1.The Role of Heparanase in the Invasion and Metastasis of Breast Carcinoma and Its Regulation Mechanism

Heparanase is the only one endogenous glucuronidase found in mammalian cells up to now,and can degrade the heparan sulfate which is the side chain of heparin sulfate proteoglycans existing in the extracellular matrix and basement membrane.Heparanase is considered as an ideal target for the treatment of cancer metastasis.High expression of heparanase gene was found in breast cancer,and was closely associated with tumor size and lymph node metastasis.In vitro,high level of heparanase promoter activity was found in breast cancer cells.In vivo,compared with negative control group,breast cancer cells transfected with heparanase could generate higher tumor volume,tumor weight,microvessel density and longer survival time of cancer cells.Blockage or silencing of heparanase gene expression through antisense technology or RNA interference technology could reduce the ability of adhesion and invasion of breast cancer cells,indicating that heparanase played important roles in the invasion and metastasis of breast cancer.Mechanisms involved in the regulation of heparanase in the invasion and metastasis of breast cancer are as following.Estrogen could bind to its recaptor and then affect the specific region of heparanase gene which could activate heparanase transcription and the expression of hepamnase Mrna and protein.Heparanase could increase the secretion of osteoclast-stimulating factor which could cause the osteolytic lesions in the absence of detectable bone metastasis.Heparanase could also induce circulating lymphocytes to produce stimulating factors which might promote the invasion and metastasis of breast cancer.Furthermore,p53 gene,ETS gene,EGR1 gene,PI-88 factor,and Cox-2 could also regulate the expression of heparanase gene.This article reviewed the role of heparanase in the invasion and metastasis of breast cancer and summarized the corresponding regulation mechanisms.
2.Renal pseudotumors: CT diagnosis and differential diagnosis

Objective: To evaluate the CT features of renal pseudotumors and the differential diagnosis with renal carcinomas and other benign tumors, emphasis on the origin of pseudotumors and the factors liable to be misdiagnosed.Methods:Unenhanced and enhanced spiral CT findings in 23 cases of renal mass were retrospectively evaluated.13 of them(inflammatory pseudotumor, 5 cases;Xanthogranulomatous pyelonephritis,2 cases;granulomatous inflammation,1 case;liparomphalus hyperplasia,1case;fibroangiolipoid hyperplasia,1 case;adenomyosis,1 case;and hematomas,2 cases) were resected and pathologically confirmed.The others\ were diagnosed by IA-DSA or followed up after the treatment.Results:Most inflammatory and hyperplasia diseases appeared as a solid or solid-cystic heterogeneous mass,with an ill-defined margin;Calcification or fat tissues sometimes could be seen.After intravenous administration of iodinated contrast media,the solid components enhanced mildly or moderately.Cystic components,infiltration of fatty renal capsule and thickening of renal fascia could be seen.Hematomas and aneurysms appeared as isoattenuation to hyperattenuation lesions,with a well-demarcated margin.Aneurysms demonstrated intense enhancement similar to the abdominal aorta.Acute hematomas could enhanced mildly.Conclusion:CT is helpful to the diagnosis and differential diagnosis of renal pseudotumors.
3.Anti-tumor mechanisms of arsenic trioxide

An overview was prepared haseed on the related articles published in recent years. The anti tumor mechanisms of AsT include: (1)inducing tumor cell apoptosis; (2)inhibiting tumor cell proliferation;and (3)inducing tumor cell differentiation.It is considesed that the study of anti tumor mechanisms of AsT in neoplasms is insufficient except for acute promyelocytic leukemia. The clinical value of AsT in neoplasm treatment is still worthy of studying.
4.Initial Study of Diagnostic Value of Diffusion Weighted Imaging in Pancreatic Cancer

Objective To evaluate the value of diffusion weighted imaging (DWI) and apparent diffusion coefficient(ADC) in diagnosing pancreatic carcinoma. Methods DWI was performed in 25 cases of pancreatic cancer, confirmed by pathology, and 20 normal persons. Five sequences of echo planar diffusion imaging were used, b value (?10-3s/mm2 ) was 50, 100, 300, 500, and 700 respectively. Then the ADC values in tumor area, non-tumor area and normal pancreatic area were calculated.Results ADC values of tumor tissue were significantly different from that of the tissue in the same pancreas, when b value were 100 and 300 s/mm2; there was statistically significantly difference between tumor group and normal group when b value were 100, 300, 500 and 700 s/mm2. ADC values of the tissue except for tumor tissue in the same pancreas were significant different from that of normal group when b value were 500 and 700 s/mm2. Conclusion DWI is a rapid and feasible method in detecting pancreatic cancer.
5.Treatment and prognosis of primary gastric lymphoma

