1.Correlation of visfatin and L-PGDS with lower extremity arteriosclerosis

Objective To investigate the relationship between lipocalintype prostaglandin dsynthase(L-PGDS) and visfatin in the lower extremity atherosclerotic plaque. Methods Collected from February 2014 to February 2016 in vascular surgery of our hospital 40 cases of femoral artery atherosclerotic plaque intima specimens (observation group), 20 cases of splenic artery, superior mesenteric artery samples (control group), the expression of visfatin and L-PGDS protein were detected by immunohistochemical staining, and the expression of visfatin and L-PGDS mRNA were determined by RT-PCR. Results The observation group visfatin protein expression was (121.42±11.07), significantly higher than the control group (72.07±12.81), and L-PGDS protein expression was (87.93±9.73), significantly lower than the control group (107.04±10.58), the difference was statistically significant (P<0.05). The relative expression of visfatin mRNA in the observation group was (0.321±0.024), which was significant higher than that of the control group (0.217±0.031), while L-PGDS mRNA was (0.203±0.018), significantly lower than the control group (0.314±0.029), the difference was statistically significant (P<0.05); The expression of L-PGDS protein was negatively correlated with the expression of visfatin protein in the plaque tissue (r=-0.617, P<0.05), visfatin mRNA and L-PGDS mRNA were negatively correlated (r=-0.645, P< 0.05). Conclusion Visfatin and L-PGDS in lower extremity atherosclerosis occurrence and development have an important relationship, both of them may play an antagonistic role.
2.Knee injury and lower extremity deep venous thrombosis

Objective To explore the relation between lower extremity deep venous thrombosis and knee injury.Methods 100 knee injury patients were operated on in our department.The difference of incidence rates of DVT,the thrombus location and incidence rates of DVT before and after operation in the two groups was compared.There were no fatal pulmonary embolism occurring in either group.Results The incidence rate of DVT in tibial plateau fracture group was 52%,that in patellar fracture was 30% (P < 0.05).DVT incidence rate of popliteal,pretibial,posterior tibial vein thrombosis in group of tibial plateau fracture was higher than that in group of patellar fracture (P < 0.05),while there was no significant difference in postoperative thrombosis between the two groups(P > 0.05).Conclusions There is a high incidence rate of DVT in knee joint injury.Which is closely related to the position of fracture and limb immobilization.
3.Clinical Effect of Four Oral Vitamins Combined with FE Complex Enzyme Rash on Recurrent Oral Ulcer and Serum Inflammatory Factors Levels

Objective:To explore the clinical effect of four oral vitamins [vitamin E (Vit E) +folic acid (FA)+ vitamin B2 (Vit B2) + vitamin B12 (Vit B12)] combined with FE complex enzyme rash on the recurrent oral ulcer (ROU) and on the serum inflammatory factors levels.Methods:126 cases with ROU in our hospital from January 2014 to February 2016 were selected as research objectives and randomly divided into two groups.FE complex enzyme was provided to the control group,while FE complex enzyme and four Vietnam (Vit E+FA+Vit B2+Vit B12) were given to the observation group.The local efficacy,long-term efficacy,serum inflammatory factor levels before and after treatment as well as the incidence of adverse drug reactions were recorded and compared between two groups.Results:Compared with the control group,the pain index was significantly decreased on 30th treating day in the observation group(P<0.01),and the average ulcer period was shorten (P<0.01).After being treated for 6 months,the overall effective rate was 95.2% in the observation group,which was significantly higher than that of the control group (81.0%,P<0.05).The serum TNF-α and IL-17 levels on the 30th treating day was significantly lower in both groups after treatment than those before treatment (P<0.01),but the serum 1L-2 level was significantly increased (P<0.01),and the improvement of each above inflammatory factors in the observation group were more significant than those of the control group (P<0.01).No significant difference was found in the incidence of adverse reactions between two groups (P>0.05).Conclusions:Four oral Vietnam combined with FE complex enzyme could promote the ulcer wound healing,reduce the pain,regulate the body to promote/anti-inflammatory factor balance and improve the long-term efficacy in the treatment of patients with ROU with high safety.
4.REPORT OF HLA DISTRIBUTION IN 2315 VOLUNTEER DONORS OF CHINESE BONE MARROW BANK FROM NORTHWEST CHINA

