1.SEX DIFFERENCES IN THE MUSCLE VOLUME OF THE ILIOPSOAS IN THE ELDERLY

The purpose of this study was to investigate sex differences in the muscle volume (MV) of the iliopsoas, psoas major, and iliacus muscles, normalized by fat free mass (FFM), in the elderly. The subjects were 15 males and 15 females 65 years of age or over. Serial transverse 1.5T magnetic images (10 mm slice) of each subject were taken between the 12th thoracic vertebrae and the lesser trochanter. FFM was measured with a bioelectrical impedance analyzer. The mean MV of each muscle on the right and left sides were calculated and used for regression analysis of the relationship between MV and FFM.A significant correlation was observed between MV and FFM of the iliacus in both sexes (p<0.01), but not between the MV and FFM of the psoas major. MV normalized by FFM and cross-sectional area (CSA) normalized by FFM2/3 showed significantly greater values for the psoas major in the males than in the females (p<0.001), but there was no sex difference in the MV or CSA normalized by FFM of the iliacus.These results suggest that elderly males tend to have a greater muscle volume and cross-sectional area of the iliopsoas than elderly females, and that these sex differences are attributable to differences in the psoas major.
2.COMPARATIVE ELECTROMYOGRAPHICAL INVESTIGATION OF THE BICEPS BRACHII, LATISSIMUS DORSI, AND TRAPEZIUS MUSCLES DURING FIVE PULL EXERCISES

To compare the effectiveness of five different muscle training movements on the biceps brachii, latissimus dorsi and trapezius muscles, eight weight-trained men (age, 20.4±0.5 years) were asked to perform three repetitions, at 70% one repetition maximum, of upright rowing (UR) and bent-over rowing (BR) exercises using a barbell ; and seated rowing (SR), front lat pull-down (LPf) and back lat pull-down (LPb) exercises using a Universal Machine. The activities of the biceps brachii, latissimus dorsi, and trapezius during the elbow flexsion and elbow extension phases of each exercise were analyzed using integrated electromyography (EMG) and normalized I-EMG. The results were as follows :1. The mean nrmsEMG values for the biceps brachii were larger during UR and LPf exercises than during BR, SR, and LPb exercises, suggesting that UR and LPf are more effective than the other movements for training the biceps brachii. The mean nrmsEMG values for the latissimus dorsi were larger during SR, LPf, and LPb exercises, followed by BR and UR exercises (in descending order), suggesting that SR, LPf, and LPb exercises are more effective than the other movements for training the latissimus dorsi.2. The mean nrmsEMG values for the upper trapezius were larger during UR and BR exercises than during SR, LPf, and LPb exercises, suggesting that UR and BR exercises are more effective than the other movements for training the upper trapezius. The mean nrmsEMG values for the middle trapezius were larger during BR and SR exercises than during UR, LPf, and LPb exercises, suggesting that BR and SR exercises are more effective than the other movements for training the middle trapezius. The mean nrmsEMG values for the lower trapezius were larger during BR exercise than during other movements, suggesting that BR exercise is more effective than the other movements for training the lower trapezius.3. In all the exercises, each muscle showed a higher nrmsEMG value during the elbow flexsion phase than during the elbow extension phase. This observation suggests that the training method examined in this study should emphasize the elbow flexsion movement.The present results suggest that UR exercise is the most effective movement for training the biceps brachii and upper trapezius, BR is most effective for training the upper trapezius, middle trapezius and lower trapezius, SR is most effective for training the latissimus dorsi and middle trapezius, LPf is most effective for training the biceps brachii and latissimus dorsi, and LPb is most effective for training the latissimus dorsi.
3.Influence of a Liaison-clinical Pathway on the Length of Stay and the Functional Independence Measure Gain of Stroke Patients in the Nakaharima and Nishiharima District Medical Facilities Network in Hyogo Prefecture

Objective : The aim of this study was to examine the influence of a liaison-clinical pathway for stroke rehabilitation. Methods : We initiated the pathway in January 2008. The patients enrolled in this study included 82 patients with acute stroke sustained during the 4 months after the initiation of the pathway and 151 patients with acute stroke sustained during the same period in the following year. Results : The mean length of stay in acute stroke centers was significantly reduced in the second year of employing the pathway. However, an evident reduction of the functional independence measure gain was observed in the convalescent rehabilitation wards, especially in severely physically handicapped patients. Conclusion : The liaison-clinical pathway is an effective approach for advancing a regional cooperation network among hospitals and for shortening the stay in acute stroke centers. On the other hand, the clinical outcome of stroke patients can deteriorate if enough efforts are not made to improve the medical management of severely physically handicapped patients in convalescent rehabilitation wards.
4.SERUM LIPID LEVELS IN MALE AND FENALE HIGH SCHOOL FRESHMEN WITH MASKED OBESITY

