. Subjects and method: A prospective, descriptive study was conducted in the 40 patients between the ages of 17 and 80 who were dignosed femoral shaft fracture caused by injury and operated for bone union by interlocking intramedullary SIGN nailing at Viet Duc hospital, from April/2004 to July/2005. Results: In this study: 5 patients with I degree fracture (12.5%), 20 with II degree (25%) and most of patients with III and IV degree fracture (62.5%), of which 14 patients with IV degree (35%). Because the SIGN nailing\u2019s structure was solid, so it had a good capacity for patient\u2019s weight. 4 weeks after operation, 85% patients were able to move around without crutches. There was no complications. Conclusion: SIGN nailing enables surgeons to drive interlocking intramedullary nailing for long bone without image intensifier. It also enables both surgeons and patients to appoarch modern, high effective methods without needing expensive machines.

Background: In 2006, the project of global fund for malaria prevention in Vietnam provided a large number of rapid diagnostic test Paracheck F for Vietnam for the purpose of rapid malaria diagnose. However, there is no study on evaluation the effect of rapid diagnostic test compared with microscopy method. Objective: To evaluate the diagnostic yield and cost of paracheck F test and microscopy in malaria diagnosis and treatment. Subject and Method: The study was carried out in 6 communes belongs to Quang Tri and Quang Binh provinces from September to November - 2006. The study was divided into 3 phases. Phase 1: diagnoses and treatments are based on clinical symptoms, phase 2: diagnoses and treatments are based on the results of paracheck and phase 3: diagnoses and treatments are based on the results of microscopy. All phases, both the common patients and malarial patients and the amount of anti-malarial drugs were treated, the amount of money was spent on transport and days work off of malarial patients and their relatives were calculated. Result: The investigation data on expenditure of malaria patients showed that: the average direct cost of malaria patient in phase 1 is VND 116.100; phase 2: VND 119.400 and phase 3: VND 120.800 per 1 treatment course. There is no significant difference between direct costs in three phases (p > 0.05). Conclusion: The expense efficiency for finding out a case of malaria by paracheck and microscopy is equivalent and lower than the expense of diagnosis based on clinical symptoms.

3.Autologous bone marrow transfusion to treat the tibia diaphyseal nonunions

Background: Bone marrow stem cells with their plasticity can be used to replace and repair the other damaged organs and tissues, so they can also be used to obtain bone healing of nonunions. Objective: to evaluate the results of percutaneous autologous bone marrow grafting to treat the tibia diaphyseal nonunions. Subjects and methods: 12 patients with noninfected nonunion of the tibia were diagnosed and treated in Viet Duc Hospital. About 250mL of marrow was aspirated, then separated and concentrated by density gradient centrifugation. The final mononuclear cell mass containing stem cells and progenitors was washed in 30ml of 0.9% NaCL and then injected into the damaged sites. Patients were evaluated by clinical and X-rays examinations with at least 6 months follow-up. Results: None of the patients had post - op complications. Bone union was obtained in eleven of the twelve patients (91,7%) at an average of 15,3 weeks (range, 9 - 30 weeks), the bone marrow grafts used for these patients who had bone union contained a mean of 5,65 \xb1 3,74 x 106 (0,95 - 11,73 x 106) CD34(+) stem cells in total. Conclusions: Percutaneous autologous bone - marrow grafting is a minimally invasive alternative and a simple, effective, safe method for the treatment of the tibia diaphyseal nonunions with the comparative bone healing rate. \r\n', u'\r\n', u'

4.Ischemic Colitis Presented as Pseudomembranous Colitis: An Untypical Case from Vietnam

Ischemic colitis (IC) is an underreported chronic disease characterized by the hypoperfusion of the bowel mucosa. The diagnosis and treatment may be challenging because its clinical course resembles other colitis or even colorectal malignancies. This paper reports an untypical case to underline the diversity of IC manifestation. A 68-year-old man with several comorbidities was admitted because of abdominal pain with a 6-month duration and a mass in the left lower quadrant. Colonoscopy revealed erosive pseudomembranous colitis narrowed colon segments with ulcerated mucosa mimicking colorectal cancer and inflammatory bowel disease. The stool cultures and Clostridium difficile toxin tests were negative. After the failure of conservative therapy, the Hartmann procedure with temporary ileostomy was performed uneventfully. The histological results of the surgical specimens revealed IC with focal pseudomembranous areas.

