2.Sensitivity and efficacy of intraoperative electromyography monitoring in combination with 3D imaging modality during complex lumbosacral procedures

MAIN OUTCOME MEASURES: Free running EMG and stimulus triggered EMG, including time, frequency, amplitude, muscle group were observed and recorded simultaneously. Never root functional injury and restoration after surgery were detected.RESULTS: 378 pedicle screws in 74 patients were monitored intraoperatively, and only 3 pedicle screw malposition (2 of L_4, 1 of L_5) was detected and then replaced with the help of C-arm fluoroscopic examination. Myoelectricity appeared when the current intensity was less than 10 mA. The correct rate of implantation was 99.2%. Nerve root impingement was found in two cases during laminectomy for L_5 and S_1 decompression and never root solution, which alerted the surgical team of critical neural structures. Nerve symptoms of the lower limb were aggravated after surgery and restored following 2-4 weeks of conventional treatment. The error injury rate of nerve root was 2.7%. In all reported cases, no irreversible neurological deficit was observed 2-4 weeks after operation.CONCLUSION: Intraoperative EMG monitoring can find improperly placed screws and detect impending nerve root injury promptly. Combined EMG and 3-D imaging modality monitoring is a reliable and practicable method that can be used to protect neural structures during complex lumbosacral surgery.
3.Long-term follow-up of Dynesys system in clinical application for the treatment of multiple lumbar degenerative disease.

OBJECTIVETo explore the clinical effects of Dynesys system for the treatment of multiple segment lumbar degenerative disease.
METHODSA total of 28 patients with lumbar degenerative disc disease treated with Dynesys system from December 2008 to May 2011 were retrospectively reviewed. There were 16 males and 12 females, aged from 27 to 75 years old with an average of 49.1 years. Thirteen patients with multiple segmental lumbar intervertebral disc protrusion, including L3-L5 in 7 cases, L2-L4 in 1 case and L4-S1 in 5 cases. Fifteen patients with multiple segmental lumbar spinal stenosis, including L3-L5 in 10 cases, L4-L5 in 4 cases and L2-S1 in 1 case. The symptoms of lumbago and (or) intermittent claudication in all patients were treated with conservative treatments for more than 6 months and these methods did not work. Visual analogue scale (VAS) was used to analyze the lumbar and leg pain, imaging data were used to measure the intervertebral space height and intervertebral motion of fixed segment and upper adjacent segment, Oswestry Disability Index (ODI) was used to evaluate the clinical effect.
RESULTSAll operations were successful and the patients were followed up from 38 to 65 months with an average 50.6 months. At final follow-up, ODI and VAS of the low back pain and leg pain were (25.10±6.52)%, (1.25±0.70) points and (1.29±0.89) points, respectively and were decreased compared with preoperative (P<0.05). Postoperative intervertebral space heights were increased and intervertebral motions were decreased in fixed segment compared with preoperative (P<0.05). There were no significant differences in intervertebral space heights and intervertebral motions of upper adjacent segment between preoperative and postoperative (P>0.05).
CONCLUSIONDynesys system may obtain long-term clinical curative effect in treating multiple lumbar degenerative disease. It can partially preserve the intervertebral motions of the fixed segments, have little effect on adjacent segments. The long-term clinical effect of Dynesys still need longer time follow-up observation.

4.Study on mutation effects of antiepileptic drugs in epileptic children

Objective To study the mutagenic action of antiepileptic drugs(AEDs), and find an effective way to prevent the mutagenesis induced by AEDs,by observing the effects of AEDs on serum folic acid(FA) level and sister chromatid exchange (SCE) frequency in epileptic children.Methods Ninty epileptic children were divided into different groups on the basis of the different drugs they had taken, then detected the two indexes at different time points.Results The serum FA level and SCE frequency of the patients significantly decreased and increased after they took carbamazepine (CBZ) and valproic acid (VPA)respectively. The two indexes went back respectively when supplied with FA.Conclusions Both CBZ and VPA possess mutagenic action, yet nitrazepam does not.FA may help repair the chromosome damage and reduce the mutagenesis effects.
5.Isolation and Identification of Thermoacidiphilic Bacteria from Apple Juice Concentrate

