1.Comparison of MELD score and Child-Pugh grading in patients with liver Cirrhosis

Objective To compare the differences between MELD and Child-Pugh grading in assessing of hepatic functional reserve in patients with cirrhosis,and analyse the correlation of two systems.Methods Retrospective analysis was made on 92 cases of hepatitis cirrhosis hospitalized with the MELD and Child-Pugh score by ROC curve,the difference and relation between two systems were compared.Results MELD and Child-Pugh system were significantly correlated(r=0.669,P < 0.000),3-month survival in patients with two systems ROC curve areas was significantly different(P < 0.05),while the MELD in patients survived more than 6 months was not significantly different compared with Child-Pugh(P>0.05).ConclusionMELD and Child-Pugh can accurately assess the prognosis of survival for patients with hepatic cirrhosis,and MELD is suitable for assessment of risk,heavy patients,not for those survived more than 6 months.
2.Current pathologic diagnosis and management of Helicobacter pylori-associated gastritis and gastric marginal zone lymphoma with a case report

Helicobacter pylori (H.pylori) associated gastritis is one of the most common infectious diseases in China and worldwide.Gastric marginal zone lymphoma,or mucosa associated lymphoid tissue (MALT) lymphoma is a rare neoplasm associated with H.pylori infection and the first neoplastic disease curable by antibiotics therapy alone.The pathologic diagnosis of gastric MALT lymphoma can be reached by histologic examination,immunohistochemical staining,and B-cell clonality studies.Eradication against H.pylori is the choice of therapy for early stage gastric MALT lymphoma.Refractory,localized disease may be treated with low-dose irradiation therapy with high rates of responding and long-term survival.Systemic chemotherapy is recommended for advanced stage gastric MALT lymphoma or high-grade transformed large B-cell lymphoma.A recent case with recurrent chronic active gastritis and gastric MALT lymphoma after H.pylori eradication therapy was reported.Recent advances in the studies of pathologic diagnosis and therapy of the disease were reviewed.
3.A novel protein peptide associated with ischemic heart disease: dermcidin

Dermcidin (DCD) was found in isolated human skin sweat glands with antimicrobial effect, and was defined as a kind of new small molecule antimicrobial peptide. It was a part of human sweat glands in the skin as the immune system's innate defense. With the studies of DCD, its extensive biological functions are gradually discovered. Since 2010, a number of studies have shown that DCD may be a new risk factor for atherosclerosis. And the role of DCD in ischemic heart disease has drawn increasing attention in particular its relationship with insulin secretion and glycemic control, nitric oxide (NO) synthesis and hypertension, platelet aggregation and acute myocardial infarction (AMI). In those experiments, it was also confirmed that aspirin had antagonistic and reverse effects on various biological functions of DCD. Further research on the role of DCD in cardiovascular and cerebrovascular diseases may lead to the prevention, early warning, prognosis evaluation and treatment breakthrough of cardiovascular and cerebrovascular events.
4.Analysis of the relationship between serum HBV-DNA And hepatic fibrosis

Objective To analyze the relationship between serum HBV-DNA and hepatic fibrosis in patients with hepatitis B. Methods Serum HBV-DNA load and hepatic fibrosis hyaluronic acid (HA),Ⅲ procollagen N terminal peptide(PⅢNP),laminin (LN),Ⅳ collagen(Ⅳ-C)of 154 cases were detected by real-time fluorescence quantitative PCR and chemiluminescence method. Statistical analysis of the correlation between HBV-DNA load and hepatic fibrosis indicator was performed.Results The data of HBV-DNA load after taking lg value was 5.49±1.39;The serum concentration of hepatic fibrosis HA was (166.95±148.20)ng/mL,PⅢNP was (12.14±7.37)ng/mL,Ⅳ-C was (65.29±45.48)ng/mL,LN was (59.52±35.67)ng/mL.The difference between experimental group and control group in hepatic fibrosis was significant(P <0.05).Serum HBV-DNA only correlated with LN(r=0.284,P =0.015).Conclusion There is no significant correlation between serum HBV-DNA and hepatic fibrosis in hepatitis B pa-tients.Anti-fibrosis treatment should also be strengthened for hepatitis B patients in the course of antiviral treatment.
5.Native Assessment to the Montreal Battery of Musical Evaluation

Objective: This research carried out the native assessment to the Montreal Battery of Musical Evaluation, including MBEA and Musical emotion test. Methods: 60 normals were tested by MBEA (including 30 university students). Also 40 students were tested by musical emotion recognition battery. The correct rates were recorded and compared with western normals’data. Results: There were significant differences in pitch judgments and musical emotion recognition be- tween Chinese and Westerners, while rhythm perception and memory of music did not have significant difference between the two groups. Conclusion: Culture difference mainly influences the musical pitch perception in brain but rhythm. Chinese have its own understanding of western musical excerpts. Although MBEA is wildly accepted abroad, native analogy testing tool is acquired.

