Building a digitized and customer-oriented hospital customer service system is conducive to better service at the hospital.Such a system is an open customer service and management system,comprising digitized customer relation management and service platform,digital intelligent IVR customer service system,video interaction service system for hospital wards,IOT-based health management and service system,and 3G-based seamless pre-hospital and post-hospital green emergency rescue system,as well as other customer service systems including the outpatient auto-calling,self-service,information release,payment management,and IOT-based customer management system.By means of thorough customer analysis,automatic service trigger-off mechanism and considerate customer service,digitized hospital customer service system realized value of the hospital while providing the customers with valuable services.
2.Research on knowledge management reform of national famous doctor studios under the background of new media technologies

The development of new media technologies and the proposal of Healthy China program have provided new technical support and policy endorsement for health services.At the same time, knowledge adaptability, service efficiency and cooperation mode are bringing forth challenges to traditional Chinese medicine(TCM) knowledge management. This study, citing the National famous doctor inheritance studios as an example, introduced the dimension of knowledge value into the knowledge management of TCM, constructed the cycle path of knowledge management, and analyzed the process of knowledge management, knowledge output and application, for the purposes of exploring the main reasons for the dilemma of knowledge sharing in the communities of National famous doctor inheritance studios.Finally, the paper put forward targeted measures in the following aspects: the creation of inheritance and innovation value chain, the expansion of social cooperation mode, the refinement of health service path and the integration of human resources. These measures aim to provide references for guiding the reform of TCM knowledge management and improving the value creation of TCM knowledge.

4.Goals, Model and Path of Construction of Key Discipline of Traditional Chinese Medicine Management

The discipline of traditional Chinese medicine (TCM) management covers the fields of medical and health services and health industries of TCM. Construction of key discipline of TCM management can help improve the level of disciplines of TCM cause management, promote academic development of TCM cause management and enhance the training level of specialties of TCM economics and management. The research on the goals, model and path of key discipline of TCM management is the core issue of the construction of key discipline of TCM management, as well as characteristics and advantages of management disciplines in the TCM colleges and universities. According to features of TCM colleges and universities, studying health management of TCM by using the theories and methods of modern management and modern economics can promote the optimal allocation of TCM resources and medical service technology and improve the theoretical system of TCM. As a result, all directions of TCM management can better achieve scientific management characteristics and advantages of health care and health rehabilitation. Also, it can enhance the level of TCM management and team building and train outstanding senior personnel of TCM cause management, so as to achieve the comprehensive promotion of specialties’ overall level of the discipline.
5.Application of improved 13-core prostate biopsy in the diagnosis of prostate cancer

Objective To investigate the clinical value of transrectal ultrasound-guided improved 13-core prostate biopsy in the detection of prostate cancer.Methods Retrospective analysis was conducted in the clinical data of 82 clinically suspected prostate cancer patients in Shaanxi Provincial People's Hospital from October 2015 to October 2016.All patients were undergone transrectal uhrasound-guided improved 13-core prostate biopsy and surgical treatment,and the prostate biopsy results were compared with the postoperative pathological results.The accuracy of the improved 13-core prostate biopsy and the standard 6-core prostate biopsy was compared.The complications of improved 13-core prostate biopsy were also discussed.Results The diagnostic results of prostate biopsy included prostate cancer in 26 patients and benign prostatic hyperplasia (BPH) in 54 patients.The diagnostic results of 80 patients are consistent with postoperative pathological examination.Two patients were diagnosed as BPH by biopsy,but confirmed to be prostate cancer by postoperative pathological examination.The accuracy of the improved 13-core prostate biopsy and the standard 6-core prostate biopsy were 97.6% (80/82) and 84.1% (69/82) respectively,and the difference showed statistical significance (P =0.023).No severe complications were found in patients who underwent transrectal ultrasound improved 13-core prostate biopsy.Conclusion Transrectal ultrasound guided improved 13-core prostate biopsy can increase the detection rate significantly.It is safe and efficacious,and can be widely used in clinic.
6.Analysis of disease characteristics and hospitalization burden of elderly patients with cerebrovascular disease based on home page of medical records