Objective To report the HLA data of volunteer donors of Chinese bank from Northwest China and characterize the distribution of HLA genes in Northwest China. Methods HLA-A, B antigens of 2315 volunteer donors were examined by the method of microlymphocytetoxicity (MLT) test .The antigen frequencies(AF) were assessed by directly counting; and based on that gene frequencies(GF) were calculated. HLA data from other population were collected and distribution characteristics were compared. With the raw data, Hardy-Weinberg equilibrium, statistical parameters of forensic medicine interest for HLA were computed. Results A total of 18 specific antigens were detected in HLA-A and the most frequent antigen was A2 . AF and GF were 0.5136 and 0.3026, respectively. A total of 42 specific antigens were detected in HLA-B and the most frequent antigen was A13. Its AF and GF were 0.1978 and 0.1044, respectively. The heterozygosity(H), polymorphism information content(PIC), discrimination power(DP) and probability of paternity exclusion (PPE) of HLA-A were 0.8215, 0.8212, 0.9356 and 0.7798 accordingly; while H,PIC, DP and PPE of HLA-B were 0.9322, 0.9322, 0.9878 and 0.9528. Conclusion The polymorphism of HLA-A,B genes is characteristic in Chinese. In this research, the genetic trait of HLA in 2315 volunteers is consistent with Northern Han population.
5.Clinical Analysis of Endovascular Treatment of TASC (Ⅱ) D-Type Femoral Artery Occlusion

Objective To investigate methods and results of endovascular treatment in TASC (Ⅱ) D-type femoral artery occlusion. Methods From January 2012 to May 2013, 26 cases (26 branches) of superficial femoral artery occlusion with endovascular treatment of TASC (Ⅱ) D-type superficial femoral artery occlusion were retrospectively reviewed. The effi-cacy was evaluated through ABI, CTA, DSA and symptoms improved. Results 26 branches were treated with endovascular methods. Technical success rate was 80.7%(21/26), including 13 branche with stent implantation, 6 branches with Silver-hawk atherectomy and 2 branches with Viabahn stent implantation. All patients were followed up for a mean period of (10.3 ± 1.2)months, primary patency rates at 6 months were 69.2%in stent group, 66.7%in Silverhawk atherectomy group and 100%in Viabahn stent group. Conclusion Endovascular treatment of TASC (Ⅱ) D-type femoral artery occlusion can lead to satisfactory short term patency rates, and Viabahn stent is the latest treatment.
6.Safety and short-term efficacy of MR guided focused ultrasound surgery for bone metastasis-induced pain palliation

Objective To discuss the safety and short-term efficacy of MR-guided focused ultrasound surgery (MRgFUS) for pain palliation of bone metastases patients.Methods Fourteen patients with painful bone metastases were recruited in this prospective study.The treating efficacy was characterized by numerical rating scale (NRS),the brief pain inventory quality of life (BPI-QOL) survey,and Karnosky performance status scale (KPS).Adverse events occurred pre-and post-treatment were analyzed.Normal distributed statistics was analyzed by using paired-samples t test or Wilcoxon rank sum test.Results Fourteen patients were treated with MRgFUS,2 patients dropped out of the study.The NRS ratings are 6.50(4.00),5.00 (5.25),2.50(5.00),2.50(4.75),2.00 (6.00) for pre-treatment,one week,one month,two months,and three months,respectively.Such variances of NRS ratings were statistically significant (Z=-2.773,-2.740,-2.769,-2.675;P<0.05).The BPI-QOL ratings were (42.42± 8.27),(30.67 ± 12.29),(29.17±15.38),(29.92± 17.67) and (35.67± 19.28),respectively.The BPI-QOL ratings decreased in the first two months after the treatment which is statistically significant (t=3.231,2.820 and 2.453;P<0.05);whereas for the third month,the BPI-QOL rating was statistically insignificant compared with the one before the treatment (P>0.05).The KPS ratings were 80(28),80(20),65(45) for pre-treatment,one week and three months after treatment,respectively.Three months after the treatment,the KPS ratings decreased which was statistically significant compared with the one before the treatment (Z=-2.204,P<0.05).After the treatment,one patient developed deep venous thrombosis,three patients reported lower extremities numbness,two patients had soft tissue edema around the lesions.Conclusions MRgFUS is effective for short-term pain palliation of bone metastases.Such noninvasive technique is safe and can improve patients' living condition.
7.The prognostic role of the programmed death-1 expression on T lymphocytes in septic patients

