Objective To investigate the pharmacological mechanism of LXHX capsule. Methods The skin specimens from psoriasis patients were examined with TUNEL (terminal deoxynucleotidyl trans-ferase-mediated dUTP-biotin nick end labeling) technique to detect the apoptosis of keratinocytes. PCNA (proliferating cell nuclear antigen) detecting kits was used, too. Then, the flow cytometry, with AnnexinV/PI and PI dyeing method, was used to analysis the effect of LXHX capsule on apoptosis in cultured keratinocytes. Results There was an increasing of both cell apoptosis and proliferation in psoriatic epidermis and LXHX capsule could induce apoptosis. Conclusions The keratinocyte apoptosis and proliferation both increased in psoriasis, which reach a new balance in a higher level. LXHX capsule could induce apoptosis in vitro, which may be one of the pharmacological mechanisms of LXHX in treating psoriasis.

Abstract: Objective　To evaluate the value of bronchoalveolar lavage fluid Aspergillus-specific fluorescent PCR assay combined with galactomannan (GM) assay in the diagnosis of patients with non-neutropenic invasive pulmonary aspergillosis (IPA). Methods From March 2022 to December 2023, 113 hospitalized patients with clinically suspected IPA were selected from the Zibo First Hospital of Zibo City, Shandong Province. Bronchoalveolar lavage fluid samples from each patient were simultaneously subjected to potassium hydroxide microscopy, fungal culture, GM assay, and Aspergillus-specific fluorescence PCR assay. According to the diagnostic criteria of IPA, patients were divided into clinically diagnosed IPA and non-IPA groups. The values of these four methods for the diagnosis of IPA were compared. Results According to the diagnostic criteria for IPA, 37 out of the 113 suspected patients were clinically diagnosed as IPA. The proportion of diabetic patients was significantly higher in the IPA group compared to the non-IPA group (χ2=7.494, P=0.006); similarly, the proportion of patients using glucocorticoids was significantly higher in the IPA group (χ2=6.981, P=0.008). Patients in the IPA group more frequently showed cavitation within consolidation areas on imaging, which was statistically significant (χ2=15.603, P<0.001). There were significant differences in the sensitivity of the four fungal detection methods in the diagnosis of IPA (χ2=45.803, P<0.001), with Aspergillus-specific PCR assay showing the highest sensitivity at 94.59%. Specificity also varied significantly across the four methods (χ2=31.511, P<0.001), with the highest specificity being seen in potassium hydroxide microscopy and fungal culture at 100.00%. There were significant differences in the clinical coincidence rate of the four methods in the diagnosis of IPA (χ2=11.768, P=0.008), with Aspergillus-specific fluorescence PCR assay having the highest coincidence rate at 90.27%. The AUC of the ROC curve of Aspergillus-specific fluorescent PCR assay combined with the GM assay was 0.976 7, higher than 0.913 8 by Aspergillus-specific fluorescent PCR assay merely. Conclusions The combination of Aspergillus-specific fluorescent PCR assay and GM assay using bronchoalveolar lavage fluid could significantly improve the accuracy of IPA diagnosis in patients without neutropenia.

Objective To investigate the effect and mechanism of transplantation of marrow multi- function stem ceils in treating femoral head necrosis.Methods Sixty japanese rabbits were divided into A,B and C groups randomly.After creation of the models of hormone induced necrosis of femoral head;A group was designated as the treatment,B as the control and C as the normal groups.The bone marrow of A group was extracted and isolated and then injected into the left femoral head and the right femoral head was decompressed by drilling only.The rabbits were killed at 8 weeks after the treatment and changes in various parameters were observed,including imaging data of molybdenum target films,CT and MRI;routine pathology with HE staining and ultrastructural alteration by election microscopy.Results Eight weeks after the treatment of transplantation of marrow multifunction stem cells,the X-ray showed only a little change but the typical appearances were revealed by CT and MRI.Pathohistologic manifestation demonstrated decrease of empty bone lacuna,increase of osteoblast and new bone formation.Election microscopy displayed abundant organelles in osteoblasts with few empty bone lacuna,in addition the tansplantation of marrow multifunction stem cells could obtain better reconstraction for the involved femoral head.Conclusions The treatment of transplantation of marrow muhifunction stem cells in femoral head necrosis could accelerate the process of repairing,worthy to be acknowledged as a good and valuable management in rabbits.(J Intervent Radial,2007,16:122-126)

