1.Risk factors associated with delayed encephalopathy occurrence in carbon monoxide poisoning

Objective To investigate the risk factors associated with delayed encephalopathy (DE)occurrence in carbon monoxide (CO) poisoning.Methods The clinical data of 249 patients with CO poisoning were retrospectively reviewed.Potential risk factors associated with occurrence of DE,including gender,age,duration of exposure to CO,time interval between onset and arrival in hospital,mental status after onset,oxygen therapy approach,were evaluated by univariate analysis of x2 test and multivariate Logistic regression analysis.Results Age and duration of exposure to CO was significandy related with occurrence of DE in patients with CO poisoning.The occurrence of DE in patients with CO poisoning with age 60-75 years was 3.236 times and 2.119 times as much as that with age 3-17 years and 18-59 years respectively.Occurrence of DE was 4.338 times in patients with duration of exposure to CO ≥12 hours compared to that < 12 hours.Conclusions Age ≥ 60 years and duration of exposure to CO ≥ 12 hours are independent risk factors for developing D E.Old patients have a inclination to develop D E.To evacuate patients from CO environment timely is important for preventing from DE.
2.The diagnostic value in bronchoalveolar lavage galactomannan antigen detection of invasive pulmonary aspergillosis in patients without granulocyte deficiency

Objective To study infection of the lung tissue by bronchoalveolar lavage for BALF and explore whether GM can help improve the he diagnosis efficiency in patients without granulocyte deficiency with IPA, and find the threshold of GM values. Methods Between January 2011 and December 2011, 173 cases of patients were considered possible invasive pulmonary aspergillosis in parallel BALF GM detection in the center of the respiratory endoscopic examination.. According to the diagnostic criteria at home and abroad, all the cases were divided into four groups, including group of IPA, group of tuberculosis, group of bacterial pneumonia and group of non-infection. Using ELISA method for determination of serum and BALF GM. The diagnostic value was made by statistical methods in BALF GM detection without patients of granulocyte deficiency with IPA. Results There were 11 cases in the group of IPA, 23 cases in the group of tuberculosis, 90 cases in the group of bacterial pneumonia and 49 cases in the group of non-infection. In patients without granulocyte deficiency with IPA,BALF GM value was higher comparing with the patients without IPA. There were significant difference between the two groups; Comparing the GM value in the serum, the detection of BALF GM was higher in IPA. ROC curve analysis found that when the BALF GM test value was 0.95 or higher, there was highest possible with IPA. Conclusion BALF GM testing is beneficial to patients of invasive pulmonary aspergillosis without granulocyte deficiency in early identification with IPA. This study found that BALF GM best value is 0.95 or higher.

5.Preliminary study on gene expression profiles of sporadic parathyroid adenoma with application of gene-chip technology

Objective To investigate the differentially expressed genes in sporadic parathyroid adenomas using genechip and provide basis for further studying the mechanism and diagnosis of sporadic parathyroid adeno-mas. Methods Total RNA was extracted from normal parathyroid tissues and adenomas tissues. The mRNA was used to prepare probes. The mixed probes were hybridized to the cDNA microarray. After washing, the DNA chips were scanned by laser scanner. The acquired image was analyzed. Results Among the 47 000 target genes, 2302 genes were identified in sporadic parathyroid adenomas tissues that had at least two-fold changes compared to normal parathyroid tissues, and 1012 genes up-regulated and 1290 down-regulated were found in this study. Of 10 genes up-regulated and 27 down-regulated, the changes were over five-fold. The function of these changed genes was involved in transport, cell adhesion, signal transduetion, transcription, ion binding and so on. Conclusions Many parathyroid adenomas associated genes have been screened by high-throughout gene chip method, Which may help to identify the mechanism and diagnosis of sporadic parathyroid adenomas.

8.Effect of overexpression of epidermal growth factor receptor on the growth and migration of a brain-metastatic H1 human melanoma cell line

