1.Analysis of the Application of Special Drugs in Our Hospital from 2009 to 2013

OBJECTIVE:To provide reference for the scientific ratio,rational application and standardized management of nar-cotic drugs and class Ⅰ psychoactive drugs(special drugs)in our hospital. METHODS:The clinical application of special drugs in our hospital from 2009 to 2013 was analyzed based on its consumption sum,DDDs,sorting and sorting ratio. RESULTS:The pro-portions of the consumption sum of special drugs in the total sum of essential medicines in from 2009 to 2013 were respectively 66.16%,67.60%,27.81%,12.11% and 6.87%,and it significantly reduced from 2011. The essential medicines’kinds in special drugs with top 10 consumption sum from 2009 to 2013 were respectively 9,8,7,6 and 5;the DDDs were respectively 9,9,8,6 and 6,consumption sum and DDDs gradually reduced,and mostly after sorting by. The sorting ratio of Sufentanil citrate injection and Transdermal fentanyl was less than 1 for several years,the sorting ratio of Morphine hydrochloride sustained-release tablet and Peth-idine hydrochloride injection was more than 1 for several years,and the sorting ratio of Oxycodone hydrochloride sustained-release tablet in 2013 changed from less than 1 to more than 1. CONCLUSIONS:The consumption sum and dosage of non-essential medi-cines in special drugs in our hospital from 2009 to 2013 gradually increased,and the essential medicines gradually reduced. The main reason is the current included kinds in the essential drug list is less so that it can not fully meet the clinical needs. It is suggest-ed that the relevant departments appropriately increase the varieties to meet the demand in the revision of essential medicine list;and hospitals take necessary measures to control the application proportion of the non-essential medicines to improve the application rate of essential medicines.
2.Analysis of the Use of Chinese Patent Medicines in Guiyang During 2004~2006

OBJECTIVE:To investigate the use of Chinese patent medicines in Guiyang in order to provide references and information about the research and development,manufacture and use and marketing etc of Chinese patent medicines.METHODS:The use of Chinese patent medicine in Guiyang area during 2004~2006 was analyzed taking DDD and sequence of consumption sum of the top 20 Chinese patent medicines as the main indexes.RESULTS:The use of Chinese patent medicines was influenced directly by dosage forms,price and DDC etc.CONCLUSION:Those Chinese patent medicines with confirmative efficiency,moderate prices and reasonable dosage forms assume precedence over the others in clinical use.
3.Investigation about ability of clinical decision-making among nursing undergraduates

Objective To explore the present state of clinical decision-making ability of nursing students in Sichuan Province. Methods The instrument for measuring the clinical decision-making ability of nursing students was adopted te investigate baccalaureate nursing students from four colleges and uni-versifies in Sichuan Province who had completed 10-12-month clinical practice. Results The overall mean score of clinical decision-making ability of baccalaureate nursing students in Sichuan Province was (72.86± 4.53). Among the dimensions, the scoring rate of Goal-setting Ability Dimension was the highest and that of evaluation-feedback ability dimension the lowest. In terms of the qualification rate of clinical decision-making ability, 99.52% of the students were qualified (4.82% good and 94.7% passed) and 0.48% was unqualified. Conclusions Baccalaureate nursing students in Sichuan Province have not showed a high level in clinical nursing decision-making ability and most of them are just qualified. No students have reached the level of excellence.
4.Investigation in knowledge rate of 120 first aid and satisfaction rate with emergency services among Nanchong citizens

Objective To investigate in knowledge rate of 120 first aid and satisfaction rate with emergency services among Nanchong citizens. Methods 360 patients who received prehospital first aid from three hospitals and their accompanies were selected from January to June 2010. The investigation content included knowledge rate of 120 first aid and satisfaction rate with emergency services. Results Results showed that about half of those surveyed still did not know the emergency phone"120" and its function, lacked self-help and self-protection knowledge, some respondents felt quite helpless upon daily life of electric shock, gas poisoning, coma and traumatic accidents. The attitude of the majority of emergency personnel was good or better, but still very stiff or irresponsible attitude existed in those few. This undermined the "120" and the image of credibility and emergency personnel. Conclusions There is an urgent need for universal coverage of on-site first aid knowledge and enhance awareness of first aid. Full-time staff in emergency department must pay close attention to the knowledge of first aid training, mastering the technique of bleeding control and bandaging, fixing, handling and CPR, as well as the use of instruments and equipment equipped in vehicles. Only in this way can we continuously improve the treatment of critically ill patients and increase survival rate.
5.Interventional effects of triptolide on the levels of cytokines in peripheral serum and articular cavity of rats with collagen induced arthritis

