1.Liquiritin improves macrophage degradation of engulfed tumour cells by promoting the formation of phagolysosomes via NOX2/gp91phox.

The incomplete degradation of tumour cells by macrophages (Mϕ) is a contributing factor to tumour progression and metastasis, and the degradation function of Mϕ is mediated through phagosomes and lysosomes. In our preliminary experiments, we found that overactivation of NADPH oxidase 2 (NOX2) reduced the ability of Mϕ to degrade engulfed tumour cells. Above this, we screened out liquiritin from Glycyrrhiza uralensis Fisch, which can significantly inhibit NOX2 activity and inhibit tumours, to elucidate that suppressing NOX2 can enhance the ability of Mϕ to degrade tumour cells. We found that the tumour environment could activate the NOX2 activity in Mϕ phagosomes, causing Mϕ to produce excessive reactive oxygen species (ROS), thus prohibiting the formation of phagolysosomes before degradation. Conversely, inhibiting NOX2 in Mϕ by liquiritin can reduce ROS and promote phagosome-lysosome fusion, therefore improving the enzymatic degradation of tumour cells after phagocytosis, and subsequently promote T cell activity by presenting antigens. We further confirmed that liquiritin down-regulated the expression of the NOX2 specific membrane component protein gp91 phox, blocking its binding to the NOX2 cytoplasmic component proteins p67 phox and p47 phox, thereby inhibiting the activity of NOX2. This study elucidates the specific mechanism by which Mϕ cannot degrade tumour cells after phagocytosis, and indicates that liquiritin can promote the ability of Mϕ to degrade tumour cells by suppressing NOX2.
2.Analysis of 32 cases of appendectomy after endoscopic retrograde appendicitis therapy in children

Objective:To investigate the related risk factors of surgical appendectomy after endoscopic retrograde appendicitis therapy (ERAT) in children.Methods:From September 2019 to November 2023 at the Children's Hospital Affiliated to Shandong University,the data from all related children with appendectomy after ERAT were analyzed. The general situation and main clinical manifestations of the children were collected. According to the effect of ERAT,the patients were divided into transfer to surgical group and appendicitis recurrence group by the reasons and time of transfer to surgical treatment. The indexes such as ERAT and reoperation process and pathological types of appendix after operation were collected,and the clinical experience was summarized.Results:Among the 242 children who underwent ERAT,32 cases underwent appendectomy again,including 19 males and 13 females,with an average age of(9.16±2.77)years,and the reoperation rate was 13.2%. The clinical manifestation was abdominal pain (32 cases,100.0%),mainly right lower abdominal pain (28 cases,87.5%),and the main duration was less than 3 months (30 cases,93.8%). Abdominal ultrasound or CT before ERAT mainly indicated appendicitis (11 cases,34.4%) and appendicitis complicated with appendiceal calculus (11 cases,34.4%). There were 9 children in transferred to surgical group,of which 5 cases were transferred to surgery because of ERAT intubation failure,and 4 cases were transferred to surgery because of appendix perforation or abscess. Postoperative pathology showed acute suppurative appendicitis in 5 cases and acute gangrenous appendicitis in 4 cases. There were 23 children in appendicitis recurrence group,whose abdominal pain was relieved after ERAT,but their symptoms were repeated after discharge. After clinical evaluation,they were diagnosed as recurrent appendicitis and underwent surgical appendectomy. Most of them were reoperated within 6 months after ERAT (21 cases,91.3%).Postoperative pathology was mainly chronic appendicitis (10 cases,43.5%).Conclusion:Acute suppurative appendicitis and acute gangrenous appendicitis are prone to catheterization or endoscopic failure during ERAT,and there is a high risk of surgical transfer. ERAT may relieve the symptoms of chronic appendicitis in children. If the symptoms are repeated,surgery is still needed to remove the appendix.
3.Paeoniflorin ameliorates chronic colitis via the DR3 signaling pathway in group 3 innate lymphoid cells

