1.Clinical characteristics of autoimmune gastritis

Objective To analyze the clinical characteristic of autoimmune gastritis (AIG).Methods From January 1990 to April 2010,the clinical data of 55 AIG patients were retrospectively analyzed,which included hemoglobin,lactate dehydrogenase (LDH),α-hydroxybutyrate dehydrogenase (α-HBDH),gastrin,intrinsic factor antibody (IFA),parietal cell antibody (PCA),gastrointestinal endoscopy examination and 24-hour esophageal pH recording.Another 31 megaloblastic anemia (MA) patients were selected as control.Statistical analysis was performed by independent-samples t test.Results Among 55 AIG patients,49 patients were associated with MA,and three out of four cases were identified of IFA.About 43.8% (21/48) patients were PCA positive.Before treatment,the levels of LDH and α-HBDH of AIG patients with MA were (1045.50±853.46)U/L and (853.71±824.23) U/L which significantly increased,than those of patients without MA [(166.67±41.03) U/L,(133.67±27.90) U/L],the differences were statistically significant (t=-4.665,-2.120,both P<0.05),however there was no significant difference when compared with the control group [(1047.52±1028.31) U/L,(1050.23±1264.37) U/L,both P>0.05)].A total of 46 patients underwent gastroendoscopy examination,63.0% (29/46) patients had gastric body atrophy while gastric antrum not involved; 34.8% (16/46) patients had neither gastric body nor antrum atrophy; seven patients gastric mucosal showed intestinal metaplasia and one patient showed intestinal metaplasia with atypical hyperplasia and 2.2% (1/46) presented both the antrum and the body atrophy.Conclusions The levels of LDH and α-HBDH increased in AIG patients might be related with MA caused marrow in-situ hemolysis.IFA is recommended as a routine test for AIG.There is still some limitations of AIG diagnosis according to histopathological features of gastric endoscopy specimen.The clinical features should be taken into consideration.

The limited-Hodgkin lymphoma (HL) treatment most preferred combined modality treatment,there are some scholars believe that nonbulky stage Ⅰ,Ⅱ of good prognosis HL patients,ABVD chemotherapy alone is acceptable,radiation therapy alone for stage Ⅰ or limited stage Ⅱ nodular lymphocytepredominant HL patients.Individual treatment by easing speed adjustment treatment in the future may make patients more benefit.
3.Construction of immortalized rat astrocyte strain by transfection of simian virus 40 large T antigen gene

Objective To immortalize rat astrocytes which could be used as cell carriers for transgenic cellular analgesia. Methods Astrocytes were isolated from cerebral cortex of newborn SD rats by trypsin digestion and cultured according to the method of differential cell adhesiveness and transfected with plasmid pCMVSV40T/ PUR containing the simian virus 40 large tumor antigen (SV40Tag) gene. The positive colonies were isolated by puromycin selection and expanded by many passages. The integration and expression of large T antigen gene were detected and the immuno-reactivity of glial fibrillary acidic protein (GFAP) was determined by PCR, RT-PCR and immuno-cytochemistry. Results Rat astrocytes were successfully cultured in vitro and positively stained for the astrocytic marker GFAP. The positive colonies were isolated and subcultured for 50 passages. PCR and RT-PCR products were analyzed using 1.5% agarose gel electrophoresis. The size of amplifacation product of target gene was identical to that of the positive control (558 bp) . There was no PCR and RT-PCR product from non-transfected cells. DNA sequencing and BLAST showed that 558 bp nucleotides were identical to the SV40Tag gene of the Genbank (100%). The transfected cells were positively stained for the SV40Tag and GFAP. Conclusion Immortalized rat astrocyte strain with SV40 tag gene is constructed successfully.
4.Biological character of immortalized rat astrocyte strain by transferction of simian large T antigen gents

Objective To study the biological character of rat astrocyte strain immortalized by simian virus 40 large T antigen gene(SV40Tag) and explore the feasibility of using it as cell vehicle for transgenic cellular analgesia.Methods Rat cerebral cortical astrocytes (AST) and immortalized rat astrocyte strain (IAST) were cultured in vitro. Morphology and growth features of AST and IAST were examined and compared after subculture, freezing and recovery. The ultrastructure of the cells was observed by transmission electron microscopy. The glial fibrillary acidic protein (GFAP) in these cells was detected by immuno-cytochemistry. The cell proliferation rate and cell cycle were determined by bromodeoxyuridine labelling and flow cytometry. AST and IAST were cultured in soft agar and inoculated in nude mice to investigate the tumorigenesis of IAST. Results LAST could be subcultured successively. The cells remained monolayer, anchorage dependent and the growth was attachment-inhibited. When AST was subcultured for only 10 passages, replicative senescense began. Subculture, freezing and recovery did not influence the shape and proliferation of IAST (94%?5%) , but decreased the vitality rate of AST (54%?4% ) (P
5.Application of double joystick technique in reduction-internal fixation for femoral shaft fracture in adults.

