1.Three-dimensional laparoscopic surgery: a revolutionary technique or a transitional technique

Three-dimensional (3D) laparoscopic surgery has been developed rapidly in China since 2012.Based on mature two-dimensional (2D) laparoscopic surgery,3D laparoscopic surgery has the same technique as 2D,with similar indication as 2D as well.However,3D laparoscopic surgery can offer better depth description,more accurate spatial location,and less error in operation.It might offer better visional experience if hybrid with virtual reality or with 4K in the future.But in current status,laparoscopy with 3D technique is only a technique with innovation,but not a revolutionary technique.
2.Difficulties and disputes about laparoscopic radical resection of rectal cancer

With the standardized training and spreading,laparoscopic total mesorectal excision for rectal cancer has been spread nationwide.The oncological adequacy and long-term outcomes have also been confirmed by series of randomized controlled trials such as COLOR Ⅱ in Europe.There were still some technical issues,such as safe surgical plane,surgical approach,lymph node dissection,and function-preserving,which were still under debate.In the current article,authors will discuss the issues with clinical experiences about preserving of Denonvilliers facial,preserving of pelvic autonomic nerve,strategy of surgical approach,dissection of lateral lymph nodes,and necessity of protective diverting stoma.
3.Protective Effect of Baicalin on Myocardial Ischemia Reperfusion Injury and its Correlation to Myocardial Cell Autophagy in Experimental Rats

Objective: To study the protective effect of baicalin on myocardial ischemia reperfusion injury and its correlation to myocardial cell autophagy in experimental rats. Methods: The animal models were established by intragastric infusion at 7 days prior operation in different groups.Atotal of 48 rats were divided into 4 groups: Sham operation group, Sham+baicalin group, Ischemia reperfusion (IR) group and Baicalin treatment group. n=12 in each group. Hemodynamics at different time points and myocardial infarction (MI) size at 45 min after reperfusion were recorded; protein expressions of LC3-II and autophagy-related Becl-1 at 30 min after reperfusion were examined by Westernblot analysis; the opening condition of mitochondrial membrane channel transition pore (mPTP) was detected by NAD+ content. Results: The MI sizes in Sham operation group and Sham+baicalin group were too small to compare; MI sizes in IR group and Baicalin treatment group were (41.32±1.85) % vs (23.30±1.60) %, P<0.001. Protein expressions of LC3-II in IR group and Baicalin treatment group were (1.051±0.005) and (0.863±0.009) which were both higher than Sham operation group (0.763±0.007), P<0.01;Becl-1 were (1.169±0.002) and (0.943±0.005) which were both higher than Sham operation group (0.647±0.014),P<0.01; LC3-II and in Becl-1 expressions in Baicalin treatment group were decreased than IR group, P<0.01. NAD+ contents (by nmol/mg) in IR group and Baicalin treatment group were (6.02±0.33) and (9.56±0.53) which were both lower than Sham operation group (11.28±0.37), P<0.001; NAD+ content in Baicalin treatment group was increased than IR group,P<0.01. Conclusion: Baicalin had no autophagy effect in normal myocardial cells, but it may decrease the MI size and reduce excessive autophagy in myocardial cells after IR which might be related to inhibiting mPTP opening.
4.Investigation on current situation and countermeasures of both Xinjiang minority and Han medical students' autonomous learning of ideological and political theory course

Objective To understand the status of Xinjiang medical students' autonomous learning of Ideological and Political Theory Course,and put forward corresponding educational countermeasures.Method By using stratified cluster sampling method 850 Xinjiang minority and Han medical students in a university were extracted as the subjects and were surveyed by college learning autonomy self-statements Questionnaire including two dimensions of attitudes and habits of learning ideological and Political Theory Course.Using SPSS 13.0 thex2-test was used for statistical analysis,the level of test=0.05.Results On the ideological and political theory course teaching attitude,positive attitude of the minority medical students and the Han medical students were 319/67.6% and 259/73.8% respectively(P=0.009 3),and the overall satisfaction rate reached 579/70.3%.In the habit of learning the ideological and political theory course,38/8.1% minority medical students selected the library to learn,while 132/37.7% of Han medical students chose Library(P=0.007 9、0.037 0).In the choice of place of study and the way to find information there was statistically significant difference between minority medical students and Han medical students.Conclusion According to the differences of minority students' and Han students' learning ideological and political theory course,we can carry out differentiated autonomous learning training strategies through the establishment of new education ideas,innovative education carrier and method to improve both the minority medical students'and Han medical students' autonomous learning ability of Ideological and Political Theory Course
5.Effect of estrogen on the permeability of retinal blood vessel

