1.Clinical study ofWenyang-Gushen-Huayu decoction on treating diabetic nephropathy

Objective To assess the clinical effectiveness ofWenyang-Gushen-Huayu decoction on treating diabetic nephropathy (DN).Methods One hundred and twenty DN patients in Hospital of Penglai Chinese Medicine were included and randomly divided into treatment group and observation group. Both groups were given conventional treatment, and the treatment group was added withWenyang-Gushen-Huayu decoction. One course of treatment lasted four weeks in both groups and each group was observed for 2 courses. The clinical symptoms, vital signs, urinary albumin excretion rate (UAER), and 24 h urinary proteinwere observed. Results The treatment group showed significantly higher effective rate (93.34%vs.73.33%) than that in the control groupa(χ2=8.640,P=0.003). TheWenyang-Gushen-Huayu decoction significantly improved the symptoms of waist chills (2.47 ± 1.11vs.2.92 ± 1.36,t=1.986), edema of face and feet (1.81 ± 0.96vs.2.14 ± 1.01,t=5.165), fatigue of the gods (2.83 ± 1.12vs.3.51 ± 1.18,t=3.237), UAER (326.4 ± 98.7μg/minvs. 421.5 ± 185.6μg/min;t=3.504) and UP (0.7 ± 0.33 g/24hvs. 1.0 ± 0.37 g/24h;t=1.845).ConclusionsTheWenyang-Gushen-Huayu decoction could improve the symptoms and reduce UAER of DN patients.
2.Pharmaceutical Care for a Patient with Sick Sinus Syndrome Complicated with Essential Thrombocythemia

Objective:To explore the main points of pharmaceutical care performed by clinical pharmacists for patients with sick sinus syndrome ( SSS) and essential thrombocythemia ( ET) . Methods:Clinical pharmacists participated in the whole process of drug treatment for a case of SSS and ET,and focused on the rational use of hydroxyurea ( Hu) and aspirin, the intervention in the medication and the positive follow-up. Results:Serious ADR such as bone marrow suppression was avoided by pharmaceutical care, and bleeding increase including pocket hematoma during the operation didn’ t come up. Conclusion: Timely medical intervention, pharmaceutical care for the key drugs and strengthened follow-up are the breakthrough points for pharmaceutical service of clinical pharmacists.
3.Gene cloning and expression of Japanese encephalitis virus E protein in Escherichia coli

Objective To express Japanese encephalitis virus (JEV) E protein in Escherichia coli. Methods The gene coding for JEV E protein was amplified by using RT-PCR, and cloned into plasmid pET-28a. The constructed plasmid was transformed into E.coli BL21 (DE3). Expression of E protein by the transformants was induced by IPTG and analyzed by SDS-PAGE and Western blot. Results Five nucleotide changes leading to amino substitutions were identify in the E protein gene of our laboratory strain of JEV when compared to a published gene sequence of JEV E protein. The yield of expressed JEV E protein with relative molecular mass approximately 52?10 3 was 35% of total bacterial proteins. Conclusions JEV E protein was expressed successfully in E.coli, which should be useful for the production of diagnostic reagents and the analysis of gene structure/function of the E protein.
4.Clinical Observations on Acupuncture Stage Treatment of Facial Spasm

Objective To validate the therapeutic effect of acupuncture stage treatment on facial spasm and seek a better method for treating this disease. Method One hundred and forty patients were randomly allocated to stage treatment (50 cases), acupuncture (50 cases) and Western medicine (40 cases) groups. In the stage treatment group, treatment was divided into early, middle and late stages according to the patients’ duration of disease. Each stage provided acupuncture point injection of different medicine plus warm needling. The acupuncture group received conventional acupuncture with 1 hour retention of needles and the Western medicine group, oral administration of carbamazepine tablets, 0.2 g 3 times a day. After the completion of treatment course, a six-month follow-up was performed to determine no relapse or aggravation in the three groups and the therapeutic effects were evaluated using Cohen and Albert spasm grading criteria.Result The complete resolution rate was 56% (28/50) in the stage treatment group, 14.0% (7/50) in the acupuncture group and 20.0% (8/40) in Western medicine group. The complete resolution rate was significantly higher in the stage treatment group than in the acupuncture and Western medicine groups (P<0.05). The total efficacy rate was 100.0% (50/50) in the stage treatment group, 70.0% (35/50) in the acupuncture group and 60.0% (24/40) in the Western medicine group. The total efficacy rate was significantly higher in the stage treatment group than in the acupuncture and Western medicine groups (P<0.05).Conclusion Stage treatment has a marked effect on facial spasm.
5.A case report of the ketogenic diet for severe cerebral complications after ependymoma operation

The ketogenic diet(KD) treatment program planned for a 2 years old and 5 months boy suffered from severe cerebral complications after ependymoma operation in the Children′s Hospital of Nanjing Medical University at December 2018 was analyzed retrospectively.The patient developed various complications, including cerebral hemorrhage, hydrocephalus and hernia after the operation of ependymoma, with coma, fever and convulsion, and 1 month of anti-infection and antiepileptic treatment was ineffective.After 2 weeks of 4∶1 KD, fever and convulsion were effectively relieved without obvious side effects.One month later, the patient regained consciousness, demonstrating that KD can be applied to treat severe cerebral complications after ependymoma operation.
6.Ruptured Achilles tendon repaired with hernia repair materials using a wrap method

