ObjectiveTo investigate the effort of MHA-bBMP implantation combined with limited contacted titanium net duct fixation on femoral shaft bone defect of rabbit.Methods48 rabbits were divided into the experimental group (treated with bionic bone MHA-bBMP plus titanium net duck) and control group (treated with iliac autograft plus titanium net duct) with 24 animals in each group. The rabbit femoral shaft bone defect model was established by cutting 10 mm bone fragment off. After operation, bionic bone MHA-bBMP/iliac autograft was implanted into bone defect area and fixed with limited contacted titanium net duck. The general condition, serum alkaline phosphatase, X-ray, histopathologic examination and electron microscope were performed.ResultsThe fixation stability of titanium net duck in two groups was good. The bone defect of two groups was repaired. The results of phosphatase, X-ray, histopathologic examination and electron microscope of two groups were not significantly different.ConclusionBionic bone MHA-bBMP is a high bioactivity substitute, and can obtain therapeutic effect equal to iliac autograft when repairing rabbit's femoral shaft defect.

Chinese Journal of Dermatology 2014;47(3):163-165
Three cases of medallion-like dermal dendrocyte hamartoma are reported. The patients included two men and one woman, and were aged from 2 to 38 years. All the patients presented with medallion-shaped,welldefined,slightly atrophic and asymptomatic congenital patches with prominently dilated superficial veins that had grown proportionately with the body length.Histological examination revealed similar changes in all of these patients, including epidermal atrophy and dermal proliferation of spindle cells, which stained positive for CD34,factor ⅩⅢa and vimentin, but negative for S100,CD68,CDla,CD31 and smooth muscle actin (SMA).None of the patients received special therapy, and the lesions enlarged slightly with body growth within 3-4 years of follow-up after the diagnosis.

Chinese Journal of Dermatology 2012;45(10):693-696
A case of the H syndrome was reported. An 18-year-old man presented with an 8-year history of progressive cutaneous sclerosis, skin hyperpigmentation, and hypertrichosis on the trunk and extremities. In addition,he suffered from short stature, hearing loss, cardiac anomaly, hepatosplenomegaly, and scrotal masses.Laboratory examination revealed hypogonadism.Cutaneous histopathologic examination showed hyperpigmentation of the basal layer with acanthosis, widespread fibrosis (moderate in dermis and severe in subcutis) and an infiltrate composed of histiocytes and plasma cells.A perivascular lymphocyte infiltrate was also observed throughout the dermis and subcutaneous fat tissue. These findings were consistent with the H syndrome.Differential diagnosis must be made with Winchester syndrome and polyneuropathy, organomegaly,endocrinopathy,M-protein and skin change (POEMS) syndrome.

Objective To investigate the replantation of complete severed fingertip when vein can't be repaired and only artery be repaired. Methods All 13 cases complete severed fingertips were replantated with only artery anastomosis but vein cannot be repaired.Low molecular weight heparin was applied to prevent blood clotting postoperation. Blood-letting by nail extraction or latero-incision were done in some cases. The survival fingers were observed. Results After 3-12 months followed-up,12 cases survived with satisfactory appearance and locomotion,2-PD of 2-7 mm,but 1 case was failed. Conclusion The replanted fingertip may survival with only artery anastomosis because of fewer tissue and faster recontruction of venous return. It would raise the survival rate of replanted fingertip by blood-letting by nail extraction or latero-incision according to the traumatic condition.

