1.An innovated surgical strategy of coronoid process fracture combined with ipsilateral zygomatic arch fracture

Objective To explore an effective and safe therapeutic strategy in the treatment of coronoid process fracture com‐bined with ipsilateral zygomatic arch fracture .Methods Through the semi coronoid scalp incision ,`an open reduction and internal fixation of zygomatic arch fracture was done .The operation was modified that the area of zygomatic arch fracture was exposed ade‐quately ,and then the fracture fragments of zygomatic arch were turned up .The coronoid process fracture pieces were isolated and removed along with the direction of muscle fibers in the temporalis muscle .Results All Cases of the surgical incisions were healed by primary intention .After 3 - 24 months following up ,the function of mouth opening and closing ,and the other movements of mandible became normal .Conclusion Through the semi coronoid scalp incision ,zygomatic arch fracture reduction and internal fixa‐tion and the coronoid process fracture pieces removing can be done simultaneously .In this way ,an effective and safe therapeutic strategy for treating coronoid process fracture combined with ipsilateral zygomatic arch fracture .
2.Advance in studies on regulatory mechanism of phenotype polarization of macrophages and intervention with traditional Chinese medicines.

Macrophages are heterogeneous and diversified, and can be polarized into different phenotypes in various microenvironments and physiological or pathological conditions. Major macrophage subpopulations including classically activated(M1) and alternatively activated(M2) macrophages, which represent different surface receptors, secret different cytokines and chemokines, are regulated by different signal paths of transcriptions and epigenetic levels, and play distinctive roles in tumor progress. TCMs may improve the microenvironment by regulating phenotype polarization of macrophages. So far, specific biomarkers and polarized molecules mechanisms generated through the macrophage polarization approach are still unclear. In this article, we merely summarize the advance in domestic and foreign studies on phenotype polarization of macrophages and regulatory mechanisms and look into the future of intervention with TCMs.

3.Effect of Ultrasound-guided transversus abdominis plane block on cyclic stress and postoperative analgesia in patients after abdominal surgery

Objective To investigate the effect of ultrasound?guided transversus abdominis plane block after general anesthesia induction on cyclic stress and postoperative analgesia in patients treated with abdominal surgery. Methods Sixty patients scheduled for elective abdominal surgery were divided into 2 groups with 30 cases in each. All were treated with ultrasound?guided transversus abdominis plane block after general anesthesia induction, and 30 cases in observation group received ropivacaine ,while those in control group saline. Anesthesia maintained by propofol combined with remifentanil during surgery ,and postoperative analgesia by sufentanil. The effect of anesthesia and operation were compared. Results Compared with control group,observation group needed less time for analepsia (P < 0.05) ,and there were lower blood pressure and heart rate at 2 min after skin incision and immediately after surgery (P < 0.05). Less propofol and remifentanil were needed in surgery and less sufentanil after surgery in observation group (P < 0.05). The VAS pain score was lower 1 h,4 h,8 h and 12 h after surgery (P < 0.05) ,and there were less times for pressing analgesic pump (P < 0.05). Patients in observation group had higher comfort degree after surgery (P<0.05). Conclusion Ultrasound?guided transversus abdominis plane block after general anesthesia induction is helpful to reduce intraoperative anesthesia used for anesthesia maintenance , and can improve patients′comfort after surgery.
5.Nutritional risk screening for preoperative patients with different parts of colorectal cancer

Objective To investigate the impact of nutritional risk of preoperative patients with different parts of colorectal cancer. Methods 385 cases of surgical patients diagnosed with colorectal cancer were selected in the retrospective study in April 2008 to March 2009, Gastrointestinal Surgery Center of West China Hospital in Sichuan University. Patients were divided into the right colon cancer group, the left colon cancer group, the rectal cancer group, using internationally recognized NRS- 2002 Scale and serum nutrition indicators to assess their preoperative nutrition. Results NRS-2002 score in the right colon cancer group showed higher than the left colon and rectal cancer group, but no statistical difference between the latter two rates; serum concentrations of HGB, TP, ALB, A/G were lower in the right colon cancer group than the other two groups, but the concentrations of ALT, AST, Ca2+, PO43-showed no statistical difference among three groups. Conclusions The proximal colon (right colon cancer) were more susceptible to the risk of preoperative malnutrition, the patients with different parts of colorectal cancer should be given to the corresponding preoperative nutritional assessment and nutritional support.
6.Molecular cloning of the survivin gene promoter and its specific expression in the human laryngeal cancer Hep-2 cell line

