1.Efficacy of freeze-dried M. vaccae vaccine in treatment of patients with multi-drug resistant pulmonary tuberculosis

AIM: To explore the immunotheraptical drugs for multi-drug resistant pulmonary tuberculosis (MDR-TB). METHODS: 79 patients with MDR-TB were randomly assigned to two groups. 46 cases in M.vaccae group were treated with "3AkPaThLevL/15PaThLevL" and M. vaccae, and 33 cases in control group were treated only with "3AkPaThLevL/15PaThLevL". The clinical effect and T-lymphocyte subsets in patients were observed after being treated for 1,2 and 3 months, respectively. RESULTS: The sputum negative rates ((41.3)%, (63.0)%, and (80.4)%) and the X-ray resolution rates ((30.4)%, (50.0)%, and (67.4)%) of the M.vaccae group were superior to those ((12.1)%, (27.3)%, and (39.4)%, P
2.Treatment and Control Status of Patients with Diabetes in Urban Communities in China

Objective To study the treatment and control status of patients with diabetes in urban communities in our country. Method structured questionnaire survey with physical examination were conducted by well-trained investigator, the participants were patient with diabetes diagnosed at least one year ago in four communities from four cities of China. Result 1047 adults with diabetes were investigated, predominantly type 2 diabetes. 50.9% of participants had hypertension, 32.7% of participants had hyperlipidemia. 70.8% of patients were treated with oral medication alone, 10.2% with insulin alone, 6.3% with oral medication plus insulin, and 9.8% without any medication. So, total about 16.5% of patients were treated with insulin. The well-performed proportion of medication, diet, physical activity, self-management education and blood sugar monitoring were 81.2%, 69.4%, 80.6%, 69.8% and 66.9%, respectively. Participants who well-practised all the five treatment strategies accounted for 34.9% of all participants. Out of all participants, 63% participants had poor FBG control and 50% had poor BP control. Conclusion Most patients with diabetes in urban community receive nonstandard treatment and most of them had poor FBG control and BP control.
3.Effect of Smad7 antisense oligodeoxynucleotide on the expression of MMP-2, TIMP-2 and proliferation in human pancreatic cancer cells SW1990

ObjectiveTo investigate the effect of Smad7 antisense oligodeoxynucleotide (ASODN) on proliferation in human pancreatic cancer cell line SW1990,with a focus on the expression of matrix metalloproteinase-2(MMP-2) and tissue inhibitor of metalloproteinase-2 (TIMP-2).To explore the underlying mechanism of the role of Smad7 in the pathogenesis and development of pancreatic cancer.MethodsSmad7 ASODN was transfected into SW1990 cells through lipofectamine.Nosense oligodeoxynucleotide (NSODN),ASODN and lipofectamine was used as control. The transfection efficiency was assessed by fluorescence microscopy and flow cytometry.The expressions of Smad7,MMP-2 and TIMP-2 in transfected cells were detectedby RT-PCR and Western blot.Cell viability was assessed by dimethyl thiazoldiphenyltetrazoliumbromide (MTT) method. Results Smad7 was expressed in SW1990 cells.The transfection efficiency of SW1990 was 81.2%.The expressions of Smad7 mRNA were 0.34 ± 0.06,0.95 ±0.07,1.03 ± 0.11 in transfected group,ASODN and lipofectamine group; and the expressions of MMP-2 mRNAwere 0.54 ± 0.08,1.15 ± 0.13,1.27 ± 0.16 ; and the expressions of TIMP - 2 mRNA were 0.26 ±0.07,0.72 ± 0.13,0.78 ± 0.17,the mRNA expressions were significantly reduced in Smad7 ASODN transfected group,compared with other two groups (P <0.01 ).The expressions of Smad7 protein were 0.14 ± 0.03,0.29 ± 0.05,0.28 ± 0.07 in transfected group,ASODN and lipofectamine group; the expressions of MMP-2 protein were 0.17 ±0.02,0.29 ±0.05,0.31 ±0.04,and the expressions of TIMP-2 protein were 0.20 ± 0.03,0.41 ± 0.11,0.43 ± 0.09,the protein expressions were significantly reduced in Smad7 ASODN transfected group,compared with other two groups (P <0.01 ).The A490 values of proliferation were 0.83 ± 0.03,1.02 ±0.02,0.99 ±0.02 in transfected group,ASODN and lipofectamine group,the proliferation were significantly reduced in Smad7 ASODN transfected group,compared with other two groups (P <0.01 ).ConclusionsSmad7 ASODN could effectively inhibit the expressions of Smad7,therefore decrease the expressions of MMP-2,TIMP-2 and reduce the proliferation.
4.A new triterpenoid fom Radix Pittospori.