BACKGROUND:The development of minimaly invasive technology significantly reduces the occurrence of surgical complications due to spinal metastases. Currently, the minimaly invasive treatment basicaly has three broad categories: vertebral cement augmentation, radiofrequency ablation combined with vertebroplasty, intraoperative radiotherapy combined with vertebroplasty. OBJECTIVE:To summarize the research progress of three kinds of minimaly invasive treatments for spinal metastases. METHODS:PubMed and Wanfang databases were searched using the keywords of spinal metastases, vertebroplasty, radiofrequency ablation, radiotherapy in English and Chinese, respectively. RESULTS AND CONCLUSION:Vertebral cement augmentation with good analgesic effect has been used widely, and the efficiency is up to 80%-90%. But its effect to kil tumors is very limited that is unable to control tumor growth. Radiofrequency ablation and radiation can kil the tumor, but cannot rebuild the vertebral stability. Therefore, the combination of different technologies can improve the therapeutic effect on spinal tumors. In recent years, intraoperative radiation and implantation of radioactive particles or radioactive bone cement have been developed as new technologies. However, there is no conclusion that these new technologies have better outcomes than the vertebral cement augmentation because of less reports and lack of long-term folow-up. Especialy in the metastatic patients with damaged vertebral posterior wal and tumors invaded into vertebral canal, the risk of nerve function damage caused by bone cement leakage is stil very high, even after many attempts. Above al, there is no a perfect minimaly invasive treatment for spinal metastases.

Purpose: To analyze the effect of the intraarticufar contact of the temporomandibular joint ( TMJ ) on three dimensional nonlinear simulation of human TMJ forces in order to simulate the distribution and transfer of TMJ forces more realistically and logically. Materials and Methods: Combining helical CT scanning technology with three dimensional finite element method, together with Auto CAD software, three dimensional nonlinear finite element models were developed in vivo for comparison with the changes in the stress within the TMJ in different contact conditions. Results: The stresses on the surfaces of articular disc and condyle in the finite element models with contact elements between the disc and condyle were mainly distributed on their anterior and medial regions, and the stresses on other regions were relatively smaller. But, the stresses of those models without contact element were mainly distributed on their midposterior and medial aspacts. Moreover, the stress values were higher. When the friction coefficient was in the range of 0 0.15, the distribution extent and magnitude of the contact stress between the disc and the condyle in the TMJ were completely same. Meanwhile, the surface stresses on the disc and the condyle were nearly coincident in three ways of disc condyle contact, which were no friction contact, rigid contact and elastic contact. Conclusion: In the issue of disc condyle contact of simulating TMJ forces with three dimensional nonlinear method, the stress distribution in the joint with complete contact relationship between the disc and the condyle conforms more to the physiological condition. The lower friction coefficient ( 0 0.15 ) in the TMJ and various contact patterns between the disc and condyle had no evident influence on the stress distribution in the TMJ. Three dimensional nonlinear analysis for the forces in the TMJ might be more realistic and reasonable because of contact simulation among various structures within the TMJ.

Objective To explore the clinical value of u ltrasound-guided percutaneous aspiration and drainage for bacterial liver absces s in children. Methods Under the guidance of ultrasonography, 15 children with bacterial hepatic abscess underwent percutaneous aspiration or drainage. Among them, 9 were given percutaneous aspiration for 1~4 times, and 6 underwent percutaneous drainage. Results Percutaneous aspirati on or drainage was successfully accomplished in all the 15 children, 14 of whom were cured. The hospital stay was 15~42 days (mean, 26.3 days). No bleeding or b ile leakage happened. Follow-up for 4~12 months (mean, 6.8 months) in 11 childre n showed no recurrence. One patient with no improvement left hospital of his own accord. Conclusions Ultrasound-guided percutaneous aspiration and drainage is a feasible option for the treatment of liver abscess in childre n.

Objective To summarize our experience in choledochofiberscopic treatment for residual stones of biliary tract. Methods 1105 cases of residual stones of biliary tract diagnosed and treated with choledochofiberscope in the past 19 years were reviewed retrospectively. Results The 1067 patients with residual stones of bile duct were treated with choledochofiberscope for 1483 times altogether. The stones were completely removed in 1035 cases, while the stones were not cleared in other 32 cases. The clearance rate of residual stones was 97.0%(1035/1067). No patient died of choledochofiberscopic treatment. Conclusions Choledochofiberscopic treatment plays an important role in the treatment of residual stones of bile duct, because it not only reduces residual stones of bile duct to avoid repeated operation, but also has some advantages such as excellent safety, minimal invasion, low cost and quicker recovery.