Objective To investigate the treatment methods for primary gastric lymphoma (PGL) and analyze the prognostic factors.Methods The clinical data of 55 patients with PGL who were admitted to the First Affiliated Hospital of Dalian Medical University from July 2002 to December 2007 were retrospectively analyzed.Operation,medication or operation combined with medication were applied to patients according to the pathological type,clinical staging,infection of helicobacter pylori and complications.Patients were followed up via phone call till February 2013,and the location,diameter,pathological type,clinical stage of the PGL and the treatment methods were recorded for prognostic analysis.The survival rate was calculated by Kaplan-Meier method,and the univariate analysis of survival was done by Log-rank test.Multivariate analysis was done by COX regression model.Results The PGL located at the gastric antrum in 26 cases,body of the stomach in 17 cases,fundus of the stomach in 5 cases,gastric cardia in 3 cases,body and antrum of the stomach in 4 cases.There were 46 patients with ulcerous PGL,5 with nodular PGL and 4 with diffused and infiltrated PGL.There were 53 B cell lympboma,1 T cell lymphoma and 1 undefined tumor.Of the 53 patients with B cell lymphoma,36 were with mucosa-associated lymphoid tissue lymphoma (MALTL) and 17 with diffuse large B-cell lymphoma (DLBCL).There were 23 patients in stage Ⅰ,23 in stage Ⅱ,4 in stage Ⅲ and 3 in stage Ⅳ.Of the 55 patients,23 received operation,14 received medication,17 received operation + medication,and 1 left untreated.Fifty-three patients had complete follow-up data.The median time of follow-up was 60 months (range,7-132 months).The 1-,3-,5-year cumulative survival rates were 91%,79% and 72%.The results of univariate analysis showed that tumor diameter,pathological type and clinical staging were risk factors influencing the prognosis of PGL patients (x2 =9.34,6.59,88.01,P < 0.05),while the treatment methods did not influence the prognosis of PGL patients (x2 =3.63,P > 0.05).The results of multivariate analysis showed that DLBCL,clinical stages Ⅲ and Ⅳ were independent risk factors influencing the prognosis of PGL patients (OR =5.758,2.231,95% confidence interval:2.536-13.073,1.370-3.625,P < 0.05).Conclusion Multi-disciplinary team treatment should be recommended for PGL patients.Pathological type (DLBCL) and clinical stages (stage Ⅲ and Ⅳ) are the independent risk factors influencing the prognosis of PGL patients.
6.To analyze the expression of HIF-1 alpha and VEGF in gastric carcinoma and its significance

Objective To investigate the expression of hypoxia - inducible factor -1 alpha and vascular endothelial growth factor in gastric carcinoma and its clinical significance. Methods The expression was detected by immunohistochemistry method in 100 cases of gastric carcinoma tissues of patients with carcinoma and HIF-1 alpha, VEGF, VEGF test results comparing the carcinoma tissue and adjacent normal tissues, HIF-1 alpha; different differentiation degree and infiltration depth of tumor tissue of HIF-1 alpha and VEGF detection results of recurrence or metastasis;group, no recurrence or metastasis detection results of HIF-1 alpha and VEGF tumor tissue cancer group. Results The positive rate of HIF-1 expression in gastric tumor tissue of patients with cancer (60.00%), the positive expression rate of VEGF (64.00%) were significantly higher than those in adjacent normal tissues (21.00%, 14.00%), the difference was statistically significant (P<0.05); high ang middle differentiation, invasive depth of tumor tissue before plasma membrane positive in patients with gastric cancer the rate of VEGF, HIF-1 alpha, was low, the data had significant differences (P<0.05); the positive rate of HIF-1 alpha and VEGF expression of gastric carcinoma tumor recurrence or metastasis was found in (70.97%), the positive expression rate of VEGF (77.42%) were significantly higher than those of HIF-1 alpha and VEGF expression in patients with gastric cancer recurrence or tumor tissue the transfer of (55.07%, 57.97%), the difference was statistically significant (P<0.05). Conclusion HIF-1 alpha and VEGF play an important role in the recurrence and metastasis of gastric cancer, is an important basis for the diagnosis and prognosis of gastric cancer patients, clinicians should understand the patients with gastric cancer HIF-1 alpha and VEGF expression, so as to reduce the recurrence and metastasis rate is of important value.

