2.Expression and relationship of basic fibroblast growth factor and platelet-derived growth factor in femoral neck fracture

Objective To investigate the expression and distribution of basic fibroblast growth factor (bFGF)and platelet-derived growth factor (PDGF)in the different phases of femoral neck fracture.Methods Immunohistochemical assays were used to determine the expression and distribution of bFGF and PDGF protein in 36 human specimen of femoral neck fracture.A was measured and analyzed by CMIAS color imaging analysis system for signals of bFGF protein were found high in the mesenchymal cells,monocyte and vascular endothelial cells at 1st week after fracture in 9 subjects,with A of (0.4076 ±0.0902).The weakly positive signals of PDGF protein were found in the mesenchymal cells,while strongly positive in the vascular endothelial cells with A of (0.2261 ±0.0636).At 2rd week,in 9 cases the expression of bFGF and PDGF was strongly expressed in fibroblasts,endothelial cells,cartilage cell and cartilage matrix,osteoblast,with A of[(0.6404±0.0920)and (0.7457±0.0756)]and significandy higher than that at 1st week (P<0.05,P<0.01).There was no significant difference between the 3rd and 3nd week with A of[(0.7168±0.1346)and (0.8033±0.0491),P>0.05 ].The expression of bFGF and PDGF protein was reduced obviously at 4th week but was positive in young and cartilage tissue,with A of [(0.5374correlation between bFGF and PDGF protein in different phases (r1week=0.792,r2week=0.834,r3week=0.880,entiation of cartilage cell and osteoblast,and induce proliferation of vascular endothelial cells and new blood vessel.③ Both bFGF and PDGF are bone growth factors, cooperating in regulating proliferation and differentiation of cartilage cell and osteoblast for fracture healing.
3.Medication treatment of 150 elderly patients with serious chronic congestive heart failure

We retrospectively reviewed clinical data of 150 elderly patients with serious chronic congestive heart failure who admitted in 1986, 1996 or 2006, including medication treatment, hospital stay,and admission interval. The results indicated that before 1990s, digitalis, diuretics, and blood vessel dilating agents were more often used in the treatment of chronic congestive heart failure(digitalis=100%,diuretics≥95%, blood vessel dilating agents>50%, β-blocker<10%, ACEI/ARB = 0% ). Since 1990s,the use of β-blocker, ACEI and ARB increased, which resulted in reduced hospital stay and increased admission interval(P<0.01). Our investigation suggests that evidence-based medication could be necessary in the treatment of elderly patients with chronic congestive heart failure.
4.Reform on training mode of clinical medicine postgraduates with professional degree

The enrollment scale of clinical medicine postgraduates with professional degree was expanded each year in order to meet the needs of the society.How to train high quality medical talents in line with modem medical education and people's health needs was the problem we were confronted with.The affiliated hospital of Inner Mongolia medical university lunched reform on training mode of clinical medicine postgraduates with professional degree; the concrete measures included increasing postgraduate pre-service training,implementing tutor responsible system,applying PBL teaching and focusing on the overall quality training.Students' clinical competence was improved after the reform.
5.Correlation between osteoporosis and degeneration of intervertebral discs in aging rats.

This study examined the correlation between osteoporosis and the degeneration of intervertebral discs. Sprague-Dawley rats were maintained up to 22 or 28 months. The femoral bone, tibial bone and lumbar vertebra were histologically studied and the expression of collagen type II and X in intervertebral discs was immunohistochemiscally determined. Several indices for the degeneration of intervertebral discs and osteoporosis and the correlation among them were then analyzed. Close correlations were found among the indices for the degeneration of intervertebral discs, including the relative area of the vascular bud, the ratio of the uncalcified and the calcified layers, the expression of collagen type II and X. The correlation with collagen type X was negative. There existed positive correlations among the indices for osteoporosis, including the thickness ratio of cortical bone, the relative area of bone trabecula, the density of femoral and vertebral body bones, and the maximum stress and strain on bone. Analysis on the relationship of osteoporosis and the disease on disc showed that the indices of osteoporosis were negatively correlated with the indices of the degeneration of intervertebral discs but the expression of collagen type X was positively correlated, with the density of vertebral body bones having the strongest dependence on collagen type X. The maximum stress and strain bore no correlation with the degeneration of intervertebral discs. These results suggest that osteoporosis was negatively correlated with the degeneration of intervertebral discs.
6.Three hundred patients with cerebrovascular disease within onset of 3 days were evaluated C-reaction protein level and cerebrovascular disease prognosis

Three hundred cerebrovascular disease (CVD) patients (disease onset <3 days) were evaluated for serum C-reactive protein (CRP) level at admission, and Scandinavian Stroke Scale (SSS) or Oxford Handicap Scale (OHS) at baseline and 3 months. Based on serum CRP levels, the participants were divided into group A [CRP(1.20 ±0.35)mg/L], group B[CRP(4.98 ± 1.08) mg/L] or group C[CRP (19.34±12.27)mg/L]. Our results showed that serum CRP level was positively correlated with SSS (r = 0.39 or0.43, both P<0.01) and OHS (r=0.40 or0.42, both P<0.01) at3 months. Thus, evaluating serum CRP level within 3 clays of disease onset might be helpful in predicting clinical outcomes of CVD patients.