Objective: To construct the eukaryotic expression vector pSurp-EGFP regulated by the survivin gene promoter and to detect the specific expression of the promoter in human laryngeal cancer Hep-2 cells by green fluorescent protein assay.Methods: Thesurvivin gene promoter was generated by polymerase chain reaction(PCR) and the CMV promoter of the pShuttle vector replaced by the survivin gene promoter to generate the plasmid pSurp.The three plasmids pShuttle,pSurp and pEGFP-C1 were respectively double-enzyme digested so as to produce the plasmids pCMV-EGFP and pSurp-EGFP carrying the CMV or survivin promoter.The purified pCMV-EGFP and pSurp-EGFP were transfected into Hep-2 cell and vascular endothelial cell ECV304 using liposome transfection reagent and the expressions of EGFP detected by the fluorescent microscope.Results: Thesurvivin gene promoter was successfully cloned by PCR,and thesurvivin gene promoter-regulated pSurp-EGFP was constructed.Green fluorescence was observed in Hep-2 cells but not in ECV304. Conclusion: The high specific activity of the survivin gene promoter in Hep-2 cells that we successfully constructed attributes to the studies of tumor specific gene therapy.

8.An empirical study on the output of scientific research papers supported by NSFC project in a teaching hospital in East China

Objective To explore the role of National Natural Science Foundation (NSFC) funding on improving medical research of China,based on the SCIE paper output of a teaching hospital in East China,in order to provide evidence-based decision-making basis for hospital management of scientific research and discipline construction.Methods SCIE output of a teaching hospital in East China during 2009-2015 was retrieved based on the web of science (WOS) platform.The papers funded by the NSFC were further analyzed.Results From 2009 to 2015,the output of SCIE of the hospital showed a steady upward trend,with an annual growth rate of 26.10%.Among them,the NSFC funded project output 835 papers SCIE,accounting for 46.62%oo of the total paper;and the absolute value of the output of the paper showed an upward trend year by year,an increase of 49.52%.The main subjects of the NSFC funded by the hospital focus on oncology,liver disease and digestive system diseases and cardiovascular and cardiovascular diseases,and the hospital's clinical status basically.The quality of the journals published by the NSFC SCIE project funded by the hospital was significantly improved.Conclusions NSFC funding plays an important role in improving the output and quality of SCIE papers in clinical medicine,which should be paid attention to in scientific research management and decision-making.
9.Study in the damage of endothelial function and administration recovery among different arteries during the developing progress of SHR

Aim To evaluate the endothelial dysfunction level of different arteries at different stages of SHR,and the recovery after administration.Methods SHR model was used,captopril(3.375 g·kg~(-1)·d~(-1)) was administered from week 7 to week 24 and the effects were observed continuously until 8 weeks post treatment.Pathological changes of aorta,mesenteric and apex cordis arteries were examined at the time points of 6,18,24,32 wk,and the endothelial-dependent relaxation of the former two preparations were tested by acetylcholine(ACh)(n=6).Results There were pathological changes in the thoracic aorta,mesenteric artery and arteriole at 18 wk,and aggravated along the age.The thoracic aorta demonstrated the most severe pathological changes appearing endothelial cells ablated and tunica media thickening.The significant decline of endothelium-dependent relaxation in aorta,and mesenteric arteries of SHR reflected an aging dependent change of vascular function with the most severe situation in the aorta(P=0.10,18 wk,P<0.01 24,32 wk);captopril increased the aorta vasodilatation of SHR at 18 wk time point,without the effect in mesenteric artery(P<0.05 vs SHR).Conclusions During the progress of SHR,endothelial damages have been observed in all three kinds of vasculatures together with the reduced endothelial-dependent relaxation.The aorta presents earlier and deeper damage than middle and small size vessels,and is sensitive towards the antihypertensive therapy such as the angiotensin converting enzyme inhibitor.

To study the mechanisms of total flavonoid from Glycyrrhizae Radix et Rhizoma (TFGR) and its ingredient isoliquiritigenin (ISL) on their regulation of M2 phenotype polarization of macrophages. IL-4 (60 μg x L(-1)) induced RAW264.7 cells for 6 h to establish the M2 macrophage model. TFGR and ISL restrained breast cancer cells migration with the aid of M2 macrophages in vitro. TFGR and ISL inhibited gene and protein expression of Arg-1, up-regulated gene of HO-1 and protein expression of iNOS, enhanced the expression of microRNA 155 and its target gene SHIP1, meanwhile down-regulated.the phosphorylation of STAT3 and STAT6. So TFGR and ISL were the bioactive fraction and ingredient in Glycyrrhizae Radix et Rhizoma to reverse M2 phenotype macrophages polarization. TFGR and ISL inhibited the promotion of M2 macrophages to breast cancer cells migration in vitro, STAT signal pathways and miR155 were partly involved.
