1.Effects of combination of Salvia miltiorrhiza and Panax notoginseng on the pharmacokinetics of their major bioactive components in Beagle dog.

After oral administration of Salvia miltiorrhiza (Danshen in Chinese), Panax notoginseng (Sanqi in Chinese) and Danshen Sanqi combination suspensions to Beagle dogs, the plasma concentration-time profiles of danshensu, tanshinone II(A), cryptotanshinone, notoginsenoside R1, ginsenoside Rg1 and Rb1 were analyzed by LC-MS/MS. Pharmacokinetic parameters were calculated and analyzed with BAPP 2.0 software. The results showed that the Cmax and AUC of danshensu, notoginsenoside R1, ginsenoside Rg1 and Rb1 in Danshen Sanqi combination group all decreased in comparison with those of Danshen or Sanqi given alone, while the CLz/F and Vz/F increased to some extent. No significant differences of the pharmacokinetics of tanshinone II(A) and cryptotanshinone were observed between groups.
2.Bipolar hemiarthroplasty for osteoporotic hip fractures in the elderly:controversy of prosthesis selection

BACKGROUND:The number of elderly patients with hip fracture is huge, and this population mainly combines with a variety of internal diseases. The general condition was bad. Compared to total hip arthroplasty, bipolar hemiarthroplasty can shorten operation time, reduce the time of anesthesia and improve surgical safety, but the operation using which fixation mode is always controversial.
OBJECTIVE:To compare the safety and curative effects of bipolar hemiarthroplasty with cementless or cement femoral stem prosthesis in elderly patients with osteoporotic complex hip fracture.
METHODS:From June 2007 to June 2010, 198 elderly patients with osteoporotic hip fracture aged more than 75 years old, who were treated with bipolar hemiarthroplasty in the People’s Hospital of Xinjiang Uygur Autonomous Region, China, were enroled in this study. Al patients were divided into cementless group (n=69) and cement group (n=129) according to the type of prognosis. Osteoporosis was treated conventionaly in both groups after replacement. Operation time, intraoperative blood loss, incidence of postoperative hip pain, out of bed activity time post surgery, Harris scores of last folow-up and loosening rate of the prognosis were compared between the two groups.
RESULTS AND CONCLUSION:176 of above 198 cases were folowed up for 48 to 84 months, including 122 in the cement group and 54 in the cementless group. Operation time was longer in the cement group than in the cementless group, but out of bed time was earlier in the cement group than in the cementless group post surgery, and there were significant differences (P < 0.05). The incidences of loosening and hip pain were more in the cement group than in the cementless group post surgery (P < 0.05). No significant difference in intraoperative blood loss and Harris scores of last folow-up was detected between the two groups (P > 0.05). The incidences of loosening rate were 5% and 4% in the cement and cementless groups, respectively, which showed significant differences (P< 0.05). These results indicate that bipolar hemiarthroplasty with or without cement for osteoporotic hip fracture in the elderly can obtain good middle- and long-term effects. Bipolar hemiarthroplasty with cement can reduce the out of bed time, but operation time was longer, and incidences of hip pain and loosening were higher in cement prosthesis than in cementless prosthesis.
3.Laparoscopic total pancreastectomy: a report of 3 cases

Objective To investigate the value of laparoscopic total pancreastectomy for the treatment of noncancerous pancreatic lesions.Methods Clinical data of 3 cases of noncancerous pancreatic lesions undergoing laparoscopic total pancreatectomy in Zhejiang Provincial People's Hospital were reviewed retrospectively.Results One patient underwent laparoscopic total pancreatectomy,one patient underwent laparoscopic resection with robotic reconstruction and one did laparoscopic resection with open reconstruction.All were spleen preserving surgery.The operation time was 310 (280-350) minutes,estimated blood loss was 483 ml(250-700)ml and postoperative hospital stay was 27 (14-38) days.One patient had postoperative bile leakage.Insulin was used to control blood glucose level in all postoperative cases.As showed by pathology there were intraductal papillary mucinous neoplasms in two patients and multiple neurocndocrine tumor in one patient.Conclusion Laparoscopic total pancreatectomy is safe and minimally invasive for the treatment of noncancerous pancreatic diseases.
4.Total pancreatic head resection with duodenum and bile duct preserving: a report of 31 patients

