OBJECTIVETo summarize laws of acupoint selection of prescriptions for treatment of cervicogenic headache by acupuncture in modern literature.

METHODSRandomized controlled trials (RCTs) involving acupuncture and moxibustion for treatment of cervicogenic headache were recruited from CBM (1978-2012), VIP (1989-2012), Wanfang Database (1998-2012), CNKI (1979-2012), PubMed (1966-2012), EMbase (1980-2012), and Cochrane Library (Volume 4, 2012). Hand recruitment was also auxiliarily used. The frequency and percentage of common acupoints, the distribution of acupoints along 14 meridians and across each part of the body, the application of specific acupoints, and features of using prescriptions for specific acupoints were statistically described.

RESULTSTotally 37 recruited papers included 42 acupoints and 159 times. Common acupoints covered Fengchi (GB20, 28 times), Jingjiaji (EX-B2, 21 times), Baihui (DU 20, 12 times), Tianzhu (BL9, 1 times), and Ashi point (11 times). Meridians along which acupoints were used mainly covered Foot-shaoyang Gallbladder Meridian, Foot-taiyang Bladder Meridian,and DU meridian. Acupoints were mainly needled from head, neck, and upper limbs. Eight confluence points and luo-connecting point were commonest used as specific acupoints. Acupuncture prescriptions were mostly composed of multiple acupoints. Filliform needle was mainly used in acupuncture methods, followed by electro-acupuncture needle.

CONCLUSIONSModern acupuncture treatment of cervicogenic headache focuses on local specific points and acupoints along meridians. Acupoints were mostly selected from head, neck, and upper limbs by syndrome typing of Chinese medicine.

To investigate the potential use of suicide gene systems in vascular targeting gene therapy, HSV-tk/GCV and EC-cd/5-FC systems were established on vascular endothelial cells in vitro by adenovirus transduction. Both modified cell lines were highly sensitive to prodrugs, the IC_(50) for GCV was 0.2?mol/L in IGll/Ad-tk cells, and IC_(50) for 5-FC was 20?mol/L in IGll/Ad-cd cells, while the parental endothelial cells were insensitive even at the highest concentrations of prodrugs in the experiment. Mixed cellular assay showed that significant bystander killing effect was exhibited in modified endothelial cells. Also, the apoptosis involved in these prodrug-mediated growth inhibitions has been shown under electron microscopy. These results indicated that both HSV-tk/GCV and EC-cd/5-FC systems could efficiently suppress vascular endothelial cell growth in vitro , suggesting the feasibility of using suicide gene in vascular targeting gene therapy.

OBJECTIVE: To investigate the pathological features and significance of myocardial contraction band necrosis in sudden cardiac death. METHODS: Using HE and PTAH staining, the distribution sites and pathological features of myocardial contraction band necrosis were observed. The data were analyzed according to the extent of necrosis. RESULTS: The locations, pathological features and the extent of necrosis were similar in different sudden death cases. The locations were related with sites of myocardial damage. The papillary muscles of left ventricular were most occurred, followed by the anterior wall of left ventricular. In the sudden death cases caused by stress cardiomyopathy, the extent of myocardial contraction band necrosis was lighter than the others. Most cases were predominantly level 1, the others were level 2. CONCLUSION Used as the diagnosis basis of acute myocardial ischemia, the myocardial contraction band necrosis has important significance to determination of death.

