1.Application of Gene Chip for Detecting Mutations of Rifampin-resistant Genes in Mycobacterium Tuberculosis Strains

Objective To explore the feasibility of detecting the mutations of rpoB gene in rifampin-resistant Mycobacterium tuberculosis strains by gene chip. Methods The DNA chip was designed according to the sequence of Mycobacterium tuberculosis rpoB gene. The DNA fragments which contained hot mutation sites of rpoB gene were labelled with cy3 fluorescence, amplified by PCR technique and hybridized with gene chip. The DNA sequencing was used as the control. Results Eighteen of the Mycobacterium tuberculosis strains were detected for the mutations of rpoB gene by gene chip. Among them, one sensitive strain was identical with the type strain, and all of 17 strains with rifampin-resistant, rpoB mutations were detected. Of the 17 strains with rpoB mutations, 14 were detected as single point mutation, one was detected as double sites point mutations with the sites of 511 and 516, and two were detected as single point mutation with the sites of 518 and 526, respectively. Because the probe of 517 rifampin-resistant did not be dotted on the gene chip, only sites of 518 and 526 mutations were detected, which were consistent with the results of DNA sequencing. Conclusion Using the gene chip could detect mass rpoB gene mutation of rifampin-resistant with higher specialty and sensitivity, which could be used for clinical detection of rifampin-resistant strains and clinical medicine.
2.Anti-neoplastic Effect of Xuezhikang in vitro

Objective To study the anti-neoplastic effect of Xuezhikang (Regulate blood-fat) Capsule in vitro and its influence on the chemotherapeutic drug 5 Fluorouracil (5-Fu).Methods Using MTT method to measure the inhibition rate of Xuezhikang Capsule used simply,5-Fu used simply and Xuezhikang Capsule with 5-Fu together on HepG2,MCF-7 and HL-60 cells.Results The inhibitory action of Xuezhikang Capsule(2.56,0.64,0.16,0.04,0.01 mg/ml) on HepG2,MCF-7 and HL-60 cells was dose-dependent and the IC50 was 14.057,19.859 and 27.771mg/ml respectively.The inhibition ratio of five different doses of Xuezhikang Capsule(0.64,0.16,0.04,0.01,0.0025 mg/ml) with 5-Fu (0.24mg/ml) together all remarkably higher than that of 5-Fu used simply.Conclusion Xuezhikang Capsule has a significant anti-cancer effect in vitro and can enhance remarkably the anti-neoplastic effect of 5-Fu.
3.The studies on therapeutic action of tuberculosis Ag85A DNA vaccines

To study the therapeutic action of tuberculosis Ag85A DNA vaccines. Methods:BALB/c mice were infected by in-traperitoneal injection with M. tuberculosis H37Rv for 8 weeks, and then treated with the saline (A), plasmid vector (B), M. bovis BCG (C), M. vaccae (D), and Ag85A DNA vaccine (E). Ag85A-specific antibodies were determined by ELISA. The lungs, livers and spleens were taken and observed their pathological changes, weighted and performed mycobacterial cultures 2 or 5 monthes after treatment. Spleen cells were tested for proliferative responses.Results:Ag85A-specific antibodies increased in the mice of group E. At 2 months after immunothera-pies, the stimulation rates of spleen cells had no significant difference among each group. The numbers of viable bacteria in the lungs and spleens of therapeutic group were lower than those in the control group. The group C and B could be observed slight .lesion of lung. At 5 months after immunotherapies, the stimulation rates of spleen cells all increased significantly in the immunotherapeutic groups. The numbers of viable bacteria in the lung and spleen had no significant difference among each group. No lesions of lung could be observed in group E and D. Conclusion:The tuberculosis DNA vaccines seem to have some immunotherapeutic actions.
4.The correlation of mild cognitive impairment and middle cerebral artery stenosis and effect of statins on mild cognitive dysfunction

Objective To investigate the correlation of the middle cerebral artery stenosis (MCAS) and the mild cognitive function impairment (MCI),and the clinical efficacy of statins in patients with MCI. Methods Six hundred and thirty-six patientse,who received transcranial color doppler ultrasound (TCD)assay, were enrolled in our hospital hospitalization or outpatients. The simple mental state examination (MMSE) and clinical dementia rating scale (CDR) were used as cognitive function assessment indexes. Forty-four cases of MCI with MCAS and 58 cases of MCI with NMCAS were used as the treatment group , who received the atorvastatin 20 mg every day , 56 cases of MCI with NMCAS were used as the control group , who only received the routine and basic diseases treatment. One yearlater,we determined the changes of MMSE and CDRagain. Results We detected 124 patients with MCAS, 512 patients with NMCAS, and 44 cases of MCAS patients with MCI, the prevalence was 35.5%,114 cases of NMCAS in patients with MCI, with the prevalence of 22.3%, the prevalence between the two groups was statistically different. One year later, the patients in the treatment group, MMSE score was improved, the score of MCI of the MCAS group improved more significantly. Conclusion The middle cerebral artery stenosis correlated with the occurrence of MCI. Atorvastatin could improve cognitive function in patients with MCI, especially for MCI which was caused by middle cerebral artery stenosis.
5.A research on effects of traditional Chinese medicine combined with early enteral nutrition for treatment of patients with severe acute pancreatitis