BACKGROUND: Common threewingnut root has the functions of anti-inflammation and immune inhibition, etc., and it has been used at present to treat various autoimmune diseases including rheumatoid arthritis.. Common threewingnut root has complex components, and triptolide is acknowledged as one of the important effective components of common threewingnut root.OBJECTIVE: To establish rat models of type Ⅱ collagen induced arthritis, and observe the effects of triptolide on the contents of interleukin-6,interleukin-10 and tumor necrosis factor alpha (TNF-α) in peripheral serum and synovial fluid.DESIGN: A randomized control animal experiment.SETTING: Department of Integrated Traditional and Western Medicine,Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology.MATERIALS: The experiments were carried out in the Tongji Hospital from November 2004 to July 2005. Fifty healthy male Wistar rats of clean degree were purchased from the experimental animal center of Tongji Medical College, Huazhong University of Science and Technology [qualification number of animal [scxk(E)2004-2007]. Triptolide (nobatch number because of temporary production) was bought from Fujian Institute of Medical Sciences, and the purity was above 98.5%.METHODS: ① Ten of the 50 rats were randomly selected as the normal controls, and the others were made into models. Type Ⅱ collagen emulsion was injected intradermally at five points along the back and tail of the rats,0.05 mL for each point, and injected intradermally at two points after 15 days. The rats in the normal control group were treated with saline in the same way. The effects of the model establishment were evaluated according to the scoring standards of arthritis index at 30 days after the first immunity, and the rats scored 6 points or above were taken as successful models and enrolled in the experiments. Twenty successful rat models were randomly divided into arthritis model group (n=10) and triptolide treated group (n=10). ② Triptolide (100 mg/L)was dispensed into parenteral solution with propylene glycol (0.05 in volume fraction), and then intramuscularly injected into hindlimb of rats in the triptolide treated group (0.04 mL/100 g), once every three days for 30 days. The rats in the normal control group were given isovolume saline, and those in the arthritis model group were treated with isovolume propylene glycol (0.05 in volume fraction). ③ The materials were removed at 30 days after administration. The contents of interleukin-6, interleukin-10 and TNF-α in peripheral serum and synovial fluid were detected with enzyme-linked immunosorbant assay(ELISA).MAIN OUTCOME MEASURES: The effects of triptolide on contents of TNF-α, interleukin-6 and interleukin-10 in peripheral serum and synovial fluid were observed.RESULTS: Fifty male Wistar rats of clean degree were selected, 10 were used as normal controls, and 20 of the other 40 rats were successfully made isto models and enrolled in the analysis of results. ① The TNF-α contents in peripheral serum and synovial fluid were the highest in the arthritis model group, and obviously decreased after treatment of triptolide [(35.09±8.82), (15.35±3.56) ng/L; (44.17±8.94), (22.54±4.76) ng/L; P< 0.01], which were similar to those in the normal control group (P > 0.05).② The contents of interleukin-6 in peripheral serum and synovial fluid were the highest in the arthritis model group, and were obviously decreased after treatment of triptolide [(76.58 ±6.81), (42.45 ±5.72) rig/L;(88.69±10.56), (48.67±5.97) ng/L; P < 0.01], but did not recover to the levels in the normal control group (P < 0.05). ③ The contents of interleukin-10 in peripheral serum and synovial fluid were the lowest in the arthritis model group, and obviously increased after treatment of triptolide[(17.53±2.07), (21.23±2.91) ng/L; (10.59±2.96), (14.74±1.85) ng/L; P< 0.01], which were similar to those in the normal control group (P > 0.05).CONCLUSION: Triptolide can treat arthritis by modulating the contents of cytokines.
6.The retrospective study for the value of serum procalcitonin to the early diagnosis of sepsis