Objective To investigate the intracranial vascular lesions in patients with subcortical infarction-induced vascular cognitive impairment(VCI), and to investigate the etiology and mechanisms of VCI. Methods Inpatients with subcortical infarction in our hospital were enrolled in this study from Nov. 2012 to Feb. 2014, with those unable to complete the cognitive evaluation eliminated. According to the diagnostic criteria of VCI the patients were divided into two groups: 49 with cognitive impairment (VCI group) and 42 without cognitive impairment (NVCI group). The clinical data, physical examinations, laboratory tests, and the Montreal Cognitive Assessment (MoCA) scale scores were collected. Cerebral blood vessels were assessed by CT angiography or magnetic resonance angiography (MRA). Results The results showed that, according to TOAST classification, VCI group had 24 (48.98%) patients with large artery atherosclerosis (LAA) and NVCI group had 22 (52.38%), showing no significant difference between the two groups. CTA or MRA indicated that 37 (75.51%) patients in VCI group had vascular stenosis, with 75.25% of the 37 patients having intracranial vascular stenosis and 28.71% with middle cerebral artery stenosis. Patients with single cerebral artery stenosis accounted for 18.37% and those with the multiple artery stenosis accounted for 57.14%. CTA or MRA indicated that 34 (80.95%) patients in NVCI group had vascular stenosis, with 60% having intracranial vascular stenosis, including 32% with middle cerebral artery stenosis, 26.19% with single cerebral artery stenosis, and 54.76% with the multiple artery stenosis, with the latter two data being significantly different from the VCI group (P <0.05). In VCI group, the number of intracranial vascular stenosis branches ranged 0-6, with a mean of (1.51±1.67), and a negative relation was found between numbers of stenosis arteries and MoCA scores in patients of VCI group (rs=-0.283, P <0.05). Conclusion Different from the common causes of small blood vessels, LAA is the most common etiology of subcortical VCI, which implied that exploring the LAA causes of small lesions is crucial for the prevention of VCI in Chinese patients.

Objective To observe the effect of the application of peripherally inserted central catheter (PICC) network platform in the information management of the patients with PICC.Methods Altogether 17 254 outpatients receiving PICC maintenance in Peking Union Medical College Hospital from April 2012 to April 2013 were enrolled as the control group,including 7 227 males and 10 027 females,with the median age of 58 years (12-85 years).A total of 20 384 outpatients from April 2013 to April 2014 with PICC were selected as the observation group,including 8 188 males and 12 196 females,~th the median age of 59 years (13-86 years).Those patients all received PICC maintenance in outpatient clinic during the intermission of therapy after PICC insertion.The time of data entry,the integrity of the data,description accuracy about complications,and normalization of wording were compared between the two groups.Results The average time of data entry in the control group was (46 ± 6) seconds,significantly longer than that in the observation group [(12 ± 5) seconds,t =562.660,P ＜ 0.05].In terms of the integrity of the data,there were 11 732 cases of complete data,3 623 cases of less complete data,and 1 899 cases of incomplete data in the control group;while the numbers of cases of complete data,less complete data,and incomplete data in the observation group were 19 568,725,and 91,respectively,showing significant difference compared with the control group (x2 =5 312.000,P ＜ 0.05).In the description accuracy about complications,the control group had 11 840 accurately described cases and 5 414 inaccurately described cases,while the observation group had 18 427 accurately described cases and 1 957 inaccurately described cases (x2 =2 814.000,P ＜ 0.05).The wording was standard in 15 280 cases but not standard in 1 974 cases in the control group,and standard in 19 659 cases and not standard in 725 cases in the observation group,with significant inter-group difference (x2 =872.600,P ＜0.05).Conclusion Simple and convenient data summary could help quality control and quality analysis,preferably guarantee the safety of catheter insertion and reduce the incidence of complications.

Objective To analyze the influence of age on the diagnosis of acute heart failure (AHF) with amino terminal brain natriuretic peptide (NT‐proBNP) ,and to explore the best cut‐off value of NT‐proBNP in the diagnosis of AHF .Methods Totally 457 inpatients suspected with AHF obtained a blood NT‐proBNP measurement within 24 hours when in charged ,including 194 patients with AHF .Patients were divided into three groups according to their age :≤50 years ,>50-70 years and >70 years . The relationship between NT‐proBNPand AHF was analyzed .The diagnostic value of NT‐proBNP was evaluated for AHF in differ‐ent age groups .Results The median NT‐proBNP levels of AHF in all age groups were significantly higher than those without (P<0 .01) .AUC of NT‐proBNP for AHF in ≤50 years ,>50-70 years and >70 years groups were 0 .95 ,0 .91 ,0 .88 ,the best diagnos‐tic cut‐off points were 1 100 ,1 800 ,3 000 pg/mL ,respectively .The diagnostic specificity was 88% ,76% and 73% ,respectively .O‐verall ,the specificity of NT‐proBNP cut‐off values for age group was higher than non‐age groups .Conclusion NT‐proBNP has a good value in the diagnosis of AHF in patients with different ages .According to the age group ,the different NT‐proBNP cut‐off values can be used to improve the specificity of diagnosis .