Inhibiting the death receptor 3(DR3)signaling pathway in group 3 innate lymphoid cells(ILC3s)pre-sents a promising approach for promoting mucosal repair in individuals with ulcerative colitis(UC).Paeoniflorin,a prominent component of Paeonia lactiflora Pall.,has demonstrated the ability to restore barrier function in UC mice,but the precise mechanism remains unclear.In this study,we aimed to delve into whether paeoniflorin may promote intestinal mucosal repair in chronic colitis by inhibiting DR3 signaling in ILC3s.C57BL/6 mice were subjected to random allocation into 7 distinct groups,namely the control group,the 2%dextran sodium sulfate(DSS)group,the paeoniflorin groups(25,50,and 100 mg/kg),the anti-tumor necrosis factor-like ligand 1A(anti-TL1A)antibody group,and the IgG group.We detected the expression of DR3 signaling pathway proteins and the proportion of ILC3s in the mouse colon using Western blot and flow cytometry,respectively.Meanwhile,DR3-overexpressing MNK-3 cells and 2% DSS-induced Rag1-/-mice were used for verification.The results showed that paeoniflorin alleviated DSS-induced chronic colitis and repaired the intestinal mucosal barrier.Simultaneously,paeoniflorin inhibited the DR3 signaling pathway in ILC3s and regulated the content of cytokines(interleukin-17A,granulocyte-macrophage colony stimulating factor,and interleukin-22).Alternatively,paeoniflorin directly inhibited the DR3 signaling pathway in ILC3s to repair mucosal damage indepen-dently of the adaptive immune system.We additionally confirmed that paeoniflorin-conditioned me-dium(CM)restored the expression of tight junctions in Caco-2 cells via coculture.In conclusion,paeoniflorin ameliorates chronic colitis by enhancing the intestinal barrier in an ILC3-dependent manner,and its mechanism is associated with the inhibition of the DR3 signaling pathway.
4.Mortality, morbidity, and care practices for 1750 very low birth weight infants, 2016-2021

Background::Very low birth weight (VLBW) infants are the key populations in neonatology, wherein morbidity and mortality remain major challenges. The study aimed to analyze the clinical characteristics of VLBW infants.Methods::A retrospective cohort study was conducted in West China Second Hospital between January 2016 and December 2021. Neonates with a birth weight of <1500 g were included. Mortality, care practices, and major morbidities were analyzed, and compared with those of previous 7 years (2009-2015).Results::Of the total 1750 VLBW, 1386 were infants born with birth weight between 1000-1499 g and 364 infants were born with weight below 1000 g; 42.9% (751/1750) required delivery room resuscitation; 53.9% (943/1750) received non-invasive ventilation only; 38.2% (669/1750) received invasive ventilation; 1517 VLBW infants received complete treatment. Among them, 60.1% (912/1517) of neonates had neonatal respiratory distress syndrome (NRDS), 28.7% (436/1517) had bronchopulmonary dysplasia (BPD), 22.0% (334/1517) had apnea, 11.1% (169/1517) had culture-confirmed sepsis, 8.4% (128/1517) had pulmonary hemorrhage, 7.6% (116/1517) had severe intraventricular hemorrhage (IVH)/periventricular leukomalacia (PVL), 5.7% (87/1517) had necrotizing enterocolitis (NEC), and 2.0% (31/1517) had severe retinopathy of prematurity. The total and in-hospital mortality rates were 9.7% (169/1750) and 3.0% (45/1517), respectively. The top three diagnoses of death among those who had received complete treatment were sepsis, NRDS, and NEC. In 2009-2015, 1146 VLBW were enrolled and 895 infants received complete treatment. The proportions of apnea, IVH, and IVH stage ≥3/PVL, were higher in 2009-2015 compared with those in 2016-2021, while the proportions of NRDS and BPD were characterized by significant increases in 2016-2021. The total and in-hospital mortality rates were 16.7% (191/1146) and 5.6% (50/895) respectively in 2009-2015.Conclusion::Among VLBW infants born in 2016-2021, the total and in-hospital mortality rates were lower than those of neonates born in 2009-2015. Incidences of NRDS and BPD increased in 2016-2021, which affected the survival rates and long-term prognosis of VLBW.
5.The Effect of Mentality-Controlling on Local Microcirculation of Acupuncture at Hegu (LI 4) by Application of Laser Speckle Contrast Imaging

