1.Effect of activation of protein kinase B and Caspase-9 signal transduction pathway of human gastric cancer cells on the cell chemosensitivity to etoposide

Objective To investigate the effect of activation of protein kinase B(PKB) and Caspase-9 signal transduction pathway of human gastric cancer cells on the cell growth and chemosensitivity to etoposide.Methods The gastric cancer cells SGC7901 were treated with etoposide or etoposide plus PKB inhibitor Wortmannin at different time.The growth rates of gastric cancer cells SGC7901 and their sensitivity to etoposide were examined by 3-(4,5-dimethylthiazol-2,1)-2,5 diphanytetrazolium(bromide) assay.Apoptosis of gastric cancer cells was(detected) by flow cytometry.PKB activity was measured by(immunoprecipitation.) Caspase-3 expression and Caspase-9 activity were determined by Western bolt analysis.Results Etoposide induced apoptosis of SGC7901 cells and inhibited its survival effectively,which was much(weaker) 12 h after treatment.PKB(activity) became higher gradually,and Caspase-3 expression,Caspase-9(activity) significantly reduced at 12 h treated with etoposide.(Conversely),after pretreated with Wortmannin,PKB activity remarkably(reduced,) and Caspase-3(expression),Caspase-9 activity markedly increased.(Wortmannin) suppressed growth and potentiated (apoptosis) caused by etoposide.Potentiation of apoptosis by Wortmannin(correlated) with etoposide-induced PKB and Caspase-9 phosphorylation.Conclusions PKB and Caspase-9 signal transduction pathway promotes(human) gastric cancer cells survival and resistance to(chemotherapy.) PKB inhibitor can enhance sensitivity of gastric cancer cells to chemotherapy.
2.Messenger RNA expression of IL-6 and IL-8 in allergen-induced late-phase cutaneous reactions (LPR) in Schneiderian membrane subjects

Objective:To detect mRNA expression of IL-6 and IL-8 in allergen-induced late-phase cutaneous reactions in schneiderian membrane subjects. Methods:Cryostat sections from rhinitis biopsies from 24 h allergen-induced late-phase cutaneous reactions (LPR) in 10human atopic subjects were hybridization with 35S-labeled RNA probes for IL-6 and IL-8.Results:mRNA was detected for IL-6 (9/10) and IL8 (10/10).Compared with the control, there were significant increases in the numbers of ce11 expreasing mRNA expression for IL-6 and IL-8(P<0.05, P<0.01). Conclusion: The augmentation of mRNA expression of IL-6 and IL-8 maybe regarded as the mark of rhinits in IL PR.
3.Comparison on short-term and long-term curative effect of modified double Kirschner wire open reduction and internal fixation and traditional surgery on patients with mallet fingers of avulsion fracture type

Objective:To compare short-term and long-term curative effect of modified double Kirschner wire open reduction and internal fixation (ORIF) and traditional surgery on patients with mallet fingers of avulsion fracture type.Methods:Eighty-five patients with mallet fingers of avulsion fracture type who were admitted to Yongkang Orthopaedic Hospital from January 2016 to October 2018 were enrolled. 45 patients received modified double Kirschner wire ORIF were included in the observation group, while 40 patients who underwent traditional extracting wire fixation were included into control group. The operation time, fracture healing time and complications were recorded. At 3 months after surgery, short-term curative effect was evaluated. After 1-year postoperative follow-up, long-term prognosis was evaluated by finger joint function scoring.Results:The operation time of observation group was shorter than that of control group ( P<0.05). The fracture healing time of observation group was shorter than that of control group ( P<0.05). The good rate of observation group was higher than that of control group (93.33% vs 77.50%) ( P<0.05). In finger joint function score, scores of finger strength, finger flexibility, effects on work and life, and total score in observation group were higher than those in control group ( P<0.05). There was no significant difference in pain degree or appearance satisfaction betwen the two groups ( P>0.05). There was no significant difference in incidence of postoperative complications between the two groups ( P>0.05). Conclusions:The modified double Kirschner wire ORIF for the treatment of avulsion fracture mallet finger has the advantages of simple operation, fast fracture healing and better long-term and short-term curative effect than the traditional wire extraction method.
4.Study on establishing a project concerning the process quality control of treating liver cancer with CyberKnife.

