1.Effects of platelet-derived growth factor-BB on the expression of matrix metalioproteinase-2 in trabecular meshwork cells in vitro

Background At present,a new drug,platelet-derived growth factor-BB (PDGF-BB),as a drug knife for the treatment of glaucoma is under study to expect it directly working on the trabecular meshwork without disrupting the normal physiological structure of anterior chamber angle,clearing the trabecular meshwork aqueous outflow channel so as to achieve the purpose of lowering intraocular pressure.Objective This study aimed to investigate the effect of PDGF-BB on matrix metalloproteinase-2 (MMP-2) expression in cultured bovine trabecular meshwork cells.Methods Trabcular tissue was obtained from fresh bovine eyes,and trabcular meshwork cells were cultured and passaged using explant method.Cultured cells were identified by morphological evaluation and neuronspecific enolase (NSE) staining.The third generation of cells were inoculated to 6-well plate,and different concentrations (0,5.0,12.5,25.0 μg/L) of PDGF-BB was added into the medium for 2 hours.Expression levels (A value) of MMP-2 mRNA (MMP-2 mRNA/β-actin) and protein in the cells were assayed by reverse transcription polymerase chain reaction (RT-PCR) and immunochemistry,respectively.Results Trabcular meshwork cells appeared 5-9 days after cultured.The third generation of cells presented with many process and showed the green influence in cytoplasm.MMP-2 mRNA/β-actin value (A) was 0.127 ± 0.026,0.147 ± 0.045,0.178 ± 0.053 and 0.222±0.062 in the O,5.0,12.5,25.0 μg/L PDGF-BB group,respectively,showing a significant difference among them (F =56.71,P<0.05),and the MMP-2 mRNA/β3-actin value in the 5.0,12.5,25.0 μg/L PDGF-BB group was elevated in comparison with that of the 0 μg/L PDGF-BB group (all P<0.05).The expression value (A value) of PDGF-BB protein in the cells was 446.12±13.81,1444.65±54.64,2086.18±73.18,3488.65±25.98 in the 0,5.0,12.5,25.0 μg/L PDGF-BB group,respectively,with a significant difference among the four groups (F=213.12,P<0.01),and the expression value (A value) of PDGF-BB protein was gradually increased with the ascend of concentration of PDGF-BB(all P<0.05).Conclusions PDGF-BB can promote the expression of MMP-2 in bovine trabcular meshwork cells in vitro in concentration-dependent manner.
3.Protein Chip Technique in Detection of Helicobacter pylori and Its Application Value

OBJECTIVE To study the application value of protein chip technique in detection of Helicobacter pylori(Hp) infection and clinical diagnosis of associated disease. METHODS The antibodies against CagA,Ure and VacA in 300 serum samples were detected by protein chip technique.The biopsy specimens obtained by gastroscopy from 170 patients with digestive system symptom were detected by modified Giemsa staining at the same time.These patients were also accepted with rapid urease test(RUT) and ~(14)C-urease breath test (~(14)C-UBT).Twenty serum samples from Hp infected patients were detected again after treatment. RESULTS The sensitivity of protein chip technique in detection of Hp infection was 92.5%,the specificity was 41.3%,the total consistent rate was 73.5%,the positive predictive value was 72.8%,and the negative one was 76.5%.The disease was serious in CagA~+ Hp.The antibody′s concentration was lower after treatment in patients infected Hp. CONCLUSIONS The method of protein chip for Hp infection detection could be used in judging the type of Hp.It is suitable for guiding of medicine and the monitoring of treatment.
4.Complications and accidents of arthroscopy

Objective To analyse the causes of complications and accidents of arthroscopy and a discussion of the preventive measures. Methods 1 540 cases of arthroscopy were carried out from May 1980 to May 1999, including 1 426 cases of the knee, 59 of the ankle, 17 of the wrist, 21 of the first metatarsophalangeal joint, 8 of the hip, 6 of the elbow and 3 of the shoulder. Results Surgical instruments (blade, tip of forceps) broken in the articular cavity took place in 8 cases, injury of the normal tissue including the cartilage, meniscus, and ligaments happened in 40 cases, nerve branch and vascular injured around the inlets in 11 cases, water seeping out in 13 cases, infection following the formation of hematoma in 5 cases, refracture of patella during operation in 1 case, myocardial infarction after operation in 1 case, totally, complications and accidents were seen in 79 cases (5.1% ). Conclusion Arthroscopic complications are mainly related to unskillful manipulation of the instruments, poor team work, inadequate exposure of intra-articular structure and negligence of timely replacing the nearly worn parts of the apparatus. The complications can all be prevented by improving above mentioned points.
5.The technique of double-tunnel with double-bundle for reconstruction of the posterior cruciate ligament with quadriceps tendon-patellar bone under arthroscopy

0.05). Conclusion The procedure of double-tunnel with double-bundle for reconstruction of posterior cruciate ligament (PCL) under arthroscopy can better improve the knee stability at the different flexion degree, and the QT-B is an optimal graft for the double-tunnel reconstruction.
6.The clinical value of 3-dimensional DSA in the diagnosis of intracranial aneurysms