Objective To investigates the role of duodenum and bile duct preserving pancreatic head resection (DBPPHR) in treatment of benign or low-grade malignant diseases located in the head of pancreas.Methods The clinical data of 31 patients who underwent DBPPHR between April 2012 to May 2016 in Zhejiang Provincial People's Hospital and Zhangzhou Municipal Hospital of Fujian Province were analyzed retrospectively.Results Of the 31 patients,4 patients underwent laparoscopic DBPPHR.One patient in the open group was converted to pancreaticoduodenectomy.For the open group,the mean operation time was (165.3 ±63.6) min;the mean estimated blood loss was (258.1 ± 156.9) ml;and the mean postoperative stay was (11.7 ± 6.3) days.The postoperative complications included 1 reoperation due to postoperative bleeding,1 bile leakage and 13 patients developed grade A pancreatic fistula (48.2%).For the laparoscopic group,the mean operation time was 350.0 (280.0 ~ 450.0) min;the mean estimated blood loss was 425.0 (250.0 ~600.0) ml;and the mean postoperative stay was 14 days.Three patients developed postoperative pancreatic fistula (grade A).The pathological diagnosis were:12 patients with pancreatolithiasis,8 patients with serous cystadenoma,4 patients with branched intraductal papillary mucinous neoplasm,5 patients with neuroendocrine tumor and 2 patients with mucinous cystadenoma.The follow-up period was 1 ~ 48 month,and there was no patient with diabetes or diarrhea.Conclusions DBPPHR was safe and efficacious.It is less invasive to treat benign or low-grade malignant diseases located in the head of pancreas.
5.Effects of TPF regimen and IMRT on immune function and survival prognosis of patients with advanced esophageal cancer

Objective:To study the effects of docetaxel, cisplatin and fluorouracil (TPF) regimen simultaneous intensity modulated radiotherapy (IMRT) on immune function and survival prognosis of patients with advanced esophageal cancer.Methods:A total of 93 patients with advanced esophageal cancer were screened in Hebei Veterans General Hospital from June 2015 to December 2017, and were divided into two groups using randomized envelope method. The observation group (47 cases) was given synchronous TPF regimen and IMRT, and the control group (46 cases) was given synchronous PF regimen (cisplatin combined with fluorouracil) and IMRT. Esophageal barium meal, chest and upper abdominal CT were reviewed within 1 month after treatment to assess the short-term efficacy and compare the immune function of the two groups before and after treatment. Kaplan-Meier survival curve was plotted to evaluate the long-term efficacy based on overall survival (OS). The incidence of adverse reactions in the two groups was collected to evaluate their safety.Results:After treatment, the T cell subgroup CD8 + level of the observation group was higher than that of the control group [(33.55±4.46)% vs. (29.06±3.61)%, P<0.05], while CD3 + [(51.29±5.22)% vs. (56.04±6.10)%, P<0.05], CD4 + [(28.27±3.63)% vs. (30.35±3.52)%, P<0.05] and CD4 + /CD8 + (0.84±0.25 vs. 1.04±0.08, P<0.05) levels were lower than those of the control group. The effective rate of recent treatment in the observation group was 82.98% (39/47), while the effective rate in the control group was only 63.04% (29/46), with a statistically significant difference ( χ2=4.70, P=0.030). The median OS of the observation group was 25.3 months (95% CI: 17.9-26.1), and that of the control group was 18.2 months (95% CI: 14.4-25.5), with a statistically significant difference ( χ2=3.28, P=0.038). Adverse reactions during the follow-up period of the two groups of patients were mainly nausea/vomiting, fatigue, anorexia, hematological toxicity, esophagitis and pneumonia, etc., which were mostly grade 1-2, and disappeared after symptomatic treatment or termination of treatment. Compared with the control group, the incidence of nausea/vomiting (46.81% vs. 78.26%, χ2=9.80, P=0.002), anorexia (44.86% vs. 71.74%, χ2=6.99, P=0.008), leukopenia (36.96% vs. 73.91%, χ2=13.37, P<0.001) and esophagitis (61.70% vs. 82.61%, χ2=5.05, P=0.025) adverse reactions was lower in the observation group. Conclusion:TPF combined with IMRT has high efficacy and low adverse reactions, which can be used as an effective treatment to improve the survival prognosis of patients with advanced esophageal cancer.
6.Evaluation changes of regional cerebral glucose metabolism in patients with Alzheimer's disease