To investigate chemical constituents from Radix Pittospori, chloroform extract of the roots was subjected to column chromatography with various chromatographic techniques. The structures were elucidated on the basis of physico-chemical property and spectral analysis. Two triterpenoids were identified as 22-acetyl-21-(2-acetoxy-2-methylbutanoyl)-R1-barrigenol(1) and 3alpha-hydroxyl-20-demethylisoaleuritolic-14(15)-ene-28, 30-dioic acid (2). Compound 1 is a new triterpene and compound 2 is isolated from this plant for the first time.
5.Effect of multi-slice computed tomography on typing and staging of bronchiolitis obliterans in children

Objective To explore the value of multi-slice computed tomography (MSCT) on the typing and staging of bronchiolitis obliterans (BO) in children.Methods Twenty-eight BO patients were recruited and underwent X-ray and MSCT from August 2012 to August 2015.The classification of MSCT signs and radiological manifestations in different stages of BO were discussed.Results Mosaic sign and bronchial wall thickening appeared in all cases and pulmonary atelectasis in 6 patients.MSCT signs were divided into 18 cascs of simple type and 10 of pneumonic type,respectively.Radiological imaging undergone after treatment for 2 ~ 4 weeks indicated that 20 patients turned normal,but 8 patients to chronic stage,including pulmonary interstitial fibrosis (7 cases),bronchoiectasis (6 cases),calcification in bronchial distal (3 cases) and pleural adhesions (2 cases).Conclusion Mosaic sign is a distinctive MSCT manifestation for BO in early phase,and CT typing and staging contribute to guiding the treatment.
6.Clinical application of the new spinal anterior screw-rod fixation system

Objective To discuss clinical application of the new spinal anterior screw rod fixation system. Methods 18 patients with burst fractures of the thoracolumbar spine, 6 with spinal tuberculosis, and 2 with spinal tumor were managed with anterior decompression instrumentation and internal fixation of screw rod system. Result 26 cases were followed up for 25 to 67 months. X ray showed that the preoperative kyphotic deformity had been corrected, and all patients had bony fusion. No complications occurred. All patients had neurological improvement. Conclusion The new screw rod fixation system has reliable stabilization and can provide rigid fixation, and it is easy to handle and the cost is low.
7.Clinical Observation on Hepatoprotective Effect of Compound Glycyrrhizin in Treating Patients with Multidrug Resistant Pulmonary Tuberculosis

OBJECTIVE:To observe the hepatoprotective effect of compound glycyrrhizin in treating patients with multidrug resistant pulmonary tuberculosis(MDR-TB).METHODS:A total of 68 MDR-TB cases were enrolled:36(treatment group) were randomly assigned to receive additional glycyrrhizin,and 32(control group) to receive additional glucurolactone and legalon,the hepatoprotective effects and side effects between 2 groups were compared before and after treatment.RESULTS:The total effective rates of the treatment group and the control group were 94.4% and 62.5%,respectively 1month after hepatoprotective treatment,and significant difference was noted between 2 groups(P0.05).CONCLUSION:Compound glycyrrhizin has a reliable hepatoprotective efficacy and few adverse drug reactions in treating MDR-TB patients complicating with lesion of liver function.
8.Preventive analgesia of Celebrex for patients receiving total knee arthroplasty

