1.Curative effect of weekly dose doctaxale for the treatment of elderly patients with advanced non-small cell lung cancer

Objective To investigate the clinical efficacy and toxicity of patients with advanced non-small cell lung cancer(NSCLC) treated with weekly dose doctaxale. Methods 50 elderly cases with histologically or cytologically diagnosed advanced NSCLC were treated in treatment group(n=28) ,using docetaxel(at a dose of 35mg/m2 infused on day 1 and 8,21 days as one cycle). Control group (n=22) , using best supportive care (BSC). Results The response rate 32. 0% in treatment group were significantly higher than 10. 0% in control group(x2=8. 156, P<0.01) ;The median time to tumor progression (TTP) , median survival(OS) ,1-year survival rate(6. 0month,12. 5 month,40. 1%) in treatment group were significantly higher(2. 5month,5. 2 month, 10. 3%) than those in control group(x2=7.234,7. 812,8.989,all P<0.01) ;The KPS score(89.5 ±10.2)points after treatment were higher than the control group after treatment (76. 0 ± 9. 9) points which was significantly different (t =2. 330, P<0. 05); The score of LCSS in cough,hemoptysis,chest pain,shortness of breath[(92.4 ±16.9) points, (93.4 ±17.5) points]after treatment,were significantly higher than the control group[(82. 4 ± 15. 0) points, (81. 3 ± 16. 0) points] which were significantly different(t = 2. 2150,2.245, all P<0. 05). Conclusion The weekly dose doctaxale was a feasible , well-tolerated, and active scheme in the treatment of elderly patients with advanced NSCLC.
2.INVESTIGATION OF HEALTH CONDITIONS OF THE MEDICAL STAFFS IN A GENERAL HOSPITAL

Objective To investigate the health status of the medical staffs in our hospital , find risk factors and make corresponding strategies to improve health of status .Methods Results of physical examination on 1203 medical staffs were analyzed by SAS 9.2.Results Total residual test ratio is 9.3%, and the residual test ratio of male was significantly higher than female (χ2=38.399,p <.0001).The prevalence ratio of all subject is 60.9%, and there was no significant difference between male and female , but the higher age category, the higher prevalence ratio (χ2=211.4037,p <.0001).The top 5 diseases our staffs suffering are waist and cervical vertebra disease , thyroid nodule, hyperlipidemia, breast hyperplasia and fatty liver, in which the prevalence ratio of thyroid nodule and hyperlipidemia was different between male and female .Conclusion The health status of our medical staffs should be improved by paying high attention to medical examination and health management .
3.Observation on Preventive Effect of Ca/Mg Infusion Combined with Glutathione and Carbamazepine on Oxaliplatin-induced Neurotoxicity

OBJECTIVE:To observe the preventive effect of Ca/Mg infusion combined with glutathione and carbamazepine on oxaliplatin-induced neurotoxicity.METHODS:65patients treated with oxaliplatin,5-fluorouracil and leucovorin for gas-trointestinal cancer were divided into treatment group undergoing treatment with Ca/Mg infusion combined with glutathione and carbamazepine,and control group without any preventive treatment.RESULTS:At the third cycle,7patients in the treat-ment group but17patients in the control group showed clinically evident neurotoxicity,which demonstrated the statistical difference between the two groups(P=0.036).After12cycles,neurotoxicity of grade2to3was observed in2patients in the treatment group but in9patients in the control group,which demonstrated the statistical difference between the two groups(P=0.022).CONCLUSION:Ca/Mg infusions combined with glutathione and carbamazepine can prevent oxaliplatin-induced neurotoxicity.
4.Progress in Study on Symptoms of Irritable Bowel Syndrome and Intestinal Gas

Irritable bowel syndrome( IBS)is a commonly seen chronic functional bowel disease,the prevalence showed a rising trend in recent year in China. At present,the etiology and pathogenesis of IBS have not yet been fully elucidated,and may be related to intestinal gas. Intestinal gas is usually composed of 59% N2 ,21% H2 ,9% CO2 ,7%methane(CH4 )and 4% O2 ,in which H2 and CH4 are produced only by intestinal bacteria metabolism. IBS patients usually manifested as having abdominal distension and symptoms of flatulence,suggesting the occurrence of accumulation of intestinal gas. In recent years,the relationship between symptoms of IBS and intestinal gas has attracted much attention. This article reviewed the progress in study on symptoms of IBS and intestinal gas.
5.Analysis of clinical characteristics and prognosis of minor solid pseudopapillary tumor of the pancreas

ObjectiveTo investigate the characteristics,treatment and prognosis of solid pseudopapillary tumor of the pancreas(SPTP)in the minor. MethodsCombining minor SPTP cases in literatures and 4 cases, analyzed the clinical and pathological features, and the relationship with sex hormone receptors, the growth and development,treatment and prognosis.ResultsThe clinical manifestations of SPTP in minor were same as adults, but the minors showed significant growth retardation which can be reversed after the tumor surgical removal, like 4 patients all showed secondary sex characteristics of retardation in the group. The protocol of surgical treatment was to ensure complete removal of the tumor and keep the healthy organs as much as possible which reduced the impact to the postoperative growth and development.The premise of operation therapy in the 4 cases was minimization of operation trauma. Minor SPTP prognosis had not been determined. ConclusionThe diagnosis and treatment of minor SPTP should be fully taken into account the characteristics of the minor to ensure that patients can continue long-term growth and development and the quality of life.
6.Establishment of in vitro metabolism of fentanyl by human liver microsomes in Chinese population