Objective To explore the effect of estrogen on the permeability of retinal blood vessel by ovariectomy. Methods Twenty-two healthy rats were divided into experimental and control group randomly. Estrogen level of rats decreased due to ovariectomy in the experimental group while stabilized by sham-ovariectomy in the control group. The results were confirmed by vaginal epithelium smearing. Retinal vein occlusion was established by photodynamic method, and leakage of Evan's blue in retina was determined by spectrophotometer. Results Mature value of vaginal epithelium decreased significantly in ovariectomy rats( t=21.008,P =0.000) while not significantly in sham-ovariectomy ones ( t=0.319,P =0.756); the mean leakage of Evans blue was (25.503 0?4.378 47) ng/mg in experimental group, and (17.830 0?4.265 69) ng/mg in the control group, and the difference between the two groups is significant( t=3.969 36,P =0.001). Conclusion Ovariectomy is an useful method to study the effect of estrogen on ocular diseases, and when estrogen level decreases, the permeability of retinal blood vessel increases.
6.The application of the triamcinolone acetonide injection in vitreoretinal surgery

Objective To discuss the role and effect of the triamcinolone acetonide (TA) injection applied in the vitreoretinal surgery for assisting to distinguish and removal residual cortex. Methods One thousand and forty-six patients ((1 056) eyes) underwent a PPV at West China Eye Center from August 2003 to February 2005. These affected eyes were selected without posterior vitreous detachment pre- and intra-operatively, and underwent intravitreal injection with TA 1-2 mg to label the residual vitreous cortex on the surface of the posterior retina after removal of the majority vitreous. Results The location and coverage of the residual vitreous cortex were clearly be shown by the white appearance of TA labeled residual vitreous, and the residual vitreous cortex was conveniently to be recognized and removed. Conclusion The application of intravitreal injection with TA is helpful for identification and elimination of the residual vitreous cortex in vitreoretinal surgery.
7.Nuclear factor kappa B,Bcl-2 and Bax in limb function after acute spinal cord compression injury

Objective To investigate the relationship of nuclear factor kappa B(NF-κB),Bcl-2 and Bax with limb function after acute spinal cord injury in rats. Methods Forty-eight rats were divided at random into a control group and an experimental group with 24 rats in each.The spinal cords of the rats in the experimental group were injured at the T8,9,10 level through moderate compression.Four hours,8 h,and 1,3,7 and 14 days after the injury,4 rats were selected randomly from each group and graded with a BBB score.They were then sacrificed and their spinal cords were collected.Immunohistochemical measurements were used to observe the expression of NF-κB, Bcl-2 and Bax. Results NF-κB,Bcl-2 and Bax were observed in the injured spinal nerve cells of rats in the exper imental group but were absent in the control group.After injury,the expression of these factors increased at first and then decreased.BBB scores for limb function increased gradually.No correlation was found between the changes in NF-κB and Bcl-2,but the expression of NF-κB was positively correlated with that of Bax.There was negative correla tion between NF-kB levels and BBB scores,and between NF-kB levels and the ratio of Bcl-2 to Bax. Conclusion In rats,there is a close negative correlation between NF-kappa B levels,the ratio of Bcl-2/Bax and limb function after acute spinaI cord iujury.
8.Significance of serum procalcitonin levels in children with respiratory syncytial virus pneumonia and sepsis

