1.Clinical observation of chronic obstructive pulmonary disease with pulmonary heart disease by modified Dingchuan zhike decoction and ambroxol hydrochloride

Objective To study the clinical effects of Dingchuan zhike decoction with ambroxol hydrochlo-ride on the patients with chronic obstructive pulmonary disease and pulmonary heart disease.Methods According to the random number table,the patients were divided into the observation group (70 cases)and the control group (73 cases).The patients were treated with ambroxol hydrochloride and Dingchuan zhike decoction in the observation group;the control group had a treatment with ambroxol hydrochloride.The lung function,sputum fluid inflammatory cytokines and clinical symptoms were compared between the two groups.Results When the two groups received treatment for 7 days,VC and FEV1 in the observation group were (2.45 ±0.38)L and (1.51 ±0.24)L,those in the control group were (1.94 ±0.38)L,(1.01 ±0.21)L,the patients′lung functions:FVC,VC,PEF,FEV1,and MVV% were improved in the observation group than those in the control group (t=10.64,15.87,12.14,10.22, 14.04,all P<0.05);After 14 days treatment,VC and FEV1 of patients in observation group were (2.83 ±0.35)L and (1.67 ±0.27)L,those in control group were (2.25 ±0.22)L,(1.21 ±0.23)L,the patients in the observation group were better than those in the control group on the lung functions:FVC,VC,PEF,FEV1 ,and MVV% (t =11.02,11.87,12.68,16.41,16.09,all P<0.05).The CRP of the observation group were (14.13 ±5.21)mg/L, (3.94 ±2.39)mg/L,after 7 and 14 days treatment respectively.And these in the control group were (20.99 ±9.77) mg/L,(9.94 ±8.22)mg/L.The PCT of the observation group were (0.31 ±0.24)μg/L,(0.27 ±0.09)μg/L. These in the control group were (0.51 ±0.21)μg/L,(0.31 ±0.18)μg/L.The levels of CRP and PCT on the obser-vation group′patients were better than those on the control group′patients after 7 days treatment (t=16.75,22.05, all P<0.05).When patients received 14 days therapy,the levels of CRP and PCT on the observation group′patients were better than those on the control group′patients (t=10.75,12.05,all P<0.05).The total effective rate was 86.30% in the control group and that of the observation group was 97.14 %.The total effective in the observation group was better than that in the control group.(χ2 =5.46,P<0.05).Conclusion The therapy of ambroxol hydro-chloride with Dingchuan zhike decoction is better than therapy of ambroxol hydrochloride alone.
2.Effects of lupeol on human co-stimulation cells and colonic cancer cell lines SW480

Objective Co-stimulation cells is a kind of natual killer (NK)-like T cells, which can kill tumor cells.Previous studies show that lupeol , an natural plant extracts , can change the growth of NK cells ,γδT cells and their effects on tumor cells .This study aimed to investigate the effects of lupeol on human co-stimulation cells and colonic cancer cell lines SW 480 . Methods The peripheral blood mononuclear cells ( PBMC) from healthy volunteers were induced in vitro by different cytokines and transferred into Co-simulation cells.After SW480 and Co-stimulation cells were incubated with different concentrations of lupeol for different durations , methyl thiazolyl tetrazolium (MTT) was used to detect the effects of lupeol on co-stimulation cells and colonic cancer cell lines SW480. Lactate dehydrogenase was used to determine the cell-killing activity of Co-simulation cells to colonic cancer lines SW 480 . Results The concentration of lupeol in 0.1-200.0 mg/L promoted the growth of Co-stimulation cells and inhibited the colonic cancer cell lines SW480.When the concentration of lupeol is at 12.5 mg/L, the cell-killing activity of Co-simulation cells to colonic cancer lines SW 480 was enhanced significantly compared with the controls (76%vs 40%, P<0.05). Conclusion Lupeol could promote the prolifera-tion of Co-stimulation cells, inhibit the growth of cancer lines SW480, and strengthen the cytotoxicity of Co-stimulation cells against co-lonic cell lines SW480.
3.Mechanism research of proliferation and killing hepatoma cell of CD3AK cell by Wogonin

