1.Clinical application of microsurgical shunt in treatment or varicocele with oligospermia

Objective To study the therapeutic efficacy and the application value of microsurgical shunt in varicocele (VC) with oligospermia. Methods Funicular vein elevatus ligation and microsurgical shunt were applied in 66 patients. Improvements of the symptom, sign,semen quality and the couple's pregnant rate in prognosis visiting were observed. Results in 71 sides by microsurgical shunt,symptom disappeared in 51 cases and obviously improved in 20 cases after 7 days. Half and one year later,symptom and sign were disappeared in 69 cases (97% );semen quality was improved in 52 cases (78% ). In 37 cases (56. 3% ),sperm density was ≥20 ? 109/L, progress sperm was ≥50% and morphologically normal sperm was ≥30 %. In 11 cases (16. 7% ),sperm density was increased from 5 ? 10,/L- 15 ? 109/L before operation to 10 ? 109/L- 19 ? 109/L after operation,and both of the sperm motility and viability were increased. In 4 cases (5% ),sperm density was increased from less than 5 ? 10s/L before operation to more than 5 ? 10,/L after operation. In 26 cases,the couple was pregnant;the pregnant rate was 39. 4%' Conclusion Vein elevatus ligation and microsurgical shunt blocked the vein blood reflux,established the new vein circuit and improved the blood supply of testis in the mean time. It was approved that the method was effective in diminishing the local symptom and sign, improving semen quality and preventing recurrence' It was especially suitable for classⅢ exudative VC patients
2.Clinical study of LigaSure vessel sealing system for surgery in gastroenteric cancer

Objective To investigate the value of LigaSure vessel sealing system for surgery in gastroenteric cancer. Methods From January to September 2009,44 cases of gastroenteric cancer were operated by using LigaSure vessel sealing system,including 13 cases of gastric cancer, 14 cases of colon cancer, 17 cases of rectum cancer. Laparoscopy was employed in 10 cases. Results All the cases were performed successfully. The operation time was 130-250 min, the blood loss was 50-250 ml, and the number of lymph nodes resected was 4-20. None of the cases had serious complications. Conclusion As an optimal hemostatic tool, LigaSure vessel sealing system is safe and feasible for gastroenteric cancer.
4.Experimental study on inhibitory effect of wogonin on proliferation and invasion of breast cancer cells.

OBJECTIVETo study the inhibitory effect of wogonin on the growth and proliferation of breast cancer cells MDA-MB-23, and observe its effect on the adhesion, migration and invasion of MDA-MB-23 cells, in order to further study its molecular mechanism.
METHODMTT assay was used to detect the effect of wogonin on MDA-MB-23 cell growth. Ki-67 assay was adopted to test the effect of wogonin on cell proliferation. Scratch test, adherence test and invasion chamber assay were taken to detect the effect on the migration and invasion abilities of MDA-MB-231 cells. Proliferation and metastasis-related proteins and relevant signaling pathways were detected by Western blotting.
RESULTWogonin could remarkably inhibit the growth and proliferation of MDA-MB-231 cells, significantly inhibit migration, adhesion and invasion abilities of breast cancer cells at a low concentration, and effectively inhibit the expression of Survivin, Bcl-2, ICAM-1, MMP-2, MMP-9 proteins of MDA-MB-231 cells.
CONCLUSIONWogonin could notably inhibit growth and proliferation of breast cancer cells, and inhibit migration, adhesion and invasion of MDA-MB-231 cells. Its invasive and adhesive effects on MDA-MB-231 cells may be related to the decrease in ICAM-1, MMP-2, MMP-9 expressions.

5.Application of artificial pneumothorax combined with intercostal nerve block in alleviating chest pain during percutaneous microwave ablation for subpleural lung malignancy: analysis of therapeutic efficacy

Objective To discuss the clinical effect of artificial pneumothorax combined with intercostal nerve block in alleviating chest pain occurring during and after percutaneous microwave ablation (MWA) for subpleural lung malignancy.Methods A total of 30 patients with subpleural lung malignancy were randomly and equally divided into group A (n=10),group B (n=10) and group C (n=10).The patients in group A received both artificial pneumothorax and intercostal nerve block before MWA.The patients in group B only received artificial pneumothorax before MWA,and the patients in group C only received intercostal nerve block before MWA.The degree of pain was evaluated by visual analogue scale (VAS) score during MWA,immediately after MWA and at 6 h,12 h and 24 h after WMA.The side effects after MWA were recorded.Results During MWA,no statistically significant differences in VAS scores existed between each other among the three groups (P=0.885).The VAS scores determined at 6 h,12 h and 24 h after MWA in group C were significantly increased (P=0.014,P=0.006 and P=0.006 respectively).No patient in group A and group B developed symptoms of chest tightness after artificial pneumothorax was performed.After treatment,a small amount of asymptomatic residual pneumothorax was still observed in 6 patients of group A and group B,which disappeared spontaneously in about one week.Another patient still showed massive pneumothorax even after thoracic gas suction,and the patient recovered after thoracic closed drainage for three days.No other serious complications related to artificial pneumothorax occurred.Conclusion Artificial pneumothorax combined with intercostal nerve block can effectively relieve the chest pain occurring during and after MWA in patients with subpleural lung malignancy,and clinically this technique is quite safe.(J Intervent Radiol,2017,26:269-273)

7.Transcriptome analysis of bioenergy plant Miscanthus sinensis Anderss by RNA-Seq.