In the present study, bioactivity-guided extraction and isolation of the n-hexane fraction of the fruits of Vietnamese Schisandra sphenanthera led to the isolation of five dibenzocyclooctadiene lignans as gomisin N (1), schisandrin C (2), gomisin H (3), gomisin D (4), and gomisin C (5). All the isolates were tested for their inhibition of NO production in LPS-induced RAW 264.7 cells. Among them, compounds 4 and 5 showed weak inhibition of NO production with IC 50 values of 25.0 ± 1.6 and 24.8 ± 2.0 µM, respectively. Compound 1 exhibited NO production inhibition with an IC 50 value of 15.8 ± 2.1 µM, meanwhile, schisandrin C (2) showed the most potent inhibition with an IC 50 value of 8.5 ± 0.5 µM. In addition, compound 2 had a concentrationdependent inhibitory effect on the protein expression of the inflammatory enzymes iNOS and COX-2. Their physicochemical properties and ADMET data were predicted by in silico, indicating favorable drug-like properties as well as low acute oral toxicity. The results suggest that the fruit of S. sphenanthera and its phytochemical constituents might be used as anti-inflammatory agents.
6.Identification of Soluble Epoxide Hydrolase Inhibitors from the Seeds of Passiflora edulis Cultivated in Vietnam

Soluble epoxide hydrolases (sEH) are enzymes present in all living organisms, metabolize epoxy fatty acids to 1,2-diols. sEH in the metabolism of polyunsaturated fatty acids plays a key role in inflammation. In addition, the endogenous lipid mediators in cardiovascular disease are also broken down to diols by the action of sEH that enhanced cardiovascular protection. In this study, sEH inhibitory guided fractionation led to the isolation of five phenolic compounds trans-resveratrol (1), trans-piceatannol (2), sulfuretin (3), (+)-balanophonin (4), and cassigarol E (5) from the ethanol extract of the seeds of Passiflora edulis Sims cultivated in Vietnam. The chemical structures of isolated compounds were determined by the interpretation of NMR spectral data, mass spectra, and comparison with data from the literature. The soluble epoxide hydrolase (sEH) inhibitory activity of isolated compounds was evaluated. Among them, trans-piceatannol (2) showed the most potent inhibitory activity on sEH with an IC₅₀ value of 3.4 µM. This study marks the first time that sulfuretin (3) was isolated from Passiflora edulis as well as (+)-balanophonin (4), and cassigarol E (5) were isolated from Passiflora genus.

7.Management of Antibiotic-Resistant Helicobacter pylori Infection: Perspectives from Vietnam

Antibiotic resistance is the most important factor leading to the failure of eradication regimens. This review focuses on the prevalence of Helicobacter pylori primary and secondary resistance to clarithromycin, metronidazole, amoxicillin, levofloxacin, tetracycline, and multidrug in Vietnam. We searched the PubMed, EMBASE, Vietnamese National Knowledge Infrastructure, and Vietnamese Biomedical databases from January 2000 to December 2016. The search terms included the following: H. pylori infection, antibiotic (including clarithromycin, metronidazole, amoxicillin, levofloxacin, tetracycline, and multidrug) resistance in Vietnam. The data were summarized in an extraction table and analyzed manually. Finally, Excel 2007 software was used to create charts. Ten studies (three studies in English and seven in Vietnamese) were included in this review. A total of 308, 412, 523, 408, 399, and 268 H. pylori strains were included in this review to evaluate the prevalence of H. pylori primary resistance to amoxicillin, clarithromycin, metronidazole, levofloxacin, tetracycline, and multidrug resistance, respectively. Overall, the primary resistance rates of amoxicillin, clarithromycin, metronidazole, levofloxacin, tetracycline, and multidrug resistance were 15.0%, 34.1%, 69.4%, 27.9%, 17.9% and 48.8%, respectively. Secondary resistance rates of amoxicillin, clarithromycin, metronidazole, levofloxacin, tetracycline, and multidrug resistance were 9.5%, 74.9%, 61.5%, 45.7%, 23.5% and 62.3%, respectively. In Vietnam, primary and secondary resistance to H. pylori is increasing over time and affects the effectiveness of H. pylori eradication.

8.Blood pressure screening results in adults in Thua Thien Hue province through the program “May Measurement Month” 2022 of the International Society of Hypertension

Background: Hypertension is a common disease in adults, also the leading cause of cardiovascular disease and premature death worldwide. The prevalence of hypertension has increased, especially in low- and middle-income countries. Therefore, hypertension screening is important to provide the basis and evidence for the development of timely intervention policies and programs. The objective of the “May Measurement Month” 2022 program is to investigate the prevalence of hypertension, drug treatment and blood pressure control in people in Thua Thien Hue province and to assess the risk factors surrounding hypertension. Subjects and Methods: This cross–sectional study collected data from volunteer adults (≥ 18 years old) in Thua Thien Hue province from 7/2022 to 8/2022. Sitting blood pressure had been measured in triplicate according to standardized specified methods of the International Society of Hypertension. Results: Average age 54.07 ± 18.17, there were 627 people (23.4%) with hypertension at the time of screening, 245 of which (39.1%) had never been diagnosed with hypertension before screening at the time. 724 people were on treatment with 1 or more types of drug, however, there were 337 people (46.5%) having their blood pressure not controlled. The statistics showed that 84.9% of volunteers were using at least 1 drug to treat hypertension. Related factors of hypertension are age, sex, overweight/obesity status, smoking, medical history of diabetes and medical history of COVID-19. The rate of subjects who had history of COVID-19 was 36.4%, including 31.6% of hypertension patients. Conclusion: The hypertension percentage of Thua Thien Hue population is still high and the rate of blood pressure control is still limited. There is a relationship between hypertension proportion and history of COVID-19. It is necessary to publish timely intervention measures to reduce the rate of hypertension and its risks to people.
