1.Healthcare Provider-Patient Communication: A Satisfaction Study in the Outpatient Clinic at Hospital Kuala Lumpur

Background: There is growing interest in research on patient satisfaction with healthcare provider (HCP) communication as a measure of healthcare quality and HCPs’ communication competency. This study aimed to determine the levels of patient satisfaction with healthcare provider-patient communication (HCP-PC) and its associated factors at the outpatient clinic at Hospital Kuala Lumpur.
Methods: A cross-sectional study was conducted on a convenience sample in July 2012 using self-administered questionnaires for the data collection. Both overall and domain-specific satisfaction were measured, with the three domains being exchanging information (EI), socio-emotional behaviour (SB), and communication style (CS).
Results: The findings show that 92.8% of the 283 respondents were satisfied with overall HCP-PC, 89.5% with EI, 91.3% with SB, and 72.2% with CS. Satisfaction was statistically higher among Malays for CS and higher among those with low education and poor health for EI, SB and CS. EI and overall communication satisfaction were also higher among patients who reported short wait times, and patients who were in gender concordance with their HCPs showed higher SB satisfaction.
Conclusion: Basic and continuous communication skills training and patient activation programs should be established to increase patient satisfaction. Health information technology use should be actively promoted to allow for structured and standardised information exchange between HCPs and patients.
2.Validation Study of the Malay Version of the Work-Family Conflict Questionnaire

Background: Work-family conflict has received increasing attention in recent decades in the area of workplace stressors, which can affect employees’ health. However, the dimensionality of the work–family conflict construct among the Malay-speaking population has not been clarified. In order to do so, it is crucial to use an instrument that is appropriate and valid for the Malay-speaking population. As such, the goal of this study was to validate and test the dimensionality of the Malay version of the Work-Family Conflict Questionnaire.
Methods: The present study conducted exploratory factor analysis, confirmatory factor analysis, discriminant validity, convergent validity, and internal consistency, using Cronbach’s alpha, of the work–family conflict construct among 332 working women in Malaysia.
Results: The results supported the existence of four dimensions in the Work-Family Conflict Questionnaire that distinguish between time based and strain-based work-family conflict and family-work conflict. The discriminant validity, convergent validity, and internal consistency of this construct are adequately supported.
Conclusion: The findings of this study supported the existence of discriminant and convergent validity, as well as adequate reliability, for the construct. Thus, the Work–Family Conflict Questionnaire is a valid and reliable instrument among Malay-speaking working women.
3.The Mediating Role Of Health Related Behaviors In The Relationship Between Work-Family Conflict And Physical Health

Purpose of the present study was to assess effect of work-family conflict (along with its four dimensions) on physical health status. In particular, we examined the mediating role of health related behaviors on the linkage between all dimensions of work-family conflict and physical health status. The current cross-sectional research was conducted among 567 women who were working in the public departments of Malaysia. Self-administrated questionnaire was used for data collection. Our findings showed that work-family conflict (along with its four dimensions) significantly and negatively correlated with physical health status. The findings from mediational analysis showed that all dimensions of work-family conflict indirectly deteriorate physical health status via increasing the chance of not using food strategies. However, our results did not support the mediating function of physical activity on the proposed relationship. Results of the current study indicated that women with greater level of work-family conflict are more likely to suffer from poor physical health status. Having multiple strategies for managing food and eating reduce the adverse effect of work-family conflict on physical health. Organizations may benefit from Family Friendly Policies (FFP) that can alleviate level of conflict and eventually avoid or decrease rate of engagement in unhealthy behavior that have detrimental effect on health status.
4.Malay Version of the Family Satisfaction Scale: Validity and Reliability among Malaysian Working Women

Family satisfaction is referred to the extent in which family members feel happy and fulfilled with each other. However, there has been lack of evidences on the family satisfaction scale within the Malaysian context. Therefore, the aim of this study was to assess validity of the Malay version of the Olson’s Family Satisfaction Scale. This is to allow Malaysian researchers to bring family satisfaction in line with the different field of studies. This study was conducted among 567 Malaysian working women. Data were collected using self-administrated questionnaires. This study conducted exploratory and confirmatory factor analysis, convergent validity and internal consistency using Cronbach’s alpha. The findings of this study support the uni-dimensionality of the Malay version of the family satisfaction scale. The 10 items of the scale account for 68.1% of the total variance and the un-rotated factor loadings ranged from 0.76 to 0.87. Confirmatory factor analysis was run and supported the structure of family satisfaction scale. The results of confirmatory factor analysis using AMOS 21 in the current study reported the following indices: RMSEA= 0.06, CFI= 0.94, NFI= 0.94, TLI= 0.93. The convergent validity (average variance extracted= 0.65) and the internal consistency (Cronbach’s alpha= 0.94) of this construct were adequately supported. The findings support the factor structure, convergent validity and the internal consistency of the examined construct. Therefore, Malay version of the family satisfaction scale is a valid and reliable instrument among Malaysian working women.
5.The Relationship Between Job Satisfaction and Psychological/Physical Health among Malaysian Working Women

