1.Observations on the Efficacy of Acupuncture at Hegu plus Fuliu in Treating Hyperhidrosis in Parkinson’s Disease

Objective To investigate the clinical efficacy of acupuncture at Hegu plus Fuliu in treating hyperhidrosis in Parkinson’s disease. Methods Fifty-seven Parkinson’s disease patients with hyperhidrosis were randomly allocated to a treatment group of 28 cases and a control group of 29 cases. The control group received conventional acupuncture and the treatment group, acupuncture at Hegu plus Fuliu in addition. The VAS score and the HDSS score were recorded in the two groups before and after treatment. The clinical therapeutic effects were compared between the two groups.Results There were statistically significant pre-/post-treatment differences in the VAS score and the HDSS score in the two groups (P<0.01). There were statistically significant post-treatment differences in the VAS score and the HDSS score between the treatment and control groups (P<0.01). The total efficacy rate was 96.4% in the treatment group and 72.4% in the control group; there was a statistically significant difference between the two groups (P<0.05).Conclusion Acupuncture at Hegu plus Fuliu can effectively reduce hyperhidrosis in Parkinson's disease patients.
2.Clinical application of low energy intracardiac cardioversion of atrial fibrillation

Objective To evaluate the efficacy and safety of low energy intracardiac cardioversion in persistent atrial fibrillation. Methods Low energy intracardiac cardioversion was performed by delivering R wave synchronized biphasic shocks in 7 patients(4 men, 3 women) with persistent atrial fibrillation. Prior to the procedure, all patients underwent transesophageal echocardiographic examinations to rule out the presence of intracardiac thrombus and received subcutaneous injection of low molecular weight heparin for 3 5 days. Two custom made 6 Fr catheters(Rhythm Technologies of Getz, USA) were used for de fibrillation shock delivery. One catheter was positioned in the lower right atrium so that the majority of the catheter electrodes had firm contact with the right atrial free wall. The second catheter was placed randomly either in coronary sinus through right internal jugular vein or in the left pulmonary artery through femoral vein. In addition, a standard diagnostic 6 F quadropolar catheter was placed at the right ventricular apex for ventricular synchronization and postshock ventricular pacing. Shocks were delivered by Implant Support Device(Model 4510, Teleceronics). After conversion, all patients were treated with intravenous amiodarone in the first 24 hours followed by oral administration. Results In all 7 patients cardioversion of atrial fibrillation to sinus rhythm was successfully obtained. A mean of 2?1 shocks per patient has been delivered with a total amount of 13 shocks. The average delivered energy was 7.8?2.2 Joules. No complication occurred. At a mean follow up of 18?9 months, 4 of the 7 patients treated successfully showed sinus rhythm there after. Atrial fibrillation recurred in 3 patients at the second, fifth day and eighth month after cardioversion. Conclusions Low energy intracardiac cardioversion is effective and safe, and can be easily performed in patients without geneal anesthesia. It offers a new option for restoring sinus rhythm in patients with persistent atrial fibrillation.
3.Relationship of hypoxia inducible factor 1? and vascular endothelial growth factor expression with angiogenesis in gastric carcinoma

Objective To investigate the expression of hypoxia inducible factor 1?(HIF-1?),vascular endothelial growth factor(VEGF) and micro vessel density(MVD) in gastric carcinoma and to explore their correlation with clinical pathological features such as cancer invasion and metastasis.Methods Forty-eight samples of gastric carcinoma tissues were examined for the expression of HIF-1?,VEGF and CD34 by immunohistochemical method.Results The positive expression rates of HIF-1? and VEGF were 66.7% and 60.4% in gastric carcinoma respectively.The mean value of MVD was 42.5?14.7 in gastric carcinoma.The expressions of HIF-1?,VEGF and the value of MVD were significantly correlated with the depth of invasion,lymph node metastasis and TNM stage.The HIF-1? expression was positively correlated with VEGF expression and MVD value.Conclusion The overexpression of HIF-1?,VEGF and MVD consist in gastric carcinoma tissue.The HIF-1? expression is positively correlated with VEGF expression and MVD value.The overexpression of HIF-1?,VEGF and MVD value are closely related with invasion,metastasis and poor biological behavior of gastric carcinoma.

