1.Clinical analysis of coagulation factors and inflammatory reaction in coronary heart disease

Objective To understand the relationship between coagulation factors,inflammatory reaction and coronary heart disease(CHD) by analyzing the changes of fibrinogen (Fib), D-dimer (DD) ,C-reactive protein(CRP) and white blood cell (WBC) count. Methods The levels of Fib, DD, CRP and WBC were measured and compared in 91eases with CHD[ineluding 34 cases of acute myocar-dial infarction (AMI group),30 eases of unstable angina peetoris (UAP group) and 27 eases of stable angina peetoris (SAP group)] and 30 healthy controls (healthy control group). Results Plasma con-centrations of Fib, DD, CRP and WBC were the highest in AMI group, followed by those in UAP group,SAP group and healthy control group. There were statistical differences in the above parameters between AMI group and UAP group; so were between UAP group and SAP group. There was no sig-nificant difference between UAP group and healthy control group (P>0. 05). Conclusion The coagu-lation factors and inflammatory reaction are closely associated with occurrence and development of cor-onary heart disease. Four parameters (Fib,DD,CRP and WBC) can be used as adjuvant indicators of differentiating types of CHD and evaluating severity of disease.
2.Effects of different moxibustion temperature on cholesterol and skin around "Shenque" (CV 8) in mice with hyperlipidemia.

OBJECTIVETo observe the effects of moxibustion with different temperatures on cholesterol and skin around "Shenque" (CV 8) in mice with acute hyperlipidemia, and to explore the correlation between moxibustion effect and transient receptor potential vanilloid 1 (TRPV1).
METHODSEight mice among 32 mice of C57BL/6J wild type (WT) were selected into a blank group, and the remaining 24 mice were made into the acute hyperlipidemia model by injection of egg yolk. After model establishment, the mice were randomly divided into a model group, a 38 °C moxibustion group and a 46 °C moxibustion group, 8 mice in each group. The temperature was (38 ± 1) °C in the 38 °C moxibustion group and (46 ± 1) °C in the 46 °C moxibustion group. "Shenque" (CV 8) and "Zusanli" (ST 36) were selected in the two groups; moxibustion was given for 10 min per time, once a day for 2 times. Mice in the blank group and model group were treated with immobilization and moxibustion was not given. After treatment, the level of serum cholesterol was tested by oxidase method; the morphology of skin around "Shenque" (CV 8) was observed by HE staining; the expression of TRPV1 in skin was measured by immunohistochemistry.
RESULTS(1) Compared with the blank group, the level of serum cholesterol was increased in the model group (P < 0.001); compared with the model group, the level of serum cholesterol was reduced in the 46 °C moxibustion group (P < 0.001); compared with the 38 °C moxibustion group, the level of serum cholesterol was reduced in the 46 °C moxibustion group (P < 0.01). (2) There was slight change of morphology and structure in skin tissue in the 38 °C moxibustion group, while obvious change was observed in the 46 °C moxibustion group, indicating specific change. (3) The difference of the expression of TRPV1 in skin was significantly different between 46 °C moxibustion group and blank group, model group (both P < 0.01); it was also different from the 38 °C moxibustion group (P < 0.05).
CONCLUSIONMoxibustion temperature is one of the important factors affecting the cholesterol and acupoint skin. The effects of moxibustion are related to TRPV1.

3.Dosimetric and clinical factors for predicting acute radiation oral mucositis in locally advanced nasopharyngeal carcinoma patients treated with intensity?modulated radiotherapy with concurrent chemotherapy

Objective To prospectively determine the dosimetric and clinical factors for predicting the risk of acute radiation oral mucositis ( ROM ) in patients receiving intensity?modulated radiotherapy ( IMRT) with concurrent chemotherapy for local advanced nasopharyngeal carcinoma. Methods Ninety?two patients who were treated with IMRT with concurrent chemotherapy from 2015 to 2016 for local advanced nasopharyngeal carcinoma were included in this study, and their acute ROM was scored according to the RTOG criteria. Grade≥3 ROM was used as a surrogate marker for severe mucositis, which was defined as a toxicity endpoint. The clinical data were reviewed, and the dose?volume histograms ( DVHs) of the patients were exported from the IMRT planning system. Optimal thresholds for predicting the incidence of severe ROM were evaluated by the area under the receiver operating characteristic ( ROC) curve ( AUC) . Results The incidence of severe ROM was 21%(19/92). Weight loss and V30 of the oral mucosa were determined as the independent predictors for severe ROM ( P=0017 and 0003, respectively) . The optimal cut?off point and AUC of V30 of the oral mucosa as a predictor for severe ROM were 7316%( 0842 sensitivity and 0671 specificity) and 0753( P=0001) , respectively. Conclusion Weight loss and V30 of the oral mucosa are predictors for severe ROM.
4.Clinical Application of Platelet-Rich Plasma in Treatment of Deep GradeⅡBurn Wounds