Objective:To investigate the effect and mechanism of Wogonin on CD3AK cell proliferation and cytotoxicity to SMMC-7721.Methods:CD3AK cells were cultured from peripheral blood mononuclear cells ( PBMC) in vitro by a variety of cytokines for 7 d,and treated with different concentrations of Wogonin for 48 h.CD3AK cells proliferation was measured by CCK-8 assay.SMMC-7721 cell growth was detected by MTT.The expression of perforin (PFP),granzyme B (GrB) and CD107a on CD3AK cells were measured by flow cytometry ( FCM).The cytotoxicity to SMMC-7721 cells was detected by LDH release assay.The expression of ERK1/2 on CD3AK cells was detected by Western blot.The mobility of SMMC-7721 cells was detected with transwell chambers.The merge of SMMC-7721 cells were measured with Wound healing assay.Results:Wogonin could significantly promote CD3AK cells proliferation, especially at 3.2 mg/L (23%higher than that of control group,P<0.05).The highest cytotoxicity to SMMC-7721 was also at the con-centration 3.2 mg/L (60.4%).The expression of PFP,GrB,CD107a were significantly higher than that of control group( P<0.05).The expression of ERK1/2 was obviously improved,especially at 12.5-0.8mg/L.After treated with Wogonin 50,12.5,3.2,0.8,0.2 mg/L for 48 h,the lowest transwell cell was at 12.5 mg/L and lowest merge rate was at 3.2 mg/L.Conclusion:Wogonin could promote CD3AK cell proliferation and enhance the cytotoxicity to SMMC-7721.Wogonin could also inhibit SMMC-7721 cell growth,migration and cell merge.The mechanism may be related to activated ERK1/2 and increase the expression of PFP,GrB,CD107a.
4.The digital artery series and parallel of island flap for repairing degloving injury of the fingertip

Objective To investigate the method and effectiveness of the digital artery series and parallel of island flap for repairing degloved injuries of the fingertip of thumb or adjacent finger Methods Between September 2008 and July 2012,the finger artery series retrograde island flaps tiled in the repair of finger degloving injury in 13 cases,repair of adjacent finger tip degloving injury in 11 cases; 8 cases of degloved injuries of the fingertip were tiled with the digital artery parallel island flap from ulnaris middle finger and radialis ring finger of arteria digitalis communis pedicled which were from the same palm side.The size of skin and soft tissue defect ranged from 2.0 cm × 1.8 cm-7.2 cm ×5.5 cm,gutted flap minimum ranged from 1.1 cm × 1.0cm-1.5 cm × 1.3cm,and the maximum ranged from 3.0 cm × 2.2 cm-5.5 cm × 4.5 cm.The donor sites were repaired with the intermediate split thickness free skin grafts and performed with pressure dressing.Results The group of 32 cases,in 1 case the flap vasospasm occurred in operation of free process,the symptoms disappeared after local application of papaverine and hot compress ; 1 case of flap occurred disturbance of blood circulation after operation because of tight suturing for pedicle,the symptoms relieved after removing the stitches at intervals; 2 cases blisters appeared after operation,disappeared after a week.All skin flaps were survived,incision and skin graft donor sites healed by first intention.Twenty patients were followed-up from 6 to 12 months after operation.All flaps presented the satisfactory appearance and texture,recovered protective feeling.At last follow-up,the two-point discrimination was 7 to 10mm,and the flexion and extension function of wounded fingers recovered to normal.According to the functional assessment criteria of upper limb formulated by the Hand Surgery Branch of Chinese Medical Association,there were excellent in 19 cases,good in 10 cases,and moderate in 3 cases.No significant loss to the donor shape and function.Conclusion The surgery by adopted the digital artery series or parallel of island flap for repairing degloved injuries of the fingertip of thumb or adjacent finger,not only has the advantages of simpleness,safety and reliability,but also can satisfy the patients who aren't willing to accept or because of physical reasons can't accept the treatment of abdominal skin tube or nail flap from hallux toe,which performs in both high-end and low level hospitals,and deserves of general application.
5.Effects of quercitrin on the proliferation and the cytotoxicity of human γδT cells

Objective To investigate the in vitro effects of quercitrin on the proliferation and the cytotoxicity of human γδT cells.Methods Peripheral blood mononuclear cells (PBMCs) were isolated from healthy subjects and cultured with isopentenyl pyrophosphate and IL -2 to induce human γδT cells.The hu-manγδT cells were cultured with quercitrin at various concentrations for 48 hours.CCK-8 kits were used to analyze the in vitro proliferation and cytotoxic activities of γδT cells.Flow cytometry was performed to meas-ure the expression of granzyme B and perforin in γδT cells.The expression of p-ERK, p-Akt and Bcl-2 at protein level were detected by Western blot .Results The percentage of human γδT cells in PBMCs was in-creased from (2.96±1.83)%to (88.94±2.36)%after 10 days of culture.The quercitrin at concentrations of 10 to 80 μg/ml could promote the growth of γδT cells and up-regulate the expression of granzyme B , per-forin, p-ERK, p-Akt and Bcl-2 in a dose dependent manner .The cytolytic activities of γδT cells against co-lonic carcinoma cells ( HCT116 ) were enhanced by quercitrin .Conclusion Quercitrin could promote the proliferation of γδT cells and enhance the expression of granzyme B and perforin at certain concentrations in vitro.ERK1/2 and Akt signal transduction systems might be involved in the process .
6.Long-term outcomes of intracavitary hyperthermia in combination with radiotherapy for locally advanced uterine cervical cancer