Objective:To investigate the value of programmed death-1(PD-1) expression on the T lymphocytes for the prognosis of septic patients.Methods:From September 2017 to May 2019, septic patients were included in Department of Intensive Care Unit at 6 hospitals. The PD-1 expression on T cells were measured by flow cytometry. Logistic regression was conducted to analyze independent risk factors related to death within 28 days,and receiver operating characteristic curve(ROC) was conducted to evaluate the prognostic value of PD-1 expression on T cells in septic patients.Results:A total of 64 septic patients were enrolled to this study,including 32 survivors and 32 deaths. The PD-1 expression on T cells in the death group was significantly higher than that in the surviving group ( P<0.05). Correlation analysis showed that the percentages of PD-1 +/CD3 +T cells and PD-1 +/CD8 +T cells were positively correlated with procalciton in ( r=0.313, P =0.015; r=0.375, P=0.003), logistic regression analysis showed that the percentages of PD-1 +/CD3 +,PD-1 +/CD4 +,PD-1 +/CD8 +T cells were independent risk factors for the death of sepsis patients. The percentage of PD-1 +/CD3 +T cell was 3.63%, with AUC 0.842, sensitivity to predict the mortality 96.43% and specificity 59.38%, ( P<0.000 1). The percentage of PD-1 +/CD4 +T cell was 4.65%, with AUC 0.847, sensitivity 96.43%, specificity 62.50%,( P<0.000 1). The percentage of PD-1 +/CD8 +T cell was 3.91%, with AUC 0.771, sensitivity 64.29%, specificity 81.25%,( P=0.000 3). Conclusions:The T cell PD-1 expression is an independent risk factor to predict the 28-day mortality in septic patients. Combining the proportions of PD-1 +/CD3 +, PD-1 +/CD4 +and PD-1 +/CD8 +T cells may further enhance the predictive value for death.
8.Analysis of sepsis-related genes through weighted gene co-expression network

Objective:To identify the Key genes in the development of sepsis through weighted gene co-expression network analysis (WGCNA).Methods:The gene expression dataset GSE154918 was downloaded from the public database Gene Expression Omnibus (GEO) database, which containes data from 105 microarrays of 40 control cases, 12 cases of asymptomatic infection, 39 cases of sepsis, and 14 cases of follow-up sepsis. The R software was used to screen out differentially expressed genes (DEG) in sepsis, and the distributed access view integrated database (DAVID), search tool for retrieval of interacting neighbouring genes (STRING) and visualization software Cytoscape were used to perform gene function and pathway enrichment analysis, Protein-protein interaction (PPI) network analysis and key gene analysis to screen out the key genes in the development of sepsis.Results:Forty-six candidate genes were obtained by WGCNA and combined with DEG expression analysis, and these 46 genes were analyzed by gene ontology (GO) and Kyoto City Encyclopedia of Genes and Genomes (KEGG) pathway enrichment to obtain gene functions and involved signaling pathways. The PPI network was further constructed using the STRING database, and 5 key genes were selected by the PPI network visualization software Cytoscape, including the mast cell expressed membrane protein 1 gene (MCEMP1), the S100 calcium-binding protein A12 gene (S100A12), the adipokine resistance factor gene (RETN), the c-type lectin structural domain family 4 member gene (CLEC4D), and peroxisome proliferator-activated receptor gene (PPARG), and differential expression analysis of each of these 5 genes showed that the expression levels of the above 5 genes were significantly upregulated in sepsis patients compared with healthy controls.Conclusion:In this study, 5 key genes related to sepsis were screened by constructing WGCNA method, which may be potential candidate targets related to sepsis diagnosis and treatment.
9.Study on effect of gastrointestinal function and portal vein hemodynamics with abdominal hot compressing with evodiae fructus and crude salt after hepatectomy