Objective To evaluate the clinical effects of platelet-rich plasma in treatment of DeepⅡburn wounds. Methods Sixty eight cases of inpatients with Deep Ⅱburns were selected, whose age were from 1 to 65 years and their burn areas were between 5%~62%of total body surface area (TBSA). Deep gradeⅡburn of each sample was divided into two parts. Part A was the treatment group and part B was the control group.The burn wounds in the treatment group were treated with platelet-rich plasma and the counterpart in the control group were treated with SD-Ag. Healing time ,recovering rate and the frequency of dressing changes,frequency of changing the most innerlayer gauze and the rate of wound infection were also analyzed. Finally the laboratory abnormalities and adverse effect were monitored regularly. Results The healing time of the treatment group (16.5±3.1 ) d was shorter than that of the control group (19.5±3.8 ) d. The recovering rate of the treatment group was higher than that of the control group on the 14th and 17th day after treatment. There were statistically sig?nificant differences between these two groups (P<0.05). After two weeks’treatment, the internal and external dressing per?meability into wet gauze layers were 20.6 ± 1.7, which were significantly decreased than that in the control group 23.3 ± 5.9. The frequency of dressing changes was(7.2±1.1)times in treatment group versus(9.2±1.4)times in control group and the frequency of changing the inner most layer gauze was( 2.3±0.6)times in the treatment group versus(5.3±1.3)times in con?trol group. There were 5 inflammation reaction cases in the treatment group, but 13 cases in the control group. However, there was no statistic significance between the two group in the outcomes of bacterial culture, the laboratory abnormalities and the adverse effect. Conclusion Platelet-rich plasma can remarkably shorten the healing time,improve healing rate,re?duce frequency of dressing change and promote wound healing for deepⅡburn wound. PRP is a potential safety reagent in treating deepⅡburn wound.
5.Results of induction chemotherapy followed by three-dimensional conformal radiotherapy and con current weekly paclitaxel for stage Ⅲ non-small cell lung cancer

Objective To evaluate the toxicity and efficacy of induction chemotherapy(ICT)followed by three-dimensional conformal radiotherapy(3 DCRT)plus concurrent weekly paclitaxel for inoperable non-small cell lung cancer(NSCLC). Methods Patients with stage Ⅲ NSCLC in favorable conditions were treated with 2 to 4 cycles of carboplatin(AUC=5-6,d1)combined with paclitaxel(175 mg/m2,d1),then followed by weekly paclitaxel(40 mg/m2)and concurrent 3DCRT within 3-4 weeks.The prescription dose of radiotherapy was given as high as possible while total lung V20≤31% and total dose of the spinal cord ≤50 Gy. Results ICT was well tolerated.During the concurrent chemoradiotherapy,the treatment of 4 patients was ended ahead of the schedule because of severe pulmonary and cardiac toxicities:the treatment of 2 patients was delayed for 7 and 12 days because of fatigue.Leucopenia(33/56)was in grade 1-2 except 1 patient in grade 3.Lymphocytopenia was severe(54/56,42 in grade 3).Three patients developed grade 3 acute radiation-induced esophagitis.and 3 developed grade 3-4 radiation-induced pneumonitis.There was one patients each who developed grade 2,3,and 4 late esophagealdamage,respectively.Nine developed grade 2 pulmonary fibrosis.The overall response rate was 69.7%.The 1-year overall survival rate was 72.3%.The 1-year local progression-free survival rate was 62.7%. Conclusions The schedule of ICT followed by weekly paclitaxel and concurrent 3DCRT can be well tolerated by most of the favorable patients with stageⅢ NSCLC.and the toxicity is tolerable. Results of this study are encouraging, though long-term results should be followed up.
6.The application value of sperm nuclear DNA integrity and sperm morphology in the treatment of ICSI

Objective To investigate the effects of sperm DNA fragmentation (SDF)and sperm morphology on the fertilization and embryo development in ICSI.Methods SDF and sperm morphology were detected in the meanwhile of taking eggs in 1 45 ICSI treatment cycle.On the basis of SDF index (DFI)divided into group A (DFI≤30%)and group B (DFI >30%).According to the normal sperm morphology divided into group C (NMSR≥4%), group D (1 %≤NMSR <4%),group E (NMSR <1 %).According to the sperm DFI and NMSR divided into group F (DFI≤30% and NMSR≥4%)and group G (DFI >30% and NMSR <4%).Statistically analyzed ICSI outcome of research group:fertilization rate,embryo utilization rate,good quality embryo rate,implantation rate and clinical pregnancy rate.Results (1 )The normal fertilization rate,embryos utilization rate,good quality embryo rate,implan-tation rate in group A were significantly higher than those in group B (χ2 =6.96,8.95,5.49,3.92,all P <0.05), but clinical pregnancy rate had no significant difference (χ2 =1 .08,P >0.05).(2)The normal fertilization rate was statistically significant in group C,group D,group E (χ2 =34.5,65.8,11 .8,all P <0.05),but there were no significant differences in embryo utilization rate,good quality embryo rate,implantation rate and clinical pregnancy rate (P >0.05).(3 )The normal fertilization rate,embryo utilization rate,good quality embryo rate,implantation rate and clinical pregnancy rate in group F were significantly higher than those in group G,and normal fertilization rate,embryos utilization rate,good quality embryo rate had statistically significant differences (χ2 =37.5,1 1 .0,4.3,all P <0.05). Conclusion Sperm abnormal morphology has negative effect on fertilization,and the high DNA fragments have negative effects on fertilization and embryo development.
7.Outcome of modified intracytoplasmic sperm injection using zona pellucida -bound spermatozoa in male ;infertility