8.Application of modified DOPS as formative assessment in clinical skills training for professional graduate students

Objective:To explore the application effect of modified direct observation of procedural skills (DOPS) as a formative assessment tool in clinical skills training of professional graduate students.Methods:A total of 130 professional graduate students of Batch 2019 were selected as the experimental group, and 127 graduate students of Batch 2018 were selected as the control group. Two groups of students received clinical general skills training after enrollment, the control group adopted the traditional skills teaching method, and the experimental group added DOPS as formative assessment on the basis of traditional teaching. The teaching effect of DOPS was evaluated by means of scores analysis and student self-assessment. SPSS 23.0 was used for t-test. Results:The score of the experimental group (84.35±3.41) was higher than that of the control group (77.58±2.68), which showed a statistically significant difference ( t=2.63, P<0.05). The scores of "skill operation ability" and "communication ability" were the lowest single indexes in the assessment of DOPS. The results of self-assessment showed that the scores of autonomous learning ability, clinical thinking ability, clinical practice ability and professional accomplishment of students in the experimental group were all higher than those in the control group ( P<0.05). Conclusion:Modified DOPS is helpful to improve clinical core ability, which is worth promoting for application.
9.Application of the need-based nursing approach in elderly rectal cancer patients after Mile surgery

Objective? To explore the effects of need-based nursing practice in elderly patients with radical resection surgery for rectal cancer. Methods? From 1 February 2017 to 3 May 2018, we selected 80 elderly patients with radical resection for rectal cancer in colon-rectum-anus surgery at the First Affiliated Hospital of Zhengzhou University as subjects by convenience sampling. All of the patients were randomly divided into control group (n=40) and observation group (n=40) with the method of random number table. Control group carried out routine nursing care. Apart from the routine nursing, observation group provided need-based nursing care. We compared the post-operative recovery, scores of pain, anxiety, depression and quality of life as well as satisfaction after intervention among patients between the two groups. Results? The time for the first exhaust, ambulation and the removal of sutures as well as hospital stay in the observation group were shorter than those in the control group with statistical differences (P< 0.05). There were statistical differences in the scores of pain, anxiety and depression of patients after surgery between the two groups (P< 0.01). After intervention, the scores of pain, vitality, social functioning, emotional function, mental health and general health of quality of life in observation group were higher than those of control group with statistical differences (P<0.05). After intervention, the patient satisfaction of observation group was higher than that of control group with statistical difference (P<0.05). Conclusions? Need-based nursing approach can improve the postoperative recovery of elderly rectal cancer patients, alleviate pain, anxiety and depression, reduce the incidence of adverse events, improve the quality of life and satisfaction of patients, hence is worthy of being widely used in clinical practice.
10.Current status and influencing factors of anxiety and depression in rectal cancer patients with permanent intestinal orifice

Objective? To investigate the current status and influencing factors of anxiety and depression in rectal cancer patients with permanent intestinal orifice. Methods? By convenience sampling, we selected the rectal cancer patients with permanent intestinal orifice who were admitted to Department of Colorectal and Anal Surgery in the First Affiliated Hospital of Zhengzhou University from January 2016 to December 2018 as our participants in the study. They were investigated by Anxiety Self-Rating Scale and Depression Self-Rating Scale. Totally 165 questionnaires were distributed with 157 valid ones retrieved. Results? In the 157 patients, 74 patients (47.13%) suffered from anxiety: 32 of them had mild anxiety and 42 patients had moderate anxiety. In the 157 patients, 95 (60.51%)of them suffered from depression:54 patients had mild depression, 36 patients had moderate depression and 5 patients had severe depression. Patients who were females, working staff, have low income, impaired ability of self-care, have not received the preoperative education, or have postoperative complications tend to develop anxiety and depression(P<0.05). Conclusions? There is a higher proportion of rectal cancer patients with permanent intestinal orifice who had anxiety and depression. The influencing factors are various and complex,and targeted intervention has positive significance in improving the patients' negative mood.