Objective To analyze the curative effect of traditional Chinese medicine(TCM)combined with early enteral nutrition(EN)for treatment of patients with severe acute pancreatitis(SAP). Methods 70 SAP patients were randomly divided into TCM plus EN group(36 cases)and conventional therapy group(34 cases). Both groups received routine treatment. Additionally,TCM+EN group received early EN and TCM decoction treatment〔the ingredients of decoction including radix bupleuri,radix paeoniae alba,radix scutellariae,fructus aurantii immaturus, magnolia bark,raw rhubarb(rhubarb was added at last during cooking the decoction)and natrii sulfas exsiccatus (dissolved in water to be administered)each 10 g,the decoction was concentrated to 150 mL and then administered via a stomach tube to the patient,afterwards the tube was clipped for 2.5-3 hours,twice a day,4-7 days constituting a therapeutic course〕. After treatment,the time for patients' symptoms improvement,the situation of intestinal recovery, the length of stay in hospital,the total medical cost,serum C-reactive protein(CRP),aspartate aminotransferase (AST), lactate dehydrogenase (LDH), amylase (AMY), acute physiology and chronic health evaluationⅡ(APACHEⅡ)score and complications,intensive care unit(ICU)transfer rate and case fatality rate in two groups were observed. Results The time for symptoms improvement of abdominal tenderness(day:1.68±1.01 vs. 3.89±1.07), abdominal distension(day:2.17±1.48 vs. 4.24±3.23),abdominal pain(day:3.12±1.14 vs. 4.94±3.21)and the intestinal recovery of exhaust defecation time(day:3.48±0.92 vs. 5.32±3.30)of SAP patients after treatment in the TCM+EN group were faster significantly than those in the conventional therapy group(all P<0.05). The length of stay in hospital(day:15.50±1.75 vs. 19.35±1.69)and total cost(wan yuan:1.812±0.424 vs. 3.292±1.081) of TCM+EN group were less than those of conventional therapy group(P<0.05 or P<0.01). After treatment,the levels of serum CRP,AST,LDH,AMY,APACHEⅡscore in TCM+EN group and conventional therapy group were all lower than those before treatment,and on day 10,the degree of descent was more prominent in TCM+EN group〔CRP(mg/L):98.972±43.384 vs. 122.392±71.621,AST(U/L):75.952±55.668 vs. 126.391±47.431, LDH (μmol?s-1?L-1):1.48±0.21 vs. 2.61±1.46,AMY(U/L):146.362±58.792 vs. 226.392±37.692,APACHE Ⅱscores:6.978±3.352 vs. 13.652±7.621,P<0.05 or P<0.01〕. There was no death in TCM+EN group,while in the conventional therapy group,there was 1 case dead. ICU transfer rate in TCM+EN group was less than that in the conventional therapy group(2.78% vs. 11.76%),but there was no statistical significant difference between the two groups(χ2=0.99,P>0.05). Among the 70 patients with SAP,the cause of the disease due to gallstone accounted for 55.72%,hyperlipidemia for 37.14%,alcoholic for 4.28%and other 2.86%. Conclusion The use of TCM combined with early EN for treatment of patients with SAP can enhance the curative effect of SAP,reduce the hospitalization time and the total cost of patients,and decrease complications and mortality,that is conducive to the faster recovery of patients.
6.Pharmacokinetics and relative bioavailability of telmisartan in male healthy Chinese volunteers

AIM: To compare pharmacokinetics and relative bioavailability of telmisartan capsule (T) and telmisartan tablet(R). METHODS: 20 male healthy Chinese volunteers were enrolled in a randomized two-way crossover designs with a single-oral dose study(80 mg once per day for each preparation). The plasma telmisatan concentration was determined by HPLC- fluorescence detector. Plasma levels of telmisatan were followed up to 96 h. Area under the telmisartan concentration time curve was calculated by variance analysis and the bioequivalent was determined by two one-side t-test. RESULTS: A two-compartment model was adopted in telmisartan plasma concentration-time data analysis. The pharmacokinetic parameters of T and R in single-dose study including Cmax (μg·L-1), Tmax (h), T1/2β (h), MRT(h), AUC0-92(μg·h·L-1) were as following: 456±253 and 760±314, 1.61±0.71 and 1.08±0.36, 22.39±6.29 and 21.08±5.24, 27.02±6.23 and 24.27±5.79, 3454±1050 and 3635±1300, respectively. Statistically significant differences were observed between the parameter values of the two products in Cmax and Tmax; whereas there was no statistically significant difference between AUC0-∞μg·h·L-1 (3601±1095 and 3767±1399). The relative bioavailability for T was 97.28%±12.74%. CONCLUSION: The test telmisartan capsule is bioequivalent to the reference tablet.
7.Significance of microvessel density in bone marrow of patients with aplastic anemia

