Objective To investigate the efficacy and significance of endoscopic sphincterotomy(EST)and endoscopic nasobiliary drainage(ENBD)combined with the continuous blood purification(CBP)in the treatment of severe acute pancreatitis(SAP).Meth-ods The clinical data of 179 patients with SAP treated in our hospital over a period of ten years were retrospectively analyzed.The cases were divided into four groups according to the treatment methods.The conventional group was treated with the routine meth-od,the CBP group received the CBP therapy combibed with routine methods,the endoscopic group was given EST and ENBD on the basis of the routine methods and the combined group was treated with CBP,EST and ENBD.The patients′vital signs,WBC,CRP, APACHE Ⅱ score,abdominal signs change,incidence of complications,mortality and hospitalization days were compared among 4 groups.Results Compared with the conventional group,the clinical indicators,WBC,CRP,APACHEⅡ scores,blood amylase lev-el,incidence of local complications,mortality and hospitalization days in the rest three groups were decreased significantly(P<0.05),the total effective rate of abdominal signs relief was significantly increased(P<0.05);compared with the rest groups,the to-tal effective rate of abdominal signs relief in the combined group was increased(P<0.05),the blood amylase level,incidence of local complications and hospitalization days were decreased significantly(P<0.05),the mortality was significantly lower than that in the conventional group(P<0.05),however there was no difference between the CBP group and the endoscopic group(P>0.05).Con-clusion CBP combined with EST and ENBD in treating SAP has the active and effective role for improving the disease condition, reducing complications and the mortality,and shortening the hospitalization time.

Objective To evaluate the effects of urodynamic studies on diagnosis and treatment of female stress urinary incontinence (SUI). Methods Urodynamic studies including the determination of stress leakage spot pressure were performed in 20 cases of SUI.Of them 6 cases (30%) had detrusor instability.Based on the types of SUI pathogenesis,7 cases were of Type Ⅲ,10 cases of mixed type (Type Ⅲ/Type Ⅱ) and 3 cases of Type Ⅱ.Of the 20 cases,16 underwent operation,including modified Gittes suspension in 12,sling with polyester pieces in 3 and MMK plus posterior vaginal wall repair in 1. Results The 16 operatied patients were followed up for 8～40 months(mean,31 months).The patients undergoing MMK or Sling operation were cured.But 3 patients (1 of Type Ⅲ and 2 of Type Ⅲ/Ⅱ) undergoing modified Gittes suspension had relapse (recurrence rate,25%). Conclusions Urodynamic studies are important to the diagnosis and differential diagnosis of SUI,and are indicative for choice of operation approaches.

Objective To analyse the factors influencing the death of patients with aortic dissection (Stanford type A arota dissection) who underwent urgent surgery.Methods The clinical data of 197 patients with aortic dissection (Stanford type A arota dissection) treated with urgent surgery from the First Affiliate Hosiptal of Xinjiang Medical University were retrospectively analyzed between January 2009 and June 2015 in order to determine the factors influencing the early-mortality.Results Of them,23 patietns dead.In univariant analysis,the rate of mortality was 11.68％.The following factors at admission were independetly associated with early-mortality:hypertension,coronary atherosclerotic heart disease,LVEF ≤ 50％,time consumed for CPB ≥ 180 min,time required for cross-clamping aorta ≥ 120 min,Cr ≥ 130 μmol/L,rethoracotomy for hemostasis (P ＜0.05).The preoperative renal failure (Cr≥130 μmol/L) and time consumed for cross-clamping aorta ≥ 120 min were strongly associated with the early-mortality based on the logistic regression analyses.Conclusions The risk of aortic dissection surgery is very high,and it is mondatory to carefully evaluate the patient' s general condition and the indications during peroperative period,and to take active measures to shorten the time of clamping aorta and total operation time in order to reduce the risk of operation and increase the survival rate of patients.

Objective To evaluate a blood test,UniCAP system(screening IgE to food or inhalant allergens,eosinophil cationic protein,ECP)in differentiation the capability of lgE-mediated allergic diseases in children. Methods 26 atopic dermatitis,33 asthma/bronchiolitis,20 allergic purpura, 15 Kawasaki disease patients and 20 normal children were tested total lgE,lgE to food or inhalant allergens,ECP by UniCAP system and compared.Resuits Fx5E positive rate are 73.1%,24.2%,25.0%,13.3%in atopic dermatitis,asthma,allergic purpura,Kawasaki disease respectively,and the Phadiatop positive rate are 23.1%、69.7%、10.0%、6.6%.The serum levels of ECP、TIgE of patients were higher than the mormal controls,with the highest level in asthma, and lowest in Kawasaki disease,the differences were significant(P<0.05).Conclusions Food orinhalant allergens plays an important role in the inflammatory process of asthma,atopic dermatitis,and some role in allergic purpura,Kawasaki disease.Food allergens are more important to atopic dermatitis.and inhalant allergens are more important to asthma.