Objective To explore the characteristics and significance signification of changes of cerebral glucose metabolism in patients with Alzheimer's disease (AD).Methods Seventeen patients diagnosed with AD based on standard clinical diagnostic criteria (AD group),and 15 healthy volunteers as normal control group (HC group)were enrolled in this study.Positron emission tomography (PET)were performed in both groups and three-dimensional reconstruction images were obtained.The visual evaluation,regions of interesting (ROI)technology and brain function analysis software were used to analyze PET results data in of each group.Results ①In AD group,glucose metabolisam was decreased and mainly distributed in the parietal lobe,temporal lobe,frontal cortex,posterior cingultate and hippocampus,but not in occipital lobe,basal ganglia,thalamus and cerebellum nuclei basales,thalamus and opisthencephalon.Ssomatic sensation and motor functionand were not affected.②Compared with mild AD patients,glucose metabolism inof moderate and severe AD patients was decreased more significantly in parietal lobe,temporal lobe,frontal cortex,posterior cingultate and hippocampus,with a wider range of lesionsthe range also expanded,and visual area and speech centerpartly followed by the disorder of visual center and speech center may bewere involved in some patients.③The built-in brain function analysis software of PET system software carried by PET system was more objectiveprecise and accurate than visual evaluation or and ROI technology,can accurately reflected the changes in brain glucose metabolism which and iwas suitable for popularization and application.Conclusion Because the distribution of impaired glucose metabolism in cerebral cortex hads certain characteristics and regularity laws in AD patients.18 F-FDG PET imaging can maybe meaningful valuable in diagnosis,differential diagnosis and therapeutic evaluation in AD.
7.Effects of the S1PR3 specific agonist peptide GPS-725.017 on acute lung injury in mice

Objective:To design a modified S1PR3 specific agonist, GPS-725.017, and investigate its protective effect on acute lung injury by promoting macrophage clearance of bacteria.Methods:A short peptide derived from the intracellular region of S1PR3 receptor was named GPS725.017, which was modified with norleucine (Nle) and myristicacid (myr) at its N terminus. Mice were divided into the sham operation group, solvent group and GPS-725.017 treatment group. The acute lung injury model was induced by endotracheal injection of E. coli (5×10 6 CFU), and the experimental group was treated with GPS-725.017 (10 mg/kg). The 48-h survival rate of mice was recorded. After 5 h of modeling, the bacterial load and inflammatory cytokines in peripheral blood and lung were detected, and Vps34 protein content in alveolar macrophages was determined by Western blot. After 12-h of modeling, lung tissues were collected for H&E staining and pathological scores. Results:Compared with the solvent group, the survival rate of mice in the GPS-725.017 treatment group was significantly improved ( P<0.01), the bacterial CFU in blood and alveolar lavage fluid was significantly lower than that in the solvent group ( P<0.001), and the levels of TNF-α and IL-1β in blood and alveolar lavage fluid were significantly lower than those in the solvent group ( P<0.001). Western blot showed that the expression level of Vps34 protein in alveolar macrophages was significantly higher than that in the solvent group ( P<0.01). Histopathology result showed that the pathological damage of lung in the treatment group was significantly less than that in the solvent group ( P<0.001). Conclusions:The modified synthetic S1PR3 specific agonist GPS-725.017 could specifically activate the S1PR3 receptor on the membrane of alveolar macrophages and up-regulate the expression level of intracellular Vps34 protein, which can promote the removal of bacteria in alveolar macrophages, significantly reduce the degree of lung injury and improve the survival rate in ALI mice.
8.The value of 18F-PSMA-1007 PET /CT in detecting the risk of metastasis in patients newly diagnosed with prostate cancer

【Objective】 To investigate the diagnostic efficiency of 18F-PSMA-1007 PET/CT in assessing the metastasis of newly diagnosed prostate cancer (PC), and evaluate its relationship with clinical risk classification. 【Methods】 The clinical data of 257 newly diagnosed PC patients who underwent 18F-PSMA-1007 PET/CT between March 2019 and April 2021 were retrospectively reviewed in this study. All images were interpreted by two senior PET/CT diagnostic specialists. According to the D’Amico risk classification, the patients were divided into low-, intermediate- and high-risk groups. According to Gleason score (GS), the patients were divided into GS≤6, GS=7, and GS≥8 groups. According to the level of serum total prostate-specific antigen (tPSA), the patients were divided into <10 ng/mL, 10-20 ng/mL, and >20 ng/mL groups. Finally, in the groups with D’Amico risk classification, the subgroups were divided according to tPSA level and GS, and the differences of 18F-PSMA-1007 PET/CT in the detection of metastasis were compared among the subgroups. 【Results】 A total of 257 patients were enrolled with a median tPSA 16.34 (3.38-783.12) ng/mL and median Gleason score (GS) 8 (range: 6-10). There were 10 (3.89%), 36 (15.01%), and 211(80.10%) PC patients in the low-, intermediate-, and high-risk groups, respectively. The rate of metastasis in high-risk group, GS ≥ 8 group, and tPSA >20 ng/mL group was 45.02%, 46.50%, and 47.02%, respectively. The rate of metastasis in low-risk group, GS ≤6 group and tPSA <10 ng/mL group was 0, 8.82%, and 15.63%, respectively. When tPSA <10 ng/mL, the rate of metastasis in low-risk group (0) was lower than that in high-risk group (33.33%). When tPSA was 10-20 ng/mL, the rate of metastasis in intermediate-risk group (7.69%) was lower than that in high-risk group (38.71%). When GS ≤6, the rate of metastasis in low-risk group (0) was lower than that in high-risk group (38.71%). 【Conclusion】 The detection rate of metastasis in patients with newly diagnosed prostate cancer by 18F-PSMA-1007 PET/CT is positively correlated with GS, preoperative tPSA level, and D’Amico risk grade.
9.The value of 18F-FDG PET/CT metabolic parameters in predicting the molecular phenotype of breast cancer