Objective To investigate the significance of serum procalcitonin(PCT)levels in children with respiratory syncytial virus(RSV)pneumonia and sepsis.Methods 43 cases of children with RSV pneumonia were divided into two groups,including 28 cases of non -sepsis and 15 cases of sepsis.30 cases of healthy children were choosed as a control group in the same period.The serum PCT levels of three groups were compared.The 29 cases of RSV pneumonia with other organ damage except lung were divided into non -severe group and severe group.The differences of serum PCT levels between the two groups were compared.Results The non -sepsis group and sepsis group serum PCT levels were (0.882 ±0.184)μg/L and (1.001 ±0.268)μg/L respectively,which were higher than the control group[(0.365 ±0.085)μg/L,(t =2.607,2.854,all P <0.05)].The serum PCT levels of the non -sepsis group and septic group were significant different(t =0.372,P >0.05);The serum PCT level of the severe group was (1.181 ±0.281)μg/L,which was higher than the non -severe group[(0.448 ±0.140)μg/L],the differ-ence was statistically significant(t =2.473,P <0.05).Conclusion The serum PCT of RSV pneumonia children was increased,the increased PCT can be an observed indicators of severe infection and multiple organ dysfunction.
9.Current application and prospect of three-dimensional laparoscopic radical resection of colorectal cancer

Most colorectal surgeries now are carried out as minimally invasive surgery along with the development and application of laparoscopy.However,the surgical imaging system does not meet the progressing requirement of laparoscopic surgeons.Thanks to the advent of three-dimensional (3D) laparoscopy,a more sophisticated surgical scene with realer images of anatomy is provided to surgeons.It makes identification and manipulation easier,so that to shorten the learning curve.Meanwhile,the 3D laparoscopy is not perfect.But authors believe it will finally be used broadly in colorectal surgery with the improvement of the technology.
10.The correlation between serum miR-21 and miR-34a expression and the efficacy of Flofox chemotherapy regimen in patients with advanced gastric carcinoma

Objective The objective of this study was to investigate the levels of serum miR-21 and miR-34a in patients with advanced gastric cancer before and after Folfox chemotherapy and its relationship with chemotherapy efficacy.Methods Forty-five patients with advanced gastric cancer who underwent Folfox chemotherapy were enrolled from May 2015 to October 2015.The levels of miR-21 and miR-34a were analyzed by Real-time PCR before and after chemotherapy.The relationship between miR-21 and miR-34a levels and chemotherapy efficacy was analyzed.The predictive values of miR-21 and miR-34a on the efficacy of chemotherapy were evaluated by the receiver operating curve(ROC).Results After chemotherapy,6 cases(13.33%)were relieved,10 cases(22.22%)were stable disease(SD),29 cases(64.44%)were disease progress(PD),the objective response rate(ORR)was 13.33%,the disease control rate(DCR)was 35.56%,the median survival time was 8.1 months.The level of miR-21 was down-regulated and up-regulation for miR-34a after chemotherapy.They were statistically significant(P<0.05).The level of miR-21 in the chemotherapy group was lower than that in the ineffective group,and the level of miR-34a was higher than that of the ineffective group(P<0.05).MiR-21 was negatively correlated with the efficacy of chemotherapy(rs=-0.431,P<0.05),while miR-34a was positively correlated with it(rs=0.504,P<0.05).The median survival time was 7.6 months for low-level patients with miR-21 and 6.0 months for high-level patients.The median survival time of miR-34a patients was 5.5 months,compared with 8.3 months for high-level patients,with a statistically significant difference(P<0.05).The Log-Rank test showed that the median survival of patients with low miR-21 was higher than that of patients with high miR-21(P<0.05).The median survival of patients with high miR-34a was higher than that of patients with low miR-34a(P<0.05).The miR-21 combined with miR-34a in predicting chemotherapy efficacy under the curve(AUC)was 0.865,the sensitivity was 80.9% and the specificity was 89.8%.Conclusion miR-21 and miR-34a are correlated with chemotherapy efficacy median survival time of patients with advanced gastric cancer.