Objective:To analyze the disease characteristics and hospitalization burden of elderly inpatients with cerebrovascular disease, so as to provide basis for disease prevention, diagnosis, treatment and rehabilitation of elderly patients with multiple chronic conditions.Methods:The data of the first page of medical records of elderly inpatients with cerebrovascular disease from 2015 to 2020 in a comprehensive tertiary hospital in Jiangsu Province were retrospectively collected.Descriptive analysis, variance analysis and multiple linear regression analysis were carried out for the research objects using SPSS statistics and Python complex network methods.Results:A total of 14 657 elderly inpatients with cerebrovascular disease were included.From 2015 to 2020, the number of hospitalizations increased from 1 268 to 4 733, the average number of hospitalizations increased from 2.0 to 2.9, the average length of stay decreased from 11.9 days to 9.1 days, and the average number of illnesses increased from 1.9 to 4.9.The five most common comorbidities associated with cerebrovascular diseases were hypertension, diabetes, ischemic heart disease, other types of heart disease and other respiratory diseases.From 2015 to 2020, the average hospitalization costs of elderly cerebrovascular inpatients decreased from 20588.1 Chinese yuan(CYN)to 15580.9 Chinese yuan(CYN). The hospitalization cost was mainly composed of drug cost(46.6%), diagnosis cost(28.2%)and treatment cost(20.2%). There were significant differences in the average hospitalization expenses among patients with different gender, age, number of admissions, length of stay and number of diseases( P<0.05 for all). Gender, age, number of hospitalizations and length of stay had an impact on hospitalization expenses. Conclusions:The number of inpatients, comorbidities, and hospitalized times of elderly patients with cerebrovascular disease showed an increasing trend, while the number of hospitalization days and the average hospitalization cost showed a downward trend.Comorbidities associated with cerebrovascular diseases should be one of the concerns of chronic disease management in the elderly.
7.Analysis of hospitalization expenses and its influencing factors of elderly patients with hypertension at tertiary general hospitals in Jiangsu province

Objective:To analyze the hospitalization expenses of elderly patients with hypertension and its influencing factors, so as to provide reference for optimizing the medical service management of elderly patients with hypertension.Methods:Medical record home page data of all hypertension inpatients of elderly patients at two tertiary general hospitals in Jiangsu province from 2017 to 2020 were retrieved. These data were used to analyze the basic information, hospitalization expenses and their influencing factors. Descriptive analysis was used for all data, variance analysis was used for one-way analysis, and multiple linear regression was used for multivariate analysis.Results:A total of 20 596 elderly inpatients with hypertension were included in this study. The number of patients was increased from 1 476 in 2017 to 10 771 in 2020. Among them, the number of inpatients with≥2 diseases increased from 1 105(74.86%) to 10 564(98.08%); From 2017 to 2020, the average hospitalization expenses of elderly inpatients with hypertension were 11 500 yuan, 13 600 yuan, 13 800 yuan and 14 100 yuan respectively, increasing year by year; Gender, age, number of hospitalizations, hospitalization days and number of diseases were the influencing factors of hospitalization expenses( P<0.05), and the hospitalization expenses increased with the increase of hospitalization days, number of diseases and age. Conclusions:The number of elderly patients with hypertension, the incidence of comorbidity, and the average hospitalization cost in tertiary hospitals in Jiangsu province were increasing year by year. There were many factors affecting the hospitalization cost. The author suggested that the treatment of elderly patients with hypertension should be shifted to prevention, so as to reduce the economic burden of disease and improve their quality of life.
8.Analysis of influencing factors of community elderly health services by general practitioners from the perspective of social ecology