OBJECTIVETo study the applied value of double joystick technique in reduction-internal fixation for femoral shaft fracture in adults.
METHODSThirty-four patients (24 males and 10 females) with femoral shaft fractures were treated with reduction assisted by double joystick technique and internal fixation with interlocking intramedullary nail from September 2010 to June 2013. The average age of the patients was 41 years old, ranged from 17 to 65 years old. The duration of the disease course ranged from 3 to 7 days, with a mean of 5 days. The fractures belonged to AO types 32A (5 cases), 32B (20 cases) and 32C (9 cases) and located in left femur for 18 patients and right femur for 16 patients. The patients were followed up, and fracture healing and complications were observed. The curative effect were evaluated according to Thorsen femur fracture evaluation standard.
RESULTSThe operative time ranged from 40 to 110 min (mean 75 min) and intraoperative blood loss ranged from 200 to 300 ml (mean 250 ml). All the patients obtained a good fracture reduction and were followed up for 12 to 24 months (mean 18 months) after the surgery. All the fractures united between 4 and 8 months with a mean of 5 months. No complications such as breakage of nail, infection, osteofascial compartment syndrome, refracture and fracture malunion were found. According to Thorsen femur fracture evaluation standard, 30 patients obtained an excellent result, 3 good and 1 fair.
CONCLUSIONIn surgery of reduction-internal fixation for femoral shaft fracture in adults, the use of double joystick technique obtains good reduction result, short operative time, less injury, high healing rate of bone fractures, less complications and good limbs function, so it is worthy of popularizing in clinic.

6.Influence of acute hypoxia on current of voltage-gated potassium channel in pulmonary artery smooth muscle cells of rats

Objective: To study influence of acute hypoxia on the current of voltage-gated potassium channel (IK) in pulmonary artery smooth muscle cells (PASMC) of rats. Methods: A total of 20 male SD rats were randomly and equally divided into normoxic control group and acute hypoxia group. The rats in acute hypoxia group were kept in hypoxic chamber for 8 h before experiment. Whole cell patch-clamp technique was used to record IK in PASMC. Results: Acute hypoxia significantly decreased the IK density in PASMC of rats. During -60mV to -10mV of resting membrane potential(RMP), acute hypoxia did not significantly decrease peak IK density in PASMC of rats, P>0.05; At 0 mV, acute hypoxia significantly decreased the peak IK density in PASMC [from(38.1 ± 5.2) pA / pF decreased to(9.82 ± 2.1) pA / pF ,P<0.05], then along with RMP increase in PASMC, the decreasing amplitude of peak IK density in PASMC gradually increased(P<0.05); From + 30 mV to+ 60 mV, the decreasing amplitude of peak IK density in PASMC further significantly increased(P<0.01); At + 60 mV the peak IK density decreased from(38.1 ± 5.2) pA / pF to(9.82 ± 2.1) pA / pF , and the decreasing amplitude reached (46.8±3.3)%. Conclusion: Acute hypoxia can decrease Kv current in PASMC of rats, leading to hypoxic pulmonary vasoconstriction.
7.Transdifferentiation of epidermal stem cells into corneal epithelial cells in vitro

Objective To explore the plasticity of transdifferentiation of epidermal stem cells (ESCs) into corneal epithelial cells. Methods Epidermal stem cells enriched by adhering to type Ⅳ collage were induced in vitro by coculture with corneal stromal fibroblasts. The method of HCC were used to show the expression of K12 which is expressed specially in corneal epithelial cells.Results ESCs with greater proliferation potential strongly expressed K19 and ?_ 1 - integrin. After two-week induction, the K12 positive cells could be detected. Conclusion The data suggested that the epidermal stem cells have the potential to transdifferentiate into corneal epithelial cells in vitro.
8.Expressions of Notch1 and Cyclin D1 in mammary ductal hyperplasia and ductal carcinoma