Objective To establish in vitro metabolism of fentanyl by human liver microsomes in Chinese population.Methods Thirty patients undergoing elective operation on liver were enrolled in the study.Normal liver specimens were obtained during removal of liver and gall for preparation of liver microsomes (by calcium precipitation) which were used for establishment of the liver microsomal incubation system for fentanyl.Fentanyl served as the metabolic substrate in the incubation reaction.The concentration of fentanyl in the incubation medium was detected at 0,5,10,15,20 and 30 min of incubation using HPLC-UV.Sufentanil served as the interior label element.The n-hexane-ethanol absolute was used to extract the sample.The chromatographic column used in this method was Grace C18 (4.6 mm × 250.0 mm,5 μm).The mobile phase was methyl cyanide-KH2PO4 buffer solution with the flow rate of 1.0 ml/min,detection wavelength of 205 nm and sample size of 20 μl.Linear regression analysis was performed by using the least-squares method.The specimens of the blank incubation system with the final concentration of fentanyl 0.6,2.4 and 10.0 μg/ml were obtained to determine the recovery,precision and stability.The metabolic rate of fentanyl in human hepatic microsomes was calculated.Results Fentanyl and the interior label element sufentanil were separated completely,and the retention time were 5.730 and 9.336 min,respectively.Endogenous matrix of microsomes did not interfere with the analysis.Regression equation was C =0.945 8A-0.140 4,R2 =0.999 2.C was the concentration of fentanyl,and A was the peak area ratio of fentanyl versus sufentanil.The recovery of incubation system with low,medium and high concentrations of fentanyl was 85%-115%,and relative standard deviation (RSD) was less than 10%.The RSD of intra-and inter-day precision and stability was less than 10%.The method was proved to meet the requirements of biological sample analysis.The metabolic rate of fentanyl was (1.6 + 0.8) nmol/min per milligram protein in human hepatic microsomes of 30 cases.Conclusion The in vitro metabolism of fentanyl by human liver microsomes is convenient,and the detectability is high,so it can be used for the research on the in vitro metabolism of fentanyl in Chinese population.
7.Application of covered esophageal stent applied in esophageal perforation (40 cases)

Objective To investigate the efficacy and safety of covered stent in treatment of esophageal perforation. Methods Observe and analyzed the effect and complications of covered metallic stent in esophageal perforation. In all of 40 cases, the most common causes were iatrogenic, esophageal cancer, and foreign body. Results All patients with esophageal perforation got successful esophageal stent placement. All of them recovered smoothly in eating. The stents were removed 4 days to 1 month later. Gastroscopy and angiography confirmed esophageal perforation has healed. 3 cases of esophageal cancer patients with dysphagia obviously relieved after stent placement. The main complications in stent and stent migration for chest pain after operation. 6 cases (15 %) of patients with stent displacement, while 2 cases (5 %) of esophageal cancer patients with food obstruction and stent displacement. Conclusion Treatment of esophageal perforation with covered metal stents endoscopically is effective and safe.
8.Studies on biomechanics of human foot: a review.

They are also the basis of the study of rehabilitation. In this paper, studies on the three aspects are reviewed, and further promising developments are proposed.

9.Experimental methods for mechanically stimulating the cells in vitro.

Cellular mechanics is a branch of tissue engineering and cellular engineering. As one of the important method, loading different mechanical stimuli to culturing cells in vitro so as to study the influence that the stress has on the cells is one of the important fields of cellular mechanics. This paper reviews the experimental methods for mechanically stimulating the cells in vitro, according to the different loading modalities, the methods can be categorized into micropipette aspiration, compression loading, substrate distention, fluid shear, etc. And it also points out their advantages and disadvantages.

10.Clinical evaluation of reliability of autologous blood withdrawal during cesarean section

Objective To evaluate the reliability of autologous blood withdrawal during cesarean section. Methods Fifteen patients preoperatively diagnosed with pernicious placenta previa and∕or accrete by using ultrasound and magnetic resonance imaging, aged 20-35 yr, weighing 55-75 kg, at≥36 weeks of gestation, were enrolled in the study. Blood containing amniotic fluid from the surgical field was collected, and the washed blood was processed using cell?salvage machine and then filtered using a leukocyte depletion filter during cesarean section. The 20 ml blood samples collected included maternal central venous blood after delivery of fetus, unwashed blood, washed blood and filtered blood. The fetal squamous cells were counted using papanicolaou staining. The concentrations of a?fetoprotein, tissue factor, endothelin?1 and histamine were measured by enzyme linked immunosorbent assay. The fetal red blood cells were counted using the acid elution method and HE staining. Results Compared with unwashed samples, the tissue factor concentrations were significantly increased, and the fetal squamous cell count, concentrations of a?fetoprotein and endothelial?1, and fetal red blood cells were decreased in the washed samples. Compared with washed samples, the fetal squamous cell count, concentrations of a?fetoprotein and fetal red blood cells were significantly decreased in filtered samples. Compared with maternal venous blood samples, the tissue factor concentrations were significantly increased, and the fetal squamous cell count and concentrations of a?fetoprotein and endothelial?1 were decreased in filtered samples. Conclusion Autologous blood withdrawn during cesarean section can be used for reinfusion in cesarean section.
