1.Comparison of survival in patients with breast cancer of triple-negative versus non-triple-negative

Objective To assess whether survival differed among patients who had triple-negative breast cancers compared with patients who had non-triple-negative breast cancers.Methods The clinicopathologic data of 684 patients with operable breast cancer,who were treated with multimodal therapy from December 1998 to June 2007 were analyzed.Of these patients,107 cases were confirmed to have triple-negative tumor histology(triple-negative group),577 cases were confirmed to have non-triple-negative tumor histology(non-triple-negative group).The 7 years cumulative survival and disease-free survival were analyzed by Kaplan-Meier method and compared by Log-rank test.The prognosis Was analyzed by COX regression model.Results The 7 years overall survival rates for triple-negative group and non-triple-negative group were 51.4%(55/107),74.2%(428/577),respectively,there Was significant difference between the two groups(P<0.01).However,the 7 years overall survival rates of patients with triple-negative group and those who with Her-2 positive were 51.4%(55/107)and 53.1%(43/81)respectively,there Was no significant difference between them (P>0.05).Univariate analysis revealed that the factors affecting the disease-free survival included age,menopausal status,tumor size,lymph nodes metastasis,estrogen,progesterone,and Her-2 receptors status.Multivariate analysis showed minor size,lymph nodes metastasis,estrogen,progesterone,and Her-2 receptors status were independent prognostic factors.Conclusions This study has shown a trend towards a poorer outcome for patients with triple-negative breast cancer similarity to patients with Her-2 positive cancers.Triple-negative breast cancer is an independent predictor factor of survival.
2.General self-efficacy and body dysmorphic disorder of patients with deformities of the maxillofacial region

Objective To observe the self-efficacy and body image changes of the patients with oral and maxillofacial deformity in pre-and post-operative.Methods 183 patients with oral and maxillofacial deformity and 200 relatives as control group were measured by applying General Self-Efficacy Scale (GSES) of Chinese version and Self-rating Scale of Body Image(SSBI).Postoperative satisfaction degree was scored by applying attitude measurement method of interval scale.Perioperative GSES,SSBI score,and postoperative satisfaction degree were statistically analyzed.Results GSES score (26.79 ± 5.26) was lower than that in control group in preoperative (29.69 ± 5.37),and the difference was statistically significant (P < 0.05).While SSBI score (24.35 ± 8.59) was higher than that in control group(11.04 ± 6.67),and the difference was also statistically significant (P < 0.05).GSES score in postoperative(29.52 ± 3.80) was significantly higher than that in preoperative (P < 0.05).SSBI score in postoperative(18.88 ± 8.19) was significantly higher than that in preoperative (P < 0.05).Postoperative satisfaction was comparatively lower for the patients whose GSES score was higher than 32 and 36 for SSBI in preoperative.Conclusion Oporation can enhance the self-efficacy of patients with oral and maxillofacial deformity and reduce the incidence of patients with body dysmorphic disorder tendency.
3.Acute closed rupture of the Achilles tendon:a Meta-analysis of nonsurgical and surgical treatment

