1.Research of Periplaneta americana L.reversing multi-drug resistance of hepatocellular carcinoma

Objective To investigate the effect of extracts from Periplaneta americana L.on reversing MDR in HepG2/ADM.Methods HepG2/ADM was induced by continuous stepwise exposer to ADM.To investigate the effect of extractive from Periplaneta americana L.on drug resistance-associated protein( P-gp、MRP、LRP和BCRP) by immunocytochemical method and RT-PCR method.Results HepG2/ADM cells to ADM shows a strong multiple drug resistance that is 10.4 times,For DDP,5-FU showed weak resistance.The extractive from Periplaneta americana L.can cut the expression of P-gp and MRP1 protein in HepG2/ADM from immunocytochemical method,and it has statistically significant.The extractive from Periplaneta americana L.can cut the expression of P-gp gene,and it has statistically significant.Conclusion The extract from Periplaneta americana L.can effectively reverse MDR in HepG2/ADM,which targets for P-gp and MRP1 protein and P-gp gene of HepG2/ADM cells.
2.Exploration of the Construction of National Regional Medical Center in Jiangxi Province from the Perspective of Stakeholder Groups

National regional medical center is a major measure to balance the distribution of high-quality medical resources.The construction of a large number of national regional medical centres in Jiangxi Province,with accurate disease selection,frequent consultation and scheduling,and rapid ideological unity.However,from the perspectives of interest groups such as the government,exporting hospitals,and dependent hospitals,there are still problems such as unclear top-level design,insufficient power of export and dependent hospitals.It is necessary to accelerate and improve the coordination mechanism,clarify the rights and responsibilities of the three parties,improve the evaluation and special shift system,optimize the green channel,strengthen the power of export hospitals,support the dislocation development of dependent hospitals,and create a sustainable model of bilateral cooperation and self-development of project hospitals.
3.Effects of mindfulness training on clinical efficacy in patients with nitrous oxide addiction

Objective:To study the effect of mindfulness-based training intervention on clinical efficacy in patients with nitrous oxide(laughing gas) addiction.Methods:From June 2019 to June 2020, sixty-six patients with nitrous oxide addiction in Beijing Gaoxin Hospital were selected and randomly divided into experimental group( n=33) and control group( n=33). The control group received Taijiquan training and physical training, while the experimental group added mindfulness-based training intervention on the basis of Taijiquan training and physical training.Symptom checklist 90 (SCL-90) scores and visual analog scales (VAS) craving scores were compared between the two groups at admission and 8 weeks after treatment.SPSS 22.0 software was used for statistical analysis.Independent sample t test and paired sample t test were used to compare the differences between groups and within groups. Results:(1)Before treatment, there were no significant differences in subscale scores of SCL-90 between the two groups except for depression factor((2.45±0.86), (2.03±0.46), t=2.474, P<0.05). After treatment, the subscale scores of somatization((1.38±0.35), (1.68±0.34), t=-3.656, P<0.05), phobic anxiety((1.49±0.37), (1.81±0.30), t=-3.993, P<0.05), paranoid ideation((1.50±0.47), (1.88±0.31), t=-3.898, P<0.05) and psychotism((1.34±0.54), (1.55±0.27), t=-3.094, P<0.05) of SCL-90 in the experimental group were significantly lower than those in the control group.(2)Before treatment, there was no significant difference in VAS craving score between the two groups( t=0.857, P=0.395). After treatment, the score of VAS in the experimental group was significantly lower than that in the control group( t=27.427, P<0.05). Conclusion:Mindfulness training intervention can effectively improve the clinical symptoms of patients with nitrous oxide addiction, which is worthy of clinical application.
4.Analysis of the efficacy of 131 I treatment for Graves′ hyperthyroidism with neutropenia

Objective:
To analyze the clinical features, the efficacy of131I therapy and its effect on granulopoiesis in Graves′ hyperthyroidism(hyperthyroidism) patients with neutropenia.
Methods:
144 hyperthyroidism patients were retrospectively studied after131I therapy, among which 42 cases(HT group) accompanied neutropenia due to hyperthyroidism itself, 52 cases(ATD group) appeared neutropenia after anti-thyroid drug(ATD)treatment, and the remaining 50 cases(control group) were hyperthyroidism patients with normal neutrophil count. The clinical features, the efficacy of131I treatment for hyperthyroidism and the change of neutrophil count after131I treatment were analyzed and compared in three groups.
Results:
Mild neutropenia was commoner in both the HT group and the ATD group. The curative rate of hyperthyroidism in the HT group, the ATD group and the control group were80. 95%, 84. 62% and 84. 00%, respectively. There was no significant difference in the efficacy of131I therapy among the three groups. Compared with the baseline value before treatment, neutrophil count increased in all three groups 2-4 weeks after131I treatment(allP<0. 05). The neutrophil recovery rates of the HT group and the ATD group were 61. 90% and 84. 62%, respectively. The ATD group had a higher recovery rate(P<0. 05).
Conclusion
Mild neutropenia is commoner in hyperthyroidism patients with neutropenia due to either hyperthyroidism itself or ATD treatment. Normative131I treatment for hyperthyroidism patients with neutropenia is safe and effective.
5.Fludarabine-based increased-intensity conditioning regimen for allogeneic hematopoietic stem cell transplantation in acquired severe aplastic anemia.

OBJECTIVETo observe the effects of increased-intensity conditioning regimen with FBCA (Fludarabine, Busulfan, Cyclophosphamide, and Antithymocyte globulin) for allogeneic hematopoietic stem cell transplantation (allo-HSCT) in acquired severe aplastic anemia (SAA).
METHODSFrom January 2000 to June 2011, twenty-two patients (male 12, female 10) with SAA underwent allo-HSCT with FBCA conditioning regimen which consisted of fludarabine (30 mg·m⁻²·d⁻¹×5 d), busulfan (3 mg/kg×2 d), cyclophosphamide (60 mg·kg⁻¹·d⁻¹×2 d) and ATG (2.5 mg·kg⁻¹·d⁻¹×5 d). GVHD prophylaxis was performed by cyclosporine and short-term course methotrexate. Nine patients received mobilized peripheral blood stem cells transplantation and 13 patients underwent mobilized peripheral blood combined with bone marrow stem cells. Fourteen cases were human leukocyte antigen (HLA)-matched related donors, while the other 8 cases were HLA-haploidentical transplantation. Engraftment was documented by short tandem repeats with polymerase chain reaction (STR-PCR) on approximately day + 30, + 90, + 180, + 1 year and + 2 year, respectively. Long-term survival and transplantation-related complications were analyzed.
RESULTSAll patients obtained prompt and sustained hematopoietic reconstitution. Median time for neutrophil and PLT engraftment was 15 (range: 11-22) days and 16 (range: 12-27) days, respectively. All patients were full donor chimerism identified by STR-PCR. 2 of the total 22 cases (9.1%) had grade I-III acute GVHD and 3 (15.8%) was chronic GVHD. Three patients (13.6%) died of transplantation related mortality and the other 19 cases were disease-free survival with a median time of 24 (range: 0.5-140.5) months. The causes of death were cytomegalovirus pneumonia (n=1), acute GVHD (n=1) and severe pulmonary infection (n=1).
CONCLUSIONIncreased-intensity of FBCA conditioning regimen could favor donor stem cell sustained engraftment for allo-HSCT in SAA.