Masked obesity is the presence of obesity based on percent body fat (%BF) when the body mass index (BMI) shows an absence of obesity. To examine the relationship between masked obesity and arteriosclerosis risk factors, we compared both serum lipid levels and the prevalence of hyperlipidemia in male and female high school freshmen with and without masked obesity. Subjects consisted of 403 male and 326 female high school students aged 15∼16 years. Of these, 34(8.4%) males and 36(11.0%) females had masked obesity, defined as 17≤BMI<23.60 and %BF≥25% in males, and 17≤BMI<24.17 and %BF≥30% in females, while the remaining 300 males and 246 females were not obese, having 17≤BMI<23.60 and %BF<25% and 17≤BMI<24.17 and %BF<30%, respectively. For both sexes, serum total-cholesterol (TC), low-density lipoprotein cholesterol (LDLC), triglycerides and the arteriosclerotic index (AI) were significantly higher (p<0.05∼0.01) in those with masked obesity. And many of the subjects with masked obesity had abnormal levels of TC, LDLC and AI, compared with those who were not obese (p<0.05∼0.01). Additionally, we compared both serum lipid levels and the prevalence of hyperlipidemia between subjects with masked obesity and control groups with the same BMI values. As a result, subjects with masked obesity had high serum lipid levels and a prevalence of hyperlipidemia. These results support the existence of masked obesity and suggest that masked obesity is associated with increased serum lipid levels, and thus could be a risk factor for arteriosclerosis in male and female high school freshmen.
5.POSITIVE EFFECTS OF SHORT-TERM EXPOSURE TO ALTITUDE ON BLOOD LACTATE AND HEART RATE

The purpose of this study was to investigate the effects of short-term living and training at an altitude of 1, 300 to 1, 800 m on physiological responses of high school elite endurance athletes. Fifteen male and seven female senior high school elite athletes, aged from 15 to 18, from three different sports (cross-country skiing, long-distance running and endurance cycling), participated in our study. The short-term (6 days) altitude exposure did not elicit abnormal responses of body tempera-ture, body weight, blood pressure or urine samples. There were also no significant changes in blood parameters examined before and after altitude exposure. Resting heart rate (HR) increased at altitude and presented an initial peak value followed by a steady decline on the following days of altitude exposure. Blood lactate concentration and exercise peak llR examined by submaximal 20-m shuttle run test decreased after the ascent to altitude and still showed lower values at postaltitude than at prealtitude. We conclude that 6-day living and training at an altitude of 1, 300 to 1, 800 m elicits positive decrements of exercise blood lactate and exercise peak HR as well as adaptive changes of resting IlR for these high school elite endurance athletes, which are probably related to an attenuation of muscle glycogen utilization and alterations in the autonomic neural system taken at altitude.
6.Thyroid hormone alterations in trauma patients requiring massive transfusion: An observational study

BACKGROUND: Although non-thyroidal illness syndrome (NTIS) is considered a negative prognostic factor, the alterations in free triiodothyronine (fT3) levels in trauma patients requiring massive transfusion have not been reported. METHODS: A prospective observational study comparing 2 groups of trauma patients was conducted. Group M comprised trauma patients requiring massive transfusions (>10 units of packed red blood cells) within 24 hours of emergency admission. Group C comprised patients with an injury severity score >9 but not requiring massive transfusions. Levels of fT3, free thyroxine (fT4), and thyroid-stimulating hormone (TSH) were evaluated on admission and on days 1, 2, and 7 after admission. The clinical backgrounds and variables measured including total transfusion amounts were compared and the inter-group prognosis was evaluated. Results are presented as mean±standard deviation. RESULTS: Nineteen patients were enrolled in each group. In both groups, 32 were men, and the mean age was 50±24 years. In group C one patient died from respiratory failure. The initial fT3 levels in group M (1.95±0.37 pg/mL) were significantly lower than those in group C (2.49±0.72 pg/mL;P<0.01) and remained low until 1 week after admission. Initial inter-group fT4 and TSH levels were not significantly different. TSH levels at 1 week (1.99±1.64 μIU/mL) were higher than at admission (1.48±0.5 μIU/mL) in group C (P<0.05). CONCLUSION: Typical NTIS was observed in trauma patients requiring massive transfusions. When initial resuscitation achieved circulatory stabilization, prognosis was not strongly associated with NTIS.
7.Association Between Body Mass Index and Functional Dyspepsia in Young Japanese People

Background/Aims:
Evidence regarding the association between body mass index (BMI) and functional dyspepsia (FD) in the Asian population is limited.Further, no study has evaluated this issue in young people in Asian and Western populations. Thus, we aim to investigate this issueamong young Japanese people.
Methods:
The study subjects comprised of 8923 Japanese university students. BMI was divided into 4 categories (quartiles) on the basis of thestudy subjects’ distribution (lowest, low, moderate, and high [reference]). The definition of lean, normal, overweight, and obesewas BMI < 18.5 kg/m2 , 18.5 ≤ BMI < 25 kg/m2 (reference), 25 kg/m2 ≤ BMI < 30 kg/m2 , and 30 kg/m2 ≤ BMI, respectively. The definition of FD was based on the Rome III criteria.
Results:
The prevalence of FD was 1.9% in this cohort. The lowest BMI was independently associated with FD after adjustment (adjusted odds ratio [OR], 2.88; 95% confidence interval [CI], 1.46-3.67); P for trend = 0.001). The lowest BMI was independently associated with FD in women but not in men (OR, 2.94; 95% CI, 1.59-5.77; P for trend = 0.001). Leanness was independently associated with FD in total and in women but not in men (total: adjusted OR, 2.01; 95% CI, 1.40-2.86) and women (OR, 2.19; 95% CI, 1.35-3.45). However,interaction analysis showed no significant difference for sex.
Conclusions
Among young Japanese people, BMI may be independently inversely associated with FD. Leanness may be an independent associated factor for FD in the young Japanese women.
8.Association Between Eating Behavior, Frequency of Meals, and Functional Dyspepsia in Young Japanese Population