ObjectiveTo explore the effect of mentality-controlling on local microcirculation of acupuncture at Hegu (LI 4). MethodsForty healthy subjects were randomly divided into simple acupuncture group and mentality-controlling acupuncture group, with 20 cases in each group. The simple acupuncture group was acupunctured at Hegu (LI 4) on the right hand of the patient. In mentality-controlling acupuncture group, an audio clip was played to guide the participants to control their mentality when needle retention for 5 min after performing acupuncture twirling mani-pulation. The microcirculatory blood perfusion units (MBPU) was captured and compared between groups of participants using laser speckle contrast imaging (LSCI) for 5 mins in each of the four stages, state before acupuncture (R1), after needle insertion (A1), after needle twirling (A2) and after needle withdrawal (R2). The participants' feelings such as relaxation, concentration, drowsiness, energy flow at acupoints were compared between groups, and the adverse reactions were recorded. ResultsThe MBPU values of R1, A1, A2 and R2 stages in mentality-controlling acupuncture group were 67.92±18.40 PU, 168.13±46.03 PU, 144.19±44.40 PU, 159.73±65.21 PU, respectively. The simple acupuncture group were 79.85 (66.80, 99.40)PU, 193.31 (173.15, 224.80)PU, 147.68 (128.12, 198.30)PU, 158.09 (126.05, 207.59)PU respectively. There was a statistically significant difference in MBPU between groups of the four stages (P<0.01). Among them, the MBPU in the A1 stage of both groups was significantly larger than that in the R1 stage, the MBPU in the A2 stage significantly smaller than that in the A1 stage, and the MBPU in the R2 stage was significantly larger than that in the R1 stage (P<0.01). MBPU in the R2 stage of the mentality-controlling acupuncture group was larger than that in the A2 stage (P<0.05). The difference of MBPU between A2 and A1 in mentality-controlling acupuncture group was significantly smaller than that in the simple acupuncture group, and the difference was statistically significant (t = 3.79, P<0.01). In mentality-controlling acupuncture group, 18 participants felt relaxed (90%), 15 concentrated (75%), 3 felt sleepy (15%), and 14 felt energy flow at acupoints (70%); while in the simple acupuncture group, 13 participants felt relaxed and sleepy (65%), 6 concentrated and 6 felt energy flow at acupoints (30%). The number of participants with concentration and energy flow at acupoints in mentality-controlling acupuncture group was higher than that in the simple acupuncture group, and the number felt sleepy was lower than that in the simple acupuncture group (P<0.05). No needle fainting, needle site infection, local haematoma and any other adverse reactions were found in both groups. ConclusionAcupuncture could affect the changes of local microcirculation, and controlling mentality during acupuncture can slow the rate of MBPU reduction and enhance the post-acupuncture effect with a favourable safety profile.
6.Research Progress on Traditional Chinese Medicine in Treating Viral Hepatitis: A Review

Viral hepatitis is an important cause of liver cirrhosis and liver cancer, which has become a major public health problem in the world. Traditional Chinese medicine has unique advantages in treating viral hepatitis, which can inhibit virus replication and enhance immunity. It can effectively prevent liver fibrosis and canceration, improving liver function and symptoms significantly with definite clinical curative effects, a high level of safety, and seldom drug resistance. In addition, it reduces the side effects of western medicine, achieving the effect of synergy and attenuation while reducing the recurrence rate of patients after drug withdrawal. Attention has been paid to the research on the treatment of viral hepatitis with traditional Chinese medicine, and great progress has been made in experimental research and clinical practice. In this paper, the research progress of traditional Chinese medicine in the treatment of viral hepatitis at home and abroad in recent five years was systematically reviewed. Modern research has confirmed that traditional Chinese medicine can play a role in the treatment of viral hepatitis by directly or indirectly inhibiting the virus, anti-inflammatory, anti-fibrosis, anti-oxidation, regulating immunity, regulating autophagy, and other signal pathways. In clinics, traditional Chinese medicine compound or combined with western medicine is often adopted to ameliorate the clinical symptoms of patients such as fatigue and loss of appetite, improve the immune mechanism of the body, enhance the antiviral ability, shorten the treatment course of patients and improve their quality of life. The research provides a reference for pharmacological research, clinical research, and new drug development for viral hepatitis.
7.Discussion on Differentiation and Treatment of Sudden Hearing Loss by Traditional Chinese Medicine Based on the Classification of Hearing Curve