The thermotolerant bacteria which was isolated from the Apple Juice Concentrate(AJC) was investigated by comparison with the standard strain of Aliyclobacillus acidoterrestris DSM3922 using Kirin-medium acidified with malic acid.The results shows the two polluted bacteria isolated from AJC can grow under the temperature of 21?C~55?C and pH of 2.4~6.2,which was corresponded with the characteristics of the thermo-acidiphilic Alicyclobacillus spp..Further more,the cell morphology,colony morphology,cultural characteristics and physiological characteristics tests indicated the two isolated strains of thermotolerant bacteria have obviously similar characteristics with the standard strain of Aliyclobacillus acidoterrestris DSM3922.
6.Comparison of abdominal CT and pathological findings in chronic schistosomiasis

Objective To retrospectivel y analyze the abdominal CT findings and pathological results of the chronic schist osomiasis so as to improve the diagnostic accuracy of the disease. M ethods The plain abdominal CT scanning was performed in 103 cases an d enhanced CT scanning in 81 cases. The pathological specimen which was consist ent with the section of CT scan was obtained in each cases. Results On CT scanning, liver cirrhosis was seen in 84 cases, various calci fication in liver in 71 cases, liver cancer in 12 cases, enlargement of sple en in 78 cases, calcification in spleen in 13 cases, wall-thickening in colon i n 27 cases, calcification in colon in 31 cases, and colon cancer in 9 cases. Pa thological examination revealed various fibrosis and formation of pseudolobule. The eggs and calcification could be seen in pseudolobule and septa, colonic sub mucosa, and regional lymph nodes. Fibrous hyperplasia in colonic wall and hyper plasia in mucous membrane were obvious. Fibrous hyperplasia and calcification w ere seen in spleen, but the eggs were not found. Conclusion The liver and colon are the major organs affected by chronic schistosomias is in abdomen, and the CT findings are obvious too. The pathological features o f spleen are accompanied with liver cirrhosis. CT is the important imaging meth od in diagnosing chronic schistosomiasis and pathological changes.
7.Case-control study on patellar fixed pin and Kirschner's nail with tension band for the treatment of patellar fracture.

OBJECTIVETo compare the clinical efficiency of patellar fixed pin and Kirschner's nail with tension band in treating patellar fracture.
METHODSFrom November 2010 to January 2012, the clinical data of 42 patients with patella fracture were treated with patella fixed needle and Kirschner's nail with tension band. The patellar fixed needle group included 21 cases (14 males and 7 females,aged 26 to 65 with an average of 43.6 years old); The Kirschner's nail with tension band group included 21 cases (12 males and 9 females,aged 32 to 58 with an average of 41.5 years old). The operation time, intra-operative blood loss and healing time were compared between two groups. Böstman score was applied to compare the therapeutic effects.
RESULTSAll patients were followed up with an average of 11.5 months ranged from 6 to 14 months. The fractures healed well without complications such as nonunion and infection. According to Böstman score, there was no significant difference between two groups in clinical efficiency (Z = -0.83, P = 0.407). There was significant difference in operation time (t = 6.67, P = 0.00), while no difference in intra-operative blood loss, bone healing time (t = 1.90, P = 0.064; t = 0.612, P = 0.544).
CONCLUSIONBoth methods can obtain good results. However, patellar fixed pin combined with tension band is one of the ideal methods for the treatment of patellar fracture, it has an advantage of simple with shorter operation time, rigid fixation, less complications and well recovery.

8.Establishment and evaluation of a mouse model of bronchial asthma with Yin deficiency syndrome.

