1.Progress in molecular targeting therapy for gastrointestinal stromal tumors

Gastrointestinal stromal tumors (GISTs) are the most common malignant tumor of abdominal soft tissue. It originates from Cahal (Cajal) interstitial cells or common precursor cells, and is driven by the mutated KIT gene or platelet-derived growth factor receptor alpha (PDGFRa) gene, all expressing type Ⅲ tyrosine kinase receptors. Imatinib mesylate, a tyrosine kinase receptor inhibitor, has been used for the standard treatment of advanced GIST, which has achieved remarkable results. Thus, GIST has become the most successful example of target therapy for solid tumors. In the context of the era of precision medicine, with the deepening in research of GISTs molecular biology,the molecular targeted treatment of GISTs has obtained a clear venation from the first-line, second-line and third-line of the advanced stage to the postoperative auxiliary and preoperative treatment, providing significant survival benefits for GISTs patients. This article systematically and comprehensively combed the preoperative and postoperative molecular targeting therapy from advanced GIST to early GIST, and analyzed the problems, proposed solutions and prospects for the future, aiming to provide reference for clinical application of molecular targeting drug therapy for GIST.
2.Clinical Application of Testing the Relation of the Respiration and the Phonation With Aerodynamics

ObjectiveTo study the relation of phonation and respiration in normal subjects. MethodsUsing Aerophone Ⅱ Model 6800 and Visipitch 6097, vital capacity was recorded first , then the simultaneous phonation volume, maximum sustain phonation time,mean airflow rate,sound pressure level and average pitch were recorded in order to induce the relation of phonation and respiration in normal subjects. ResultsThere was a linear correlation between phonation volume and vital capacity. Maximum sustain phonation time varied with changes of mean airflow rate and phonation volume and mean airflow rate varied little in different subjects and had no relation with average pitch and sound intensity. ConclusionMaximum phonation time, the mean airflow rate and ratio of phonation volume to vital capacity can be used as indicator in clinical application.
3.Clinical therapeutic observation of doxazosin mesylate in treatment of vesical spasm in 256 patients undergoing surgery for relieving bladder outlet obstruction

Objective To study the preventive and therapeutic effect of doxazosin mesylate (Cardura) in the management of vesical spasm. Methods Two hundred and fifty six cases were randomized into two groups: the study group was given Cardura 4 mg at bedtime, while the control group was given probanthine 15 mg twice a day. All the patients were given 8 ng of Vitamin Ks three times a day. Results There was significant difference in the incidence of vesical spasm between study group and control group (P<0.01). Conclusions Cardura can decrease the urethral closure pressure, and reduce excitability of bladder detrusor muscle. So it is effective for the relief of vesical spasm.
4.Clinical study of the correlation of obesity with chronic periodontitis

Objective:To investigate the relationship between obesity and chronic periodontitis.Methods:2 1 36 subjects (1 091 males and 1 045 females)were included.Body mass index(BMI)was calculated.Gingival index (GI),tooth pocket probing depth (PD)and attachment loss (AL)were measured at 6 ponint of each tooth according the quartation method.The correlation between obesity and the indexes of periodontitis was studied by Spearman analysis.Results:There was a significant positive correlation of BMI degree with chronic periodontitis;there was no correlation between BMI and GI,PD in men,but BMI was positively correlated with AL,BMI and GI in women.Conclusion:Progress of chronic periodontitis may be related to obesity,the relation is more significant in women.

7.Effects of caveolin-1 on the expression of epidermal growth factor in fibroblasts co-cultured with breast cancer cells

BACKGROUND: Caveolin-1, as the most important functional protein in caveolae, is involoved in a variety of cell biological processes, and is closely related to the occurrence and development of breast cancer. OBJECTIVE: To explore the effect of caveolin-1 on epidermal growth factor (EGF) in fibroblasts after co-cultured with breast cancer cells.METHODS: Caveolin-1 expression in fibroblast lines ESF-1 was interfered with siRNA, and the optimal effect was determined through QRT-PCR and western blot assay. (1) The optimal silencing model of ESF-1-Caveolin-1 SiRNA-N.2 was obtained, which was co-cultured with breast cancer cells BT474 as experimental group, single-cultured ESF-1 and ESF-1-Caveolin-1 SiRNA-N.2 as controls. The expression level of EGF in ESF-1 was detected by QRT-PCR at 24 and 48 hours of culture; the expression level of EGF in the culture medium was detected by ELISA at 48 and 72 hours of culture. (2) ESF-1-Caveolin-1 SiRNA-N.2 (experimental group) and ESF-1 (control group) were respectively co-cultured with BT474, and single-cultured BT474 as blank control group. The proliferation of BT474 was detected by cell counting-kit 8 assay after 24- and 48-hour culture.RESULTS AND CONCLUSION: The mRNA expression level of EGF in the experimental group was significantly higher than that in the other two groups (P < 0.05), and the EGF expression level in the ESF-1-Caveolin-1 SiRNA-N.2 group was significantly higher than that in the ESF-1 group (P < 0.05). The protein expression level of EGF was ranked as follows: experimental group > ESF-1-Caveolin-1 SiRNA-N.2 group > ESF-1 group (P < 0.05). The proliferation of BT474 cells was significantly increased after co-cultured with BT474 cells and ESF-1 siRNA Caveolin-1 cells, especially with BT474 cells (P < 0.05). Our findings suggest that Caveolin-1 siRNA can promote the expression of EGF in fibroblasts, especially co-cultured with breast cancer cells. Furthermore, caveolin-1 siRNA accelerates the proliferation of breast cancer cells after co-cultured with fibroblasts.
8.A clinical study on the relationship between neuropeptide substance P in gingival crevicular fluid and chronic periodontitis