Objective To investigate the role of epidermal growth factor receptor (EGFR) in the growth and migration of melanoma.Methods A brain-metastatic human melanoma cell line H1,which had been developed in our laboratory,was used in this study.Some H1 melanoma cells were stably transfected with wild-type EGFR via lentiviral vectors to overexpress EGFR (H1EGFRwt),and some H1 cells transfected with green fluorescent protein (GFP) served as the control group (H1GFP).After transfection,the cells were sorted and purified.Then,scratch wound healing assay was performed to estimate the migratory activity of H1 cells; colony-forming assay was conducted to investigate the effects of EGFR on the survival ability and colony-forming ability of H1 cells,and resazurin reduction test was used to evaluate colony formation results; Western blot was carried out to measure the expressions of signaling pathways activated by EGFR.Statistical analysis was done by repeated measures analysis of variance (ANOVA) for the comparison of wound-area healing rate,and by paired t test for the comparison of metabolic activity,between the two groups of H1 cells by using SPSS software version 20.0.Results Flow cytometry showed that H1 cells were successfully transfected with EGFR and GFP respectively,and purified H1EGFRwt and H1GFP cells were obtained after cell sorting.The wound-area healing rate at 6,18,24 and 48 hours after epidermal growth factor (EGF) stimulation was 0.145 ± 0.066,0.479 ± 0.096,0.571 ± 0.198 and 0.672 ± 0.199 respectively for H1EGFRwt cells,0.051 ± 0.036,0.254 ± 0.038,0.303 ± 0.077 and 0.498 ± 0.111 respectively for H1GFP cells.The degree of wound healing in H1EGFRwt cells was significantly higher than that in H1GFP cells (F =68.49,df=5,P < 0.05).Colony-forming assay revealed that the average metabolic activity score of H1EGFRwt cells was significantly higher than that of H1GFP cells (92 225.2 ± 6 632.1 vs.62 935.7 ± 8 159.2,t =2.26,df=9,P < 0.01).Western blot showed that EGF might induce the phosphorylation of downstream signaling proteins such as phosphatidylinositol-specific phospholipase C-γ (PLCγ),signal transducer and activator of transcription 5 (STAT5) and 3 (STAT3) in H1EGFRwt cells,but not in H1GFP cells.In addition,the expressions of phosphorylated serine/threonine protein kinase (pAKT) and mitogen-activated protein kinase (pMAPK) were significantly higher in H1EGFRwt cells than in H1GFP cells after EGF stimulation.Conclusions EGFR may play an important role in the metastasis of H1 melanoma cells,and might serve as a target in the treatment of metastatic melanoma.
9.Expressions of IGF-1R and IGFBP-3 in colon cancer metastasis and their correlations with lymphatic metastasis

Objective To investigate the expressions of insulin-like growth factor receptor-1 (IGF-1R) and insulin-like growth factor binding protein-3 (1GFBP-3) and their correlations with clinicopathological parameters in the primary colon cancer,as well as their roles in lymph node metastasis of colon cancer.Methods The expressions of IGF-1R and IGFBP-3 in 78 cases of colon cancer tissues and 78 cases of normal colon mucosa tissues were detected by SP immunohistochemical technology and the correlations between the expressions and the clinical pathological parameters of colon cancer were analyzed.Results The positive rate of IGF-1R in colon cancer (66.7%,52/78) was significantly higher than that in control group (24.4%,19/78),x2 =28.150,P =0.000.The positive rate of IGFBP-3 in colon cancer (73.1%,57/78) was significantly lower than that in control group (89.7%,70/78),x2 =7.158,P =0.007.IGF-1R expression in colon cancer was significantly correlated with the invasion (x2 =5.804,P =0.016),TNM stage (x2 =5.246,P =0.022) and lymph node metastasis (x2 =12.955,P =0.000).IGFBP-3 expression in colon cancer was signi-ficantly correlated with the TNM stage (x2 =7.096,P=0.008),lymph node metastasis (x2 =5.893,P =0.015) and distant metastasis (P =0.003).Both with other factors had no significant correlation (P > 0.05).1GF-1R expression and IGFBP-3 expression showed a negative correlation (r =-0.245,P =0.03).Conclusion The over expression of IGF-1R and low expression of IGFBP-3 are associated with TNM stage and lymph node metastasis in colon cancer.IGF-1R and IGFBP-3 may become new targets of the treatment of colon cancer.
10.Study on the character and risk factors of coronary artery changes in patients with coronary heart disease and impaired glucose tolerance

Objective To investigate the character and risk factors of coronary artery disease in prediabetes. Methods The 193 patients with coronary artery disease were divided into 3 groups according to the American diabetes associations (ADA) standard of diabetic diagnoses. (1) Normal glucose tolerance group(NGT group,122 cases). (2) Impaired glucose tolerance group (IGT group,39 cases). (3) Diabetes mellitus group (DM group,32 cases). All patients were checked by the coronary angiographic semiquantitative (CAG) and the 75g oral glucose tolerance test. Blood glucose, insulin, TG, TC, HDL C, blood pressure, BUN and Cr were determined. The coronary artery changes were analysed by CAG, and the severity of coronary artery stenosis were evaluated by AHA standards. The risk factors in coronary artery disease were analysed by pluralized straight line statistics.