Objective To analysis the relationship between thyroid function with gastrointestinal motility and plasma motilin levels, and to provide the basis for clinical application.Me thods We selected 30 cases of patients with hyperthyroidism, 30 cases of hyperthyroidism group and hypothyroidism as hypothyroidism group, and 30 healthy volunteers as control group who were treated in our hospital from January 2013 to January 2016 in;The radioimmunoassay was used to test serum motilin levels, isotope single photon emission computed tomography was used to detected the gastric liquid technology half time, esophageal transit time.Re s ults The esophagus passage time of the subjects in the hypothyroidism group was significantly higher than that of the hyperthyroidism group and the control group, and the difference was statistically significant (P＜0.05);The level of motilin in serum of hyperthyroidism group was significantly higher than that of control group (P＜0.05), and the level of motilin in serum of hypothyroidism group was significantly lower than that of control group (P＜0.05);Hyperthyroidism was positively correlated with motilin level (r = 0.542, P＜0.05), and negatively correlated with the time of passage of the esophagus and gastric emptying time (r =-0.126 and r =-0.481, P＜0.05);Hypothyroidism was negatively correlated with motilin level (r =-0.617, P＜0.05), and positively correlated with esophageal transit time and gastric emptying time (r = 0.627 and r = 0.416, P ＜0.05).Conclus ions Hyperthyroidism can lead to the increase of gastrointestinal function and motilin level. Hypothyroidism can lead to the decrease of gastrointestinal function and motilin level.

Objectives To assess the plasma levels of acyl ghrelin (AG) and dys-acyl ghrelin (DG) in chronic kidney disease (CKD) and hemodialysis (HD) patients and analyze their relationships with different stages of CKD and hemodialysis. Methods Forty-six CKD stage 1-5 patients and 15 hemodialysis patients were enrolled into the study. Body weight, height, hemoglobin, biochemical parameters, inflammatory parameters, preprandial, postprandial and 3 hours after hemodialysis plasma AG and DG levels were measured. Appetite and food intake were assessed. Body mass index (BMI), and estimated glomerular filtration rate (eGFR) were calculated. Results There were no significant differences in BMI, SGA, appetite, food intake and malnutrition among CKD patients of different stages. eGFR was declining with the progression of CKD stages and patients received a three-week hemodi-alysis. Compared with that in CKD stage 1-2 patients, the level of preprandial and postprandial DG was remarkably increased in stage 3-5 patients (P<0.01). The level of DG was significantly decreased after a standard breakfast in CKD patients (P<0.01). CKD stage was positively correlated with preprandial (r=0.31, P<0.05)a nd postprandi-al DG (r=0.34, P < 0.05), TNF-α (r=0.33, P < 0.05), IL-6 (r=0.40, P < 0.05), leptin (r=0.34, P < 0.05), and age (r=0.41, P<0.05). CKD stage was also highly and positively correlated with the proportion of preprandial and postprandial DG (r=0.61, P<0.01;r=0.55, P<0.01). Multivariate partial-correlation analysis showed that CKD was independently associated with the proportion of preprandial and postprandial DG (r=0.55, P < 0.01; r=0.43, P < 0.01).There was no decrease in AG postprandially, nor any changes in AG resultant from dialysis (P > 0.05);levels of DG decreased slightly postprandially and were markedly decreased by hemodialysis (P<0.01), even lower than those seen postprandially in CKD stage 1-2;Both preprandial and postprandial DG were negatively correlated with serum albumin levels (r=-0.64, P < 0.05; r=-0.59, P < 0.05), while there was no correlation between AG and serum albumin levels. Conclusions There is a strong and independent correlation of DG with CKD stage. Postprandial suppression of ghrelin is impaired with reduced renal function. Hemodialysis removes DG but not AG.

Objective To observe the effect of the application of peripherally inserted central catheter (PICC) network platform in the information management of the patients with PICC.Methods Altogether 17 254 outpatients receiving PICC maintenance in Peking Union Medical College Hospital from April 2012 to April 2013 were enrolled as the control group,including 7 227 males and 10 027 females,with the median age of 58 years (12-85 years).A total of 20 384 outpatients from April 2013 to April 2014 with PICC were selected as the observation group,including 8 188 males and 12 196 females,~th the median age of 59 years (13-86 years).Those patients all received PICC maintenance in outpatient clinic during the intermission of therapy after PICC insertion.The time of data entry,the integrity of the data,description accuracy about complications,and normalization of wording were compared between the two groups.Results The average time of data entry in the control group was (46 ± 6) seconds,significantly longer than that in the observation group [(12 ± 5) seconds,t =562.660,P ＜ 0.05].In terms of the integrity of the data,there were 11 732 cases of complete data,3 623 cases of less complete data,and 1 899 cases of incomplete data in the control group;while the numbers of cases of complete data,less complete data,and incomplete data in the observation group were 19 568,725,and 91,respectively,showing significant difference compared with the control group (x2 =5 312.000,P ＜ 0.05).In the description accuracy about complications,the control group had 11 840 accurately described cases and 5 414 inaccurately described cases,while the observation group had 18 427 accurately described cases and 1 957 inaccurately described cases (x2 =2 814.000,P ＜ 0.05).The wording was standard in 15 280 cases but not standard in 1 974 cases in the control group,and standard in 19 659 cases and not standard in 725 cases in the observation group,with significant inter-group difference (x2 =872.600,P ＜0.05).Conclusion Simple and convenient data summary could help quality control and quality analysis,preferably guarantee the safety of catheter insertion and reduce the incidence of complications.