Objective:To identify the influencing factors for community elderly health services provided by general practitioners (GPs) using the social ecological theory, for reference in improving their participation and satisfaction.Methods:According to the social ecological theory, an ecological model for GPs to carry out community elderly health services was constructed from four levels: public policy ecology, community health service ecology, interpersonal relationship ecology, and individual characteristics ecology of general practitioners. A survey questionnaire was designed with six latent variables: public health policy support, public health service and basic medical service supply, doctor-patient relationship, individual participation and individual satisfaction. The questionnaire was distributed to 220 GPs from 11 primary healthcare institutions in Jiangsu province, China, who were randomly selected between October and November 2021. Exploratory and confirmatory analyses of the model were conducted using AMOS 25.0.Results:A total of 207 valid questionnaires were collected, and all the KMO values of the six latent variables were greater than 0.7, while the composite reliability values and average variance extracted values greater than 0.7 and 0.5, respectively. Both the reliability and validity of the data met the analysis requirements. Exploratory analysis revealed that public health policy support had a direct positive effect on both public health service and basic medical service supply (both effect sizes being 0.37). Public health service had a direct positive effect on doctor-patient relationship, individual participation and individual satisfaction (effect sizes being 0.52, 0.22, and 0.31, respectively). The direct effect of basic medical service supply on doctor-patient relationship was not significant (effect size being 0.03), but it had a direct positive effect on public health service (effect size being 0.46). Doctor-patient relationship had a direct positive effect on individual participation (effect size being 0.51), but its direct effect on individual satisfaction was not significant (effect size being 0.06). Individual participation had a direct positive effect on individual satisfaction (effect size being 0.52). Conclusions:By optimizing the public policy ecosystem, community health service ecosystem, and interpersonal relationship ecosystem, the participation and satisfaction of general practitioners can be systematically improved.
9.Exploring the Cultural Origins and Developmental Patterns of Academic Schools in Traditional Chinese Medicine from a Constructivist Perspective

Using constructivism theory, this study explored the relationship between the "origin" and "flow" of academic schools in traditional Chinese medicine (TCM), clarified the developmental patterns of schools, and provided insights for the modernization, inheritance, and innovation of current schools. Academic schools originate from different cultural sources, and their differentiation and development are similar to the replication, spread and variation of biological "genes". The theoretical characteristics of constructivism align well with the principles of formation and differentiation of academic schools. The construction of academic thoughts and core diagnostic and therapeutic concepts within each school can be seen as a multilevel unity of self-construction, mutual construction among individuals, and social construction. Firstly, individual medical practitioners choose and inherit from the origins of TCM, integrating personalized understanding, which then differentiates into various schools in the history of academia. Secondly, during the process of cultural inheritance, medical practitioners from different times and regions gradually form academic schools and local medical schools through the method of "tailoring strategies to three categories of etiological factors" and mutual construction among individuals. Finally, in the context of the scientific, standardized, and intelligent development of modern medicine, the further evolution of academic schools needs to follow the evolutionary laws of traditional medicine, combine with the health needs of the new era, adopt a social construction approach, facilitate multi-party participation in the inheritance and innovation of academic thoughts and clinical experiences, and utilize the internet and intelligent technology means to empower modern development.
10.Tenofovir vs. entecavir on recurrence of hepatitis B virus-related hepatocellular carcinoma beyond Milan criteria after hepatectomy.

BACKGROUND:
Hepatectomy for hepatocellular carcinoma (HCC) beyond the Milan criteria is shown to be beneficial. However, a high rate of post-operative HCC recurrence hinders the long-term survival of the patients. This study aimed to investigate and compare the impacts of tenofovir (TDF) and entecavir (ETV) on the recurrence of hepatitis B viral (HBV)-related HCC beyond the Milan criteria.
METHODS:
Data pertaining to 1532 patients who underwent hepatectomy and received antiviral therapy between January 2014 and January 2019 were collected from five centers. Recurrence-free survival (RFS) analysis was performed using the Kaplan-Meier method. Cox proportional hazards regression analysis was performed to determine prognostic factors for HCC recurrence.
RESULTS:
The analysis incorporates 595 HBV-related HCC patients. The overall 5-year RFS was 21.3%. Among them, 533 and 62 patients received ETV and TDF treatment, respectively. The 1-, 3-, and 5-year RFS rates were 46.3%, 27.4%, and 19.6%, respectively, in the ETV group compared with 65.1%, 41.8%, and 37.2%, respectively, in the TDF group (P < 0.001). Multivariate analysis showed that TDF treatment (hazard ratio [HR]: 0.604, P = 0.005), cirrhosis (HR: 1.557, P = 0.004), tumor size (HR: 1.037, P = 0.008), microvascular invasion (MVI) (HR: 1.403, P = 0.002), portal vein tumor thrombus (PVTT) (HR: 1.358, P = 0.012), capsular invasion (HR: 1.228, P = 0.040), and creatinine levels (CREA) (HR: 0.993, P = 0.031) were statistically significant prognostic factors associated with RFS.
CONCLUSIONS
Patients with HCC beyond the Milan criteria exhibited a high rate of HCC recurrence after hepatectomy. Compared to the ETV therapy, TDF administration significantly lowered the risk of HCC recurrence.