Pure-tone audiometry can be performed to evaluate the type and degree of hearing loss, whose results can be divided into four types including low-frequency descending, high-frequency descending, flat descending and total deafness. The low-frequency descending type of sudden hearing loss (SHL) is more likely to be spleen deficiency and dampness exuberance, the high-frequency descending type is often due to yin deficiency of liver and kidney, the flat descending type is commonly associated with qi and blood depletion, and the type of total deafness is often linked to blood stasis. Our team has further developed a comprehensive diagnostic and therapeutic approach for SHL, emphasizing "the integration of disease and syndrome, the combination of acupuncture and herbal medicine, and dynamically administering treatment". Firstly, it advocates integrating disease diagnosis with syndrome differentiation. Secondly, it recommends combining acupuncture and herbal medicine, with local acupoints such as Ermen (TE 21), Tinggong (SI 19), Tinghui (GB 2), and Yifeng (TE 17) used to unblock the auditory orifice, and herbal prescriptions tailored to the hearing curve patterns. For the low-frequency descending type, it is recommended to fortify the spleen and percolate dampness by taking distal points of spleen channel and stomach channel and using Shenling Baizhu Powder (参苓白术散). For the high-frequency descending type, the method of nourishing kidney and calming liver is recommended, using distal points of kidney and liver channels and taking Erlong Zuoci Pills (耳聋左慈丸). Regarding the flat descending type, tonifying qi and nourishing blood is advised, for which acupoints of Conception Vessel, spleen, stomach and large intestine channels can be needled, and Yiqi Congming Decoction (益气聪明汤) can be administered. For the total deafness type, it is recommended to activate blood and dissipate stasis, often with Xuehai (SP 10) and Geshu (BL 17) needled and Tongqiao Huoxue Decoction (通窍活血汤) administered. All these are conducted to treat the root and branch simultaneously. At the same time, it emphasizes the need to consider complex syndrome presentations and disease progression, dynamically analyze the disease causes and mechanisms, and adjust treatment according to the changing syndromes. In conclusion, this article is expected to inspire clinical diagnosis and treatment of SHL.
8.Effect and Mechanism of Osthole on Proliferation and Apoptosis in Human Intrahepatic Cholangiocarcinoma HuCCT1 Cells

ObjectiveTo investigate the effect and mechanism of osthole on the proliferation and apoptosis in human intrahepatic cholangiocarcinoma HuCCT1 cells. MethodThe effect of 10, 20, 40, 80, and 120 μmol·L-1 osthole on the proliferation of HuCCT1 cells was detected by the cell counting kit-8 (CCK-8). A blank group, and low-, medium-, and high-dose osthole groups (16, 32, and 64 μmol·L-1) were set up. The effect of osthole on cell clone formation rate was detected by colony formation assay. The effect of osthole on cell cycle and apoptosis was detected by flow cytometry. The effect of osthole on cell apoptotic morphology was detected by Hoechst 33342 fluorescent staining. The effect of osthole on cell cycle protein cyclin B1, proliferating cell nuclear antigen (PCNA), cysteine-aspartic acid protease (Caspase)-9, Caspase-3, cleaved Caspase-9, cleaved Caspase-3, cleaved poly(ADP-ribose) polymerase (cleaved PARP), B-cell lymphoma-2 (Bcl-2), phosphorylated protein kinase B (p-Akt), phosphorylated mammalian target of rapamycin (p-mTOR), and phosphorylated ribosomal protein S6 (p-RPS6) was detected by Western blot. ResultThe cell viability in the osthole group(40,80,120 μmol·L-1) decreased (P<0.05,P<0.01), with the half maximal inhibitory concentration (IC50) of 63.8 μmol·L-1 as compared with that in the blank group. Compared with the blank group, the osthole groups(32,64 μmol·L-1)showed reduced clone formation rate (P<0.01), increased number of cells in the G2 phase (P<0.05,P<0.01), decreased number of cells, increased pyknosis and fragmentation, increased apoptosis rate (P<0.05,P<0.01), down-regulated expression of cyclin B1, PCNA, Bcl-2, Caspase-3, Caspase-9, p-Akt, p-mTOR, and p-RPS6 (P<0.05,P<0.01), and up-regulated expression of cleaved Caspase-3, cleaved Caspase-9, and cleaved PARP (P<0.05,P<0.01). ConclusionOsthole can inhibit the proliferation and promote the apoptosis of HuCCT1 cells, and its mechanism may be related to the Akt/mTOR signaling pathway.
9.Pharmacokinetics effect of 8 components of Baizhu Shaoyao San with processing method based on UPLC-MS/MS