Objective To investigate the treatment outcome of modified intracytoplasmic sperm injection (ICSI)using zona pellucida(ZP)-bound sperm.Methods 82 patients with less,weak,abnormal sperm disease who were conformed to ICSI,were divided into traditional ICSI group and the group of modified ICSI using ZP-bound sperm according to ICSI case number.The results of normal fertilization rate,cleavage rate,high-quality embryo rate, planting rate,clinical pregnancy rate and early abortion rate were compared.Results The women's age,the sterility year,mature egg rate,normal fertilization rate,cleavage rate in the two groups had no statistically significant differ-ences (all P>0.05).The planting rate and clinical pregnancy rate of the observation group (46.6%,63.3%)were higher than those of the control group(38.5%,53.6%),but there were no statistically significant differences(all P>0.05).The using embryo rate and high-quality embryo rate of the observation group (73.9%,51.0%)were signifi-cantly higher than those of control group(65.8%,38.6%),the differences were statistically significant(χ2 =5.84,χ2 =11.6,all P<0.05).Conclusion Modified ICSI using ZP-bound sperm can effectively improve the embryos quality in ICSI.

Taxillus chinensis,belonging to hemiparasitic herbal medicine and featuring its wild host range,is used for relief from rheumatic conditions,reinforcement of liver and kidney,strengthening of tendon and bone,and prevention of abortion.It showed that host trees presented significant impacts on the quality of Taxillus chinensis in the modern researches.In this study,both domestic and foreign published articles over Taxillus chinensis from various host trees were reviewed to summarized its herbalogy,chemical constituents,toxicity,and pharmacological action.In regard to the influences on the quality of medicinal materials,ideas and references for the quality control of Taxillus chinensis were provided by analyzing the quality of Taxillus chinensis impacted by hosts.
9.An evaluation of Mandard tumor regression grade system in patients with locally advanced rectal cancer treated with preoperative radiotherapy

Objective To explore the clinical and imaging factors influencing the patients' prognosis after preoperative radiotherapy for local advanced rectal cancer.Methods We retrospectively analyzed 106 locally advanced rectal cancer patients from June 2004 to September 2015 in our institution.All patients underwent preoperative radiotherapy.According to the Mandard score,patients were divided into 5 groups (TRG1-5).All patients were divided into two groups according to the TRG,which including good responder (TRG1 + 2) and poor responder (TRG3 + 4 + 5) groups.All of the tumor ADC values of post-RT were measured by Diffusion-weighted MRI technology,and the relationship between tumor ADC values of post-RT and TRG was analyzed.Results In univariate analysis,age,chemotherapy,pT,pN,differentiation degree,vascular invasion and TRG were significantly associated with overall survival (x2 =3.945-8.110,P < 0.05).Multivariate analysis indicated that differentiation degree and TRG were the independent prognostic factors for OS (x2 =5.221,6.563,P < 0.05).No significant difference was found between long-course and short-course radiotherapy group (P > 0.05) in OS.The good responder group had a favorable survival in 5-year OS compared to the poor responder group (x2 =8.110,P < 0.05).Preoperative radiotherapy,preoperative chemotherapy,pathological type,differentiation degree and gross type,vascular tumor thrombus and tumor ADC values of post-RT were significantly associated with TRG (x2 =4.189-18.139,P < 0.05).The best critical point of tumor ADC values of post-RT was 1.7 x 10-3 mm2/s by using ROC curve.The accuracy of tumor ADC values of post-RT in predicting TRG1 + 2 was 70%.Conclusions The TRG can predict the efficacy of preoperative radiotherapy in patients with locally advanced rectal cancer based on the Mandard score.There was no significant difference in OS between long-course radiotherapy group and short-course radiotherapy group.The tumor ADC values of post-RT might become a potential factor to predict TRG in patients with locally advanced rectal cancer after preoperative radiotherapy.

Objective To explore the extensively drug resistant mechanism and clinical treatment strategy of Klebsiella pneu-moniae .Methods The isolate was identified by Vitek2 Compact System.Antimicrobial susceptibility testing was conducted by Kirby-Bauer method.KPC-2 carbapenemase was detected by modified Hodge test.The gene encoding KPC-2 carbapenemase was amplified by polymerase chain reaction (PCR)and then sequenced.Results The strain was resistant to all antibiotics used in routine antimicrobial susceptibility testing except amikacin.Modified Hodge test showed positive result.KPC-2 gene was detected by PCR.The sequence was consistent with that of 11844849 in GenBank.After treatment for one month,no exten-sively drug resistant K.pneumoniae strain was detected from the patient.Conclusions It is necessary to strengthen the monito-ring and improve the awareness of extensively drug resistant K.pneumoniae for better control of such infections.