OBJECTIVETo establish and evaluate a mouse model of bronchial asthma with Yin deficiency syndrome.
METHODSThe mouse model of bronchial asthma with Yin deficiency syndrome was established by the treatment with injecting ovalbumin (OVA) two times to sensitize, inhaling OVA 14 times to stimulate, and using thyroxin through lavage during late stimulation. This model was evaluated through body weight, asthmatic behaviors, respiratory function, autonomous activity, lung pathology, and pulmonary fluid clearance.
RESULTSOVA combined with thyroxin was an appropriate method to induce the mouse model with increased food and water intake, autonomous activity, asthmatic behaviors score, and respiratory rate, decreased body weight, tidal volume, and wet/dry ratio of lung, and changed with pathology of lung tissue. The changes of the above mentioned parameters indicated that the model was the bronchial asthma with Yin deficiency syndrome.
CONCLUSIONThe OVA combined with thyroxin is a good pattern to establish a mouse model of bronchial asthma with Yin deficiency syndrome successfully, which can highly simulate the clinical symptoms of this disease.

9.Effect of high frequency electrocoagulation by ultrafine-needle on the cutaneous capillary hemangiomas: a preliminary study on cock combs.

OBJECTIVETo investigate the injury effect of high frequency electrocoagulation (HFE) by ultrafine-needle on the cutaneous capillary hemangiomas by histopathologic study of cock comb.
METHODS32 male cocks were randomly divided into 4 groups with 8 animals in each group. 3 experimental groups received HFE with different power (6W, 8W, 10W) on the cock comb. Specimens were taken from cock combs for histologic study immediately after treatment and 7, 14, 21, 28 days later.
RESULTSAfter treatment, the cock combs turned pale and followed with scar formation. The shape of combs changed a lot after high power treatment. Immediately after treatment, the capillary number in the experimental groups decreased markedly under light microscope, compared with that in control group (P < 0.05). After that, the capillary number in experimental group increased. 7 days after treatment, the capillary number in experimental groups was still significantly lower than that in control groups (P < 0.01), but not markedly different between the three experiment groups (P > 0.05). Collagen formation also increased after treatment (P < 0.05), which was higher in experimental groups with high power (P < 0.05, correlation coefficient r > 0.5).
CONCLUSIONSHFE with ultrafine needle can effectively decrease the capillary number in cock comb. The 6W treatment is mini-invasive and will not destroy the comb shape with good cosmetic results. The technique may be used for the treatment of cutaneous capillary hemangioma.

10.Mechanism of improving effect of losartan on insulin sensitivity of non-insulin-dependent diabetes mellitus rats.

The specific inhibition of angiotensin II action at AT(1) receptors by losartan has been shown to decrease peripheral insulin resistance in type 2 diabetic patients and animal models. We examined the effect of losartan on the expression of insulin receptor substrate 1 (IRS-1), protein kinase B (PKB) and glucose transporter 4 (GLUT4), as well as the phosphorylation status of IRS-1 and the association between IRS-1 and phosphatidylinositol (PI) 3-kinase in skeletal muscle from fat-fed and-streptozotocin (STZ)-treated rats, an animal model of type 2 diabetes mellitus. In addition, the effects of losartan on GLUT4 translocation in muscle cells and on insulin sensitivity were also evaluated. Muscle tissues were isolated from male losartan-treated and untreated normal or non-insulin-dependent diabetes mellitus (NIDDM) rats with a dose of 4 mg/kg per day for 6 weeks. Oral administration of losartan improved insulin sensitivity, which was determined by an oral glucose tolerance test (OGTT). In skeletal muscles, the protein levels of IRS-1, PKB and GLUT4 in NIDDM rats were not significantly different from those of the control rats, and they were not affected by losartan. The levels of IRS-1 tyrosine phosphorylation, PI 3-kinase activity associated with IRS-1 and PKB activation after stimulation with insulin in muscle tissue of NIDDM rats were significantly decreased (P<0.01) compared with those in the control rats, while they were not increased by losartan. Losartan had a major effect on GLUT4 translocation in myocytes, as it significantly increased (P<0.05) the insulin-induced amounts of GLUT4 in plasma membrane (PM) and T-tubules (TT) in myocytes from NIDDM rats. Consistent with these results, the plasma glucose level in losartan-treated NIDDM rats was decreased (P<0.05) compared with that in untreated NIDDM rats. Our results suggest that losartan may exert beneficial effects on insulin resistance by increasing the translocation of GLUT4 in muscle tissue, which is probably associated with a non-PI 3-kinase-dependent mechanism.