Objective: To study the effects of retinoids (retinol? retinoic acid? retinyl acetate) in vitro on the production of interleukin 1 (IL-1)? interleukin 2 (IL-2) of peripheral blood mononuclear cells (PBMC) from the children with recurrent respiratory infection (RRI). Methods: The activities of IL-1? IL-2 of the PBMC from RRI children and the changes of IL-1? IL-2 activities after cultured with 10 -9 mol/L retinoids were measured. Results: The activities of IL-1? IL-2 of PBMC from RRI children were lower than normal control and could be restored to normal level after cultured with retinoids. Conclusion: The production of IL-1? IL-2 of PBMC from RRI children can be enhanced by retinoids and restored to normal.

Objective To explore the kinds and levels of phthalates leaching from disposable plastic products. Methods Samples of peritoneal dialysis solution, blood preservative solution, infusion instruments, preservative film, disposable plastic bags and water in plastic bottles were analyzed for phthalates by RP-HPLC after liquid-liquid extraction and/ or solid phase extraction. Results Di(2-ethylhexyl) phthalate (DEHP) was leached from all medical instruments, the maximum level of which reached 77.51?g/ L. Di-n-butyl phthalate was leached from disposable plastic bags, the level of which reached 91.45?g/ kg. Phthalates were not found in samples of preservative film and water in plastic bottles. Conclusion As DEHP leaching from the medical instruments might directly enter the human body, attention should be paid to its health hazards.

Objective To study the toxicokinetics of phthalates in male rabbits. Methods Concentrations of DEHP or DBP in plasma of 6 healthy male rabbits were determined by RP-HPLC after constant rate infusion of 0.5 g/kg DEHP or DBP(IV). The toxicokinetic parameters were computed by 3P87 program. Results The toxicokinetic model of DEHP and DBP were both first-order elimination and two-compartment model with constant rate infusion. The main toxicokinetic parameters of DEHP were as follows: distribution phose t1/2(?)=0.101 h; elimination phase t1/2(?)=12.701 h; CLs=0.013 g?kg-1?h-1. The main toxicokinetic parameters of DBP were as follows: t1/2(?)=0.441 h; t1/2(?)=31.311 h; CLs=0.021 g?kg-1?h-1. Conclusion DEHP and DBP were both first order elmination and two-compartment medol with constand rate intusion(IV). DEHP and DBP could be rapidly eliminated in male rabbits.

Objective To study the effect of Interleukin-22 (IL-22) on murine asthmatic airway inflammation and airway remodeling, observe the effect of budesonide on IL-22 of asthmatic mouse model, explore the mechanism of budesonide in the treatment of asthma.Methods Ovalbumin(OVA) was used as an allergen to sensitize and challenge the mice.24 female specific-free (SPF) BALB/c mice aged four weeks were randomly divided into 3 groups:control group, asthma group and budesonide treatment group (BUD group).For Histopathological Examination, HE staining was used to measure the inflammation scores, AB-PAS staining was used to measure the hyperplasia of goblet cells and mucin.Enzyme-linked immunosorbent assay(ELISA) was used to analyze IL-22 levels in bronchoalveolar lavage fluid (BALF).Quantitative Real-time PCR was performed to analyze the effects of budesonide on IL-22 mRNA levels in lung tissue.Results The inflammation scores of asthma group were elevated compared with the control group.An overall change towards less severe asthmatic airway inflammation by the end of the trial was observed in the BUD group.IL-22 levels in BALF were significant decreased after the treatment of budesonide, the mRNA levels of IL-22 were obviously decreased in BUD group, too.A significant positive correlation was observed between the mRNA levels of IL-22 and airway inflammation.Conclusions The increasing IL-22 secretion can lead to the occurrence of airway inflammation of asthma.Budesonide can inhibit the expression of IL-22, thereby Budesonide could inhibit the development of airway inflammation of asthma.

Objective To investigate the changes in the abilities of invasion and metastasis in surviving human lung adenocarcinoma (A549) cells after X-ray irradiation and the related mechanism.Methods The change in radiosensitivity after X-ray irradiation was determined by colony formation assay.The abilities of invasion and metastasis were evaluated by MTF adhesion assay and Transwell invasion and migration assay in vitro.The mRNA and protein expression of E-cadherin,vimentin,tumor growth factor (TGF)-β1,matrix metalloproteinase (MMP)-2,and MMP-9 was measured by real-time RT-PCR using SYBR Green fluorescence and Western blot.Results The colony formation assay showed that the surviving A549 cells after X-ray irradiation were more resistant to irradiation (ratio of D0 values =0.94).Their abilities of adhesion,invasion,and migration were significantly increased by 1.46 times,1.40 times,and 1.45 times,respectively.In addition,the surviving cells showed enlarged intercellular spaces and had many long pseudopodi.Compared with those in the control group,the mRNA expression levels of vimentin,TGF-β1,MMP-2,and MMP-9 of surviving cells were increased by 1.37 times,2.37 times,1.80 times,and 1.50 times,respectively,the protein expression levels of the above substances were increased by 1.60 times,1.80 times,1.10 times,and 1.20 times,respectively,and the mRNA and protein expression levels of E-cadherin were decreased by 59.4％ and 74.6％.Conclusions The surviving A549 cells after X-ray irradiation have significantly increased abilities of invasion and metastasis.This may be due to radiationinduced TGF-β1 expression increase that in turn promotes epithelial-mesenchymal transition and also due to radiation-induced MMP-2 and MMP-9 expression increase.