In order to guarantee the safety and accuracy of the whole treating process as well as better link up of each section during the treatment, we did research in order to establish a project concerning the process quality control (PQC) of treating liver cancer with CyberKnife. From the safety and accuracy point of view, we divided the whole process of treating liver cancer with CyberKnife into ten links, i.e. the registration of patients' information, the im plantation of fiducial markers, fixation of body posture, CT localization, target delineation, design of the treatment plan, quality assurance in physics, implementation of the treatment plan, inspection on the correctness and data archiving. We analyzed the possible mistakes in each link and the consequences brought by them. To smoothly connect all the links, a special part "Attention" was added between every two links. Various wrong operations which may influence the safety and accuracy of treatment were illustrated, and the consequences brought by them were also ex plained. The "Attention" part among links offers important information for the next step, and gives us reminding and warnings. The project of quality control covers all the important links when treating liver cancer with Cy berKnife. It offers regulations, reminding and warning for us so that the safety and accuracy of treatment can be guaranteed, and the work of all staff could be closely connected.

5.Effect of tooth wear on morphological features of the mandibular first molars

Objective:To investigate the effect of tooth wear on the morphological features of mandibular first molars. Methods:15 extracted mandibular first molars with tooth wear and 15 without wear were investigated. Data about the morphological features of tooth were collected through the general measurement and the image measurement by electronic meter. Results:①In non-tooth-wear group, mesiodistal width of tooth was greater than buccolingual width(P0.05); ②The vertical distance between the lowest point of pulp chamber roof and occlusal surface in non-tooth-wear group was greater than that in tooth-wear group(P05); ④The distance between distolingual pulp horn and distal proximal plane was greater than that between mesiolingual pulp horn and mesial proximal plane in both two groups(P0.05). Conclusion:For different parts of tooth, the effect of tooth wear differs. As for mandibular first molars, the distal part of occlusal surface and the buccal part of distal proximal plane are susceptible to tooth wear, which is mainly due to occlusal factors and the features of biomechanical system.
6.Clinical observe of transcatheter arterial chemombolization combined with functional splenic embolization treating hepatocellular carcinoma with hypersplenism

Objective To observe the therapeutic effects and comphcations of treatment by using transcatheter arterial chemombolization(TACE) combined with functional splenic embolization(FSE) on patients with primary hepatocelluar carcinoma (HCC) combinded with hypersplenism.Methods Eighty HCC patients with hypersplenism were randomly divided into two groups by computer program.Observation group was treated with TACE and FSE,while control group was treated by only TACE.The iodine oil deposition,blood routine parameters and related complications were observed.Results (1) After one-month' s treatment,abdominal CT scan showed that 38 cases(79.17%) of patients in the observation group and 27 cases (84.38%) of patients in control group had iodine oil deposition in over 50% of the tumor area.(2) The white blood cell counting in observation group were (7.65 ± 2.3) × 109/L,(5.89 ± 1.9) × 109/L,(5.02 ± 1.7) × 109/L in the follow-up examination 1 week,1month and 3 month after splenic embolization,respectively,which were significantly increased from the preoperative level of (2.21 ± 1.1) × 109/L(P < 0.05).Platelets counting were (93 ± 29)× 109/L,(127 ± 32) × 109/L and (119 ± 26) × 109/L in observation group at 1 week,1 month and 3 month after splenic embolization,respectively,which were significantly higher than the preoperative level of (39 ± 20)× 109/L (P < 0.05).In the control group,there were no statistically significant difference between the preoperative and postoperative levels (P > 0.05).(3) Both of the the two groups showed no serious complications.Conclusion TACE combined with FSE is a safe and an effective method to treat hypersplenism combined in HCC patients.
8.Evaluation of the effect of human biological stimulation feedback instrument for screening of metabolic syndrome

