Objective To investigate the influence of low bilirubin level on hepatic stellate cells (HSCs) in vitro. Methods HSCs were isolated and cultured from the liver of SD rats. The effect of bilirubin in different concentration on reactive oxygen in HSCs was determined by DCFH-DA kit. The proliferation of HSCs was tested by CCK-8 test kit.Smoothmuscle α-actin (α-SMA) expression of HSCs was tested by Western blot.The apoptosis of HSCs was tested by flow cytometry.The fibrosis-related genes were tested by PCR. Results HSCs were isolated and cultured successfully.Bilirubin in low concentration (0,1,10,20 mg/L) inhibited the generation of the reactive oxygen.Proliferation and α-SMA expression of HSCs was inhibited by bilirubin (0.624 ± 0.092,0.536 ± 0.127,0.407 ± 0.033,0.399 ± 0.022,F =13.454,P ＜0.05 ; 339 ± 2,336 ± 10,246 ± 7,242 ± 5,3.7 ± 0.3,F =191.107,P ＜ 0.05 ) and the apoptosis of HSCs was promoted by bilirubin(2.69 ±0.07％,2.95 ±0.10％,4.41 ±0.22％,4.91 ±0.86％,F =34.731,P ＜0.05 ).Bilirubin in low concentration changed the expression of fibrosis-related genes in HSCs.The ratio of TIMP-1mRNA/MMP-2mRNA decreased (54 ± 2,65 ± 2,47 ± 2,44 ± 2,F =73.400,P ＜ 0.05).Conclusions Bilirubin in low concentration inhibits the proliferation and activation of hepatic stellate cells,orobablv bv a mechanism in which bilirubin promoted antioxidative function.

ObjectiveTo explore the effect of ultrasonic harmonic scalpel on stress after radical resction of esophageal carcinoma.MethodsAll 115 patients of esophageal carcinoma undergone radical esophagectomy were analyzed retrospectively.60 patients were performed with ultrasonic harmonic scalpel,the other 55 patients were performed with electrotome.The drainage volume of 72h after operation,postoperative complications and hospital days were analyzed.White blood cell count,Neu,CRP and blood glucose level were tested 2 days after operation.ResultsThere was significant difference in the drainage volume of 72h after operation (P ＜ 0.05 ),but the postoperative complications and hospital days had no obvious difference.White blood cell count,Neu,CRP and blood glucose were obviously increased after operation( all P ＜ 0.05 ) ;the barmonic scalpel group was dramatically decreased as compared with the electrotome group except blood glucose ( all P ＜ 0.05 ).ConclusionRadical resection of esophageal carcinoma using ultrasonic harmonic scalpel appears to decrease the drainage volume of 72h after operation and less the stress.

Objective To evaluate the advantage of video-assisted mini thoracotomy for patients with esophageal cancer compared with routine open thomcotomy.Methods Perioperative clinical results were compared between 24 csses received VAMT(test group)and 26 cases underwent routine open thoracotomy(control group)during the same period.All the cancer stages were T2-3N0-1M0.Results This study showed that VAMT Was a minimally invasive swgew with shorter operation time(40.0±3.3)min vs.(70.3±3.2)min,P＜0.05)and had less bleeding volume both during and after operation compared to the control group(108.1±15.2)ml V8.(200.1±11.3)ml,(380.8±45.4)ml vs.(780.1±52.2)ml,P＜0.05).No significant differences were observed in the two groups with respect to complication occurrence rate(4.2%vs.19.2%,P＞0.05).Conclusion It is suggested that VAMT is safe,reliable and less invasive in the treatmem of esophagus carcinoma.

Objective To explore the feasibility,necessity and the skill of total mesoesophageal excision (TME ) during thoracoscopy combined with laparoscopy in radical resection of esophageal carcinoma.Methods 69 patients with esophageal carcinoma were divided into the TME group(40 cases)and the thoracotomy with triple incisions group(29 cases)according to the admission sequence.The operation time,intraoperative blood loss,total lymph nodes removed,postoperative complication rate and disease -free survival were compared between the two groups.Results The operation time of TME group was (182.85 ±26.73)min,which was significantly shorter than (295.71 ±19.50)min of the thoracotomy group (t=-19.301,P<0.001).The intraoperative blood loss in TME group was (86.43 ±59.34)mL,which was significantly less than (163.47 ±58.82)mL in the thoracotomy group (t=-5.342,P<0.001 ).No significant differences were detected between the two groups in total lymph nodes removed and incidence rate of postoperative complication (all P>0.05 ).The disease-free survival period in TME group was (14.78 ±2.14)months,which in the thoracotomy group was (13.10 ±4.09)months,the difference was significant (t=2.200,P<0.05).Conclusion TME is safe and feasible during thoracoscopy combined with laparos-copy in radical resection of esophageal carcinoma.TME is better in improving the regional control in esophageal carci-noma.

OBJECTIVE:To provide reference for the promotion of work efficiency of PIVAS.METHODS:The theory of bat-ch processing of PIVAS in our hospital was interpreted and its effect and problems were analyzed.RESULTS & CONCLUSIONS:Batch processing principal of background decision and foreground adjustment were adopted to allocate workload of different periods scientifically and improve working efficiency.Some problems existed in processing period require improvement of batch processing.

OBJECTIVE:To explore how to strengthen self-protection of chemotherapy drug dispensing staff in PIVAS.METHODS:The protective equipment of PIVAS,caution of drug dispensing and solution for emergency were introduced.The way of strengthening self-protection of staff in PIVAS was explored.RESULTS & CONCLUSIONS:Standardized operation procedures and clean operation environment are of great significance for the enhancement of professional protection of drug dispensing staff.