BACKGROUND:There is always a controversy about nonsurgical and surgical repair of acute Achiles tendon rupture. Except retrospective analysis, there is a lack of high-level evidence-based medicine data.
OBJECTIVE:To systemicaly evaluate the clinical effectiveness of the nonsurgical versus surgical treatment for acute closed rupture of the Achiles tendon.
METHODS:The randomized controled trials about the nonsurgical versus surgical treatment for acute Achiles tendon rupture were searched in PubMed, EMbase, CBM, CNKI, Ovid, and WanFang Data by computer as wel as in relevant journals by hand, which were reported before February 2014. The Cochrane Library (Issue 1, 2014) was also retrieved. Two reviewers independently screened studies, extracted data, and evaluated the methodological quality according to the inclusion and exclusion criteria. Then Meta-analysis was conducted using RevMan 5.2 Software.
RESULTS AND CONCLUSION:Nine randomized controled trials were included, and totaly 874 patients were involved, 441 cases in the nonsurgical group and 433 in the surgical group. The results of Meta-analysis showed that compared with the surgical group, the incidence of complications was lower [OR=0.41, 95%CI(0.26, 0.63), P< 0.000 1], but the rate of tendon re-rupture [OR=2.86, 95%CI(1.62, 5.02),P=0.000 2] and incidence of cicatricial adhesion [OR=0.07, 95%CI(0.03, 0.19),P < 0.000 1] were higher in the nonsurgical group. However, there were no significant differences between the two groups in patient satisfaction, superficial infection, motion recovery, and deep infection. Compared with the surgical treatment, the nonsurgical treatment can reduce the incidence of complications, but has a higher incidence of re-rupture. Because of sample-size and methodological quality restrictions, this conclusion needs further verification of large-sample, multicenter, and high-quality randomized controled trials.
4.EFFECT OF CHITOSAN ON DELAYED OUTWARD POTASSIUM CURRENT IN SINGLE VENTRICULAR MYOCYTES OF GUINEA PIG

To study the effect of chitosan on delayed outward potassium current(IK) in single ventricular myocytes of guinea pig and investigate its antiarrhythmic mechanism from ion channel view. Patch clamp technique with whole-cell configuration. Holding potential was -40mV,commanding potential was -60- + 70mV ,step pulse +10mV,stimulating frequency 1 Hz,duration 300 ms and stimulating interval 6s. The result showed that Chitosan inhibited IK in a dose -dependent manner. Conclusion :Chitosan can inhibite IK in single ventricular myocytes of guinea pig.
5.Application of phage-displayed peptide libraries

Phage display is a new technology developed in recent years. Random peptides displayed on the filamentous phage surface offer a rich source of molecular diversity in searching for ligands that bind an antibody, receptor and other proteins. The phage display system has been effective in the discovery of various ligands. Potential applications of the peptide library include investigation of the protein protein interactions and discovery of mimetic drug candidates.
6.Application of ultrasonic harmonic scalpel in open radical gastrectomy of proximal gastric cancer

ObjectiveTo explore the practicability and security of the ultracision harmonic scalpel in radical gastrectomy for patients with proximal gastric cancer.Methods115 patients of stage Ⅰ ~ Ⅲ proximal gastric cancer undergone radical gastrectomy were analyzed retrospectively.65 patients were performed with ultracision harmonic scalpel,the other 50 patients were performed with conventional operation.The operative time,volume of intraoperative hemorrhage,length of incision,and postoperative complications were analyzed.ResultsThere were significant difference in operative time ( 126.9 ± 20.7 ) min and ( 150.6 ± 31.7 ) min ( P < 0.05 ),volume of intraoperative hemorrhage (105.7 ±40.3)ml and (171.7 ±63.2)ml(P<0.01) and length of incision(17.2 ±2.1)cm and (20.3 ±2.3)cm (P < 0.05 ).There was no significant difference in the postoperative complications between ultracisiou harmonic scalpel group and conventional group.All patients recovered without severe complications.ConclusionRadical resection of proximal gastric cancer using ultracision harmonic scalpel could shorten operative time,decrease intraoperative hemorrhage,shorten incision length,and the technique is convenient and safe.
7.The effect of ultrasonic harmonic scalpel on stress after radical resection of esophageal carcinoma