Objective To explore the clinical value of 3-dimensional digital subtraction angiography(3D-DSA)in the diagnosis and therapy of intracranial aneurysms.Methods 2-dimensional digital subtraction angiography (2D-DSA)and 3D-DSA was performed respectively on 1 53 patients,including 1 5 1 cases with spontaneous subarachnoid hemorrhage (SAH),1 case with headache and 1 case with cerebral infarction;Meanwhile,to compare the difference in diagnosis of intracranial aneurysms between 2D-DSA and 3D-DSA.Results 1 12 patients(125 pieces)with intracranial aneurysms were detected on 2D-DSA,its detecting rate was 73.2%(1 12/1 53 );136 patients(1 52 pieces)with intracranial aneurysms were detected on 3D-DSA,its detecting rate was 88.9%(136/1 53)(χ2 =12.254,P <0.05).The displaying rate of 3D-DSA for the relationship between carrying tumor artery and aneurysms and the necks of aneurysms was 100%(1 52/1 52),however,the displaying rate of 2D-DSA for that was 61.6%(77/125)(χ2 =63.993,P <0.05). Conclusion 3D-DSA can improved the diagnostic accurate rate of intracranial aneurysms,it is important in chosing the therapeutic regimen.
7.Differences of clinical results of anterior cruciate ligament reconstruction between two different transplants

8.Effect of Arginine Vasopression on the Contents of cAMP and cGMP in the Cardiac Muscle in Vitro

The effect of arginine vasopressin (AW) on the contents of cAMP and cGMP in the isolated right atria and ventricular muscle strips was observed at the concentration possessing negative inotropic effect on the cardiac muscle (0.8ng/ml). Results showed that although there were accumulation of both cAMP and cGMP stimulated by AVP in the cardiac muscle, the increase of cGMP was more pronounced with the rise of cGMP/cAMP ratio from the control value 024 (atria) and 0.29(ventricle) to 0.45 and 0.41 respectively after treatment with AVP. Coupled with the known effect of cGMP in mediating the negative inotropic effect in the heart, cGMP is also a possible mediator of the negative inotropic effect of AVP in the heart. The significance of increase in cAMP stimulated by AVP awaits further studies.
9.Establishment of rabbit models of corticosteroid-induced avascular necrosis of femoral head

BACKGROUND: Hormone use has become the primary cause of steroid-induced avascular necrosis of femoral head (SANFH). OBJECTIVE: This study used a combination of injection of horse serum and a large dose of corticosteroid to develop a hormone-induced rabbit model of early avascular necrosis of femoral head (ANFH), and preliminary discussed the pathogenesis of ANFH. METHODS: New Zealand rabbits were randomly divided into three groups. Methylprednisolone with horse serum group: horse serum (10 mL/kg) was injected. Three weeks later, 6 mL/kg of horse serum was injected. Two weeks later, 45 mg/kg of methylprednisolone was daily injected for 5 consecutive days. Methylprednisolone group: 45 mg/kg of methylprednisolone was daily injected for 5 consecutive days. Control group: no treatment was given. Serum cholesterol and triacylglycerol levels were detected at 1, 3, 7 and 14 days before and after hormone injection. MRI and histopathological detection was done in femoral head at 2, 4 and 8 weeks after hormone injection. RESULTS AND CONCLUSION: The serum triglyceride and total cholesterol in methylprednisolone with horse serum group and methylprednisolone group were higher than control group at 1 and 3 days after hormone injection (P < 0.01). MRI results displayed abnormal signal in femoral head at 4 weeks in methylprednisolone with horse serum group, but in the methylprednisolone group at 8 weeks. Histological detection results exhibited that at 4 weeks, some trabeculae were broken into fragments, and the empty bone lacunae increased. At 8 weeks, the trabeculae showed thinning and broken. There were large amount of empty bone lacunae with bone cell atrophy and larger fat cells which were fused into bubbles. In methylprednisolone group, the level of necrosis was lighter than methylprednisolone with horse serum group during each period. Results suggest that hormone combined with horse serum can successfully prepare early-stage hormone-induced ANFH.
10.Efficacy and safety of tranexamic acid on reducing blood loss in bilateral total knee arthroplasty

BACKGROUND:The blood loss and the need of blood transfusion after bilateral total knee arthroplasty are very high. Therefore, the use of medicine such as tranexamic acid in total knee arthroplasty is overheated in recent years. Tranexamic acid has been proved to be able to decrease the blood loss after total knee arthroplasty, while the study of its use in bilateral total knee arthroplasty is seldom. OBJECTIVE:To observe the effect and safety of tranexamic acid on perioperative blood loss and the change of hemoglobin in patients undergoing bilateral total knee arthroplasty. METHODS: We selected 69 patients who received bilateral total knee arthroplasty and divided them into two different groups randomly. The patients in the experimental group were given 1 g of tranexamie acid dissolved in 100 mL of 5% glucose solution through intravenous infusion. Those in control group were only given 5% glucose solution 100 mL. Total blood loss, intraoperative blood loss, the hidden blood loss, amount of postoperative drainage, the ratio of blood transfusion, hemoglobin, D-dimer, prothrombin time and activated partial thromboplastin time were studied and compared between the two groups. We also observed the incidence of thrombotic events between the two groups. RESULTS AND CONCLUSION:The blood loss of the experimental group was significantly lower compared to the control group (t=2.194-2.908,P < 0.05). The blood transfusion rate of experimental group was significantly lower compared to the control group (χ2=5.219,P=0.022). The hemoglobin of the experimental group was significantly higher than that of the control group at 3 and 5 days after replacement (t=-3.481, 2.319,P < 0.05). No significant difference in prothrombin time and activated partial thromboplastin time was detectable between the two groups before and after replacement (P > 0.05). Thrombotic events were not visible in both groups. Venous ultrasonography in the lower extremity did not reveal deep venous thrombosis. Above results suggest that tranexamic acid can significantly reduce intraoperative blood loss in patients undergoing bilateral total knee arthroplasty, decreases the requirement of blood transfusion and the dynamic change of hemoglobin, and does not increase the risk of thrombosis.