【Objective】 To analyze the correlation between metabolic parameters of 18F-deoxyglucose-labeledpositron emission tomography/computed tomography(18F-FDG PET/CT) scan and the immunohistochemistry (IHC) expression in patients with primary breast cancer so as to explore the predictive value of the metabolic parameters for molecular subtypes. 【Methods】 We retrospectively recruited 97 patients who underwent 18F-FDG PET/CT scan from November 2016 to June 2020 with breast cancer. The clinical stages (Ⅰ,Ⅱ,Ⅲ and Ⅳ) and menstrual status (pre-menopause or menopause) were collected. Metabolic parameters, including the maximum standardized uptake value (SUVmax), mean standardized uptake value (SUVmean), metabolic tumor volume (MTV) and total lesion glycolysis (TLG) of the primary tumor, were calculated by physician according to the 40% SUVmax principle. The IHC expressions (positive or negative) of the estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor 2 (HER2), proliferating cell nuclear antigen (Ki-67) and p53; the ipsilateral axillary lymph node metastasis (with or without), and the molecular subtypes (Luminal A, Luminal B, HER2 overexpression, and triple negative) were determined by an experienced pathologist. The correlations between the metabolic parameters and IHC expression were analyzed by Pearson test or Spearman test, and were further stratified by different menstrual status and ipsilateral axillary lymph node metastasis. Finally, we analyzed metabolic parameters among different molecular subtypes. 【Results】 For all the patients, SUVmax and SUVmean had significantly negative correlation with ER and PR expressions (P<0.05); SUVmax, SUVmean, and TLG were significantly positively correlated with Ki-67 expression (P<0.05). SUVmax, SUVmean, and TLG of premenopausal patients (n=57) were negatively correlated with ER and PR expressions (P<0.05), but positively correlated with Ki-67 expression (P<0.05). MTV, TLG and PR expressions in postmenopausal patients (n=40) were positively correlated (P<0.05). In patients with (n=27) or without (n=57) ipsilateral axillary lymph node metastasis, Ki-67 was negatively correlated with SUVmax and SUVmean (P<0.05); in patients without ipsilateral axillary lymph node metastasis, SUVmax, SUVmean, and TLG were negatively correlated with PR (P<0.05). Among different molecular types, SUVmax of HER2 overexpression was significantly higher than that of Luminal A (P<0.05). 【Conclusion】 The 18F-FDG PET/CT metabolic parameters of breast cancer patients have a good correlation with the expression of immunohistochemistry, and SUVmax has predictive value for the expression of hetergeneous molecular types.
10.Effect of interleukin-33 on Th1/Th2 cytokine ratio in peripheral lymphocytes in asthmatic mice.

BACKGROUNDAllergic asthma is a chronic airway inflammatory disease partly characterised by high concentration of T help 2 (Th2) cytokines in bronchoalveolar lavage fluid (BALF). There is no report on the relation of peripherally circulating blood lymphocytes and asthma. We explored the balance of Th2/Th1 cytokines in asthmatic mice. Exogenous recombinant interleukin (IL) 33 acted on murine peripheral circulating blood lymphocytes, IL-5 cytokine was selected for assessing Th2 cytokines and interferon-gamma (IFN-γ) for Th1 cytokines.
METHODSFemale specific pathogen free BABL/c mice were sensitised by intraperitoneal injection of 20 µg of ovalbumin emulsified in 1 mg of aluminium hydroxide gel in a total volume of 200 µl, and challenged for 30 minutes in 7 consecutive days with an aerosol of 2 g ovalbumin in 100 ml of PBS. Then we collected BALF and isolated lymphocytes from the peripheral blood. The lymphocytes were divided into two groups: asthmatic group and normal group. Th1/Th2 cytokines was detected by enzyme-linked immunosorbent assay (ELISA) kits.
RESULTSIn the asthma group, we found numerous eosinophils and lymphocytes on the glass slides. We then confirmed that the optimal concentration of IL-33 was 10 ng/ml and time of IL-33 stimulating lymphocytes was 24 hours. In the asthma group, the production of IL-5 was significantly increased over normal group after stimulation with IL-33 (P < 0.05) and the production of IFNγ was supressed from IL-33 stimulated lymphocytes (P < 0.05).
CONCLUSIONIL-33 acts on lymphocytes of peripheral blood increasing secretion of Th2 cytokines and inhibiting secretion of Th1 cytokines.