Objective To investigate the relationship between mammary ductal hyperplasia and ductal carcinoma by detecting the expressions of Notch1 and Cyclin D1 in various histotypic mammary ductal diseases.Methods A total cases of 44 normal ductal tissues,44 usual ductal hyperplasias.40 atypical ductal hyperplasias,42 ductal carcinomas in situ and 51 invasive ductal carcinomas were included in this study.Expressions of Notch1 and Cyclin D1 were detected by S-P immunohistochemistry.Results There was significant difference of expression of Notch1 among the five subgroups of this study(P<0.01);there were significant differences between every two subgroups(P<0.05)except for normal ductal tissues versus usual ductal hyperplasias and ductal carcinomas in situ versus invasive ductal carcinomas(P>0.05).There was also significant difference of expression of CychnD1 amongthe five subgroups of this study(P<0.01),and there were significant difference between every two subgroups(P<0.05)except for normal ductal tissues versus usual ductal hyperplasias and normal ductal tissues versus atypical ductal hyperplasia(P>0.05).The correlation between Notch1 and Cyclin D1 was negative (r=-0.428,P<0.01).Conclusions There is a relationship between mammary ductal hyperplasia and ductal carcinoma.Decrease of Nowhl or increase of Cyclin D1 expressions may promote nmnmmry ductal hyperplasia advancing to manunary ductal carcinoma.
9.Causes and managements of dislocation after total hip arthroplasty

[Objective] To approach the causes and managements of dislocation after total hip arthroplasty.[Methods]850 total hip arthroplasties were done in our department from 1996 to 2004,in which 7 cases experienced post-operative dislocation.Whith 4 males and 3 females,the average age was 67 years old.Causes and managements have been studied retrospectively through analysis of history,surgical approach,timing and direction of the dislocations and predisposing factors of the dislocations occurred.[Results]There wer 7 cases of dislocations of which 5 patients(71%)were primary cases and 2 patients(29%)were revision cases;4 patients(57%)had history of hip surgery;1 case had mental disorder after cerebral surgery.Direct lateral approach was used for all these patients,and only anterior dislocation was occurred in this group.Through study the AP X-rays postoperatively,the abduction angle were more than 55 degree in 2 of these cases.The time of dislocation was from right after operation to 27 months after operation;5 of them experienced their dislocation within 3 months after operation.Closed reduction and 6 weeks traction were selected for all these patients,and 6 of them were successful.Recurrent dislccation occurred twice in another one patient within 3 months after reduction,and the acetabular cup was loosened during closed reduction.A stable joint was achieved after revision and modifying the abduction angle of the cup.[Conclusion]Causative risk factors for dislocation after total hip arthroplasty include surgery history on the same hip,inability to comply with instructions of rehabilitation,approach of surgery and malposition of the prosthesis.Closed reduction and traction for 6-week afterwards are usually successful for most of the dislocated cases.Revision could be done in the base of analyzing the reasons for recurrent dislocation,and then stale joint could be achieved.
10.Laryngeal mask insertion conditions at different target plasma concentrations of propofol combined with fentanyl in patients scheduled for minor surgery

Objective To compare the laryngeal mask insertion conditions at different plasma target concentrations (Cp) during the induction of anesthesia with target-controlled infusion (TCI) of propofol.Methods Forty-five ASA I - II patients of both sexes aged 18-60 yr, weighing 50-80 kg undergoing minor surgery in which the use of laryngeal mask (LM) was indicated were randomly divided into 3 groups ( n = 15) according to Cp of propofol set during induction of anesthesia: 4, 6 and 8 ?g ? ml-1 . The patients were premedicated with intramuscular scopolamine 0.3 mg. Fentanyl 3 ?g?kg-1 was given i.v. . TCI of propofol was started 3 min later with Diprifusor (Graseby 3500 infusion pump). LM was inserted when the effect-site concentration (EC) of propofol reached 2.5 ?g ? ml -1 as displayed on the infusion pump. LM insertion conditions (mouth opening, gagling, coughing, head and limb movement, overall ease of insertion) were assessed. The total dose of propofol, insertion time, the time needed to reach EC 2.5 ?g?ml-1 were recorded. SP, DP, HR and BIS value were recorded at 6 time points: baseline before induction (T1 ) , at the loss of consciousness (T2), at EC 2.5 ?g ? ml-1 (T3), immediately (T4), 3 min (T5) after insertion of LM. Results The SP, DP, HR and BIS value were decreasing with increasing depth of anesthesia in the 3 groups. The decrease in BP and BIS value after insertion of LM was significantly larger in group 3 (8 ?g ?ml-1) than in group 1 and 2 (P