Miscanthus sinensis Anderss is a perennial C4-grass. It is a promising bioenergy plant, which has been proposed as general feedstock for biomass and lignocellulosic biofuel production. In this study, the flower and leaf buds transcriptomes of Miscanthus sinensis Anderss were sequenced by the platform of Illumina HiSeq 2000. In total 98 326 Unigenes were generated by de novo assembly with an average length of 822 bp and N50 of 1 023 bp. Based on the NR, NT, Swiss-Prot, KEGG, GO and COG databases (Evalue < le-5), 74 134 (75.40%) Unigenes were annotated. A total of 45 507 Unigenes were mapped into different GO terms. In KEGG pathways identification, 36 710 sequences were assigned to 128 KEGG pathways. Sorghum bicolor (37 731, 60.86%), Zea mays (16 258, 26.22%), and Oryza sativa (3 065, 4.94%) showed high similarity to Miscanthus sinensis Anderss. And 24 photosynthesis-related enzyme genes were identified. The result provides a foundation for further characterizing the functional genes in Miscanthus sinensis Anderss.

8.Application of high hydrostatic pressure in development of melanoma vaccine

Objective: To explore the application of high hydrostatic pressure in the development of melanoma vaccine.Methods:The high hydrostatic pressure,liquid nitrogen freezing and thawing were used to break the murine B16 melanoma cells and then the cell suspension was mixed with Freund's adjuvant to prepare vaccine for immunizing the mice.Results:After immu-nization,the murine B16 melanoma cells were injected intravenously and subcutaneously.The immune results of the vaccines were evaluated,by comparing survival time of the mice, subcutaneous tumor volume, DTH experiments and the fluorescence imaging of tumors.Conclusion:Compared with the method of liquid nitrogen freeze-thaw broken,high hydrostatic pressure crushing cells has more advantages in the development of tumor vaccine.
9.Effect of early enteral nutrition with ω-3 polyunsaturated fatty acid on nutritional status and body immunity of postoperative patients with colorectal cancer

Objective To study the effect of early enteral nutrition (EEN) with ω-3 polyunsaturated fatty acid on nutritional status and body immunity of postoperative patients with colorectal cancer.Methods Forty patients with colorectal cancer were randomly divided into EEN group and parenteral nutrition (PN) group by systematic sampling method with 20 cases of each.The patients in EEN group were given complete enteral nutrition with ω-3 polyunsaturated fatty acid 1-7 d after operation,while the patients in PN group only were given nutrient by regular PN.Nutritional status,body immunity before operation and 1-7 d after operation,and intestinal function recovery time,untoward reaction were determined.Results The postoperative anus first exhaust time and defecate time of patients in EEN group were significantly shorter than those in PN group [(48.0±5.6) hvs.(51.0±6.7) h,(53.0±4.8) h vs.(72.0±3.6) h,P<0.05].Serum albumin and prealbumin 1 d after operation in 2 groups were significantly lower than before operation (P <0.05),but 7 d after operation were significantly higher than 1 d after operation (P <0.05).Serum albumin and prealbumin 7 d after operation in EEN group were significantly higher than PN group(P < 0.05).IgG,IgA,IgM,CD3,CD4,CD8,CD4/CD8 1 d after operation in 2 groups were significantly lower than before operation (P < 0.05).IgG,IgA,CD4,CD4/CD8 7 d after operation in EEN group were significantly higher than PN group (P< 0.05).Conclusions Compared with the PN,EEN with ω-3 polyunsaturated fatty acid can remarkably improve nutritional status and body immunity in postoperative patients with colorectal cancer.It can accelerate the recovery of intestine function and worthy of recommending.
10.Operation skills of endoscopic retrograde cholangiopancreatography in pancreaticobiliary patients with periampullary duodenal diverticula

ObjectiveTo investigate the operation skills of endoscopic retrograde cholangiopancreatography (ERCP) in patients with periampullary duodenal diverticula. MethodsA retrospective analysis was performed for the clinical data of 867 patients with periampullary duodenal diverticula who underwent ERCP in Department of Hepatobiliary Endoscopy in Shandong Jiaotong Hospital from March 2013 to February 2015. The rate of successful intubation, therapeutic effect, and development of complications were analyzed. ResultsThe rate of successful intubation was 99.8%. Of all patients, 793 underwent successful intubation with an incision knife, 44 were treated with double guide wires and pancreatic duct stenting, 3 underwent fixation of the papilla with hemostatic clips, 25 underwent the pre-cut technique, and 2 underwent failed intubation. There were 5 cases of pancreatitis, l case of perforation, l case of delayed bleeding, and all the patients were cured after conservative treatment. ConclusionWhen ERCP is used in the treatment of pancreaticobiliary diseases complicated by periampullary duodenal diverticula, the indications for ERCP and endoscopic sphincterotomy should be clarified. Targeted operation skills selected based on the features of the papilla help to improve the success rate and reduce complications, and therefore, it is a safe minimally invasive therapy.