Background: The workplace environment has a great influence on employees’ health. Job dissatisfaction has been widely recognised as a workplace stressor that can influence employees’ psychological and physical health statuses. However, job satisfaction is a multi-dimensional concept, and it is necessary to investigate its different facets and their unique consequences. Therefore, the aim of this study was to assess the relationship between the nine facets of job satisfaction and psychological health and somatic complaints (i.e., sleep disorders, headache, gastro-intestinal and respiratory problems).
Methods: This cross-sectional study was conducted among 567 Malaysian women working in the public sector. Data collection was conducted using a series of self-administered questionnaires.
Results: The results of this study show that there is a link between job satisfaction and psychological distress as well as four somatic complaints. Satisfaction with the nature of work was the strongest predictor for psychological distress, sleep disorders, headaches and gastro-intestinal problems.
Conclusion: From the results of this study, we conclude that there is a link between job satisfaction and the health status of employees. In addition, job satisfaction levels vary across different dimensions and can even differ from an individual’s feelings of global job satisfaction. Policies and practices should focus on improving working conditions to enhance the fit of the job and the employee.
6.Validation of the Malay Version of the Parental Bonding Instrument among Malaysian Youths Using Exploratory Factor Analysis

Background: Parenting behaviour is culturally sensitive. The aims of this study were (1) to translate the Parental Bonding Instrument into Malay (PBI-M) and (2) to determine its factorial structure and validity among the Malaysian population.
Methods: The PBI-M was generated from a standard translation process and comprehension testing. The validation study of the PBI-M was administered to 248 college students aged 18 to 22 years.
Results: Participants in the comprehension testing had difficulty understanding negative items. Five translated double negative items were replaced with five positive items with similar meanings. Exploratory factor analysis showed a three-factor model for the PBI-M with acceptable reliability. Four negative items (items 3, 4, 8, and 16) and item 19 were omitted from the final PBI-M list because of incorrect placement or low factor loading (< 0.32). Out of the final 20 items of the PBI-M, there were 10 items for the care factor, five items for the autonomy factor and five items for the overprotection factor. All the items loaded positively on their respective factors.
Conclusion: The Malaysian population favoured positive items in answering questions. The PBI-M confirmed the three-factor model that consisted of care, autonomy and overprotection. The PBI-M is a valid and reliable instrument to assess the Malaysian parenting style. Confirmatory factor analysis may further support this finding.
7.Limited Health Literacy And Its Associated Factors Among Overweight And Obese Housewives Living In Klang Valley Low Cost Flats: Findings From The My Body Is Fit And Fabulous At Home (Mybff@Home) Study

Limited health literacy is highly associated with poor health status. The concept of health literacy in public health relates to the concern that people with lower health literacy were also less likely to engage in health promoting behaviours. This is an analysis on health literacy and its associated factors among housewives and also a sub analysis of the Malaysian My Body is Fit and Fabulous at Home (MyBFF@home) study involving 328 overweight and obese housewives living in low cost flats in Klang Valley, part of a weight loss programme recruited using convenient sampling. Health literacy was measured using the Malay version of Newest Vital Sign (NVS) test with scores ranging from 0 to 6. Total score of ≤2 indicate limited health literacy and total scores of >2 indicate adequate health literacy. Descriptive analysis, chi-square test and multiple logistic regression were used to analyse the data.The mean(SD) NVS score was 1.07(1.19) with most (87.5%) housewives having limited health literacy. Factors that were significantly associated with limited health literacy were older age of 45 to 59 years old (p = 0.040), primary education and below (p = 0.001) and absence of internet connectivity (p = 0.001). In the final model, absence of internet connectivity (OR 2.61; 95%CI 1.31 to 5.22) was associated with limited health literacy. Majority of the housewives have limited health literacy and providing internet connectivity to housewives may increase their health literacy.