Objective: To investigate the changes and significance of microvessel density(MVD) in bone marrow of patients with aplastic anemia. Methods: 30 aplastic anemia cases,20 hyperplastic anemia cases,and 20 normal individuals′ paraffin-embedded bone marrow biopsys were stained with HE and CD34 by immunohistochemistry method respectively;and the vol% of hematopoietic tissue area in bone marrows were detected accordingly. Results:The difference between the MVD of HE and CD34 was not significant in aplastic anemia cases ,and the difference between the MVD of HE and CD34 was not significant in normal individuals either. While the MVD of HE was significantly lower than that of CD34 in hyperplastic anemia cases. The difference of the MVD between light and heavy pathogenetic condition in aplastic anemia cases was significant.The MVD of CD34 were significantly different among the cases of aplastic anemia, hyperplastic anemia,and normal individuals.The MVD showed significant linear correlation with the vol% of hematopoietic tissue area (r=0.74,P

Aim To investigate the effects of co-administration of Astragalus Saponin Ⅰ(ASI)and bendazac lysine(BDL)on hypertrophy of cultured rat mesangial cells and its mechanism.Methods The levels of collagen Ⅳ and laminin,the percentages of cells in S phase,the relative quantity of transforming growth factor ?1(TGF-?1) mRNA and indexes of oxidative status were assayed after the cells were incubated in different agents for 36 h.Results The percentage of S phase cells in high glucose group(HG) was greatly decreased while those of vitamin E group(VE) and co-administration groups were increased.The relative quantity of TGF-?1 mRNA and the collagen Ⅳ level in co-administration groups were significantly decreased,and the levels of total anti-oxidative capability(T-AOC),activity of catalase(CAT),GSH-PX,and SOD were greatly increased.Furthermore,the significant differences were found between low ASI(AL)group,low BDL(BL) group and co-administration of low ASI and low BDL(AL+BL) group for TGF-?1 mRNA,T-AOC and GSH-PX;the high ASI group(AH),high BDL group(BH) and co-administration of high ASI and high BDL group(AH+BH) for TGF-?1 mRNA and collagen Ⅳ,respectively. Conclusion Co-administration of ASI and BDL has synergetic effects on regulating TGF-?1,collagen Ⅳ,and radical oxidative stress,therefore,is beneficial to protecting rat mesangial cells against hypertrophy.
9.Effect of TRAIL on Expression of Multidrug Resistance Gene GST-π in Drug-resistant Human Gastric Cancer Cell Line SGC7901/VCR

Background:Multidrug resistance of tumor cells is one of the important factors that cause failure of chemotherapy in advanced gastric cancer. Tumor necrosis factor-related apoptosis-inducing ligand(TRAIL)may enhance the killing effect of chemotherapeutics on tumor cells,and reverse drug-resistant cell lines to sensitive cell lines,but its mechanism is not yet clear. Aims:To study the effect of TRAIL on expression of multidrug resistance gene glutathione S-transferase-π(GST-π)in drug-resistant human gastric cancer cell line SGC-7901 / VCR and the potential mechanism of TRAIL in reversing multidrug resistance of gastric cancer cells. Methods:SGC-7901 / VCR cells were treated with TRAIL in different doses (50,100,200 and 400 μg/ L)for 48 hours. After treatment,expression of GST-π mRNA in SGC-7901 / VCR cells and concentration of GST-π in culture supernatant were detected by RT-PCR and ELISA,respectively. Results:TRAIL could inhibit mRNA expression and protein secretion of GST-π in SGC-7901 / VCR cells in a dose-dependent manner within a certain range(≤200 μg/ L). The relative expression levels of GST-π mRNA in 50,100,200 and 400 μg/ L TRAIL groups were 0. 89 ± 0. 04,0. 77 ± 0. 08,0. 65 ± 0. 06 and 0. 61 ± 0. 03,respectively,and the concentrations of GST-π in culture supernatant in these groups were(57. 56 ± 1. 19)ng/ mL,(56. 30 ± 0. 80)ng/ mL,(31. 41 ± 1. 65)ng/ mL and (30. 80 ± 1. 34)ng/ mL,respectively,all were significantly lower than those in control group[1. 01 ± 0. 13 and(58. 62 ± 1. 38)ng/ mL,P all < 0. 05]. Conclusions:TRAIL may play a potential role in reversing multidrug resistance of gastric cancer cells through down-regulating GST-π expression.
10.Setting up a management operating mechanism that combines responsibilities, powers and interests

Reform in personnel matters and the distribution system began to be implemented from October 2001 in line with the guiding principle of putting quality at the core of medical practice, taking doctors as the axis, putting emphasis on profits, and using distribution by merit as a lever. A series of reform measures was adopted: the employment of the entire staff on the basis of a contract system and transformation of identity management into post management; division of medical service units based on medical groups, with the attending doctors being in charge; and intra hospital distribution by the principle of more pay for more and better work. As a result, a management operating mechanism that specifies clear cut job responsibilities and combines responsibilities, powers and interests has been set up, bringing about good results.