Objective To study the value of serum procalcitonin(PCT) to the early diagnosis of sepsis .Methods From June 2014 to June 2015 ,a total of 686 cases were enrolled in this retrospective study .PCT tests were assayed within 2 days of bacterial culture .Results In this study ,56 cases ,67cases ,and 567cases were classified into the positive blood culture group ,positive body fluid culture group ,and negative all culture group ,respectively .Median PCT values were 4 .26 2 .78 ,0 .46 ng/mL ,respectively .Me-dian PCT values in the gram-positive bacterial culture group and gram-negative bacterial culture group ,respectively ,were 2 .35 and 4 .56 ng/mL .Median PCT values in the positive hydrothorax culture group ,positive ascites culture group ,and positive bile culture group ,respectively ,were 1 .91 ,5 .23 ,3 .64 ng/mL .In all ,Median PCT values of 47 cases of sepsis and 16 cases of severe sepsis were 5 .32 and 10 .25 ng/mL ,respectively .Conclusion PCT level is correlated with the severity of sepsis ,pathogenic bacteria type ,and the site of infection ,and can be used in the early diagnosis of sepsis .
7.Discussions on the progress and problems of public hospital payment system reforms in China

Based on a description of the evolution,main reform practices and effects of public hospital payment system reforms,this paper focused on analyzing the existing problems found in the reforms.Such problems namely include lack of scientific and reasonable design of payment standards,lack of systemic and coordinated system reforms,and failure to fully mobilize the enthusiasm of medical workers to participate in the reforms,as well as other underlying causes.Based on such suggestions were put forward to promote public hospital payment system reforms,as negotiation of the payment standard between purchasers and providers,combined approaches in innovation in payment methods,advancing systemic payment system reform,and establishing a transmission mechanism of incentive to the medical personnel.
8.Studies on the reform of payment system of grass-roots health care institutions in China

The paper presented the payment system theory of the primary health care service in China,the current status of the health service system,and analyzed main challenges for the time being.Based on such studies,the authors made the following policy proposals.The first is adjustment of the fiscal payment method of the government to such institutions in line with the classification guidance principle; the second is to shift the payment method of primary public health services to the post payment of fee-for-service; the third is reform of the performance-based salary system to link payment to medical workers directly with the amount and quality of their services; the fourth is a set of result-oriented performance appraisal indicators,with rising proportion of performance pay; the fifth is to integrate the outpatient clinic fund covered by medical insurance and the primary public health service fund,into a capitation payment.
9.Job satisfaction degrees and influencing factors among community nurses

Objective To explore job satisfaction degrees and the influencing factors among community nurses. Method General information questionnaire and the revised job satisfaction questionnaire were adopted in the investigation among 124 nurses from 28 community health service centers in Nanchong City between February to October 2016. Results The total score was (119.62 ± 15.37), at the moderate to high level. Of the eight dimensions, relationship with colleagues was scored the highest, and personal growth was the lowest. The influencing factors of the nurse's job satisfaction were found to be employment type, work positions, service duration in the community and whether they have been trained on community nursing position (all P<0.05). Conclusions The job satisfaction degree of community nurses is generally at middle level. Therefore, measures should be taken to improve the job satisfaction for the development of community nursing services.
10.Dynamic MRI for assessment of the anatomical and functional significance of the bladder and urethra in female stress urinary incontinence

Astract:Objective To assess the anatomical and functional significance of the bladder and urethra in female SUI by dynamic MRI and urodynamic. Methods The clinical data were retrospectively analysed on 60 cases of female stress urinary incontinence from Feb 2011 to Nov 2013. All the women received pelvic floor dynamic MRI and Urodynamic study. Measured the urethral angle,the distance of the H line, M line and the descent of the bladder,cervical,anorectal junction from the PCL in the mid-sagittal dynamic MR image of the pelvic. Pearson 's correlation coefficient was used to examine the association among the distance of the H line,M line and the urethral angle,the descent of the bladder,cervical,anorectal junction from the PCL and the Urodynamic data. Results The distance of the H line,M line were significantly and positively associated with urethral angle and the descent of the bladder,cervical,anorectal junction from the PCL. The distance of the H line,M line were significantly and negatively associated with functional profile length, maximum urethral closure pressure,VLPP and detrusor opening pressure. Conclusion Pelvic floor dynamic MRI combined with Urodynamic study is useful for assessing the anatomical and func-tional significance of the bladder and urethra in female SUI.
