1.Surgical treatment for intra-articular calcaneal fracture

Objective To investigate the effects of the locking compression plates on the intra-articular calcaneal fracture.Methods The lateral foot incision and locking plate was used for treatment of calcaneal fracture 54 cases.Results 36 patients were followed average 18 months(6-36months).The overall results to the Maryland Scale were evaluated as excellent in 26 cases,good in 5 cases.The rate of excellent and good was 86%.Conclusion The internal fixation with locking compression plate in treatment of calcaneal fractures can achieve good clinical results.It is a kind of ideal treatment method.

Fiber type of skeletal muscle and its multiple and specialized protein isoform are the structure and molecular basis of the skeletal muscle's function and adaptability.Myosin heavy chain(MHC)isoform,as the factor determinating the type of the skeletal muscle to be fast or slow one,has become molecular biomarker of muscle fiber type and adaptability.Exercise may lead to the transform between myosin subtypes.This article reviewed not only the relation between myosin heavy chain and muscle fiber type of skeletal muscle,but also the influences of different motor patterns' on the transform of skeletal muscle fiber MHC isoform.
3.Pharmaceutical Care for a Patient with Sick Sinus Syndrome Complicated with Essential Thrombocythemia

Objective:To explore the main points of pharmaceutical care performed by clinical pharmacists for patients with sick sinus syndrome ( SSS) and essential thrombocythemia ( ET) . Methods:Clinical pharmacists participated in the whole process of drug treatment for a case of SSS and ET,and focused on the rational use of hydroxyurea ( Hu) and aspirin, the intervention in the medication and the positive follow-up. Results:Serious ADR such as bone marrow suppression was avoided by pharmaceutical care, and bleeding increase including pocket hematoma during the operation didn’ t come up. Conclusion: Timely medical intervention, pharmaceutical care for the key drugs and strengthened follow-up are the breakthrough points for pharmaceutical service of clinical pharmacists.
4.Analysis of risk factors for stroke-associated pneumonia

Clinical data of 595 patients of stroke admitted to the First Affiliated Hospital of Dalian Medical University, Dalian, Liaoning were analyzed retrospectively, 310 (52. 1% ) of them with strokeassociated pneumonia (SAP). Early-onset pneumonia occurred in 225 patients (72.6%), and late-onset pneumonia in 85 (27.4%). Patients with length of hospital-stay equal to or more than two weeks accounted for (113 cases) 50. 3% of those with early-onset pneumonia, whereas patients with length of hospital-stay less than one week (60 cases) accounted for 70. 6% of those with late-onset pneumonia. Age above 65 years ( OR = 1. 037 ), diabetes ( OR = 1. 724), Glasgow coma scores equal to or less than eight ( OR = 0. 098 ),nasal feeding ( OR = 6. 640 ), administration of gastric mucosal protective drugs ( OR = 3. 581 ) and antibiotic prophylaxis ( OR = 2. 433) all were risk factors for SAP.

Objective To study the preventive and therapeutic effects and the mechanism of Chinese traditional compound medicine to subcilincal hepatic encephalopathy(SHE)induced by endotoxin in liver cirrhotic rats.Methods The liver cirrhosis model of rats were made in subcutaneous injection of 40% carbon tetrachloride,the SHE model was made by infused with endotoxin into the tail vein.Treatment group was given low dose or moderate dose or high dose of Chinese traditional compound medicine,Control group was treated with lactulose.The morbidity of SHE,hepatic function,blood ammonium(BA),endotoxin,liver tissue pathology were observed.Results Above indexes were improved by every group of Chinese traditional compound medicine.The curative effect of moderate and high group of Chinese traditional compound medicine to the hepatic function,BA,endotoxin,liver tissue pathology were better than that of lactulose.Conclusions The effects of the Chinese traditional compound medicines(compose of Polygonum cuspidatum,raw hawthornfruit,seed of Zizyphus jujuba Mill,Mongolian Dandelion Herb and Rhizoma Acoori Graminei)in SHE prevention and treatment is significant.Its mechanism might be as follows:① recovering liver function,decreasing inflammatory changes of liver tissue;② preventing the absorption of BA,lowering the content of BA;③ reducing the sensitivity of the body to endotoxin.

8.Comparison of Efficacy between Paroxetine and Estazolam on Chronic Insomnia

Objective: To determine whether paroxetine would be effective in the treatment of patients with chronic insomnia and to compare efficacy of paroxetine and estazolam on chronic insomnia. Methods: 74 patients with chronic insomnia were divided into 2 groups, receiving treatments of paroxetine and estazolam respectively. All of the subjects were assessed with Pittsburgh Sleep Quality Index (PSQI) and sleep diary. Results: 1. On the eighth day of treatment, estazolam group showed improvement, all the indexes of sleep were better than those before treatment and those of paroxetine group; 2. On the fifteenth day of treatment, paroxetine group showed improvement, all the indexes of sleep were better than those before treatment and those of estazolam group, while the indexes of the estazolam group returned to pre-treatment level. 3. At the end of treatments and 3 months' follow up, paroxetine group still showed better effect in sleep indexes than estazolam group and the pre-treatment level. Conclusion: paroxetine is effective in the treatment of chronic insomnia and shows long-term effect.
9.Preventive Effects of Antidepressants on Post-stroke Depression

Objectives: To test whether early prophylactic antidepressant treatment by paroxetine has any beneficial influence on the rate of post-stroke depression (PSD) and rehabilitation of stroke patients. Methods: A randomized, placebo controlled study involved 64 consecutive patients admitted to hospital for an acute stroke; they were treated with placebo, paroxetine (20mg/d) respectively. Before and at the end of the observation, we assessed the rate of PSD and activities of daily living (ADL), degree of neurological deficit (modified Edinburgh-Scandinavia scale, MESS) of all the subjects.Results:At the end of 12-week treatment, the treatment group had lower rate of PSD (10.7%) than control (41.4%); and they had lower score in ADL (27.6?4.8/32.8?4.1) and lower score in MESS (11.1?4.3/13.6?3.2).Conclusion:Prophylactic antidepressant treatment by paroxetine can decrease the rate of PSD and improve the neurological function of post-stroke patients.
10.Effects of contrast media on the apoptosis of glomerular endothelial cells with expression change of Bcl-2/Bax mRNA

Objective To investigate the effects of non-ionic low osmolar contrast media (LOCM) on the apoptosis and mRNA expression of Bcl-2/Bax in rat glomerular endothelial cells cultured in vitro. Methods Rat glomerular endothelial cells were exposed to ioversol with different concentrations for different time. Cell viability was then assessed by MTT method and morphological changes of cells were observed by inverted microscopy. In order to analyze cell apoptosis, DNA electrophoresis was applied. Meanwhile, the expression changes of Bcl-2/Bax at mRNA level were determined by RT-PCR. Results The viability of rat glomerular endothelial cells was inhibited by ioversol in a concentration and time dependent manner. After being exposed to ioversol(90 mL/L)for 24 h, the cells began to become crimple, round, and even floating. A typical DNA ladder determined by agarose gel electrophoresis was observed. Noticeably, the mRNA level of Bcl-2 in ioversol treated cells was down-regulated, while Bax was up-regulated, so that the ratio of Bcl-2 mRNA to Bax mRNA increased significantly. Conclusion Non-ionic LOCM can induce glomerular endothelial cells apoptosis, which may be partly due to the disturbance of Bcl-2/Bax balance. This is probably related to the pathogenesis of contrast media nephropathy.