BACKGROUND:The incidence of complications folowing conventional repairing for the ruptured tendon is up to 73%, such as delayed healing, adhesion, secondary rupture. These complications severely affect ankle joint function of the patients. OBJECTIVE:To reduce the complications folowing the repair and investigate the role of hernia repair materials in the repairing of ruptured Achiles tendon. METHODS:Eleven patients who underwent repair of ruptured Achiles tendon were retrospectively studied. Among them there were eight males and three females with the mean age of 42 years (26-58 years). Three cases were injured in left side and eight cases were in right side; four cases were open injury by sickle-induced damage and seven cases were closed injury by exercise-induced damage. The hernia repair materials after pruning was applied with a “wrap” method to strengthen the anastomosis repair of Achiles tendon after the conventional repair surgery had been completed. At the time of folow-up, al patients were observed for postoperative local appearance and function recovery, and assessed with Arner Lindholm standards. RESULTS AND CONCLUSION:The folow-up interval was 6-45 months (average 24.5 months). Two cases were lost to folow-up. According to the Arner Lindholm standards, the effects were excelent in seven cases (78%) and good in two cases (22%). Local appearance and function recovered wel postoperatively, and no complications occurred such as wound infection, secondary rupture and so on. A “wrap” method with hernia repair materials is suggested to strengthen the anastomosis repair of ruptured Achiles tendon after the conventional repair surgery is completed, it can significantly promote the healing of Achiles tendon ruptured end anastomosis, prevent Achiles tendon recurrent ruptures, local adhesion and infection.
7.Clinicopathological characteristics of colorectal cancer presenting at young age

Objective The aim of this study was to analyse characteristics of CRC in a cohort under the age of 40 .Methods Using single center retrospective cohort study ,we reviewed the prospectively collected database of 2 897 colorectal cancer patients who had undergone curative CRC resections in Chongqing Medical University between 2010 and 2014 .175 patients (5 .8% ) were under 40 ,in which six patients for various reasons (including recurrent colorectal cancer hospital ,incomplete information ,etc .) were excluded .A group of 180 consecutive patients over the age of 40 undergoing surgery for colorectal cancer in the same centre was used as control .Results There had no difference in tumor classification and tumor location between the younger group (<40) and the older group(>40) ,but the lymph node positive rate in younger group was higher ,unable to accurately grasp the preopera‐tive lymph node status ,lead to lack of preoperative staging ,and that made it difficult to preoperative treatment options .Conclusion Therefore ,to young people in colorectal preoperative neoadjuvant chemoradiation indications and the assurance of intraoperative re‐section range ,we need to do more consideration.
8.Antibiotic Application in Internal Medicine Department Inpatients:Analysis of Rationality and Frequency

OBJECTIVE To analyze antibiotic usage of inpatients in an internal medicine department,probe into the features of antibiotic usage and provide rference for the rational clinical drug-use.METHODS We selected randomly 3102 samples from inpatients in our Internal Medicine Department during the period from Jan to Jun,2007,and analyzed the situation of the antibiotic application.RESULTS The percentage of antibiotic application was 66.18% among 3102 patients,totally 28 kinds from 10 species of antibiotics were used.Cephalosporins and quinolones were mostly applied,and more often they were used in combination.CONCLUSIONS Medication frequency of cephalosporins and quinolones ranks the first in the DDDs,and remains as the primary choice of anti-infectives used.
9.Isolation and preliminary identification of 74×103 molecule as putative Japanese encephalitis virus receptor

Objective To isolate and identify the putative Japanese encephalitis virus (JEV) re-ceptors from C6/36 and Vero cells. Methods Molecules binding with JEV were isolated from C6/36 and Vero cells by co-immunoprecipitation (Co-IP) approach, identified by mass spectrometry, and detected by Western blot. The location of putative JEV receptor on cells membrane and the binding with JEV were ob-served by laser scanning confocal microscopy (LCM). Results Several molecules binding with JEV were isolated from C6/36 and Vero cells by Co-IP, and only one molecule was identified as heat shock cognate 70 (HSC70) by mass spectrometry. Antibody against HSC'70 was able to detect a 74 ×103 protein isolated by Co-IP from C6/36 and Vero cells membrane in Western blot assays. It was observed by LSCM that when JEV attached on the surface of C6/36 cells, JEV and HSCT0 protein were co-localization. Conclusion 74 x 103 molecular identified as HSC70 protein from C6/36 cells may be JEV receptor.
10.Establishment of L1210 cell line resistant to cis-diamminedichloro- platinum and a preliminary observation on its biology

AIM To establish the expermental model in vitro for the study of mechanism of antitumor drug resistance and the screening of antitumor drug. METHODS By continuously exposing cells to gradually increasing concentration of drug and agar cell colony forming technique, using dye exclusive method for determing cytotoxic effect, a murine leukemia L1210 cell subline were established,which exhibited 40 fold resistance to Cis diamminedichloro platinum(DDP). RESULTS The doubling times, plate efficiency, cell cycle, DNA index and cell morphology of DDP resistant L1210 subline were similarto those of its parant cell line. When the cell subline was stored with DDP at -196℃ for 6 months and then was recovered, its characterization of antitumor drug resistance was still maintained to a period of 5 months without DDP. DDP resistant L1210 subline was characterized with cross resistance to Carboplatin, Mitomycin, Thio Tepa, Methotrexate, Vincristin and Mustine Hydrochloride, but with no cross resistance to Harringtonine and Adriamycin. It was seemed more sensitive to Cytarabine and Fluorouracil. CONCLUSION DDP resistant L1210 subline is a good experimental model in vitro for the study of mechanism of antitumor drug resistance and the screening of antitumor drug.