OBJECTIVE:To promote the safety and effectiveness of clinical medication.METHODS:In drug dispension process,operation procedure was carried out strictly and different dispensing methods were adopted according to the characteristics of drugs.RESULTS & CONCLUSIONS:Dispensing guidelines for common injection,insoluble drug and chemotherapy drugs were summarized and improved to promote quality of intravenous drugs dispension.

Objective To investigate the significance of preoperative and postoperative enteral nutrition support in gastric cancer patients with nutritional risk.Methods The clinical data of 72 gastric cancer patients with nutritional risk admitted in our department were analyzed.All the patients were randomly allocated into two groups.36 patients in the control group received only intravenous nutrition postoperatively (PN group).36 patients in the treatment group received enteral nutrition support from the fifth day before operation to eighth day after operation (EN group).The body weight,total protein,albumin,transferrin and prealbumin were measured on the fifth day before operation,the operation morning,the first day and the eighth day after operation.Results On the operation morning,the transferrin and prealbumin of the EN group were higher than those in PN group significantly(P ＜0.05).In the eighth day after operation.the treatment group patients showed significant increase in the body weight,total protein,albumin,transferrin and prealbumin compared with those in the control group(P ＜ 0.05).On the operation morning,the transferrin and prealbmnin of EN group were increased significantly than on the fifth day before operation(P ＜ 0.01).Conclusion The preoperative and postoperative enteral nutritional support can improve the nutritlonal condition in the gastric cancer patients with nutritional risk.The preoperative enteral nutritional support can improve the patients' nutritlonal condition and reserves before operation,and reduce adverse reaction of enteral nutrition.

Objective To investigate the effects of low carbohydrate diet in treating non-alcoholic fatty liver disease (NAFLD) patients.Methods 58 male NAFLD patients selected in Renmin Hospital of Wuhan University from September 2010 to October 2012 were divided with random number table into low-carbohydrate diet group (L group,n =31) and medium-carbohydrate diet group (M group,n =27).Waistline,weight,serum glucose level,insulin secretion,glutamic-pyruvic transaminase (ALT),aspartate transaminase (AST)、high-density lipoprotein (HDL),low-density lipoprotein (LDL),total cholesterol (TC),and triglyceride (TG) of the patients were measured.Results Six patients were excluded from this research,2 in L group and 4 in M group.After 6-week's dietary intervention,blood glucose level and insulin secretion were significantly lower in L group than in M group [(4.3±1.4) mol/Lvs.(5.0±0.9) mol/L,P=0.004; (6.1 ±1.5) U/mlvs.(8.9 ± 1.4) U/ml,P =0.001].The levels of ALT and AST in L group were significantly lower than those in Mgroup[(30.23±3.34) U/Lvs.(42.33±4.46) U/L,P=0.003; (31.19±4.13) U/Lvs.(45.21±3.73) U/L,P =0.001].The levels of LDL and TG in L group were also significantly lower than those in M group [(1.13±0.22) mmol/Lvs.(2.71±0.67) mmol/L,P=0.001; (0.99±0.74) mmol/Lvs.(1.42±1.06) mmol/L,P =0.001].Conclusion In NAFLD patients,low-carbohydrate diet can improve blood glucose level,insulin secretion,liver function,and lipid metabolism disorders.

Objective To study the protective effects of resveratrol against hepatic stellate cells (HSCs) and liver fibrogensis.Methods HSCs were isolated from liver of SD rats.The reactive oxygen output in HSCs under resveratrol in different concentrations was tested by DCFH-DA kit.The proliferation of HSCs was tested by CCK-8 test kit.Smoothmuscle α-actin (α-SMA) expression of HSCs was evaluated by Western blotting.The activity-related genes were measured by PCR.The models of liver fibrogenes were established.Resveratrol in different concentrations was administrated intraperitoneally.Liver was studied by pathology and SMA staining.Hydroxyproline content of liver and levels of collagen Ⅲ and hyaluronic acid in serum were tested.Results HSCs were isolated from liver and cultured successfully.Resveratrol inhibited the generation of the reactive oxygen.Proliferation and activation of HSCs was inhibited by resveratrol (0.536 ±0.052,0.411 ±0.047,0.327 ±0.063,0.312 ±0.032,F =12.776,P ＜0.05) (103 ±7,90 ±7,63 ± 4,53 ± 3,F =62.179,P ＜ 0.05).Resveratrol inhibited the expression of genes (myogenic determination gene MyoD,collagen 11 and collagen Ⅰ) in HSCs(122 ± 5,96 ± 3,68 ± 3,60 ± 3,F =180.600,P＜0.05) (100±8,82 ±3,53 ±3,51 ±2,F=77.451,P ＜0.05) (170 ±3,147 ±4,92 ±3,90 ±2,F =462.878,P ＜ 0.05).Resveratrol downregulated the level of hydroxyproline,collagen Ⅲ and hyaluronic acid (358.3 ± 20.2,320.5 ± 15.3,290.3 ± 24.5,F =23.929,P ＜ 0.05) (32.8 ± 3.1,28.9 ±1.3,25.3±1.8,F=20.050,P＜0.05)(276.3 ±17.8,225.3 ±28.3,195.4 ±11.2,F=18.585,P＜0.05).Conclusions Resveratrol can inhibit the proliferation and activation of HSCs and downregulate the fibrogensis level of the liver of rats.