Objective:To study on the effects of gastrointestinal function and portal vein hemodynamics applicated with abdominal hot compressing with evodiae fructus and crude salt after hepatectomy.Methods:A total of 60 patients who underwent hepatectomy were randomly divided into 2 groups by random number table method, with 30 in each group. The control group was treated with conventional basic western medicine, while the treatment group was treated with medicinal evodiae fructus and crude salt hot compress on abdomen on the basis of the control group. The portal vein diameter (PVD), portal venous flow velocity (PVV), recovery time of main clinical indexes, clinical symptom scores and liver function indexes were dynamically monitored at different observation time points.Results:The scores of abdominal distension, nausea and vomiting in the treatment group were significantly lower than those in the control group 3 days after treatment ( t values were -3.489 and -2.740, respectively, all Ps<0.05). The recovery time of bowel sounds, first exhaust time and first defecation time in the treatment group were significantly earlier than those in the control group ( t values were -3.622, -4.297 and -4.151, respectively, all Ps<0.01). With the extension of treatment time, ALT in 2 groups showed a gradual downward trend ( P<0.05 or P<0.01), DBIL in control group was significantly higher at 3 days after treatment than before ( t=-2.157, P=0.039), and TBIL was significantly lower at 7 days after treatment than before ( t=2.175, P=0.038). The PVD ( t values were 3.528, 2.160) and PVV ( t values were 11.096, 4.264) of the treatment group were significantly higher than those of the control group 3 and 5 days after treatment ( P<0.01 or P<0.05). Conclusion:Early application of abdominal hot compressing with evodiae fructus and crude salt hot compress on abdomen after hepatectomy can improve the portal vein blood circulation and promote the rehabilitation of gastrointestinal function in patients with hepatectomy.
10.Study on compatibility of traditional Chinese medicine active constituents with antioxidant activity.

OBJECTIVETo find a research method suitable to multi-component multi-index composition-activity relationship of traditional Chinese medicines (TCMs) with the antioxidant activity of four compatible active constituents of TCMs as the subject.
METHODLARS-based regression algorithm and comprehensive weight coefficient method were adopted to study in vitro clearance of DPPH and polyaromatic hydrocarbon with different doses of four compatible active constituents--total flavonoids of Glycyrrhizae Radix et Rhizoma (TFG), Ginkgo Folium extract (GBE), total flavonoids of Epimedii Folium (TFE) and astragaloside (AST) according to the comprehensive efficacy assessment procedures, that is test design, efficacy test and mathematical modeling (model verification).
RESULTLARS and comprehensive weight coefficient method was adopted to assess the optimal comprehensive efficacy of clearing DPPH and polyaromatic hydrocarbon. The results showed that the optimal dose combination of compatible constituents is TFG-GBE-TFE-AST 1: 0.2545: 0.007 6:0.011 5.
CONCLUSIONThe above compatibility of TCMs constituents can effectively clear polyaromatic hydrocarbon and DPPH with a good antioxidation. The uniform design adopted in this experiment, in combination of synthetic weight method, is suitable to the data analysis of "non-linear and small-sample" biotic experiment and the study on screening and assessment of compatibility of TCMs constituents.