Objective: To study the relationship between occurrence and development of chronic periodontitis and neuropeptide substance P (SP) in gingival crevicular fluid. Methods: There were 48 patients in experimental group and 42 patients in the control group. Gingival crevicular fluid samples were collected from the patients′ intraoral chronic periodontitis inflammatory positive sites and healthy sites. Neuropeptide substance P levels in gingival crevicular fluid from two groups were measured by radioimmunoassay. Gingival index, pocket depth, attachment loss, alveolar bone resorption were recorded and their correlations with SP were analyzed. Results: SP levels of the experimental group were significantly higher than those of the control group(P<0.01). There were significant differences(P<0.01)between SP level and periodontal clinical index in slight periodontitis and the control group. There were significant correlations(P<0.05)between GCF SP content and all PD, AL, ABL but GI in slight periodontitis, and there were significant correlations in between GCF SP content and GI for both moderate periodontitis and severe periodontitis. There were significant correlations(P<0.01) between GCF SP content and PD,AL,ABL for both moderate periodontitis and severe periodontitis. Conclusion:GCF SP level is closely related to occurrence and development of chronic periodontitis.GCF SP level can reflect the degree of the damage of periodontium.Determination of patients′ GCF SP level has important significance for the patients′ condition judgement and treatment guidance.
9.The value of studying liver function reserve in hepatic carcinoma by 13C-methacetin breath test

Objective To investigate the characteristic of 13C-methacetin breath test (13C-MBT) as a tool to monitor the hepatic function of patients with hepatic carcinoma by comparing with Child-Pugh classification and general liver function. Methods Thirty-nine patients with primary liver cancer, 16 patients with hepatic metastasis and 14 healthy volunteers serving as controls were included in this study. According to Child-Pugh classification, the primary liver cancer patients were divided into A, B and C subgroups. All subjects received 13C-MBT and routine liver function tests after an overnight fast. The three major parameters of 13C-MBT i.e. maximum excretion rate before 40 min (Mwnax40), 13CO2 cumulative excretion of 40 min(CUM40) and that of 120 min(CUM120) were recorded and the two metabolism curves (DOB curve, MV curve) were made. Results (1) In the control, hepatic metastasis and primary liver cancer groups, both the DOB curve and MV curve were similar in shape; the peak time occurred at about 20 min after administration and then the curves lowered progressively. There were significant differences between the primary liver cancer group and the other two groups, but it was not statistically different between the hepatic metastasis group and the controls. The shape was obviously distinct in the groups A, B and C of primary liver cancer. The group A had a single sharp peak curve, group B a relatively flat peak curve with a lower level for a long time after the ascending phase and group C no clear excretion peak or even a negative curve. (2) As to the three parameters of 13C-MBT, there were statistical difference between the primary liver cancer group and the other two groups(P <0. 05). Between hepatic metastasis group and controls,there was statistical difference about CUM120 (P <0. 05), but no statistical difference about Mvmax40 and CUM40. While in the three groups with primary liver cancer groups, there was statistical difference between group A and B in Mvmax40 and CUM40 ( P < 0. 05 ), but no statistical difference between group B and C. As to CUM120, there was statistical difference only between group A and C. (3) Comparing the three parameters of 13C-MBT with routine liver function tests, there was negative correlation with TBA, positive correlation with Alb, PA, ChE and no correlation with ALT, AST, TBil, γ-GT, ALP and PT. (4) There was a good consistency between 13C-MBT and Child-Pugh classification in the evaluation of liver function of patients with liver cancer ( Kappa = 0. 647, P < 0. 05 ). Conclusion The value of the three parameters of 13C-MBT is decreased with severity of the liver disease and 13C-MBT may be used to evaluate the reserved hepatic function in patients with primary liver cancer with a diagnostic value equivalent to Child-Pugh classification. The study further confirms that 13C-MBT has correlation with TBA,AIb, PA and ChE.