Background/Aims:
Functional dyspepsia (FD) may be a common digestive disease worldwide and reduces the quality of life of patients. However, only a few studies have investigated the association between eating behavior and FD. The purpose of this cross-sectional study is to examine the association between eating behavior and prevalence of FD in a young Japanese cohort.
Methods:
In this study, we enrolled 8923 Japanese university students. FD is diagnosed based on the Rome III criteria. Eating habits and frequency of meals were investigated using a self-administered questionnaire.
Results:
The FD subjects had a younger mean age, a lower body mass index, and a lower proportion of men compared to the non-FD subjects.An independent positive association between skipping breakfast and/or lunch and FD was found (adjusted ORs were 1.60 [95% CI, 1.10-2.32] for breakfast and 2.52 [95% CI, 1.04-5.18] for lunch). Skipping dinner, extra meals (snacks) or midnight snacks was not associated with FD. The prevalence of FD in subjects eating 1, 2, and 3 meals per day was 4.8%, 2.2%, and 1.7%, respectively. The frequency of meals was independently inversely associated with prevalence of FD (adjusted ORs were 1 per day: 2.72 [95% CI, 1.19-5.42], and 2 per day: 1.69 [95% CI, 1.16-2.43], P for trend = 0.001).
Conclusions
In the young Japanese people, the frequency of meals may be independently inversely associated with prevalence of FD. In particular, skipping breakfast and/or lunch was associated with the prevalence of FD.
9.Multimorbidity of Allergic Diseases Is Associated With Functional Gastrointestinal Disorders in a Young Japanese Population

Background/Aims:
Although certain allergic diseases have been reported to be associated with the prevalence of functional dyspepsia (FD) and irritable bowel syndrome (IBS), it is unclear whether the presence of multiple allergic diseases further increases the prevalence of FD and IBS.The aim of this study is to determine this issue in young people.
Methods:
A cohort of 8923 Japanese university students was enrolled and diagnoses of FD and IBS were confirmed using Rome III criteria.Allergic disorders diagnosed at medical institutions were obtained by means of a self-administered questionnaire.
Results:
The prevalence of FD, IBS, and their overlap was found to be 1.9%, 6.5%, and 1.1%, respectively. Pollen allergy was independently positively correlated with FD, IBS, and overlap of FD and IBS. Allergic rhinitis was positively linked to IBS. Drug allergy was positively associated with FD. The presence of multiple allergic diseases was positively correlated with FD and IBS (FD: adjusted OR for 2 allergic diseases: 1.95 [95% CI, 1.24-2.98], P for trend = 0.003; and IBS: adjusted OR for 1 allergic disease: 1.40 [95% CI, 1.15-1.69], 2 allergic diseases 1.47 [95% CI, 1.12-1.91], and 3 or more allergic diseases: 2.22 [95% CI, 1.45-3.28], P for trend = 0.001). Additionally, the concomitant existence of multiple allergic diseases was also demonstrated to have a trend that correlated with the overlap of FD and IBS (P for trend = 0.018).
Conclusion
Allergic disease multimorbidity is positively correlated with the prevalence of FD and IBS in a young population.
10.Discriminant analysis for predictor of falls in stroke patients by using the Berg Balance Scale.

INTRODUCTIONAn observational study was carried out to estimate the strength of the relationships among balance, mobility and falls in hemiplegic stroke inpatients. The objective was to examine factors that may aid in the prediction of the likelihood of falls in stroke patients.
METHODSA total of 53 stroke patients (30 male, 23 female) aged 67.0 ± 11.1 years were interviewed regarding their fall history. Physical performance was assessed using the Berg Balance Scale (BBS) and the Functional Independence Measure (FIM) scale. Variables that differed between fallers and non-fallers were identified, and a discriminant function analysis was carried out to determine the combination of variables that effectively predicted fall status.
RESULTSOf the 53 stroke patients, 19 were fallers. Compared with the non-fallers, the fallers scored low on the FIM, and differed with respect to age, time from stroke onset, length of hospital stay, Brunnstrom recovery stage and admission BBS score. Discriminant analysis for predicting falls in stroke patients showed that admission BBS score was significantly related to the likelihood of falls. Moreover, discriminant analysis showed that the use of a significant BBS score to classify fallers and non-fallers had an accuracy of 81.1%. The discriminating criterion between the two groups was a score of 31 points on the BBS.
CONCLUSIONThe results of this study suggest that BBS score is a strong predictor of falls in stroke patients. As balance is closely related to the risk of falls in hospitalised stroke patients, BBS might be useful in the prediction of falls.