8.Influence of an intervention program promoting voluntary fasting practices and its perceived barriers among overweight or obese Muslim women working in the public sector, Malaysia

Introduction: This article aims to describe the effect of an intervention to promote the practice of voluntary Islamic fasting and its barriers among a group of overweight and obese Muslim women working in the public sector in Malaysia. Materials and methods: In this quasi-experimental study, an intervention to encourage voluntary fasting was delivered in a half day seminar, supported by relevant booklets prepared. A self-administered questionnaire was used to capture data on voluntary fasting practices and its perceived barriers before and four months after the intervention. Data was analysed using Mc Nemar test to compare the proportion before and after the intervention. Significant level was set at p<0.05. Results: A total of 56 women were recruited. Their mean age was 36.65 years, most were married, had formal education up to secondary level and median income of RM 3000 per month. At baseline, 60.7% of them have practiced voluntary fasting. After intervention, the number of participants who practices voluntary fasting increased with a significant increase in three types of voluntary fasting (i.e. Mondays only, Thursdays only and 6 days in Shawal). The highest barrier to practice voluntary fasting was having no motivation to do so, followed by work commitment, health problems and family commitment. Discussion and Conclusion: Most of the respondents could practice voluntary fasting regularly if motivated to do so and backed by social support both at work and at home.
9.Weight Perception among Non-pregnant Urban Malaysian Women: Is It Reliable?

In maternal healthcare, pre-pregnancy weight is used to predict pregnancy outcomes. Since no recorded data on pre-pregnancy weight, perceived weight is used alternatively. This study examines the relationship between perceived and actual weight among non-pregnant urban Malaysian women of childbearing age and identifies differences in perceived and actual weight by selected socio-demographic characteristics. A cross-sectional study was conducted between April and June 2013 among urban Malaysian women attending public health clinics in the Klang Valley. Information on height, perceived current weight and time when their weight was last taken were obtained and actual weight was the average of two measurements (TANITA-HD-323-digital-scale). Socio-demographic data collected were age, ethnicity, education level, marital and employment status and total household income. Mean age of 371 women in this study was 28.81±5.65, 82.2% were Malays, 62.8% had tertiary education, over 75% were married and employed, with more than half from middle-income households. Overall, the mean perceived and actual weight was 59.29±11.59 and 59.20±11.90 respectively. Pearson’s Correlation test showed a very strong positive correlation between perceived and actual weight (r=0.957;p<0.0001), ranging between 0.852 to 0.994 among subgroups; 258 (69.5%) perceived their weight accurately (±2.0 kg of actual weight), 49 (13.2%) under and 64 (17.3%) overestimated their weight. Main outliers were among younger women, Malays, tertiary educated, employed, middle-income and had weight last measured a month or more ago. Strong correlation between perceived and actual weight among women in this study reassured weight perception can be used more confidently in patients’ history taking and future research among urban Malaysian women using public health services.
10.Prevalence of ‘Researcher’s defined’ and ‘Self-rated’ Successful Aging among Pre-Retirement Public Servants

Introduction:
The socioeconomic impact of aging population can be reduced if majority of people achieve successful aging. This study aimed to determine the prevalence of researcher's defined successful aging (RDSA) and self-rated successful aging (SRSA) among pre-retirement public servants and their predictors.
Methods:
The sample included 1,064 pre-retirement public servants (50 to 60 years old) from nine government agencies. Data was analyzed using Multiple Logistic Regression to test for the association between the studied factors and SA.
Results:
The prevalence of RDSA and SRSA was 37.5% and 98.7%, respectively. Results showed four (4) significant factors with higher odds of having RDSA were not obese, good social support, being physically active and younger age. Meanwhile, five (5) factors highly selected by respondents as predictors for SRSA were having good spiritual or religious practice, happy family, good psycho cognitive function, social support and good physical function.
Conclusions
The prevalence of SRSA was higher despite the presence of self-reported chronic diseases and physical limitation identified among respondents. The discrepancy in both prevalence of SA reflects the differences that exist between the criteria for SA perceived by respondents and researchers. Misperception among respondents of their aging process as 'successful' despite having diseases or disability may worsen their health status because they continue practicing unhealthy lifestyles without action to improve it. Promotional activities on SA, regular health screening since young and healthy working environment should be implemented by various agencies.