5.Clinical Observation of Ordinary Acupuncture plus Zhigou (TE6) and Zhaohai (KI6) for Constipation Due to Yin Deficiency in Parkinson's Disease

Objective To observe the clinical efficacy of ordinary acupuncture plus Zhigou (TE6) and Zhaohai (KI6) in treating constipation due to yin deficiency in Parkinson's disease. Method Sixty eligible patients were randomized into a control group and a treatment group, 30 cases each. The control group was intervened by ordinary acupuncture by selecting the Dance-tremor Control Zone, Baihui (GV20), Fengchi (GB20), Fengfu (GV16), Quchi (LI11), Yanglingquan (GB34), Taixi (KI3) and Taichong (LR3). Based on the ordinary acupuncture, Zhigou (TE6) and Zhaohai (KI6) were added in the treatment group. The treatment was given three times a week, 12 sessions as a course, and the therapeutic efficacy was evaluated after 2 successive treatment courses by using Patient Assessment of Constipation Quality of Life Scale (PAC-QOL), Bristol Stool Scale (BSS), Cleveland Clinic Score (CCS) and Unified Parkinson's Disease Rating Scale (UPDRS) as the observation indexes. Result Before the intervention, there were no significant differences in comparing the PAC-QOL, Bristol, CCS and UPDRS scores between the two groups (P>0.05);after the treatment, the scores of PAC-QOL, Bristol, CCS, and motor function and activities of daily living in UPDRS dropped significantly in the two groups (P<0.05); the PAC-QOL, Bristol and CCS scores in the treatment group were significantly lower than those in the control group after the treatment (P<0.05); the treatment group had a significantly higher effective rate in improving constipation symptoms than the control group (P<0.05); there was no significant difference in comparing the UPDRS score between the two groups after the intervention (P>0.05). Conclusion Ordinary acupuncture plus Zhigou and Zhaohai can significantly improve the quality of life, stool form and difficulty in defecation in constipation of yin-deficiency type in Parkinson's disease, and the therapeutic efficacy is superior to that of ordinary acupuncture.

Acupuncture anesthesia is a technique by partially or completely replacing anesthetics with acupuncture in surgery based on the traditional acupuncture analgesia mechanism.It emerged in the 1950s,went viral in the 1970s and then gradually fell into decline.In the recent years,this technique has regained attention and further research.Acupuncture anesthesia can be classified as either pure acupuncture anesthesia or acupuncture-medication combined anesthesia.To expand the application of this technique,a patient with non-ST elevation acute coronary syndrome in urgent need of percutaneous coronary intervention (PCI) received pure acupuncture anesthesia because of an allergy to lidocaine,and the operation went successfully.This is the first time that pure acupuncture anesthesia and coronary arteriography were combined,which is of great significance in further study and development of acupuncture anesthesia.

OBJECTIVETo study the chemical constituents of Psammosilene tunicoides.
METHODThe two chemical constituents were separated by various chromatographic methods, and their structures were identified on the basis of spectroscopic analysis.
RESULTTwo beta-carboline alkaloids were separated from normal butanol fraction of P. tunicoides, and identified as 1-acetyl-3-methoxycarbonyl-beta-carboline (1) and 1-acetyl-3-methoxycarbonyl-4-hydroxyl-beta-carboline (2).
CONCLUSIONCompound 1 is separated from this plant as natural products for the first time, and compound 2 was a new compound.

10.Optimization of atrioventricular delay by surface electrocardiography during dual chamber pacing.

BACKGROUNDNominal atrioventricular (AV) interval in dual chamber pacemaker (DDD) is not the best AV delay in the majority of patients with atrioventricular block. To find a simple method for optimizing AV delay adjustment, we assessed surface electrocardiography (ECG) for optimizing AV delay during dual chamber pacing.
METHODSDDD pacemakers were implanted in 46 patients with complete, or almost complete, AV block. Optimal AV delay was achieved by programming an additional delay of 100 ms, to the width of intrinsic P wave or to the interval between pacing spike to the end of P wave on surface ECG. Left ventricular (LV) end diastolic and end systolic volumes, ejection fraction and diastolic parameters were measured by Doppler echocardiography during both nominal and optimal AV delay pacing.
RESULTSCompared to nominal AV delay setting, LV end diastolic volume increased [to (53.2 +/- 11.3) ml from (50.2 +/- 10.2) ml, P < 0.05], end systolic volume decreased [to (26.1 +/- 9.0) ml from (27.9 +/- 8.2) ml, P < 0.05] during adjusted AV delay pacing, resulting in an increase in LV ejection fraction [to (68.2 +/- 5.3)% from (64.5 +/- 4.3)%, P < 0.05]. LV diastolic filling and isovolumic relaxation time were not significantly changed.
CONCLUSIONOptimization of AV delay by surface ECG is a simple method to improve LV systolic function during dual chamber pacing.