Objective To assess the effect of a noninvasive equipment (the human biological stimulation feedback instrument) for the screening of metabolic syndrome (MS). Methods The samples were selected from the International Health Care Center of the Second Affiliated Hospital of Zhejiang University College of Medicine. From January to December 2012, according to the inclusion criteria, a total of 5674 consecutive participants (3 437 men, 2 237 women) without medical history of diabetes mellitus underwent general health screening in this hospital, which included questionnaire investigation, all MS components and the risk assessment of the human biological stimulation feedback instrument. MS was defined according to 2004 the Chinese Diabetes Society (CDS) criteria. Results The 5 674 subjects were divided into three groups in accordance with the human biological stimulation feedback instrument risk score: the normal group, metabolic abnormalities low risk group, metabolic abnormalities high risk group. The detection rate of MS corresponding to the three groups was 8.73% (251/2 876), 29.32% (612/2 087), 32.77% (233/711), respectively (trend χ2=372.14,P<0.001). With the increase of risk score, the detection rate of MS increased. A total of 1 096 (male 879, female 217) were diagnosed MS by CDS criteria. The area under the curve (AUC) of the test was 0.674 (95%CI 0.658-0.691, P<0.000 1)for predicting MS. The corresponding cutoff value was 25.00%, the sensitivity, specificity, positive predictive value and negative predictive value of diagnosis of MS was 77.80%, 56.00%, 30.20%and 91.30%, respectively;409 subjects who had no history of MS and diabetes mellitus follow up for two years, the AUC of the test was 0.719 (95%CI 0.644-0.794)for predicting MS. The corresponding cutoff value was 25.00%, too. And the sensitivity and specificity of diagnosis of MS was 78.00%and 59.80%, respectively. The detection rate of MS was positively correlated with the risk score. Conclusion We found that the human biological stimulation feedback instrument is a potential screening approach for early predicting MS in health checkup with simple and noninvasive procedure and high negative predictive value, although its diagnostic efficiency is not so good. And it might have a warning effect on the progress of MS.
9.Airway management of a series of patients with inhalation injury caused by smoke from smoke pot based on grade classification

In this study,we summarized airway management of a series of patients with inhalation injury caused by smoke from smoke pot based on grade classification,which were:establishment of a special team for these classified patients,classifying the patients into four sub-groups including extremely severe,severe,moderate and mild,establishment of a program for trachea management and relevant measures on these classified sub-groups.Key points of management on extremely severe patients were as follows:protective isolation,mechanical ventilation,nursing of extracorporeal membrane oxygenation and pneumothorax and mediastinal emphysema,prevention of tracheo-esophgeal fistula,nursing cooperation of fiberoptic bronchoscopy.Key points of management on severe patients were as follows:disinfection and isolation,rational oxygen therapy,sputum elimination management,observation of illness status,preparation of emergency treatment,prevention and nursing of complications.Key points of management on moderate patients were as follows:ventilation,oxygen uptake,aerosol inhalation,sputum elimination guidance,respiratory function training.For mild patients,there was no special management except ventilation,aerosol inhalation and regular pulmonary function examination.One patient died due to multiple organ failure complicated with massive hemoptysis,four patients recovered with airway scar proliferation and lung fibrosis,and fifty-five patients fully recovered.
10.Clinical effect of conversion to Rapamycin on chronic allograft nephropathy

Objective To study the effect and safety of conversion from calcineurin inhibitors to rapamycin in kidney transplantation recipients with chronic allograft nephropathy.Methods In 82 kidney transplant recipients enrolled in this study,72 cases were diagnosed as having chronic allograft nephropathy by biopsy.Recipients (SRL group) were administered with rapamycin after withdrawal of calcineurin inhibitors.The doses of CNI in other recipients (non-SRL group) were not changed.Renal function,proteinuria,blood pressure,blood fat,hepatic function and hemogram were observed for 24 months in each group.Results During the follow-up period,serum creatinine level was dropped significantly in SRL group (P<0.05),but it was increased in non-SRL group (P<0.05).SRL group showed increased proteinuria,serum cholesterol and triglycerides (P<0.05),and reduced Plt (P<0.05).According to the renal function before conversion,the recipients who were administered rapamycin divided into four groups.In group A (Scr < 120 μmol/L),there was no significant difference in diverse variables before and after conversion.In group B (Scr 120-200 μmol/L and Banff Ⅰ-Ⅱ),renal function was improved,and proteinuria alleviated.In group C (Scr 120-200 μmol/L and Banff > Ⅱ),and group D (Scr >200 μmol/L),renal function was damaged to varying degrees and proteinuria was deteriorated.Conclusion It is safe and effective for patients with chronic allograft nephropathy to convert from calcineurin inhibitors to rapamycin.