BACKGROUND:The preemptive analgesia is stil a controversial issue. Existing studies have not paid much attention to effects of preoperative factors on the hypersensitivity of peripheral and central mechanisms. Visual analog scale scores cannot subjectively and repeatedly reveal patient’s pain.
OBJECTIVE:To investigate the validity of the preventive analgesia effect of Celebrex in patients with total knee arthroplasty.
METHODS:Patients with osteoarthritis of the knee who received total knee arthroplasty were accessed by Pittsburgh sleep quality index, self-rating depression scale and self-rating anxiety scale. In al , thirty patients were enrol ed in the study. They were randomized into Celebrex group and vitamin C group, and each group had 15 patients. The patients in the Celebrex group and vitamin C group took 200 mg Celebrex and vitamin C, respectively, twice a day from day 2 to day 4. Both of their knees were evaluated by resting visual analogue scale and moving visual analogue scale in the evening of day 1 before treatment and day 3 after treatment. Meanwhile, the pain threshold and pain tolerance were accessed by a pain-threshold machine.
RESULTS AND CONCLUSION:No statistical significance of the changes of resting and moving visual analogue scale scores was found in both knees in the Celebrex group (P>0.05). The pain threshold of both knees were significantly increased (P<0.05), and the severe knee, which had less visual analogue scale scores than the minor one, turned out to increase more obviously than the minor knee (P>0.05). There were no significant changes in the pain tolerance in both knees (P>0.05). The changing values of resting or moving visual analogue scale were not significantly correlated with the pain threshold and pain tolerance (P>0.05). There were no significant changes in visual analogue scale scores, pain threshold and pain tolerance in both knees of the vitamin C group (P>0.05). Celebrex could increase the pain threshold of patients receiving total knee arthroplasty, especial y the severe knee, which indicates that the Celebrex is good for the preventive analgesia. Comparatively speaking, the pain threshold might be more sensitive than visual analogue scale in revealing the change of pain after analgesia. There is no significant correlation between visual analogue scale score and the hypersensitivity of pain.
9.Effects of ursolic acid at different concentrations on proliferation and differentiation of osteoclasts and their significances

Objective:To investigate the effects of ursolic acid (UA) on the proliferation and differentiation of osteoclasts (OC), and to explore its role in orthodontic force-induced root resorption and its relationship with OC.Methods:The mononuclear / macrophage cells RAW264.7 were induced to the OC.Tacrolimus acid phosphatase (TRAP) staining and bone resorption lacunae observation were used to identify the induction.CCK-8 method was used to select the appropriate concentration of UA for RAW264.7 cell-free biotoxicity and to observe its effect on the proliferation and differentiation of RAW264.7.In experimental groups, UA with gradient concentrations (1.0,2.5,5.0,10.0,20.0and 40.0 μmol·L-1)were added.UA was not added in control group.Results:The TRAP staining and bone resorption lacunae observation showed that after the RAW264.7 cells were induced for 5 d, the TRAP staining positive cells were found;the resorption lacunae were rounded,and oval, etc,the bottom wall was coarser,and the boundary was clear,which indicated that the RAW264.7 cells were successfully differentiated into the osteoclasts.The CCK-8 detection results showed that high concentration of UA (> 10.0 μmol·L-1) significantly inhibited the proliferation of OC;the appropriate concentration of UA (5.0 μmol·L-1) was in the biological safety concentration range and could inhibit the OC proliferation;low concentration of UA (<2.5 μmol·L-1) had no effect.Conclusion:RANKL can induce the differentiation and maturation of RAW264.7 cells.UA is correlated with the proliferation and differentiation of OC;UA has inhibitory effect on OC at the appropriate concentration (5.0 μmol·L-1) in a time-dependent manner.
10.Retroperitoneal laparoscopic nephrectomy for kidney tuberculosis

Objective To discuss the feasibility and safety of retroperitoneal laparoscopic nephrectomy for treatment of kidney tuberculosis. Methods From March 2005 to February 2009, 28 patients with kidney tuberculosis underwent retroperitoneal laparoscopic nephrectomy. The patients′ data were reviewed and analyzed. Results There were 18 men and tencwomen with an average age of 36 (26-51) in the cohort. Sixteen patients had lesions on the left kidney and 12 on right kidney. All patients had a normal renal function on the contra lateral side. The severely impaired renal function of the lesion side was confirmed before operation. Anti-tuberculosis chemotherapy was administered to patients for two weeks to six months in advance of the surgery. No active lesion of tuberculosis was found and ESR level was normal before operation. All the operations were successfully performed without switching to open surgery. The average operative time was 170 (121-258) minutes, blood loss was 110 (70-250) ml and average postoperative hospital stay was 5.7 (5-14) days. Peritoneum injury was seen in three patients and incision infection in two patients. No severe complications were observed. Anti-tuberculosis chemotherapy was continued for three months. Twenty-four patients were followed-up, and the average follow-up time was 12.5 (6-20) months. All patients recovered without any lesion remaining. Conclusions Retroperitoneal laparoscopic nephrectomy could be a safe and reliable method for the treatment of non-functioning kidney due to tuberculosis.