Objective To explore the association between XPD codon 751 polymorphism and esophageal cancer (EC) by systematically reviewing the risk of the original studies. Methods A comprehensive search was conducted to identify all case-control studies of XPD codon 751 polymorphism and EC risk. Meta-analysis was applied with Rev Man 4.2 software for calculation of pooled OR value (with 95%C1)of EC, esophageal squamous cell carcinoma (ESCC)and esophageal adenocarcinoma (EAC). Results Of the 12 case-control studies selected for this Meta-analysis, a total of 2558 EC cases and 5122 controls were included. Compared with the wild-type homozygote Lys/Lys, the pooled Odds Ratios (with 95% CI) of Lys/Gln, Gin/Gin, (Lys/Gln + Gln/Gln) genotypes of XPD codon 751 polymorphism for EC risk were 1.19(1.05, 1.34), 1.22(0.86, 1.74), 1.20(1.01,1.42), respectively. In a stratified analysis, a total of 1417 ESCC cases and 2312 controls were included, and individuals carrying Lys/Gln genotype or (Lys/Gln+Gln/Gln) had 1.22-fold or 1.24-fold excess risks for ESCC compared with those carrying Lys/Lys genotype. A total of 935 EAC cases and 2604 controls were included, and none of the genotype of XPD codon 751 genetic polymorphism was found to be related to EAC. Conclusion Both heterogyzote Lys/Gln and (Lys/Gln + Gln/Gln) for XPD codon 751 genetic polymorphism were associated with an increased risk of developing esophageal cancer. Furthermore, heterogyzote Lys/Gln and (Lys/Gln + Gln/Gln) for XPD codon 751 genetic polymorphism might have increased the risk of ESCC, but have no association with EAC.

Objective To examine the association between CYP1A1 polymorphisms (MspI and Ile/Val) and esophageal cancer (EC) by systematically reviewing the risk of the original studies. Methods Data from 16 papers (8 for MspI, 14 for Ile/Val) regarding case-control studies on the association of cytochrome P450 polymorphisms and risk of esophageal cancer was analyzed by dominant model (variant genotype vs. wild-type genotype) through meta-analysis. Stratified analysis was carried out according to the pathological types. Results In systematical analysis, CYP1A1 MspI variant genotype (TC+CC) had no association with EC risk (OR=1.17,95%CI: 0.82-1.66). Similar results were observed in esophageal squamous-cell carcinoma(ESCC) (OR=1.17,95%CI: 0.82-1.69) and esophageal adenocarcinoma (EAC) (OR=1.39,95% CI: 0.67-2.09). Individuals with the CYP1A1 Ile/Val variant genotype (Ile/Val + Val/Val) had an increased risk for EC, when comparing with wild type (Iie/Iie ), with an OR of 1.39 (95 %CI: 1.07-1.80). CYP1A1 Ile/Val variant genotype could increase the risk of ESCC (OR=1.43,95%CI:1.07-1.91) but no significant association was found with EAC (OR=1.20,95%CI:0.62-2.30). Conclusion CYP1A1 gene polymorphism Ile/Val might have played a role in the development of ESCC but CYP1A1 MspI polymorphism might not be associated with the susceptibility of EC.

Objective To evaluate the effect of 3D printed skeletal model in orthopedic clinical in-ternship teaching . Methods From October 2015 to October 2016 , 30 undergraduate clinical medical internship students were divided into traditional teaching group and 3D model teaching group randomly. 1 teaching ward round was conducted for each student. Teaching rounds were conducted according to routine procedures in traditional teaching group, while the 3D model teaching group used the patient's 3D print skeletal model to assist in teaching rounds and perform simulated surgery. All students were tested in theory and practice, and the teaching evaluation was carried out by questionnaire. Data were imputed into SPSS 19.0 for comparison between the two groups using t-test. Results Students in 3D model teaching group significantly outperformed than those in traditional teaching group in theoretical achievement [(92.27±4.56) vs. (86.87±5.30)] and practical achievement [(90.07±4.61) vs. (82.53±5.03)] (P<0.01). In respects of pro-moting theoretical knowledge, enhancing surgical participation, hands-on ability exercise, teaching attraction and overall satisfaction, students in 3D model teaching group gave a better evaluation of teaching they had received than those in the traditional teaching group (P<0.01). Conclusion 3D printed skeletal model is an effective teaching tool, and its application in clinical internship teaching at basic-level hospital can signifi-cantly improve teaching effectiveness and satisfaction. It is worth further exploration and promotion.