Objective To evaluate the long-term clinical efficacy and toxicities of combined intracavitary hyperthermia and radiotherapy fur locally advanced uterine cervical cancer. Methods 310 patients with locally advanced uterine cervical cancer were assigned into intracavitary hyperthermia + radiotherapy group(TRT, 181 patients) and external-beam radiotherapy + traditional intracavitary radiation group (RT,129 patients). The external-beam radiotherapy were given with 60Co γ-my or 6-8 MV X-ray in traditional fractionation. In TRT group,radiotherapy was 40 Gy using the anterior-posterior pelvic fields and additional 20-25 Gy using the lateral fields. Hyperthermia was delivered by the 915 MHz microwave hyperthermia device within 15-60 min after external radiotherapy for 10-12 times(40 min each time,1-2 times per week). The temperature of tumor surface was 46-47℃. In the RT group, the external-beam radiotherapy of 40 Gy was delivered using the anterior-posterior pelvic fields. The intracavity radiotherapy of radium was delivered before 1989 ,with 50 mg radium in the vagina and 30 mg in uterine cavity for 24 hours ,weekly for 3 times to a total dose of 7200 mg·h. After 1989,intraeavity radiotherapy of 192Ir was delivered to a total dose of 30-36 Gy to point A in 5-6 Gy fractions,2 fractions per week. Results The 5-year survival of patients in TRT group and RT group was 67.4% versus 52.1% for stage Ⅱ disease (χ2=7.55,P=0.006), and 60.0% vemus32.3% forstage Ⅲ (χ2=7.06,P=0.007) . The 10-year survival was46.5% versus42.6% for stage Ⅱ (χ2=3.90,P=0.058), and 43.7% versus 20.6% for stage Ⅲ(χ2=17.28,P=0.000). Cox regression analysis showed that the tumor stage(P=0.023) and intracavitary hyperthermia( P=0.019) were prognostic factors. According to the RTOG criteria, the rate of mild to moderate late side effects of rectum and bladder in TRT and RT group was 17.7% and 33.1%, respectively (χ2=9.18, P=0.002). Rectovaginal fistula was developed in5 patients(3.9% ) in RT group and I patient (0.6%) in TRT group(χ2= 4.38,P=0.036). Conclusions The long-term survival of patients with stage Ⅲ uterine cervical cancer is better of TRT group than RT group. The TRT is well tolerated and the late toxicity rate is obviously low. It is necessary to carry out large randomized clinical trials to confirm these outcomes.
7.Curative Effect of Tegafur Gimeracil Oteracil Potassium Capsules Combined with Oxaliplatin in the Treat-ment of Advanced Gastric Cancer and Influence on MMP-9 Expression in Cancer Tissue

Objective:To observe the efficacy of Tegafur gimeracil oteracil potassium(Gio) capsules combined with oxaliplatin in the treatment of advanced gastric cancer,and explore its effect on matrix metalloproteinase-9 (MMP-9) expression in cancer tissues. Methods: Totally 120 patients with advanced gastric cancer were randomly divided into the observation group and the control group, and both groups were treated with neoadjuvant chemotherapy for 3 cycles as follows:the observation group was given Gio capsules and oxaliplatin,and the control group was given 5-fluorouracil combined with oxaliplatin. The short term efficacy,adverse reactions and ex-pression of MMP-9 in cancer tissue before and after the chemotherapy were observed in the two groups. Results:After the treatment, the objective response rate in the two groups had no significant difference(P>0.05);the clinical benefit rate of the observation group was significantly higher than that of the control group(P<0.05);the incidence of severe bone marrow suppression and liver and kid-ney dysfunction in the observation group was significantly lower than that in the control group(P<0.05);after the treatment,the pos-itive expression of MMP-9 in the observation group was significantly lower than that in the control group(P<0.05). Conclusion:Gio capsules combined with oxaliplatin can improve the clinical benefit rate of the patients with advanced gastric cancer,and effectively re-duce the expression of MMP-9 in cancer tissue.