7.Efficacy of Letrozole in the Treatment of Breast Cancer in Postmenopausal Women:A Systematic Review

OBJECTIVE:To evaluate efficacy of letrozole in the treatment of breast cancer in postmenopausal women. METHODS:RCTs about letrozole in the treatment of breast cancer in postmenopausal women were collected using Cochrane systematic evaluation method by retrieving from Cochrane Library,PubMed,CBMdisc and CNKI. The quality evaluation of those trials and data extraction were performed and Meta-analysis was conducted by RevMan 5.0 software. RESULTS:6 RCTs involving 6 316 patients were included. In neoadjuvant endocrine therapy,letrozole was more effective than tamoxifen in PFS but there was no statistical significance in difference of total survival rate;in rescue therapy for recurrence and transform,letrozole was more effective than long-term therapy of tamoxifen in OS,PFS and OR;in new neoadjuvant endocrine therapy before operation,letrozole was more effective than tamoxifen in OR. CONCLUSION:The current evidence shows that letrozole is more effective than tamoxifen for breast cancer in postmenopausal women.More high-quality clinical trials are expected for further study.
8. Treatment of displaced intra-articular calcaneal fractures with cannulated screws and plates: A metaanalysis of 15 randomized controlled trials

OBJECTIVE: Displaced intra-articular calcaneal fractures are the most common type of calcaneal fracture. Cannulated screw fixation and plate fixation are commonly used methods. However, the differences in therapeutic effectiveness between cannulated screw fixation and plate fixation are still unclear. Thus, this study compared the effect of percutaneous cannulated screw fixation and plate fixation in the treatment of intra-articular calcaneal fractures. METHODS: The authors searched for randomized controlled trials on displaced intra-articular calcaneal fractures fixated with cannulated screws or plates in the following electronic databases: PubMed, Cochrane Library, Embase, Web of Science, and CNKI. The retrieval time limit was from the establishment of the database to December 2019. The retrieved literatures were screened according to inclusion and exclusion criteria. Quality evaluation was performed to extract effective data. Meta-analysis was conducted using RevMan 5.3 software. RESULTS: Fifteen randomized controlled studies were included with 1 438 patients with calcaneal fractures. The risk bias evaluation of the included literature showed that the overall quality of literature was above average. Meta-analysis results show that (1) plate fixation group was better than cannulated screw fixation group in terms of Bohler’s angle (MD=0.69, 95%CI:0.27-1.10, P=0.001) and Gissane’s angle (MD=0.95, 95%CI:0.16-1.74, P=0.02) (P < 0.05). (2) Compared with plate fixation, cannulated screw fixation showed a significant reduction in the operation time (MD=-23.92, 95%CI:-25.40 to-22.44, P < 0.000 01) and postoperative complications (OR=0.28, 95%CI:0.19-0.41, P < 0.000 01) (P < 0.05). (3) There was no statistically significant difference between the cannulated screw fixation group and the plate fixation group in terms of calcaneal height (MD=0.43, 95%CI:-0.17-1.03, P=0.16) and AOFAS score (MD=0.61, 95%CI:-0.35-1.58, P=0.21). CONCLUSION: Above data concluded that Bohler’s and Gissane’s angles in the plate fixation group are better than those in the cannulated screw fixation group, while the cannulated screw fixation technology has the advantages of short operation time and low complication rate.
10.The ambitious but achievable goal: to end preventable childhood deaths due to pneumonia by 2025.

Globally, pneumonia is the leading cause of childhood mortality. Pneumonia is the second killer of children less than 5 years of age in China. The World Heath Organization and United Nations Children′s Fund launched the integrated Global Action Plan for the Prevention and Control of Pneumonia and Diarrhea (GAPPD) in 2013. The ambitious goal is to end preventable childhood deaths due to pneumonia by 2025. Countries or regions should achieve the following goals: (1) reduce mortality from pneumonia in children less than 5 years of age to fewer than 3 per 1 000 live births; (2) reduce the incidence of severe pneumonia by 75% in children less than 5 years of age compared to 2010 levels. If the implementation of key interventions is accelerated, the mortality rate of childhood pneumonia will drop substantially every year, which makes the goal achievable.