ObjectiveTo explore the effect of ultrasonic harmonic scalpel on stress after radical resction of esophageal carcinoma.MethodsAll 115 patients of esophageal carcinoma undergone radical esophagectomy were analyzed retrospectively.60 patients were performed with ultrasonic harmonic scalpel,the other 55 patients were performed with electrotome.The drainage volume of 72h after operation,postoperative complications and hospital days were analyzed.White blood cell count,Neu,CRP and blood glucose level were tested 2 days after operation.ResultsThere was significant difference in the drainage volume of 72h after operation (P < 0.05 ),but the postoperative complications and hospital days had no obvious difference.White blood cell count,Neu,CRP and blood glucose were obviously increased after operation( all P < 0.05 ) ;the barmonic scalpel group was dramatically decreased as compared with the electrotome group except blood glucose ( all P < 0.05 ).ConclusionRadical resection of esophageal carcinoma using ultrasonic harmonic scalpel appears to decrease the drainage volume of 72h after operation and less the stress.
8.The efficacy of cognitive-behavioral intervention on anxiety and oral health-related quality of life in adult orthodontic patients

Objective To explore the influence of cognitive-behavioral intervention on the anxiety and oral health-related quality of life in adult orthodontic patients.Methods 152 adult orthodontic patients whose MDAS scores were greater than 11 were divided into the intervention group and the control group according Multistage Grouping Process.The former were treated with MBT appliance combined with cognitive-behavioral intervention,and the latter were treated with MBT appliance only.The State-Trait Anxiety Inventory(STAI) was adopted to evaluate anxiety in all patients during orthodontic treatment,and the UK oral health-related quality of life questionnaire(OHRQoL-UK) was adopted to evaluate the oral health-related quality of life in all patients before and after orthodontic treatment,respectively.Results 1.After 3 and 6 months cognitive-behavioral intervention,the scores of SAI in the intervention group(50.37±9.18,35.65±8.92) were significantly reduced compared to the control group(56.62± 10.52,54.13± 10.03) (P<0.01).After 6 months cognitive-behavioral intervention,the scores of TAI in the intervention group(47.92±6.82) were significantly reduced compared to the control group(51.52±7.41) (P <0.05).2.After the orthodontic treatment,the scores of OHRQoL-UK in the intervention group(58.27±13.43) were significantly improved compared to the control group (51.99± 11.69) (P< 0.01).3.The oral health-related quality of life after the orthodontic treatment were significantly related to post-treatment anxiety of adult orthodontic patients(r=-0.192 ~-0.459,P< 0.05).Conclusion Cognitive-behavioral intervention can effectively reduce anxiety and improve the oral health-related life quality in the adult orthodontic patients.
9.The elinical study of median-rib-section and sub axillary median-rib-section in thoractomg

Objective To compare the effect of two operative methods,median-rid-section and sub axillary median-rib-section.Methods 152 cases underwent by median-rib-section and among them 12 cases underwent by sub axillany median rib section.Results The median-rib-section procided a satisfact incision(the area of ineision was 14cm?30cm),without renobing the ribs,with the musculature.Complete,and less trauma.And the method of sub axillary median-rub-section had more intrathoracre esposure(the area of inclsion was 14cm?20cm),and the thoractomy could be done directly and satisfactory.Conclusion The median-rib section is a good method in thoraxoctomy which makes the ribs,made less trauma and provides satisfactory exposure,and after surgery,the chest is closed tighly.The sub axillary media rib section provides a wider application while remaining the good charactery of common sub axillary section.
10.Creating database with Microsoft Access and its clinical application for patients with breast cancer

Objective The purpose of this study was to develop a database program based on Microsoft Access 2003 for patients with breast cancer to save and manage clinical data.This makes it easier to analyze data of the database.Methods A total of 1177 cases with breast cancer who were performed surgical treatment from December 1998 to June 2007,the clinical data collection which included regular fouow-up data of these patients after therapy,the database was designed by using Microsoft Access 2003 included the creation of tables,forms and queries according to the therapy guidelines for breast cancer.Results The clinical data for patients with breast cancer can be stored in an Access database that is both user-friendly with reliable data administration and that can be shared in network,and it is capable of recording regular updated data.Conclusion This program will not only convenient for researchers to statistic and analyze their outcome data,but also for minimizing data entry errors and reducing the time spent on data query.