Objective:To explore the pharmacokinetics effect of 8 components of processed Baizhu Shaoyao San on rats.Methods:The rats were divided into processing group and unprocessing group, administered with decoction of Baizhu Shaoyao San by gavage respectively. Then, blood was collected from fundus vein at certain time to obtain the plasma. Finally, the contents of 8 components in plasma were detected and compared by UPLC-MS/MS method, and the methodology of the experiment was tested. The drug concentration in blood and the collection time of blood were analyzed by DAS software, and the time curves of different groups were obtained, the pharmacokinetic parameters were calculated.Results:The blood peak concentration, peak time, area under the drug time curve, and average residence time of 8 components in the serum of rats in the raw product group and the fried product group were different to varying degrees.Conclusion:Processed Baizhu Shaoyao San could influence the behavior of the components measured in rats, which may affect the clinical therapeutic effect of Baizhu Shaoyao San.
10.Study on Identification of Ginseng Flos and its Confounding Products Based on High-throughput Sequencing Technology

OBJECTIVE To identify Ginseng Flos and their confounds by using the high-throughput sequencing technology, and to verify the accuracy of high-throughput sequencing technology in species identification by using ITS2 sequencing technology. METHODS High-throughput sequencing was performed on the amplified products of Ginseng Flos adulterated samples, use cutadapt, PEAR, PRINSEQ, Usearch, RDP classifier, SINTAX software to obtain operational taxonomic unit(OUT) sequences, remove fungi, unclassified and other non-green plant sequences. To avoid false positives, delete OTU sequences with a sequence number <100 or base numbers <200 bp. The ITS2 amplification products of Ginseng Flos, Quinquefolii Flos, and Notoginseng Flos were sequenced. To verify the accuracy of high-throughput sequencing technology for species identification, MEGA 11.0 was used to construct neighbor joining system cluster tree, genetic distance, interspecific information loci and Blast analysis of ITS2 and OTU base sequences of Ginseng Flos, Quinquefolii Flos, and Notoginseng Flos. RESULTS A total of 54 653 valid sequences were obtained by high-throughput sequencing, the serial numbers of Ginseng Flos, Quinquefolii Flos, and Notoginseng Flos were OTU1, OTU2, OTU3, respectively, and the corresponding effective sequences were 31 325, 857 and 442, respectively. By performing a Blast search of ITS2 and OTU base sequences of each species, each species was supported. The genetic distance between Ginseng Flos and Quinquefolii Flos and Notoginseng Flos was 0.010 and 0.033, respectively. Ginseng Flos and Quinquefolii Flos, Notoginseng Flos had 2 and 7 information sites, respectively. The neighbor join system cluster tree showed that the species were clustered independently into one branch, with Ginseng Flos, and Quinquefolii Flos clustered as a large branch and juxtaposed with Notoginseng Flos. Ginseng Flos was the same as Quinquefolii Flos secondary structure, but with Notoginseng Flos there were three different positions but there were A, B and C differences between arm Ⅳ and arm Ⅰ of Notoginseng Flos. CONCLUSION The high-throughput sequencing technology can accurately identify Ginseng Flos, Quinquefolii Flos and Notoginseng Flos, and has a strong ability to identify adulterated samples, which provides a certain idea for the identification of commercial Ginseng Flos.