8.Magnetic Resonance Venography for Diagnosis of Cerebral Venous Sinus Thrombosis:A Meta-analysis

Purpose cerebral venous sinus thrombosis (CVST) has no specific manifestations in clinic, it is often misdiagnosed and the treatment is delayed. This paper aims to evaluate magnetic resonance venography (MRV) in the detection of CVST by meta-analysis. Materials and Methods The articles concerning the diagnosis of CVST by using MRV were searched in the databases such as Cochrane Library, PubMed, Embase, Web of Science, CBM, VIP, Wanfang and CNKI. The QUADAS items were used to evaluate the quality of the included studies. The results were analyzed with software Metadisc1.4 and RevMan 5.0. The pooled weighted sensitivity, specificity, and the diagnostic odds ratio were calculated, and complete summary receiver operating characteristic curve (SROC) was drawn. Results Twelve studies of eight articles met the inclusion criteria. The pooled sensitivity, specificity, diagnostic odds ratio and area under SROC curve for the MRV were 0.86 (0.80-0.91), 0.87 (0.83-0.93), 29.65 (12.48-70.47), and 0.9125, respectively. Conclusion MRV can be used as an effective method with its high value in the diagnosis of CVST.
9.Effects of ropivacaine on GABA-activated membrane currents in isolated dorsal root ganglion neurons in CCI model rats *

Objective To investigate the effects of ropivavaine on gamma -aminobutyric acid(GABA)-activated membrane cur-rents in isolated dorsal root ganglion(DRG) neurons of the rats with ischiadic nerve chronic constriction injury (CCI) and to discuss the possible analgesia mechanism of ropivacaine .Methods The whole-cell patch clamp technique was used to record and compare the changes of GABA receptor activation currents of acute isolated DRG neurons after 30 s of ropivacaine preperfusion in the oper-ating side and the operative opposite side of the CCI model rats and the sham-operation group .Results (1)Compared with the oper-ative opposite side ,the sham-operation group and the control group ,the thermal withdrawal latency in the operative side group of the CCI model rats was notablely shortened(P<0 .05);(2)the amplitude of GABA-activated currents with different concentration GABA(0 .1-1 000μmol/L) in the operative opposite side group of the CCI operation was significantly greater than that of the op-erative side group and the sham-opeartion group ;(3)DRG neurons after ropivacaine preperfusion (0 .1-1 000μmol /L) showed va-rying degrees of enhancement effect on the 100 μmol/L GABA-activated currents ,the enhancement amplitude in the CCI operative opposite side group was significantly greater than that in the operative side group and the sham-operation group ;(4)The dose-re-sponse curve of DRG neurons GABA (0 .1-1 000μmol/L) activated current in the operative side group of the CCI rats after ropiva-caine pre-perfusion (100 μmol/L) was shifted to the left ,the difference between two EC50 had no statistical significance (P>0 .05) .Conclusion Ropivacaine has the enhancement effect on GABA activated currents in the DRG neurons of the CCI model rats , which could be one of reasons for ropivacaine producing the anesthetic and analgesic effect .
10.Association between KCNJ11 polymorphism and essential hypertension in Xinjiang Kazak population

Objective To investigate the relationship between the single nucleotide polymorphism (SNP)of KCNJ1 1 (inwardly rectifying potassium channel,subfamily J,member 1 1)gene and essential hypertension (EH)in Xinjiang Kazak population.Methods The polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP ) method was used to detect genotypes of rs2285676 polymorphism of KCNJ1 1, including 126 hypertensives (EH group)and 126 normotensives (NT group)in Xinjiang Kazak population.Multiple-factor unconditional Logistic regression analysis was used to evaluate the risk factors for hypertension in Xinjiang Kazak. Results Logistic regression analysis showed that KCNJ1 1-rs2285676 genotypes,sex,weight,total cholesterol and triglyceride were not associated with hypertension;body mass index (BMI)and high-density lipoprotein (HDL) were protective factors affecting hypertension while low-density lipoprotein (LDL ) was a risk factor for hypertension.Genotypes (TT,CT and CC)and allele frequencies (T and C)of the SNP of rs2285676 in KCNJ1 1 gene were 50.00%,48.41%,1.59%,74.21% and 25.79% in EH group,respectively.The differences between EH and normal control were not significant (P >0.05).Conclusion The rs2285676 polymorphism of KCNJ1 1 is not associated with EH in Xinjiang Kazak population.
